1.Determination of Circulating Antigen in Neurocysticercosis Patients by Monoclonal Antibody

Objective To detect circulating antigen (Cag) for diagnosing neurocysticercosis. Method ELISA was performed with monoclonal antibody 4B6 against the cyst fluid antigen of Cyticercus cellulosae for detecting Cag in serum and/or cerebrospinal fluid (CSF) of patients with neurocysticercosis or with other diseases. Results In the group of 82 cases of neurocysticercosis, the positive rate of serum Cag was 79.2% (65/82) and the positive rate of CSF Cag was 100% (26/26). After chemotherapy for 20 cases with positive serum Cag, the titer of serum Cag in 17 cases dropped to zero(85% ). Cag could not be detected in specimens from patients with other diseases. Conclusion These results indicate that the determination of Cag, especially of the CSF Cag, is useful for the diagnosis of neurocysticercosis and the drop in serum Cag is a good parameter for the evaluation of the effectiveness of chemotherapy.
2.Priliminary outcomes of ARCH plate in cervical expansive open-door laminoplasty for OPLL

Objective To evaluate the short-term results of ARCH plate in cerical expansive open-door laminoplasty(ELAP)for OPLL.Methods From June 2010 to September 2011,a total of 12 cases with OPLL underwent open-door ELAP by ARCH plate fixation and were followed up in our hospital.8 cases were males and 4 cases were female.The average age was 60.5 years.The neurological effect was evaluated by use Japanese Orthopedic Association(JOA)scoring criteria for cervical myelopathy.The ana-tomical effect was analyzed by compare roentgenogram and CT before and after surgery.Results The JOA scores were improved for 39% after the operation,while the saggital diameters of the cervical spinal canaldural after operation was enlarged for 82%(P<0.05).X-ray films and CT scan after operation that there was no occurrence of re-close of door,there was no occurrence of anchor loosing.Conclusion For OPLL,ELAP using ARCH plate fixation bring less occurrence of re-close of door,slight postoperative neck AS,and satisfactory clinical outcomes.

AIM To research effects of sodium ferulate(SF) on transforming growth factor ?_1 inhibited hepatocyte growth. METHODS Human cultured hepatocytes(L02 cells) served as control group ,TGF?_1(5 ?g?L -1 ) was used to inhibited L02 cells to construct the cell model. MTT methods were used to study the growth inhibitory effect of TGF?_1 on L02 cells. DNA synthesis was analyzed by measuring 3H-TdR incorporation. The effects of SF on the changes of cell cycle were analyzed by flow cytometry. Generation of intracellular ROS was determined by the emission of fluorescence in L02 cells proloaded with DCFH-DA fluoresin and then treated them with TGF?_1 or SF. 4.Synthesis and antifungal evaluation of chalcone derivatives combined with fluconazole against drug-resistant Candida albicans

Chalcone is a common scaffold in natural products with optimal properties and biological activities.In this study, we designed and prepared eight new coumarin-chalcone derivatives (5a-5h), and confirmed their structures by 1H NMR and 13C NMR. Their in vitro antifungal activity combined with fluconazole (FLC) against drug-resistant Candida albicans was tested by microdilution method.The results indicated that most chalcone derivatives showed good antifungal activity against drug resistant Candida albicans with FLC, particularly with compound 5g displaying better antifungal activity (MIC50 = 5.60 μg/mL) than FLC (MIC50 = 200 μg/mL) when combined with FLC, so, these derivatives could be used as synergists of antifungal drugs.
5.Application value of SPECT-CT fusion imaging in moyamoya disease revascularization:a clinical study

Objective To investigate the application value of identification of the scalp surface locations of cerebral ischemia lesions before direct revascularization for moyamoya disease and to design surgical approaches according to this by using the fusion of single photon emission computed tomography ( SPECT) cerebral perfusion imaging with CT imaging. Methods The clinical data of 13 adult patients with ischemic-type moyamoya disease underwent superficial temporal artery-middle cerebral artery bypass surgery were analyzed retrospectively. SPECT cerebral perfusion imaging was fused with CT imaging of the same machine before procedure. The lesions of ischemia were located on the cortical surface. The surgical approaches were designed at the center of the ischemic lesions. The patients were followed up for 6 to 12 months after procedure. The improvement of clinical symptoms and cerebral perfusion of the patients were observed after operation. Results One patient had perioperative cerebral hyperperfusion syndrome,and the others did not have any perioperative complications. At one-month follow-up, the improvement of symptoms in 4 patients were excellent,in 5 were good,in 4 were fair,and none was poor. At 6 to 12 month follow-up,the improvement of symptoms in 9 patients were excellent,in 4 were good,and none was poor. The postoperative visual SPECT cerebral perfusion imaging analysis suggested that the cerebral perfusion was improved significantly as compared with before procedure in all patients. Quantitative analysis:There was significant difference in target ischemic lesions between preoperative Fb and postoperative Fb ([2. 13±1. 06]% vs. [4. 13±2. 09]%;P<0. 05). There was significant difference between preoperative Fb and Fe ([2. 46±1. 97]% vs. [2. 13±1. 06]%;P<0. 05). The postoperative BFCR was [67. 57±3. 78]%( >50%) , which indicated that the efficacy of the procedure was remarkable. The superficial temporal arteries fed to brain of the patients were observed after procedure by using the head CT angiography. The postoperative head MRI reexamination showed no new infarcts occurred at 6 months. Conclusion Combine SPECT cerebral perfusion imaging with CT imaging to design surgical approach for superficial temporal artery-middle cerebral artery bypass surgery may improve the efficacy and reduce the risks of operation.
6.The evaluation value of severity and prognosis of septic shock patients based on the arterial-to-venous carbon dioxide difference

Objective To assess the value of central venous-to-arterial carbon dioxide difference [ P( cv-a) CO2 ] in evaluation of disease severity and prognosis in patients with septic shock.Methods There were 45 consecutive resuscitated septic shock patients from April 2009 to October 2010 included immediately after their admission into our ICU.The patients were divided into low P(cv-a) CO2 group and high P(cv-a) CO2 group according to a threshold of 6 mm Hg ( 1 mm Hg=0.133 kPa).All patients were treated by early goal directed therapy (EGDT).The parameters of hemodynamics,lactate clearance rate,the Acute Physiology and Chronic Health Evaluation Ⅱ (APACHE Ⅱ ) score,the sequential organ failure assessment (SOFA)score,6 h rate of EGDT achievement,the ICU mortality and 28 days in-hospital mortality were recorded for all patients.Results There were 30 patients in the low P(cv-a) CO2 group,and 15 in the high P(cv-a)CO2group.There were no significant differences between low P(cv-a) CO2 and high P(cv-a) CO2 patients in age,APACHE Ⅱ score and SOFA score (all P > 0.05 ).Compared with the high P(cv-a)CO2 group,the low P(cv-a) CO2 group had higher cardiac index ( CI ) and 24 h CI,higher delivery O2 ( DO2 ) and 24 h DO2,higher central venous oxygen saturation ( ScvO2 ) [ (74 ± 9) % vs (67 ± 8) % ],lower lactate [ ( 3.4 ± 2.1 )mmol/L vs (5.7 ± 4.5 ) mmol/L] and higher △SOFA score [ (0.7 ± 1.8 ) vs ( - 0.4 ± 1.1 ) ],lower 24 h SOFA score [ (7.8 ± 2.0) vs (9.8 ± 2.0 ) ],higher 6 h rate of EGDT achievement ( 83.3% vs 53.3 % )(P < 0.05 ),however,there were no differences in 28 days mortality and ICU mortality between the two groups ( P > 0.05 ).Conclusion P(cv-a) CO2 might be an indicator for predicting the severity of patients with septic shock and evaluating tissue perfusion.
7.Analysis of the effect of different surgical methods in the treatment of uterine prolapse in the reproductive period

Objective:To investigate the therapeutic clinical effect of laparoscopic high uterosacral ligament suspension (LHUS) combined with cervical amputation and vaginal sacrospinal ligament fixation (SSLF) in the treatment of prolapse of uterus in childbearing period.Methods:From October 2011 to December 2016, the clinical data of 78 patients with uterine prolapse above grade Ⅱ treated in Department of Gynecology, Jizhong Energy Fengfeng Group Hospital were retrospectively analyzed.According to different surgical methods, the patients were divided into LHUS group (40 cases) and SSLF group (38 cases). The operation time, intraoperative bleeding volume and hospital stay of the two groups were compared by independent sample t test.Before and 6 months after the operation, the prolapse quality of life questionnaire (P-QOL), pelvic organ prolapse/incontinence sexual function questionnaire-12 (PISQ-12) and pelvic floor distress inventory-short form 20 (PISQ-20) were used.PFDI-20 and pelvic floor impact questionnaire-short 7 (PFIQ-7) were used to evaluate patients′ quality of life, sexual function and pelvic floor function.Paired t test was used to compare the above scores before and 6 months after operation in the same group.Results:There were no significant differences in operation time, intraoperative bleeding volume and hospital stay between the two groups ( t value were 1.593, 1.203 and 0.535, , respectively, all P>0.05). The preoperative P-QOL, PISQ-12, PFDI-20, and PFIQ-7 scores of patients in the LHUS group were (55.4±11.1), (25.1±4.6), (15.0±4.9), (8.9±2.8) points, and (53.7±10.5), (23.9±3.7), (14.1±4.2), (9.2±3.0) in the SSLF group.There was no statistically significant difference in the indexes between the two groups before operation (t value were 0.694, 1.265, 0.869 and 0.457, respectively, all P>0.05). The scores of P-QOL, PISQ-12 at 6 months after operation in LHUS group((87.9±12.0), (39.1±6.1)) and SSLF group((81.3±11.7), (35.6±4.0)) were significantly higher than those before operation( t value were 12.574, 11.589, 10.823 and 13.236, respectively, all P<0.001). The scores of P-QOL and PISQ-12 at 6 months after operation in the two groups were higher than those before operation((87.9±12.0), (39.1±6.1)), and the P-QOL score of LHUS group was higher than that of SSLF group 6 months after operation, the difference was statistically significant ( t value were 12.574, 11.589, 10.823 and 13.236, respectively, all P<0.001). At 6 months after operation, PFDI-20 and PFIQ-7 scores in LHUS group((2.1±0.3), (1.3±0.2) points) were significantly lower than before operation ( t value were=16.619 and, 17.123, all P<0.001). The PFDI-20 and PFIQ-7 scores in LIHUS group were lower than those in SSLF group((2.7±0.3), (1.9±0.2)) at 6 months after operation (t values were 10.096 and 13.073, respectively, all P<0.001). Conclusion:LHUS combined with cervical resection in the treatment of prolapsed uterine prolapse can effectively improve the quality of life, sexual function, and pelvic floor function.The effect is better than SSLF, which is worthy of clinical promotion.
8.Analysis on result of genetic screen and gene types of 10854 cases with thalassemia

Objective To investigate the gene carrying rate,gene type and composition ratio of thalassemia among pre-pregnant population in Chongqing area.Methods A total of 1054 people were enrolled in the hospital from April 2014 to March 2016 for thalassemia screening.The content of screening included mean corpuscular volume (MCV),mean corpuscular hemoglobin (MCH) and hemoglobin electrophoresis.Thalassemia gene was examined in people with any abnormal term of screening result.Results In 10854 cases,1117 cases showed positive in thalassemia primary screening,and the positive rate was 10.29%.458 cases were tested positive of thalassemia gene,the carrying rate of thalassemia was 4.21%.In which,253 cases of pure a-thalassemia were tested.The carrying rate of α-thalassemia was 2.33%.The most common kind in α-thalassemia was--SEA whose constituent ratio were 52.17 %.197 cases of pure β-thalassemia were tested,the carrying rate of β-thalassemia was 1.81%.The most common kind in β-thalassemia was CD17 (A→T),whose constituent ratio were 31.47 %.11 cases were diagnosed with αβ-thalassemia.Conclusion Chongqing is high-prevalence area of thalassemia.It is important to conduct thalassemia genetic screen before pregnancy which plays a vital role in improving population quality and achieving prepotency.
9.Effect analysis of dynamic blood glucose monitoring and clinical intervention in the middle and late pregnant women with diabetes mellitus

Objective To investigate the clinical value of dynamic glucose monitoring in the middle and late pregnant women with diabetes mellitus,and the related clinical and nursing measures.Methods One hundred admitting cases of the middle and late pregnant women with diabetes mellitus were grouped according to the random number table,50 cases of the control group were treated with finger tip blood glucose monitoring and conventional treatment,50 cases of dynamic blood glucose monitoring and comprehensive clinical and nursing intervention for the observation group.The blood glucose control situation with different time,the incidence of hypoglycemia,the actual control range of the glycosylated hemoglobinblood and the length of hospital stay and so on were evaluated in two groups.Results The incidence of hypoglycemia in the observation group was 6.0% (3/50),the control group was 24.0% (12/50),the difference was statistically significant (P < 0.05).In the early morning,after breakfast 2 h,before and after lunch 2 h,before and after dinner 2 h and before bed and other different time,the blood glucose level with the observation group was lower than that in the control group (P < 0.05).At the same time,in the blood glucose effective controlling time,hospitalization time,the observation group was better than the control group (P < 0.05).The blood glucose level and the detection range of the glycosylated hemoglobin were also detected in the safe state for outpatient follow-up after hospital discharge.Conclusions The intervention of dynamic blood glucose monitoring and comprehensive clinical and nursing measures in middle and late pregnant women with diabetes mellitus is conducive to control blood glucose level timely and accurately and to improve their prognosis.It is the key to the success of the future obstetric prevention and diagnosis and treatment of the disease,and is recommended clinical popularization.
10.Optimization research on preparation conditions of polymer-shelled microbubble contrast agents

BACKGROUND:In previous experiments, a polymer-shel ed microbubble contrast agent was successful y prepared using L-polylactic acid-polyethylene glycol methyl-covered liquid fluorocarbon perfluoropentane, which developed wel in vivo and in vitro under a second harmonic contrast-enhanced ultrasound with a low mechanical index OBJECTIVE:To optimize the conditions for preparing polymer-shel ed microbubble contrast agents with high yield and suitable size. METHODS:The polymer-shel ed microbubble contrast agent containing perfluoropentane was prepared by single emulsion method. Three preparation conditions, including polymer mass/perfluoropentane volume ratio (4/1, 2/1, 1/1, 1/2), hemogenate speed (18 000, 26 000, 35 000 r/min) and time (15, 30, 60, 120 seconds), were optimized, and the microbubble yield and size were set as judge standard. The contrast-enhanced ultrasonography of rabbit kidney was performed using the optimized microbubbles, and the arrival time, time to peak, half time of descending and peak intensity were analyzed by TCA software. RESULTS AND CONCLUSION:The preparation conditions were optimized as a polymer mass/perfluoropentane volume ratio of 2/1, homogenate speed of 26 000 r/min and homogenate time of 60 seconds, which presented a high microbubble yield of (1.8±0.4)×109/mL and mean size of (3.7±1.3)μm with uniform distribution. The arrival time, time to peak, half time of descending and peak intensity were detected as (3.1±0.6) seconds, (2.9±0.5) seconds, (4.0±0.7) seconds and (4.7±1.1)×10-5 AU, respectively. The polymeric microbubbles prepared by optimized conditions were of high yield, with suitable size and good contrast-enhanced effect in vivo.
