1.Effect of Prophylactic Use of Vitamin B6 with Chlorpheniramine on the Adverse Reactions in Fundus Fluores-cein Angiography

OBJECTIVE:To explore the effect of prophylactic use of vitamin B6 with chlorpheniramine on the adverse reactions in fundus fluorescein angiography (FFA). METHODS:326 patients with FFA were randomly divided into observation group and control group. Observation group was orally given Vitamin B6 tablet 10 mg 30 min before angiography+Chlorpheniramine maleate tablet 4 mg;control group was orally given Metoclopramide tablet 10 mg+Chlorpheniramine maleate tablet 4 mg. The occurrence time and incidence of adverse reactions in 2 groups were observed and correlation analysis was conducted. RESULTS:There was no significant difference in the occurrence time of adverse reactions between 2 groups(P>0.05);the incidence of adverse reactions in observation group was significantly lower than control group,the difference was statistically significant(P<0.05). The correlation analysis of adverse reactions in control group showed the occurrence of adverse reactions had no correlation with age and gender of patients(P>0.05). CONCLUSIONS:Prophylactic use of vitamin B6 with chlorphenir arnine can reduce the occurrence of adverse reactions in FFA.
2.Therapeutic Effect of Tonglieshu capsules for Nonbacterial Prostatitis

[ Objective ] To investigate the therapeutic effect of Tonglieshu capsules (mainly composed of Radix Linderae, Semen Litchi, Semen Vaccariae, Rhizoma Corydalis, Rhizoma Sparganii, Rhizoma Curcumae, Herba Taraxaci, etc. ) for nonbacterial prostatitis. [Methods] Forty SD rats were randomized to 4 groups: normal control group (group A) , model group (group B), Qianlietong group (group C) and Tonglieshu, Capsules (group D). Except those in group A, rats in other groups were injected with Xiaozhiling Injection directly into the prostatic glands to establish nonbacterial prostatitis. After treatment with Tonglieshu capsules for 4 weeks, white blood cells counting, density of lecithin corpuscle and inflammatory cells infiltrating into interstitium and glandular lumen of prostatic tissues were observed. [Results] WBC counting was decreased and density of lecithin corpuscle increased in group D (P 0.05) . [Conclusion] Tonglieshu capsules has an effect in treating chronic prostatitis by decreasing the WBC counting, reducing infiltration of inflammatory cells and increasing the density of lecithin corpuscle in prostatic tissues.
4.Frequency of c-kit mutation and prognosis in t(8;21) acute myeloid leukemia patients with trisomy 4

Objective To investigate the frequency of c-kit mutation and prognosis in t (8;21) acute myeloid leukemia (AML) patients with trisomy 4. Methods A total of 145 de novo t(8;21) AML patients from February 2005 to January 2013 were analyzed retrospectively. Detection of exons 8 and 17 mutation of c-kit by PCR and cytogenetic analysis by R-banding technologies were performed on bone marrow samples of all patients at diagnosis. Clinical data were collected and analyzed statistically. Results Among 145 t (8;21) AML patients, 12 cases (8.3 %) were trisomy 4, 91.7 % (11/12) of them were identified with c-kit mutation, which was significantly higher than that without trisomy 4 [26.3 % (35/133), P< 0.01]. The follow-up data showed that the patients with trisomy 4 were correlated with the lower overall survival (OS) rate (15 % vs 56 %, P< 0.01) and disease-free survival (DFS) rate (0 vs 51 %, P< 0.01) when compared with patients without trisomy 4. Furthermore, the subgroup of patients with both trisomy 4 and c-kit mutation had a worse OS and DFS (P< 0.05). Conclusions Trisomy 4 is associated with high frequency of c-kit mutation and demonstrates poor prognosis in t(8;21) AML patients. Trisomy 4 or it combined with c-kit gene mutation is the main influencing factor on the survival of the patients with t(8;21) AML.
5.Bioequivalence and relative bioavailability of domestic and imported repaglinide tablets in healthy volunteers

Objective:To evaluate bioequivalence and relative bioavailability of domestic and imported repaglinide tablets in healthy volunteers. Methods: Twenty two healthy male volunteers were randomized into A and B groups. A single dose (4 mg) of domestic and imported repaglinide tablets were given respectively according to an open 2-way crossover study design. The washout period was 1 week. Plasma concentrations of repaglinide were determined by HPLC method. Results:The pharmacokinetic parameters of domestic and imported drugs were as follows: t1/2 were(0.86±0.24) and (0.83±0.31) h;tmax were( 0.79±0.37) and (0.75±0.41) h;cmax were (52.43±20.92) and (53.32±24.94) μg/L. AUC0-t were (79.87±36.48) and (74.95±30.57) μg*h*L-1,respectively. The relative bioavailability of domestic formulation was (106.55±16.15)%. Conclusion: The results of variance analysis and two one-side t test show that 2 formulations are of bioequivalence.
6.Comparation of Two Different Methods for Rotavirus and Adenovirus Antigens Detection

Objective To compare the detecting results of rotavirus (RV)and adenovirus (AdV)antigens using auto stool pretreatment system (machine method)and manual method.Methods A total of 100 stools collectecd from diarrear patients admitted in gastroenterology outpatient department from September 2014 to Octorber 2014 in Peking University Medical College Hospital were detected to identify RV and AdV antigens using machine method and manual method respectively,and the nucletic acids of positive samples were detected by liquid chip method to verify the results.Results The RV,AdV and co-infection antigen positive detection rate using machine method were 17.0% (17/100),25.0% (25/100)and 12.0% (12/100)respectively,whereas those using the manual method were 4.0% (4/100),13.0% (13/100)and 2.0% (2/100),re-spectively.Taking the nucletic acids detection as the golden method,the false positive detection rate of RV,AdV and co-in-fection antigen using machine method were 23.5% (4/17),20.0% (5/25)and 33.3% (4/12)respectively,whereas those u-sing the manual method were 75.0% (3/4),69.2% (9/13)and 50.0% (1/2),respectively.χ2 test for paired data for RV and AdV positive detection rate,false positive detection rate of RV and false positive detection rate of AdV using two meth-ods were statistically significant (χ2 =15.0,52.8 and 47.5,P values <0.05).Two methods for detecting RV and AdV had poor consistency (Kappa value was 0.25,Kappa values <0.4).Conclusion Machine method has much more advantage on RV and AdV positive detection rate and false positive detection rate than manual method,which is good for clinical applica-tion.
7.Research on Construction Methods of Traditional Chinese Medicine Health Maintenance

This article was aimed to explore the method of traditional Chinese medicine (TCM) health maintenance knowledge base building. TCM health maintenance knowledge classification method was studied under the guidance of TCM health maintenance classification system to construct a special literature database to store TCM health maintenance knowledge. Based on TCM literature resources, TCM health maintenance knowledge service platform was designed and developed for the static display and dynamic query support of TCM health maintenance knowledge. The content of knowledge base will be accessed to the whole society. The results showed that the construction method can be a system arrangement and modern interpretation of TCM health maintenance knowledge, in order to support TCM health maintenance knowledge base and its update and service system. It was concluded that under the guidance of TCM theories, the construction of TCM health maintenance knowledge base will realize the health maintenance knowledge-sharing and service requirement of TCM.
8.Evaluation of an automated morphological analysis system in white blood cell identificaton

Objective To evaluate the clinical performance of an automated image analysis systems named CellaVision DM96 in classifying White Blood Cells.Methods A total of 2267 peripheral blood samples (male 1 235, female 1 034, average age 46) were obtained from outpatient and inpatient in Peking Union Medical College Hospital ( PUMCH ) . These samples were selected to evaluate the precision, sensitivity, specificity and the analytical error of the system.We first evaluated the coincidence rate of reclassification and manual microscopy.On the base of favourable coincidence rate, we then evaluated the correlations between the pre-classification and reclassification of segmented neutrophil, band neutrophil, lymphocyte, monocyte, eosinophile, basophile, blast cell, promyelocyte, myelocyte, metamyelocyte, plasma cell and reactive lymphocyte.Results The sensitivity and specificity of pre-classification of White Blood Cell were 46% -100% and 24%-92%, respectively.When studied on the cell level, the total coincidence rate of pre-classification was 88%.And the coincidence rates of pre-classification and reclassification of White Blood Cell were 6%-95% and 25%-100%, respectively.When assessed on the sample level, the coincidence rates of pre-classification and reclassification of leukocytes were 64%-98%and 84%-100%, respectively.The correlations of pre-classification and reclassification of leukocytes in order from high to low were: lymphocyte, segmented neutrophil, eosinophile, band neutrophil, monocyte, basophile, when r were 0.943 9, 0.915 2, 0.785 4, 0.775 6, 0.676 2 and 0.289 1, respectively.The correlations between reclassification and manual microscopy of White Blood Cell were higher than those between pre-classification and manual microscopy.Order from high to low was: eosinophile, segmented neutrophil, lymphocyte, monocyte, band neutrophil, basophile.And r were 0.972 1, 0.968 5, 0.957 0, 0.831 9, 0.800 6 and 0.648 7, respectively.The ability of this automated image analysis systems at pre-classification in distinguishing between band cell and segment cell, atypical lymphocyte and normal lymphocyte was not good. Conclusion The performance of reclassification was better than pre-classification.The reclassification can be substitute for the microscopy inspection, and be used in the Clinical practice.
9.Serum levels of miR-134 and miR-146b in elderly patients with acute ischemic stroke and their prognostic value

Objective:To investigate the prognostic value of serum levels of miR-134 and miR-146b in elderly patients with acute ischemic stroke(AIS).Methods:A total of 162 elderly patients with AIS admitted to the Third People's Hospital of Haikou from January 2017 to October 2019 were enrolled.According to modified Rankin Scale(mRS)scores, they were divided into the good prognosis group(n=98, mRS score≤2)and the poor prognosis group(n=64, mRS score>2). Based on the National Institutes of Health Stroke Scale(NIHSS), patients were divide into the mild group(n=46, NIHSS score<5), moderate group(n=75, 5≤NIHSS score≤20), and severe group(n=41, NIHSS score>20). Real-time fluorescence quantitative PCR was used to detect serum levels of miR-134 and miR-146b in each group.Multivariate logistic regression was used to analyze risk factors for poor prognosis in elderly patients with AIS.The receiver operating characteristic(ROC)curve was used to analyze the value of serum miR-134 and miR-146b levels in predicting poor prognosis of elderly patients with AIS.Pearson correlation was used to analyze the correlations of serum levels of miR-134 and miR-146b with NIHSS and mRS scores in elderly patients with AIS.Results:Serum levels of miR-134(3.26±1.13 vs.0.85±0.38)and miR-146b(2.27±0.93 vs.0.56±0.21)were higher in the AIS group than in the control group( t=14.360 and 12.527, P<0.01). Serum levels of miR-134(4.35±1.46 vs.2.28±0.85)and miR-146b(3.07±1.04 vs.1.51±0.66)were higher in the poor prognosis group than in the good prognosis group( t=13.520 and 11.242, P<0.01). Serum levels of miR-134 and miR-146b in the severe group were higher than in the moderate and mild groups( t=10.815 and 9.462, P<0.01), and they were also higher in the moderate group than in the mild group( t=13.627, 11.611, P<0.01). Multivariate Logistic regression analysis showed that serum miR-134( OR=2.470, 95% CI: 1.603-4.927)and miR-146b( OR=1.914, 95% CI: 1.350-3.406)were risk factors for poor prognosis in elderly AIS patients( P<0.05). The ROC curve analysis showed that the optimal cut-off values of serum miR-134 and miR-146b to predict poor prognosis in elderly AIS patients were 3.84 and 2.68, respectively.The area under the ROC curve(AUC)of combined serum miR-134 and miR-146b(0.926, 95% CI: 0.865-0.987)for the prediction of poor prognosis was higher than that of either marker alone, with a sensitivity and specificity of 92.4% and 86.2%, respectively.The correlation analysis showed that serum levels of miR-134 and miR-146b were positively correlated with NIHSS and mRS scores in elderly AIS patients( r=0.806, 0.871, 0.785 and 0.842, all P<0.01). Conclusions:Increased serum miR-134 and miR-146b levels are correlated with the severity of neurological impairment and prognosis in elderly patients with AIS.Serum miR-134 and miR-146b levels together have a high value in predicting poor prognosis in elderly AIS patients.
10.Application of iterative reconstruction in upper limb intravascular fistula CT angiography

Objective To assess the image quality (IQ) of an iterative reconstruction (IR) technique(sinogram affirmed iterative reconstruction,SAFIRE) for upper limb intravascular fistula CT angiography on a second generation dual-source CT scanner and determine the optimal dose level using IR with IQ comparable to filtered back projection(FBP).Methods The experiment was a prospective study.A total of 150 patients were averagely divided into 5 groups to perform upper limb intravascular fistula CT angiography.The control group was scanned with conventional tube output (120 kV,110 mAs) and images were reconstructed with FBP.The tube current of the other 4 groups (reconstructed with SAFIRE) was successively reduced to 70%,60%,50% and 40% of that of control group.Quantitative measurements of signal-to-noise ratio (SNR) and contrast-to-noise ratio (CNR) were obtained in two regions of interest (ROI) in each group.The ROIs were placed at the subclavian artery near shoulder joint,and the stoma of radial artery and cephalic vein.Analysis of variance (ANOVA) was used for comparisons of objective evaluation parameters (SNR,CNR) and radiation dose (CTDIvol,DLP,E) among 5 groups.Two radiologists graded IQ in a blinded fashion on a 4-point scale (4-excellent,3-good,2-fair and 1-poor).The Kruskal-Wallis nonparametric test was performed for detection of differences in subjective evaluation of IQ among groups.Results The radiation doses of groups 77,66,55 and 44 mAs were 70%,55%,40% and 25% of the control group respectively.The differences of SNR and CNR among groups 77,66,44 mAs and the control group were statistically significant in ROI1 (F =24.018,20.386,P < 0.05),SNR and CNR of group 77 and 66 mAs were higher than the control group while group 44 mAs was lower.The difference of SNR and CNR among group 55 mAs and the control group was not statistically significant in ROI1 (P > 0.05).The differences of SNR and CNR among groups 77,66,55 mAs and the control group were statistically significant in ROI2 (F =15.934,13.818,P < 0.05),and groups 77,66 and 55 mAs had higher SNR and CNR.The difference of SNR and CNR between group 44 mAs and the control group was not statistically significant in ROI2 (P > 0.05).Groups 77,66,55 mAs and the control group had higher scores of the subjective IQ (≥ 3) which was descended in group 44 mAs.The differences of contrast,sharpness,noise and general impression of images among the 5 groups were statistically significant by Kruskal-Wallis nonparametric test(H =10.268,14.542,15.840,11.014,P < 0.05).Using the criterion of excellent IQ(score 4),the ROC curve of dose levels and IQ acceptability established a reduction of 60% of effective dose [Group 55 mAs:(3.545 ± 1.396) mSv] as an optimum cutoff point (AUC:0.844,95% CI:0.705-0.982).Conclusions Iterative reconstruction technique could provide a dose reduction to 40% (3.545 mSv) compared with the filtered back projection in upper limb intravascular fistula CT angiography without reducing equivalent image quality.
