1.Research on recovery of the pelvic floor muscle by using electrical stimulation biofeedback system in first-time mothers after natural birth

Objective:To observe recovery of the pelvic floor muscle by using electrical stimulation biofeedback system in first-time mothers after natural birth.Methods: One hundred an seventy-two cases of primipara by transvaginal full-term natural birth, less than 35 years old, myodynamiaⅢ or less level were choosed for research object between January 2013 and January 2014 in our hospital. According to admission order, an odd number into the control group, even into the treatment group, 93 cases in each group. Kegel exercise method was used to pelvic floor muscle rehabilitation in control group, on the basis of the treatment in control group, recovery of postpartum pelvic floor muscle was conducted by using electrical stimulation biofeedback apparatus in treatment group. Strength of maternal pelvic floor muscle after recovery was tested with neuromuscular stimulation therapy apparatus and to investigate rate of urinary incontinence and sexual satisfaction in two groups.Results:①The cases ofⅠ,Ⅱ class A and BⅡ pelvic floor muscle fiber restoring toⅣ andⅤ increased significantly after electrical stimulation biofeedback system treatment in treatment group, compared with control group, the difference was statistically significant.The pelvic floor muscle strength increased significantly after treatment in treatment group (P<0.05).②Urinary incontinence rate after treatment for 2 months and 6 months in treatment group was lower significantly than that in control group, sexual life satisfaction was higher significantly than that in control group, differences between groups were statistically significant (P<0.05).Conclusion: Application of electrical stimulation biofeedback system in the treatment of female pelvic floor muscle dysfunction can enhance the enthusiasm of the patients in treatment and curative effect is reliable, and no obvious complications occurred, is worth further promotion in clinical.
2.Clinical effects of locking proximal humerus plate for humeral proximal fractures in elderly patients

Objective To discuss the clinical effects of locking proximal humerus plate (LPHP)for humeral proximal fractures in elderly patients.Methods A total of 84 elderly patients with humeral proximal fractures were retrospectively analyzed.Patients were randomly divided into LPHP group and control group (n=42,each group).Patients in LPHP group were treated with open reduction and internal fixation using LPHP.Patients in control group were treated with open reduction and Kirschner wire fixation.All patients were followed up for 1 year and the shoulder joint function was evaluated.Results There were no significant differences in bleeding volume and the wound healing time between LPHP group and control group [(189.2±27.5) ml vs.(177.6±28.2) ml,(102.3±6.2) days vs.(99.9±6.8) days,t=1.78,1.33,P=0.079,0.105,respectively].One year after the follow-up,there were 22 cases (52.4%,22/42) with excellent clinical evaluation,18 cases (42.9%,18/42) with good clinical evaluation and 2 cases (4.8%,2/42) with poor clinical evaluation in LPHP group,and 15 cases (35.7%,15/42) with excellent clinical evaluation,22 cases (52.4%,22/42) with good clinical evaluation,and 5 cases (11.9%,5/42) with poor clinical evaluation in control group.There was a significant difference in the evaluation rate of excellent and good clinical efficacy between in LPHP group and in control group (95.2 % vs.88.1 %,x2 =3.18,P 0.028).Conclusions The internal fixation using LPHP has a good clinical effect in elderly patients with humeral proximal fractures,and it can well improve the recovery of shoulder joint function.
3.Impact of electroacupuncture on the life quality in patients of female mixed urinary incontinence.

OBJECTIVETo observe the impact of electroacupuncture (EA) on the quality of life in the patients of female mixed urinary incontinence (MUI) and evaluate its clinical efficacy.
METHODSEA was adopted in 25 cases of MUI at Zhongliao (BL 33) and Huiyang (BL 35), needle retaining for 30 min each time. The treatment was given once every two days, three treatments a week, totally for 12 weeks of treatment. The incontinence quality of life questionnaire (I-QOL), the incontinence impact questionnaire-7 (IIQ-7), the urogenital distress inventory short form (UDI-SF) and the visual analogue scale (VAS) were adopted in the comparison before and after treatment. The efficacy was evaluated.
RESULTSThe curative rate was 52.0% (13/25) and the total effective rate was 96.0% (24/25) in 25 cases. In comparison before and after treatment, in I-QOL, the score of limiting behaviors was increased up to (30.32 ± 3.39) from (11.52 ± 2.45), the score of psychosocial impacts was up to (36.48 ± 2.90) from (13.52 ± 2.25) and the score of social isolation was up to (21.52 ± 1.61) from (9.84 ± 2.23). The score of IIQ-7 was decreased, down to (2.48 ± 1.36) from (11.96 ± 3.45), the score of UDI-SF was down to (2.92 ± 0.95) from (8.84 ± 2.58) and VAS score was down to (1.40 ± 0.91) from (6.96 ± 1.65). The differences were all significant statistically in comparison (all P < 0.05).
CONCLUSIONEA improves effectively the quality of life in MUI patients and the clinical efficacy is apparent.

Aim To investigate the effect of sulfur dioxide (SO2) on blood pressure and aortic structure of the spontaneous hypertensive rat(SHR).Methods Sixteen male SHRs at the age of 4 weeks were randomly divided into SHR control group and SHR+Na2SO3/NaHSO3 group,respectively.Another eight male WKY rats served as WKY control group.Five weeks later, the blood pressure was determined.The rat aortas were dyed with Hart's Method. Objective To evaluate if the difference between oversea predicted values and healthy adult measurements of spirometry in shenyang.Methods 287 healthy adult volunteers underwent pulmonary function test.The measurements of spiromenty of 230 selected objects were compared to conventional oversea predicted values.Results In male group(154 objects),the measured values were as follows:VC(4.4?0.7)L,FVC(4.3?0.7)L,FEV1(3.5?0.6)L,FEF25%~75%(3.6?1.1)L/s,PEF(7.1?1.9)L/s,IC(2.8?0.7)L,respectively.In female group(76 objects),the measured values were as follows:VC(3.1?0.7)L,FVC(3.1?0.7)L,FEV1 2.5?0.6(L),FEF25%~75%(2.6?0.9)L/s,PEF(5.0?1.4)L/s,IC(2.1?0.7)L respectively.The measurements were 2%~26% and 2%~21% lower than the predicted values in male and female respectively.The major compared values had significant difference.Conclusion The major measured values of spirometry in healthy adult in shenyang were significantly lower than the oversea predicted values.Knudson or ECCS regression equations with appropriate correction factor were suggested to use in shenyang.
6.Endobronchial ultrasound-guided transbronchial needle aspiration for the diagnosis of thoracic tuberculosis

Objective To evaluated the role of endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) in the diagnosis of thoracic tuberculosis.Methods The study was retrospective,from September 2009 to September 2012,38 patients who underwent EBUS-TBNA were finally diagnosed of thoracic tuberculosis,with enlarged hilar or mediastinal Iymph nodes on chest enhanced computed tomography(≥ 1.0 cm).Patients in whom EBUS TBNA was nondiagnostic subsequently underwent surgical biopsy.All the patients had a minimum of 6 months clinical and radiologic follow-up.Results EBUS-TBNA was performed on a total of 88 lymph node stations in 38 patients.Of the enlarged lymph nodes,60(68.18%) were located in the mediastinal region and the remaining 28 (31.82 %) around the hilum or interlobar area.Of the 38 patients,EBUS-TBNA achieved definitive diagnosis in 34 patients(89.47%).EBUS was well tolerated by all of the patients with no complications.Conclusion EBUS-TBNA is a safe procedure with a high yield for the diagnoses of thoracic tuberculosis.
7.Exploration and Application of Simulation Teaching Management in Obstetrics and Gynecology Practice Teaching

Objective To this study is intended to discuss its feasibility and advances in clinical techniques of obstetrics and gynecology. Methods Based on our hospital clinical teaching platform, a total of 348 medical students or clinical practicum doctors were randomly divided into PBL (Problem-based learning) traning group and clinical practice teaching mode group,respectively, after a period of three weeks of practices,the training effects are estimated by questionnaires, theoretical and clinical operations. Results After training, 174 students in simulation training group showed better in theory test,clinical operations and standardized patients examination than traditional clinical students. Conclusion The establishment of standardized simulation teaching system can effectively complement the lack of clinical practice, teaching resources, solve the visualization is not enough, the students hands-on opportunities for small problems,can enhance the training effect and improve the effectiveness of teaching.
8.Effective observation of electroacupuncture with different courses for female stress urinary incontinence.

OBJECTIVETo verify the effect of electroacupuncture (EA) for female stress urinary incontinence (SU).
METHODSForty-two patients were randomly divided into an observation group(20 cases) and a control group (22 cases). EA at Zhongliao (BL 33) and Huiyang (BL 35) was used in the observation group. Sham acupuncture at non meridian points, one can beside Zhongliao (BL 33) and Huiyang (BL 35), was applied, and placebo EA was adopted in the control group. Treatment with needle retained for 30 min a time was given once every other day and three times a week for continuous six weeks in the two groups. Urinary and reproductive simple score (UDI),visual analogue scale (VAS) and the frequency of nocturnal enuresis were observed before treatment, at the second, forth and sixth week of treatment in the two groups, and the efficacy was compared.
RESULTSThe effective rates of the observation group were 80.0% (16/20), 95.0% (19/20) and 95.0% (19/20), which were better than 40.9% (9/22), 31.8% (7/22) and 27.3% (6/22) of the control group at the second, forth and sixth week of treatment. The differences were statistically significant between the two groups (all P < 0.05). After treatment in the observation group, the results of UDI, VAS and frequency of nocturnal enuresis were improved compared with those before treatment (all P < 0.05). Along with treatment, all indices were gradually improved, and the change at the sixth week was the most obvious in observation group. The results of the observation group were better than those of the control group at all times (all P < 0.05).
CONCLUSIONEA achieves obvious effect for female SUI, and effectively improves the lower urinary trace symptoms, the degree of urinary incontinence and the frequency of nocturnal enuresis. The effect becomes increasingly better along with treatment within six weeks.

9.Study on relationship between lower extremity venous thrombotic disease and seasons

Objective To investigate the relationship between the onset of lower extremity venous thrombotic disease and seasons.Methods A retrospective study was conducted, 300 patients with lower extremity venous thrombotic disease admitted to HandanCity Hospital of Traditional Chinese Medicine (TCM) from August 2012 to February 2014 were enrolled, the incidences and TCM syndromes of patients with lower extremity venous thrombotic disease in different seasons were observed, and the pathogenesis and relationships between the types of TCM syndrome and seasons were analyzed.Results There were 142 patients with lower extremity superficial thrombophlebitis, and 158 cases with lower extremity deep venous thrombosis, the incidence of lower extremity venous thrombotic disease in spring was significantly higher than that in summer and autumn [32.8% (86/262) vs. 21.3% (54/254), 18.4% (50/272), bothP < 0.01], but lower than that in winter [32.8% (86/262) vs. 37.2% (110/296)], the difference was not statistically significant (P > 0.01); while the incidence of lower extremity venous thrombotic disease in winter was significant higher than those in summer and autumn (allP < 0.01). The incidences of damp and heat downward flow type in autumn and summer were increased compared with those in winter and spring [55.6% (30/54), 60.0% (30/50) vs. 20.0% (22/110), 23.3% (20/86), allP < 0.01], while the incidences of lower extremity venous thrombotic disease with damp heat and stasis syndrome in winter and spring were increased compared with those in summer and autumn seasons [80.0% (88/110), 76.7% (66/86) vs. 44.4% (24/54), 40.0% (20/50)].Conclusions The incidence of lower extremity venous thrombotic disease is related to seasons, and the onset is high in winter and spring, damp heat and stasis syndrome being the main type; according to different seasons, clinical treatment can direct to different pathogenic factors to adopt different preventive measures interfering with the patient's constitution in order to eliminate or reduce the risk factors, achieving the effect of the disease prevention.
