1.Treatment of gemcitabine combined with nedaplatin and paclitaxel for patients with metastatic cisplatin-resistant nasopharyngeal carcinoma

Objective To assess the short-term effects and adverse reactions of combination regimen of gemcitabine, nedaplatin and paclitaxel for patients with metastatic cisplatin-resistant nasopharyngeal carcinoma (NPC). Methods A total of 15 patients with metastatic cisplatin-resistant NPC were enrolled. All patients were treated with a combination regimen including gemcitabine with 1000 mg/m2 on day 1st and 8th, nedaplatin with 70 mg/m2 on day 1st and paclitaxel with 135 mg/m2 on day 1st, repeated every 21 days. Response was evaluated according to Response Evaluation Criteria in Solid Tumors (RECIST) criteria 3.0. Results The overall response rate was 40.0 %, with a complete response rate of 6.7 % (1/15) and a partial response rate of 33.3 % (5/15). Six patients (40.0 %) had stable disease and 3 patients (20.0 %) had progressive disease. The median time to progression (TTP) was 4.7 months and the median overall survival was 6.3 months. Hematological toxicities were the adverse reaction with 40.0 % of leucopenia, 6.7 % of anemia and 20.0 % thrombocytopenia. One patient needed for platelet transfusion. Other adverse reactions were mild. Conclusion The combination regimen of gemcitabine, nedaplatin and paclitaxel is feasible as second-line chemotherapy in patients with metastatic cisplatin-resistant NPC.
2.Regulative effect of collagen and related protease gene of betulin on human ESF-1 cells

Objective To explore the mechanism and regulative function of betulin on anti-aging genes related of normol human dermal fibroblasts (ESF-1), and to reveal the repairing mechanism of betulin on wrinkles. Methods The cultured cell in vitro were divided into the control group, the estradiol group and the betulin high, medium and low dose groups. Competence of ESF-1 cells was detected by MTT. Expression of mRNA of collagen type I and Ⅲ (ColⅠand Col Ⅲ), tissue inhibitors of matrix metalloproteinase 1 and 2 (TIMP-1 and TIMP-2), matrix metalloproteinase 1 (MMP-1) was detected by RT-PCR. Results Compared with the control group, the proliferation of ESF-1 cells (0.455 ± 0.037, 0.445 ± 0.040 vs. 0.385 ± 0.601) in the estradiol group and the medial-dose betulin group were increased (P<0.05). Compared with the control group, the expression of mRNA of ColⅠ (0.960 ± 0.012, 0.929 ± 0.015 vs. 0.842 ± 0.014), Col Ⅲ (0.892 ± 0.009, 0.824 ± 0.022 vs. 0.768 ± 0.025), TIMP-1 (0.938 ± 0.026, 0.878 ± 0.035 vs. 0.796 ± 0.022), TIMP-2 (0.557 ± 0.025, 0.506 ± 0.036 vs. 0.436 ± 0.063) in the estradiol group and the medial-dose betulin group were increased (all P<0.01). Conclusion Betulin based on increasing the competence of ESF-1 cells, promoting collagen synthesis and the expression level of related-proteinase is to suspend the development of wrinkles.
3.Relationship between tumor differentiation and related pathologic factors in Esophago-gastric junction adenocarcinoma

Objective To investigate the relationship between the tumor differentiation and the related pathologic factors in Esophago-gastric junction adenocarcinoma. Methods The pathological data from 163 patients of Esophago-gastric junction adenocarcinoma underwent radical surgical resections were randomly divided two groups (group1: well-differentiated,group2: poor-differentiated). The composition of N stages,T stages,morphological type and vessel carcinoma embolus were compared between the two groups ,respectively. The tumor sizes and the number of metastases lymph nodes were compared between the two groups. Results The cases with lower N stages or T stages in group1 were dominant(T1 and T2:55. 74%,N1 and N2:75.41%). Oppositely,those with higher N stages or T stages were dominant(T3 and T4:59.81%,N1 and N2:57.85%)in group 2. The differences were significant Incidence of vessel carcinoma embolus was 44. 26%(27/61)in group 1 and 63. 73%(65/102)in group 2,with a significant difference. There was no significant difference for morphological type in the two groups. The tumor sizes and the number of metastases lymph was 4. 27 ±2. 00 cm and 4. 15 ±5.27 respectively in group 1 ,and 5. 87 ± 3. 26 cm and 8. 80 ± 7.65 respectively in group 2. The differences were significantly different(Ps < 0. 01).Conclusions The tumor differentiation has significant effect on N stages,T stages,vessel carcinoma embolus,tumor size and the number of metastases lymph nodes in Esophago-gastric junction adenocarcinoma.
4.Activation of TLRs/NF-κB signal pathway and occurrence of different functional cytokines during invasive pulmonary aspergillosis

Objective To study the activation of TLRs/NF-κB signal pathway and production of different functional cytokines during invasive pulmonary aspergillosis( IPA) , in order to probe the pathogene-sis of IPA. Methods Mouse were randomly divided into normal, normal + inoculated with Aspergillus fumigatus( normal inoculation group), and immune suppression + inoculation with Aspergillus fumigatus (IPAmodel group) , the mouse were killed at different time points after inhaling Aspergillus fumigatus spores by nose. Removing the lung tissue in a sterile manner and making pathological section respectively, counting Aspergillus fumigatus colony, dynamiclly detecting the expression of TLR2, TLR4 mRNA, variation of NF-κB p65 protein, pro-inflammatory cytokines TNF-α, IL-1β and anti-inflammatory cytokines IL-10 levels in the lung tissue by RT-PCR and Western blot method during Aspergillus fumigatus infection in mouse. Results (1) When it's 72 h after inhaling Aspergillus fumigatus by nose, IPA model emerged severe lung tissue inflammation, and generated a large number of hyphae, meanwhile, burthen of Aspergillus fumigatus was higher than normal inoculation group at each time point. (2)Compared with the normal inoculation group, IPA group whose TLR2 mRNA was low expression at early stage of infection (24 h), and emerged high expression at late stage of infection (120 h, 144 h); and TLR4 mRNA has been at a state of low expression in the infection process; NF-κB p65 suddenly increased at early stage of infection(24 h) and then continued to decline. (3) After infected by Aspergillus fumigatus in normal mouse, proinflammatory cytokine TNF-α, IL-1β in lung exhibited high expression at the early stages of infection, and the highest expression levels appeared at 48 h or 72 h, then decreased and recovered to normal level. And the expression level of anti-inflammatory cytokine IL-10 rised at late stage of infection; The IP A mouse released a lot of anti-inflammatory cytokine IL-10 at early stage of infection, which significantly reduced at late stage, and released pro-inflammatory cyto-kines TNF-α, IL-1β at slow and low level. Conclusion The abnormal activation of TLRs/NF-β signaling pathway caused the loss of dynamic balance between pro-inflammatory cytokines and anti-inflammatory cytokines, leading to the occurrence and development of IPA.
5.Inhibiting effect of IL-24 combined with targeting attenuated Salmonella vector SL 7207/pBud-Vp3 on growth of gastric cancer cells

Objective To investigate the inhibiting effect of interleukin (IL)‐24 combined with targeted attenuated Salmonella typhimurium vector SL7207/pBud‐VP3 on the growth of gastric cancer cells .Methods The co‐expression eukaryotic expression plasmid pBud‐VP3‐IL‐24 was constructed .The plasmid pBud‐VP3‐IL‐24 was transformed into attenuated Salmonella typhimurium SL7207 by using the high voltage electroporation for constructing the SL 7207/pBud‐VP3‐IL‐24 strain .The mouse gastric cancer transplantation tumor model was established and randomly divided into the normal saline control group ,SL7207/pBud group , SL7207/pBud‐VP3 group and SL7207/pBud‐VP3‐IL‐24 group .The tumor‐bearing mice were fed by oral administration of bacterial strain .The tumor volume was measured and the tumor inhibition rate was calculated .The expression of IL‐24 was detected by Western blotting .The levels of IFN‐γ,IL‐6 and TNF‐αin tumor tissue were detected by using RT‐PCR .The expression of Caspase‐3 and VEGF were detected by using immunohistochemistry .Results The plasmids attenuated Salmonella typhimurium vector carrying the gene IL‐24 was successfully constructed .The IL‐24 protein expression was detected in gastric cancer tissue after 14 d treatment .The tumor volume after 28 d treatment in the SL7207/pBud‐VP3‐IL‐24 group was reduced compared with the other groups ,moreover the tumor growth was significantly inhibited ,and the differences were statistically significant (P<0 .05) .RT‐PCR and immunohistochemistry results showed that IL‐24 combined with SL7207/bBud‐VP3 could significantly increase the expression levels of immune factor IL‐6 ,IFN‐γ and TNF‐αin tumor tissue ,.in addition ,up‐regulated the expression of Caspase‐3 and down‐regulated the VEGF expression(P<0 .05) .Conclu‐sion IL‐24 combined with SL7207/pBud‐VP3 can synergically play the inhibitory effect on the growth of gastric cancer cells ,its mecha‐nism is related with the tumor apoptosis promotion ,tumor vessel inhibition and immune regulation .
6.Mutation analysis of a Chinese family with Alport syndrome and genetic diagnosis before embryo implantation

Objective To study the gene mutation of a Chinese family with Alport syndrome and to perform preimplantation genetic diagnosis before embryo implantation.Methods Next generation sequence analysis was done for checking COL4A3,COL4A4 and COL4A5 genes in the Alport syndrome family members.Array comparative genomic hybridization(CGH) was used to detect the embryos.Results A mutation c.2605G > A was found and identified in COL4A5 gene of all of the Alport syndrome patients in the family,but COL4A3 and COL4A3 genes were normal in all of the detected people.After searching for the mutation database,the mutation c.2605G > A of COL4A5 gene was related to the X-linked dominant Alport syndrome.Three embryos were detected by using the preimplantation genetic diagnosis.Among these embryos,there were two male and one female.One of the male embryos was chromosomal aneuploidy,which was 45,XY,-16 and the other was normal.This normal embryo was implanted,and after 20 weeks the prenatal amniocentesis diagnosis approved that the fetus was normal.Conclusions The mutation of COL4A5 gene (c.2605 G > A) is the cause of Alport syndrome in this family,which indicates that next generation sequence analysis proves to be an accurate and rapid method to detect Alport syndrome disease.Meanwhile array CGH can be used to reduce birth rates as a useful preimplantation genetic diagnosis method.
7.Observation of topography and analysis of surface contamination of titanium implant after roughness treatment.

The roughness treatment of dental implant surface could improve the bone bonding and increase the success rate of implant, but the difference of diverse treatments is still unknown. In this study using scanning electron microscopy (SEM), energy disperse spectrometer (EDS) and the test of contact angle, we studied the microstructure, surface contamination and surface energy, and hence conducted a comparative analysis of the following surface roughness treatments: Polished Treatment (PT), Sandblasting with Alumina(SA), Sandblasting with Aluminia and Acid-etched (SAA), Sandblasting with Titanium Acid-etched (STA), Electro-erosion Treatment(ET). The result of SEM showed that the surface displayed irregularities after roughness treatments and that the surface properties of different roughness treatments had some distinctions. SAA and SA had some sharp edges and protrutions; the STA showed a regular pattern like honeycomb, but the ET sample treated by electric erosion exhibited the deeper pores of different sizes and the pores with a perforated secondary structure. The EDS indicated that the surface was contaminated after the treatment with foreign materials; the SA surface had some embedded contaminations even after acid etching. The measurement of water contact angle indicated that the morphology correlated with the surface treatments. These findings suggest that the distinction of surface structure and composition caused by different treatments may result in the disparity in biological behavior of dental implant.

8.Clinical re-evaluation of effects of different treatments to prevent from phlebitis induced by Chansu injection.

OBJECTIVETo re-evaluate the effects of different treatments to prevent from phlebitis induced by Chansu injection.
METHODPatients treated with Chansu injection were divided randomly into 4 groups with 50 per group, control group, the magnesium sulfate group, phentolaminum group, and anisodamine group. Patients in the control group only received the routine nursing treatment, and patients in the various experiment group received different interventions. The comparison was made in the morbidity and the starting time of occurrence of phlebitis, the severity of pain, duration of pain.
RESULTThe morbidity of phlebitis was 8%, 8%, 6% respectively. The starting time of phlebitis occurrence was (21 +/- 9.31) , (22.34 +/- 10.15), (20.19 +/- 11.23) h, respectively. The NRS of pain was (4. 15 +/- 1.03), (3.26 +/- 1.17), (4.32 +/- 1.36), respectively. The duration time of pain was (4.05 +/- 1.21), (3.37 +/- 1.17), (3.19 +/- 1.67) d, respectively. In control group, the morbidity of phlebitis, the starting time of occurrence of phlebitis, the severity of pain, duration of pain was 24%, (17 +/- 6.32) h, (6.58 +/- 1.29), (5.32 +/- 1.12) d, respectively. As compared with the control group, a significance difference was found between every group in three test groups and control group respectively (P<0.05).
CONCLUSIONThe morbidity and the starting time of occurrence of phlebitis, the severity of pain, duration of pain was significantly reduced respectively by external appication of magnesium sulfate, anisodamine, and intravenous drip infusion of phentolaminum.

9.Application of walking combined with Buerger exercise in patients with grade 0 diabetic foot

Objective To study the application of walking combined with Buerger exercise in the treatment of grade 0 diabetic foot.Methods Eighty patients with grade 0 diabetic foot were randomly divided into observation group and control group.The control group was trained by walking exercise .The observation group was trained by walking combined with Buerger exercise through 12 months of training.The changes of self-sensory symptoms,ABI and DAWV were observed.Results The self-sensory symptoms of the two groups were significantly decreased (observation group: markedly effective 15 cases,effective 19 cases;control group: markedly effective 5 cases,effective 23 cases).The effective rate of the observation group was higher than that of the control group (85% vs.70%,χ2 =7.831,P <0.05).The ABI ratio of the two groups were higher than those before treatment ,and the ABI ratio of the observation group was higher than that of the control group [(0.889 ±0.113) vs.(0.842 ±0.124),t =1.772 P <0.05].The ratio of DAWV in the observation group was higher than that in the control group [(6.772 ±0.435)cm/s vs.(6.543 ±0.552)cm/s,t =2.061,P <0.05].Conclusion Walking combined with Buerger exercise can improve the blood flow velocity and blood flow of the collateral circulation of the lower limbs of patients with grade 0 diabetic foot,and improve the self-sensory symptoms of peripheral neuropathy .
10.Mechanism of Ermiao power in the treatment of rheumatoid arthritis based on urine metabolomics technology

Objective To study the anti-rheumatoid arthritis mechanism of Ermiao powder by high-throughput urine metabolomics.Methods The rats were randomly divided into control group,model group and administration group with 8 rats in each group.The rat model of rheumatoid arthritis was established by intradermal injection of complete Freund's adjuvant.Rats in control group were given Ermiao power solution 0.108 g/ml by gavage.The degree of joint swelling in rats was observed and scored.On this basis,metabolic data of rat urine samples were collected for metabolomic analysis.Unsupervised and supervised pattern recognition technology was used to analyze the high-throughput biological information data and reduce the dimension.Metabolic information related to rheumatoid arthritis was screened and focused to clarify the pathogenesis of rheumatoid arthritis and the therapeutic mechanism of Ermiao power.Results Compared with the model group,the swelling degree of the foot (1.93 ± 0.11 vs.2.36 ± 0.19) in Ermiao power group significantly decreased (P<0.01).Metabolic profiles showed that the metabolic distribution of healthy rats was significantly separated from that of model rats,and the treatment group was in the middle of the two groups.From the macro-metabolic point of view,the metabolism of model rats changed dramatically.The Ermiao power had a good intervention effect on rheumatoid arthritis.Thirteen biomarkers related to rheumatoid arthritis were identified by database matching,including linolenic acid,arachidonic acid,5,6-EET,alpha-lactose,sucrose,trehalose,prostaglandins and leukotriene C4.It involved linoleic acid metabolism,arachidonic acid metabolism,starch and galactose metabolism and sphingolipid metabolism.Conclusions The Ermiao power has significant therapeutic effect on rheumatoid arthritis rats.Regulation of the linoleic acid metabolism,arachidonic acid metabolism,starch and galactose metabolism may be the mechanism of its treatment of rheumatoid arthritis.
