OBJECTIVE:To study the dissolution rate of total puerariae flavones(TPF)bioadhesive sustained-release tablet,and to determine its bioadhesive force to animal stomach and small intestine in vitro METHODS:Using rotating basket method,the dissolubility summation was determined with 0 1mol/L HCl as dissolution medium at speed of 100r/min,single-index model,Weibull distributing model,Higuchi equation and zero-class model were used to imitate the dissolution curve The biggest absolute error,the biggest relative error and AIC were used as comprehensive indices to select the best imitating model A new apparatus made by ourselves was used to compare the adhesive force between the sustained-release tablets and popular tablets in adhering to rabbits'stomach or small intestine RESULTS:The results showed that the dissolution rate in vitro imitated in single-index model was the best Bioadhesion study showed that there were obvious differences between bioadhesive sustained-release tablets and non-bioadhesive tablets in adhering with gastric mucosa and small intestines mucosa(P

OBJECTIVE:To study the main factors affecting in vitro dissolubility of total puerariae flavones(TPF)bioad?hesive tablets.METHODS:Using HPMC,Carbopol(CP934NP)as bioadhesive and base materials,lactose as porogenic agent to prepare bioadhesive tablets;Basket-rotating method was adopted to determine the dissolubility while0.1mol/L HCl was used as dissolution medium,Rotational speed was100r/min.The accumulated dissolution was detected and the influence of the amount of HPMC,CP,kind of porogenic agent,amount of lactose,size of granules in pressed tablets and medium pH on dissolubility was observed.RESULTS&CONCLUSION:The amount of HPMC,CP and lactose,kind of porogenic agent,size of granules in tablets and medium pH can affect the dissolubility of bioadhesive tablets.
4.The effects of γ-IFN on the expression of HLA-G mRNA in primary cultured astrocytoma cells

Objective To study the effects of γ-IFN on the expression of HLA-G mRNA in primary cultured astrocytoma cells. Methods Different concentrations of γ-IFN were added to primary cultured cells, and HLA-G mRNA were detected by RT-PCR. Results After γ-IFN treatment, HLA-G mRNA can not be determined from HLA-G originally negative astrocytoma cells. The expression of HLA-G are up - regulated in all original HLA-G positive astrocytoma cells in a dose-dependent manner. Conclusion γ-IFN can increase the ex-pression of HLA-G gene in the primary cultured astrocytoma cells which HLA - G are originally positive.
5.Effects of Rehmanniae Radix Preparata and Poria on decoction amounts of three constituents in Corni Fructus

AIM To study the effects of Rehmanniae Radix Preparata (A) and Poria (B) on decoction amounts of loganin,morroniside and 5-hydroxymethylfurfural in Corni Fructus (C).METHODS These three medicinal materials were combined one another and divided into seven groups (A,B,C,A + B,A + C,B + C and A + B + C).Then the contents of three constituents were determined by HPLC.RESULTS Compared with the single decoction of Corni Fructus,the decoction amounts of loganin and 5-hydroxymethylfurfural were decreased,and that of morroniside was increased at the time of mixed decoction of Rehmanniae Radix Preparata and Corni Fructus,or Rehmanniae Radix Preparata,Poria and Corni Fructus.This situation was just the contrary at the time of mixed decoction of Poria and Corni Fructus.CONCLUSION The mixed decoction of Corni Fructus,Rehmanniae Radix Preparata and Poria increases the decoction amount of morroniside,which may make mixed decoction liquid show better efficacy.
6.Effects of Combined Decoctions of Alismatis Rhizoma and Curcumae Radix and Rehmanniae Radix Praeparata on the Decoction Amount of Acteoside in Rehmanniae Radix Praeparata

OBJECTIVE:To study the effects of combined decoctions of alismatis rhizoma and curcumae radix and rehmanniae radix praeparata on the decoction amount of acteoside in rehmanniae radix praeparata,and provide reference for studying the effec-tive ingredients of three-drug effect. METHODS:Single decoction-1(rehmanniae radix praeparata 20 g),single decoction-2(alis-matis rhizoma 20 g),single decoction-3(curcumae radix 20 g),combined decoction-1(rehmanniae radix praeparata 20 g,alisma-tis rhizoma 20 g),combined decoction-2(rehmanniae radix praeparata 20 g,curcumae radix 20 g),combined decoction-3(alisma-tis rhizoma 20 g,curcumae radix 20 g)and combined decoction-4(rehmanniae radix praeparata 20 g,alismatis rhizoma 20 g,cur-cumae radix 20 g)were respectively taken to prepare dry extracts after extracting by refluxing,HPLC was used to detect the acteo-side content and calculate its decoction amount in sample. RESULTS:The decoction amounts of acteoside in single decoction-1, combined decoction-1,combined decoction-2 and combined decoction-4 dry extracts were 0.0354,0.0223,0.0228,0.0110 mg/g,respectively. Compared with single decoction-1 group,the last 3 groups had statistical significances(P<0.01);combined decoc-tion-4 showed lowest decoction amount in three-drug combined decoction group. Acteoside was not detected in the negative control groups(single decoction-2,single decoction-3 and combined decoction-3). CONCLUSIONS:The decoction amount of acteoside is reduced when alismatis rhizoma,curcumae radix and rehmanniae radix praeparata were decocted together,indicating that it may not be the main ingredient of playing effects in three-drug combined decoction liquid.
7.The accuracy of bony resection and component size planning of total knee arthroplasty assisted with a novel designed patient-specific instrumentation

Objective:To investigate the accuracy of bony resection and component size planning of total knee arthroplasty (TKA) assisted with a novel designed patient-specific instrumentation (PSI).Methods:Thirty-eight TKAs assisted with a novel designed PSI of thirty-five patients from March 2018 to April 2019 were retrospectively analyzed. There were 11 males (10 knees) and 27 females (25 knees), aged 67.7±6.9 years (range 49-81 years). Intraoperative bone resection thicknesses at medial distal femur (MDF), lateral distal femur (LDF), medial posterior femur (MPF), lateral posterior femur (LPF), anterior femur (AF), medial tibial (MT), lateral tibial (LT) were measured with vernier caliper and compared with the preoperative planned bone resections as a primary outcome. The femur and tribal component sizes used intraoperatively were recorded and compared the preoperative planning.Results:The difference of thickness was -0.1±1.6 mm at MDF, -0.5±1.6 mm at LDF, 0.8±1.7 mm at MPF, 0.0±1.7 mm at LPF, 0.0±1.4 mm at AF, -0.3±1.4 mm at MT and 0.3±1.5 mm at LT. The ratio of differences <2 mm and <3mm were 87.8% and 93.94% at MDF, 87.8% and 93.94% at LDF, 71.3% and 85.71% at MPF, 77.14% and 91.43% at LPF, 88.46% and 100% at AF, 85.29% and 97.06% at MT, 83.78% and 94.59% at LT. 89.47% of femoral components were the same size with preoperative planning, 10.53% femoral components were within 0.5 size compared to the preoperative planning, no femoral components were over 0.5 size compared to the preoperative planning; 36.84% of tibial components were the same size with preoperative planning, 60.53% tibial components were within 0.5 size compared to the preoperative planning, 2.63% tibial components were within 1 size compared to the preoperative planning, notibial components were over 1 size compared to the preoperative planning.Conclusion:TKA assisted with a novel designed PSI shows good accuracy with bone resection and component size planning. The procedure of TKA could be simplified with this technique.
8.The predictive value of the quick sequential organ failure assessment score in septic shock after percutaneous nephrolithotomy

Objective:To evaluate the predictive value of the quick sequential organ failure assessment(qSOFA) score in septic shock after percutaneous nephrolithotomy(PCNL).Methods:309 patients who underwent PCNL at the First Affiliated Hospital of Soochow University between May 2018 and October 2019 were retrospectively reviewed. Among them, there were 192 men and 117 women, whose mean age was (51.4±12.8)years (range from 20 to 79 years). There were 82 cases(26.5%) of hypertension and 23 cases(7.4%) of diabetes. There were 88 patients(28.5%) with positive preoperative urine culture.102 patients(33.0%) were diagnosed with staghorn calculi by abdominal CT and urinary tract abdominal plain film(KUB).78 patients(25.2%) had a history of urinary surgery. The qSOFA and SIRS were evaluated to all patients within 24 h after PCNL and the best diagnostic criteria was considered as qSOFA≥2 and SIRS≥2. Receiver operating characteristic(ROC) curves were constructed and the areas under the curve(AUC) were calculated to compare the discriminatory ability of qSOFA and SIRS with the post-PCNL septic shock. A univariate logistic regression analysis was used to identify the covariates associated with post-PCNL sepsis. Then adjusted multivariate analysis was used to identify the predictive value of positive qSOFA and SIRS for the postoperative clinical outcomes including postoperative hospitalization days, postoperative blood transfusion, postoperative re-intervention, residual stone, planned readmission within 30 days and unplanned readmission within 30 days.Results:Among the 309 patients who underwent PCNL, 23 patients(7.4%) met the positive qSOFA criterion while 84 patients(27.2%) developed to SIRS. 7 patients(2.3%) were admitted to ICU after operation and were eventually diagnosed as septic shock, among which 6 patients met the criteria of qSOFA and SIRS. 8 patients(2.6%) underwent multi-channel operation. The median operative time of 309 patients was 85(56, 115) min. Postoperative calculus composition analysis showed that 64 patients(20.7%) were infectious calculi. Postoperative KUB showed residual calculi in 179 patients (57.9%). The median postoperative hospital stay was 7(6, 9) days. 10 patients(3.2%) received blood transfusion. 9 patients(2.9%) received re-intervention after surgery. There were 41 patients (13.3%) of planned readmissions and 16 cases (5.2%) of unplanned readmissions within 30 days. The AUC of qSOFA and SIRS was 0.900 and 0.799 respectively. The qSOFA had a higher specificity, positive likelihood ratio and positive predictive value(94.4%, 15.23, 26.1%)than that of SIRS(74.2%, 3.32, 7.1%)for septic shock. In univariate logistic regression analysis significant associations were observed between positive urine culture, stone size, staghorn stones, struvite stones, surgery history, operation time and sepsis after PCNL. Multivariate logistic regression analysis revealed that postoperative length of stay( OR=1.237, 95% CI 1.048-1.459, P=0.012) and postoperative transfusion( OR=8.265, 95% CI 1.409-48.481, P=0.019) were closely associated with qSOFA after adjusting for covariates shown to be related to post-PCNL sepsis mentioned above. Conclusions:The qSOFA could be superior to SIRS in predicting septic shock after PCNL.
9.Elevated vascular endothelia growth factor-A in the serum and peritoneal fluid of patients with endometriosis.

There has been emergence of evidence suggesting that specific variants of the vascular endothelia growth factor (VEGF) family, based on their ability to regulate angiogenesis, would be pivotal in the pathogenesis of endometriosis. This study was aimed at determining whether high levels of VEGF-A could be found in the serum and peritoneal fluid (PF) of patients with endometriosis. VEGF-A levels were measured by enzyme-linked immunosorbent assay (ELISA) in serum and PF from 46 patients with surgically confirmed endometriosis, and 40 controls with no clinical evidence of the disease or detectable endometriotic lesions at the time of surgical examination. The results showed the mean VEGF-A levels were significantly higher in the serum and PF of patients with endometriosis than in the controls. The VEGF-A levels in the serum and PF of patients with severe endometriosis (stages III-IV) were significantly higher than in those with minimal endometriosis (P<0.001). It was concluded that endometriosis was associated with significant modulation in the levels of circulating VEGF-A.
10.Analysis of clinical characteristics of 22 906 gravidas with Hepatitis B Virus infection

Objective To analyze the clinical characteristics of gravidas with HBV in Nanfang Hospital from 2008 to 2014. Methods 22 906 gravidas were retrospectively investigated. Results The HBsAg positive rates were 11.64% and 6.16% when the gravidas were divided into Cantonese and non-Cantonese groups (χ2 =193.370, P < 0.005). The ALT abnormal rates in HBeAg positive and HBeAg negative gravidas were 17.96% and 6.68% (χ2=62.594, P<0.005). Conclusion The HBsAg positive rate of gravidas in Guangdong and the ALT abnormal rate of HBeAg positive gravidas are higher.
