1.Analysis of the influencing factors for screening primary aldosteronism

Objective To confirm the most effectively screening conditions by comparing the ARR values of different posi‐tions after saline load test .Methods After exclusion the other secondary hypertension ,the completion of saline dressing test ,84 ca‐ses diagnosed hypertensive patients under saline load test results were divided into primary aldosteronism group (pH ,52 cases) and primary hypertension group (32 cases) .Analyzed plasma aldosterone (PAC) ,renin (PRA) values of upright and decumbent body positions ,their ARR values were calculated and the urine aldosterone was tested .Results Those whose upright position ARR rati‐os higher than 240(pg/mL)/(ng · mL -1 · h-1 ) were accounted for 80 .77% (42/52) and 43 .75% (14/32) ,respectively in the both PA group and PH group;Those whose upright position ARR ratios higher than 240(pg/mL)/(ng · mL -1 · h-1 ) and decumbent position PAC higher than 150 pg/mL were accounted for 65 .39% (34/52) and 3 .13% (1/32) ,respectively in the both PA group and ph group;The specificity of this method for PA was 96 .88% .Those whose upright position ARR ratios higher than 240(pg/mL)/(ng · mL -1 · h-1 ) and decumbent position PAC higher than 150 pg/mL and urine aldosterone higher than 10 μg/24 h ,The specificity of this method for PA was 100 .00% .Conclusion It is an effective screening method for primary aldosteronism when ARR values of upright position higher than 240(pg/mL)/(ng · mL -1 · h-1 ) ,PAC of decumbent position higher than 150 pg/mL and urine aldosterone higher than 10 μg/24 h .

3.Study on perioperative use of antibiotics in cesarean section

Objective To analyze the use of antibiotics in perioperative period of cesarean section,and to summarize the experience of scientific intervention to promote rational drug use.Method 192 cesarean section included in the study were inpatients in December 2015 to December 2016 in our hospital,were grouped according to the order of admission,divided into control group(95 cases)and observation group(97 cases),were given routine perioperative antibiotic treatment.The observation group was treated with scientific medication intervention,and the control group was not intervened.The two groups of white blood cell count,postoperative body temperature and hospitalization time were counted,the infection types and infection rate were counted,and the treatment costs of the two groups were counted.Results The white blood cell count of the observation group was significantly higher than the control group,postoperative body temperature recovery time and hospitalization time was significantly shorter than the control group,the difference was significant(P<0.05); after two groups appeared infection phenomenon,including urinary tract infection,incision infection and puerperal infection as well as intrauterine infection,and two groups in different types of infection and the infection rate by comparison there was no statistically significant difference; statistical analysis and comparison of treatment related costs of the two groups of patients,to observe antibacterial drug group,the total cost of expenses and hospitalization costs were significantly lower than that of the control group,and patients with antimicrobial drugs accounted for the total proportion of drugs were significantly lower than the control group,the difference was statistically significant(P<0.05).Conclusion Cesarean section perioperative use of antibiotics in the process,through the implementation of scientific intervention can obtain ideal effect,promote rational and normative clinical medication,reduce hospitalization expenses and antimicrobial drug costs,promote the recovery of patients after surgery,and does not increase the infection after operation.
4.Analgesic effect of continuous epidural infusion of ropivacaine supplemented by patient-controlled intravenous analgesia with lornoxicam after hysterectomy

Objective To compare the analgesic and side effects of patient-controlled intravenous analgesia(PCIA) with lornoxicam and morphine supplemented by continuous epidural infusion of 0.2% ropivacaine. Methods Sixty ASA I - II patients undergoing elective hysterectomy under epidural anesthesia were randomly divided into two groups: lornoxicam group (L, n = 30) and morphine group (M, n=30). For postoperative analgesia both groups received continuous epidural infusion of 0.2% ropivacaine 4 ml'h . In group L the patients received PICA with 0.008% lornoxicam and in group M 0.001% morphine. The bolus dose was 1 ml, lock-out interval 5 min and maximal amount within 1 h was 12 ml in both groups. If the patient still felt pain after pressing PCA pump 12 times, an additional epidural bolus of 0.2% ropivacaine 5 ml and morphine 0. 5mg was given. The clinical effects were assessed by (1) VAS score of pain( 0 = no pain, 10 = severe pain), (2) Bruggrmann comfort score (0 = persistent pain, 4 = no pain at coughing), (3) modified Bromage score, (4)patients satisfaction, (5) the number of pressing of PCIA pump by the patient (D1 ) at 30 min, 1,4,8,12,16,20,24h and the number of bolus dose delivered (D2), (5) the time when the patient passed gas and (7) complications such as nausea and vomiting and respiratory depression.Results The demographic data were comparable between the two groups. The amount of ropivacaine consumed was 192 mg in both groups.The amount of lornoxicam consumed was (3.4 +2.8) mg in group L and the amount of morphine consumed was (4.7 + 3.5) mg in group M. There was no statistically significant difference in VAS and Bruggrmann comfort score between the two groups. There were less side-effects in group L which were also milder as compared with those in group M.Conclusion The results suggest that continuous epidural infusion of 0.2% ropivacaine combined with lornoxicam in PCIA can provide better analgesia and reduce drug consumption in PICA. The efficacy of analgesia providedby lornoxicam is similar to morphine but lornoxicam causes less side-effects than morphine.
5.The analgesic effect of continuous infusion of ropivacaine with epidural catheter placed at different interspaces after gynecologic surgery

Objective To assess the analgesic effect of continuous infusion of 0.2% ropivacaine with epidural catheter placed at T11-12 or L2-3 after abdominal hysterectomy. Methods Eighty ASA Ⅰ -Ⅱ patients undergoing elective abdominal hysterectomy were randomly divided into 4 groups with 20 patients in each group : in group Al and A2 the epidural catheter was placed at T11-12 and in group Bl and B2 at L2-3 . After surgery two infusion pumps were used. The first pump was used for continuous epidural infusion of 0.2% ropivacaine in the 4 groups. The second pump was used for patient controlled intravenous analgesia (PCIA) with 0.08% lornoxicam in group Al and Bl or with 0.1% morphine in group A2 and B2. The PCIA bolus dose was 1 ml with a lockout time of 5 min. The analgesic effect (assessed using VAS) and the consumption of lomoxicam / morphine were compared among the four groups. Results The ropivacaine consumption was 192 mg during the 24 h after operation in the 4 groups. The lornoxicam and morphine consumption were (3.9?2.8) mg and (4.6?3.5) mg in group Al and A2 with the epidural catheter placed at T11-12 and (7.7?2.5) mg and (7.8?2.4) mg in group B2 and B2 with catheter placed at L2-3.The consumption of lomoxicam or morphine was significantly less with epidural catheter placed at T11-12 than that with epidural catheter at L2-3 (P
6.Preliminary experience of percutaneous transforaminal endoscopic spine system in thoracic discectomy for disc herniation

Objective To investigate the feasibility of percutaneous transforaminal endoscopic spine system in thoracic discectomy for disc herniation. Methods One patient with thoracic disc herniation involved the level of vertebral segment in T11/12 was treated with percutaneous transforaminal endoscopic spine system and followed up for 1 month. The targeted puncture was performed under local anesthesia and fluoroscopic guidance with patient in prone position. The foramen of T 11/12 was enlarged gradually with four trephinations, and the working cannula was inserted transforaminal into the canal. Then the herniation was exposed and removed with full endoscopic technique, including the loosen nucleus pulposus. The dural sac was exposed and released adequately. Drainage was placed during operation. Results The procedure was successfully carried out and the dural sac was completely released. The drainage was removed in the second day of operation. The patient could walk in the third day after operation with obvious relief of back and leg pain. At the follow-up of one month postoperation, the visual analogue scale of leg pain decreased from 8 to 1, and the Oswestry disability index (ODI) decreased from 64 to 4. According to MacNab scale, excellent result was acquired. Conclusion There is the feasibility of the percutaneous transforaminal endoscopic spine system in thoracic discectomy for disc herniation. It is a good minimal invasive technique with good results and high technical requirements for surgeons.
7.Optimization of Ethanol Reflux Extraction Process for Aitong Cataplasm by Weighted Score Orthogonal Test

Objective:To optimize the ethanol reflux extraction process for Aitong cataplasm .Methods: Orthogonal design was used to investigate the effect of solid-liquid ratio,extraction time and times on the extraction technology of Aitong cataplasm with the content of asarinin and dry extract yield as the indices .Results:The best extraction conditions were as follows:extracted 3 times with 8-fold amount of 70%ethanol, and 1 hour for each time.Conclusion: The optimized extraction process is stable and feasible , which can be used to extract Aitong cataplasm .

Objective To explore body mass index(BMI),glucose and lipid metabolism status of high -nor-mal blood pressure that selected Uygur,Kazak and Han nationalities in parts of Xinjiang.Methods Some representa-tive regions were selected,including Fuhai County of Altay,Xinyuan County of Yili,Yanqi County of Korla and Pasto-ral of Urumqi South Mountain in August 2012 -2014 August,each region was randomly selected two survey points respectively,400 -450 people were expected to conduct on -site investigations in every survey point,and 1 046 cases were collected containing Uygur,Kazak and Han nationalities who were conformed to diagnostic standard,206 cases in which the Kazak (115 men and 91 women),Han 518 cases (277 men and 241 women),322 cases of Uygur (174 men and 148 women).in order to realize the BMI,waistline,lipid metabolism and fasting glucose status.Results (1)BMI was descending:Kazak >Uygur >Han,between the three groups respectively were statistically significant differences[(26.07 ±3.23)kg/m2 ,(25.85 ±4.09)kg/m2 ,(24.63 ±3.00)kg/m2 ,F =19.829,P <0.05],sub-group analyzes:BMI for men descending:Kazak >Uygur >Han,comparison between groups of Han,Uygur and Kazak provided results which were statistically significant(F =6.752,P <0.05);but statistical significance merely existed in Han and Kazak female comparison(F =2.248,P <0.05).Waist comparison showed a decreasing sequence:Uygur>Kazak >Han,there were no statistically significant difference between every three groups[(86.32 ±8.63)cm, (85.50 ±10.11)cm,(84.06 ±7.57)cm,F =1.675,P >0.05],gender comparison was found the same characteristics. (2)Blood fat comparison between different ethnic:Descending order of serum TG:Kazak >Han >Uygur,comparison among groups of Han,Uygur and Kazak differences were statistically significant(F =5.829,P <0.05),comparison among groups of Han,Uygur and Kazak differences were statistically significant(F =5.829,P <0.05).Comparison of Serum TC:Han >Uygur >Kazak.Statistical significance indwelled in Han,Uygur and Kazak comparison (F =5.298, P <0.05);The result of comparing serum HDL -C,LDL -C in different ethnic groups:there was no statistically significant difference between different group (F =1.026,0.744,all P >0.05).Descending order was presenting in fasting glucose:Han >Uygur >Kazak.Statistical significance appears in Han,Uygur and Kazak comparison (F =8.989,P <0.05).Conclusion High -normal blood pressure of Uygur,Kazak and Han nationalities who appeared glucose metabolic disorders,BMI and waistline increase(overweight and obesity),combined with the characteristics of different national regional life,it is of great significance to prevent and delay the occurrence of hypertension through strengthening the management and intervention of high -normal blood pressure crowd,giving medical care to the people suffering from overweight,obesity,and glucose metabolic disorders,and making full preparation for the primary of hypertension.
9.Investigation on States of Depression and Anxienty of Military Policemen Who Taking Part in Anti-Terrorism Practices

Objective To explore the effect of anti-terrorism practices on the psychological health of military policemen.Methods The health statement of 101 soldiers and officers who took part in the anti-terrorism practices was estimated with Zung Self-Rating Anxiety Scale (SAS) and Self-Rating Depression Scale (SDS).ResultsThe total raw score, positive titles average score and negative titles score of SAS of these military policemen were all higher than national norm ( PP<0.05). The total raw score and standrad score of SDS were also higher than national norm, but without statistical meaning( P>0.05); while, the scores of hopelessness and impractical life feeling were significantly lower than national norm ( P<0.01), the scores of apptite decreasing, the weight lighten, easy tired were significantly higher than national norm ( P<0.01).ConclusionThe military policemen have higher anxietical feeling and some level of somatizational depression.
10.The design and clinical application of MED-LIF with mobile microendoscopic discectomy technique

Objective To evaluate the feasibility and clinical efficacy of microendoscopic discectomy-lumbar interbody fusion (MED-LIF) with mobile microendoscopic discectomy (MMED) technique. Methods The MMED includes outer working canal and inner operating canal, and large working canals (12 mm and 14 mm) are fabricated for this operation. The operation was designed as follow:an incision was made between pedicle projection sites and spinous process on the side with prominent symptom. Working canal was inserted along spinous process and a fenestration was performed. After discectomy and ipsilateral decompression, contralateral nerve was decompressed in case of contralateral stenosis. Then the intervertebral space was prepared and grafted. The inner operating canal was removed and the suitable cage was inserted, followed by percutaneous pedicles screws installation, reduction and fixation. A total of 102 patients with lumbar degenerative disc disease were treated by this technique. The index levels included L34 (n=11), L45 (n=64), L5S1 (n=21), L3-5 (n=3), and L4-S1(n=3). The operative data and follow-up results were recorded and evaluated. Results Surgery was successful in all patients, with no nerve injury or conversion to open surgery. The mean operative time was ( 120 ± 30) min (range, 90-200 min), with a mean blood loss of (120 ± 80) mL (range, 50-300 mL). The post-operative X-ray and CT scans showed improvement of spinal alignment with sufficient decompression. Patients were followed up for 6 to 36 months. The Oswestry disability index (ODI) score decreased from the pre-operative 44.2%±16.3%to the last follow-up 4.9%±4.7%. The visual analog pain score (VAS) of lumbar decreased from the pre-operative 5.3±4.1 to the last follow-up 2.1±1.7, and VAS of leg decreased from the pre-operative 6.7 ± 3.5 to 1.0 ± 0.8 at final follow-up. The clinical results were excellent in 46 cases, good in 50 cases and fair in 6 cases according to the Macnab standard. Conclusion MED-LIF can be easily performed with MMED technique, with sufficient decompression and reduction, providing satisfactory results with less invasive procedure.
