1.Effects of edaravone and early rehabilitation on neurological deficit in patients with acute cerebral infarction

Objective To investigate the effect of edaravone and early rehabilitation on neurological deficit in patients with acute cerebral infarction. Methods Fifty patients with acute cerebral infarction were enrolled in this study. Fifty patients were grouped using the random number table method as the subject of this study. A single group of 25 patients treated with edaravone treatment, combined group of 25 patients with edaravone and early rehabilitation treatment, compared the two groups of patients with acute cerebral infarction neurological deficit score, quality of life score and clinical efficacy. Results The scores of neurological deficits after treatment were (14.24 ± 3.42), which were significantly lower than those in the single group (P<0.05). The quality of life scores of the patients in the combined group were (104.24 ± 3.25) The total effective rate was 92.00% in the combined group, which was significantly higher than that in the single group (64.00%, P<0.05). Conclusion Edaravone and early rehabilitation therapy in patients with acute cerebral infarction can significantly improve the neurological function of patients, the effect is more significant.
2.Construction and identification of eukaryotic co-expression vector carrying Myod1 and Myog

BACKGROUND:Now it has cooperation and facilitative effete between myogenic regulatory factors through a long time study. So, gene therapy of double genes of Myod1 and Myog can obtain better effect, and can provide a new way for preventing denervated skeletal muscle atrophy.
OBJECTIVE:To construct eukaryotic co-expression vector carrying Myod1 and Myog genes.
METHODS:Ful-length Myod1 gene and Myog gene cDNA were amplified by reverse transcription PCR, and then inserted into pVAX1 vector after digested to establish the recombined Myod1 and Myog eukaryotic co-expression vector pVAX1-Myod1-IRES2-Myog-IRES2-EGFP, and then identified with gene sequencing. The in vitro cultured 3T3 cel s were transfected with pVAX1-Myod1-IRES2-Myog-IRES2-EGFP, and the expressions of Myod1 and Myog genes in the 3T3 cel s were detected with western blot assay in order to identify whether the 3T3 cel s could express the target protein correctly.
RESULTS AND CONCLUSION:The sequencing results showed that the sequence length and base sequence of Myod1 and Myog cDNA in eukaryotic co-expression vector pVAX1-Myod1-IRES2-Myog-IRES2-EGFP were identical with the reported sequences. Myod1 and Myog protein band expressions were detected in 3T3 cel s by western blot after transient transfection. The pVAX1-Myod1-IRES2-Myog-IRES2-EGFP, a eukaryotic co-expression vector of Myod1 gene and Myog gene is successful y constructed.
3.Clinical features of 9 cases of Candida parapsilosis septicemia

Objective To analyze the clinical characteristics of Candida parapsilosis septicemia of prematurity and provide a reference for both diagnosis and treatment.Methods The clinical data of 9 premature suffering with candida parapsilosis septicemia were retrospectively analyzed.Results All 9 cases (0.73 percent in the corresponding period) were premature infants with gestational age less than 37 weeks.The average weight of the patients was (1 621.00 ±372.22) g,among which 5 cases were of very low birth weight and 4 cases were of low birth weight.The average time of the fungi infection was (22.00 ± 7.58) d.Of the 9 cases,7 cases had been treated with broad spectrum antibiotics previously,9 cases were given nutrition intravenously,7 cases were treated with mechanical ventilation,7 cases adopted peripherally inserted central catheter(PICC) and 1 case received surgery.The symptoms included poor response,unstable serum oxygen saturation,jaundice,poor perfusion,abdominal distention,fever and no bodyweight increase.Laboratory tests results presented platelet decrease(6 cases),C-reaction protein increase (6 cases),high plasma (1-3)-β-D glucan level(9 cases) and white blood cells in the normal range(6 cases).All 9 infants recovered after receiving fluconazole and supporting treatment.Two cases failed to complete the hospital treatment but after discharge from hospital continued treatment by administering prescribed medicine,and through regular follow-up was done,the prognosis of all cases turned out to be good,and no case died.Conclusions Gestational age,birth weight,application of broad spectrum antibiotics,mechanical ventilation and peripherally inserted central catheter are common high risk factors of Candida parapsilosis septicemia in premature infants.The disease was featured with normal white blood cell,low platelet,high serum levels of C-reaction protein and (1-3)-β-D glucan.Anti-fungi therapy with fluconazole may be a good choice for it.
4.Comparison of the early effects of bone cement versus cementless prostheses in very elderly patients undergoing total hip arthroplasty

Objective To evaluate the early effects of bone cement prostheses versus cementless prostheses in very elderly patients undergoing total hip arthroplasty.Methods A total of 110 patients aged 70 years and over who had undergone hip replacement because of hip disease from March 2011 to March 2013 were selected.104 cases were followed up for 3 years,with 46 cases treated with the bone cement prostheses(Group A) and 58 cases with cementless prostheses(Group B).The conditions before and after surgery were recorded.Postoperative Harris scores were collected to evaluate the function of the hip joint,and the loosening of hip prostheses was detected by x-ray.Results Intraoperative blood loss and postoperative drainage volume were lower in Group A than in Group B [(236.0±26.3) ml vs.(296.0±34.5) m1,(123.0±20.3) ml vs.(156.0±26.4) ml,both P <0.05].Harris scores before,6 months,1,2 and 3 years after surgery were(40.2±7.2),(80.3±5.2),(90.2±4.5),(90.6±4.2),and(90.8±3.5) in Group A,and(39.8±6.3),(76.5±8.7),(86.5±5.1),(89.3± 4.8),and(91.0± 4.1) in Group B,respectively.Harris scores 6 months and 1 year after surgery were higher in Group A than in Group B,and there was no significant difference in Harris scores between the two groups 2 and 3 years after surgery(P>0.05).Pain scores before,6 months,1,2 and 3 years after surgery were(13.5± 5.3),(37.5± 6.2),(38.4±5.3),(39.1 ±2.5) and(39.0±2.1) in Group A and(12.9±6.2),(30.2±8.7),(35.9±6.5),(38.6±4.8) and(39.2±2.6) in Group B,respectively.Pain scores 1 year after surgery was better in Group A than in Group B,and there was no significant difference in pain scores between the two groups 2 and 3 years after surgery(P>0.05).Bone cement implantation syndrome occurred in 6 cases in Group A,of which 1 case had respiratory and cardiac arrest and was successfully rescued after cardiopulmonary resuscitation.In addition,in Group A,1 case had osteolytic disease 2 years after surgery,and,in Group B,4 cases had proximal femoral fractures and 1 case had osteolytic disease 3 years after surgery.Conclusions For very elderly patients undergoing total hip replacement,satisfactory early therapeutic effects can be achieved by using bone cement or cementless prostheses.Intraoperative blood loss,postoperative drainage volume and Harris scores 1 year after surgery are better with bone cement prostheses than with cementless prostheses.Close attention should be paid to possible development of bone cement implantation syndrome during operation.
5.Modification study of cyanoacrylate medical adhesive

Objective To perform modification study of cyanoacrylate (CA) medical adhesive and to select the optimal mo-difying material and the best ratio.Methods Carboxylic multi-walled carbon nanotubes (MWCNTs-COOH), multi-walled carbon nanotubes (MWCNTs), hydrophobic nano-silica, nitrile rubber, epoxy resin and polymethyl methacrylate (PMMA) were used to modify 5% triclosan/cyanoacrylate antimicrobial adhesive respectively. The bond strength, toughness and viscosity of the modified adhesive were examined in different concentrations.Results 0.064% MWCNTs-COOH, 6% nano-silica, 4% nitrile rubber and 6.4% epoxy resin all had good effects in strength modification. The bond strength were (14.71±1.48)MPa, (14.03±1.92)MPa, (14.6±1.78)MPa and (14.05±1.46)MPa respectively. 8% nano-silica had the lowest Tg of (1.1±0.24)℃ and the strongest viscosity of (15 536.68±28.4)cP. When the nano-silica concentration was 8%, the compound was very viscous and sticky. When the antimicrobial adhesive modified with 6% nano-silica, the bond strength was (14.03±1.92)MPa, the Tg was (3.6±0.68)℃, and the viscosity was (5 278.87±31.68)cP.Conclusion 6% nano-silica is the best modifying material, and has the optimal effect of modification.
6.Determination of Oridonin in Rabdosia nervosa by HPLC

Objective To develop a HPLC method for the determination of oridonin in the leaves and stems of Rabdosia nervosa.Methods All of reference substances and samples were separated with the mobile phase of methanol:water (45 ∶55) under isocratic elution for 20 min on a Kromasil C18 reversed-phase column(250 mm?4.0 mm,5 ?m),the flow rate was 1.0 mL?min-1,the detection wavelength was 235 nm,and the column temperature was 30 ℃.Results The content of oridonin was 0.047 mg/g in stems and 0.791 mg/g in leaves,the content in stems being about one seventeenth of that in leaves.The average recovery of oridonin was 98.33 %.Conclusion This method is simple,sensitive,accurate and suitable for quantitative determination of oridonin in Rabdosia nervosa.
7.Determination of Lasiokaurin in the Stems and Leaves of Rabdosia nervosa by HPLC

Objective To develop a HPLC method for the determination of Lasiokaurin in the stems and leaves of Rabdosia nervosa.Methods HPLC was performed on a Kromsil C18 reversed-phase column(250mm?4.2mm,5?m).All of the reference substances and samples were separated with the mobile phase of methanol :water(55:45) under isocratic elution for 30min,flow rate was 1.0ml/min,the detection wavelength was 234nm,and the column temperature was 30℃.Results The content of Lasiokaurin was 0.005mg/g in the stems and 0.372mg/g in the leaves,the content in the stems were almost 1/75 times as much as that in the leaves.The average recovery of Lasiokaurin was 96.21%.Conclusion This developed method,which is simple and with good precision,high sensitivity and selectivity,can be used for quality evaluation of Lasiokaurin in Rabdosia nervosa.
8.Surgical Treatment of Prolapse of Lumbar Intervertebral Disc and Segmental Instability

Objective To evaluate the operative results of the prolapse of lumbar intervertebral disc and segmental instability.Methods37 patients with prolapse of lumbar intervertebral disc and segmental instability were treated surgically, including nucleus gelatinosus extirpation,!lumbar fusion and transpedicular screw fixation with an average 9-month follow-up after the operation.ResultsAfter the treatment, the completely spine arthrodesis was obtained in all cases. Of 37 cases, 28 cases showed excellent clinical results, 7 cases were good, 2 cases improved, and the total fineness rate was 95.6%.ConclusionIt is an effective therapy for patients with prolapse of lumbar intervertebral disc and segmental instability that fusing and fixing the lumbar with transpedicular screw after nucleus gelatinosus extirpation.
9.The relationship between CHADS2 score and prognosis in acute ischemic stroke patients with nonvalvular atrial fibrillation

Objective To explore the relationship between the CHADS2 score and short-term prognosis in acute ischemic stroke patients with nonvalvular atrial fibrillation. Methods Consecutive ischemic stroke patients with nonvalvular atrial fibrillation who were hospitalized within 7 days after stroke were registered.Patients were divided into 3 groups on the basis of CHADS2 pre-stroke score (0 to 1,2,3to 6).And recovery was assessed by modified Rankin Scale (mRS) at 3 months follow-up (mRS ≤ 2reflected good prognosis,and mRS ≥ 5 implicated unfavorable outcome).After screening the risk factors affecting prognosis using univariate analysis,the relationship between CHADS2 score and prognosis was estimated using logistic regression model.Results Of 203 patients enrolled,the CHADS2 score of 0-1,2,3-6 were present in 72,53 and 78 respectively.Patients with the higher CHADS2 score had a higher initial NIHSS score (9.8,12.6,13.0,F =3.404,P =0.008 ),more likely to receive nosocomial pulmonary infection( 12.5%,34.0%,39.7%,x2 =14.643,P =0.001 ),negatively related to good prognosis (x2 =28.542,P =0.000 ) and positively related to unfavorable outcome ( x2 =23.575,P =0.000 ).Logistic regression model results suggested that CHADS2 score was an independent predictor of good prognosis and unfavorable outcome. Compared with CHADS2 score 3-6, CHADS2 score 0-1 was independently associated with good prognosis ( OR =5.018,95 % CI 2.055-12.560 ).And compared with CHADS2 score 0-1,CHADS2 score 3-6 was independently associated with unfavorable outcome ( OR =6.197,95% CI1.670-22.996 ).Conclusions CHADS2 pre-stroke score appears to be useful in prediction of stroke outcome for patients with nonvalvular atrial fibrillation.After acute ischemic stroke,the patients with the lower CHADS2 score (0-1) are more likely to achieve good prognosis,inversely,the patients with the higher CHADS2 score (3-6) are more likely to achieve unfavorable outcome.
10.Assessment of aortic stenosis with dual-source CT

Objective To discuss the feasibility and accuracy of dual-source CT ( DSCT) in the evaluation of aortic stenosis ( AS) with transthoracic echocardiography ( TTE) as reference.Methods A total of 53 patients who underwent both DSCT and TTE were prospectively evaluated.All of them were assessed by TTE for aortic stenosis.Maximum aortic valve area ( AVA) in systolic phase was measured with DSCT , and was compared to that index obtained from the continuity equation on TTE.The severity of AS was graded as mild , moderate , or severe according to the AVA.Linear regression analysis and Bland-Altman plots were used to compare the AVA measured by using CT and TTE.Agreement on semi-quantitative grades of AS severity between the two methods was tested by using Kappa statistics.Results The mean AVA using DSCT was (1.45 ±0.35 ) cm2 compared to the mean AVA of ( 1.33 ±0.36 ) cm2 using TTE, with a significant correlation between them (r=0.92,P<0.01).Bland-Altman analysis demonstrated good inter-modality consistency between DSCT and TTE.However , DSCT demonstrated a slight overestimation of the AVA compared to TTE.As identified by TTE, there were 53 patients with AS, 13 with mild AS, 21 with moderate AS , and 19 with severe AS.In 3 patients DSCT showed no AS , TTE detected mild AS.In 6 cases, TTE had graded the stenosis as moderate , but the stenosis degree was graded as mild using DSCT.Kappa analysis showed a good agreement between the two methods on semi -quantitative grades of aortic stenosis severity (Kappa=0.75,P<0.01).Conclusion AVA measurements using DSCT is feasible and reasonably accurate for those patients with moderate to severe aortic stenosis.
