1.Quality of life in renal transplant patients versus hemodialysis patients

BACKGROUND: Renal replacement is a therapy for patients with nephropathy at final stage. Hemodialysis and renal transplantation are two effective treatments for nephropathy at final stage. OBJECTIVE: Using Brief Form of World Health Organization Quality of Life Questionnaire (WHOQOL-BREF) to compare and analyze the quality of life (QOL) and the factors influencing QOL in hemodialysis and renal transplant patients. DESIGN, TIME AND SETTING: The survey was performed at the Department of Urology, Beijing Friendship Hospital, Capital Medical University from January 2005 to January 2006. PARTICIPANTS: Sixty hemodialysis outpatients and 60 renal transplant patients from Beijing Friendship Hospital of Capital Medical University were enrolled. All patients were voluntary to participate in the questionnaire investigation. METHODS: There were two scales: ①General condition scale that evaluated the demographic data, clinical specifications, and complication conditions. ②WHOQOL-BREF involved four domains of physical capacity, psychological state, social function and environment, as well as the self-rating total scores by patients. A higher score indicated a better QOL. MAIN OUTCOME MEASURES: All patients were scored by each scale, and compared with the norms. The correlation between general condition of patients and the scores for each domain of QOL was analyzed. RESULTS: A total of 120 patients were all included in the final analysis. The overall score of QOL in hemodialysis patients was significantly lower than that in renal transplant patients (P=0.001). 2.The analysis of application value in minimally invasive surgery technology on orthopedic trauma

Objective:To explore the application value of minimally invasive surgery techniques in orthopedic trauma.Methods: From January 2012 and January 2015, 100 cases of orthopedic trauma patients were selected according to surgical methods, and were divided into traditional surgery group with 49 cases and minimally invasive surgery group with 51 cases, comparing two groups with incision, operation time, intraoperative blood loss, hospital stay, postoperative1 week incision pain score, the incidence of postoperative complications and treatment satisfaction.Results: The minimally invasive surgical incision length, operative time, intraoperative blood loss, pain score, incision healing time and hospital stay in the minimally invasive surgery group were better than that in the traditional surgery group (t=5.452,t=11.358,t=28.560, t=9.210,t=6.820;P<0.05). The incidence of postoperative concurrent with minimally invasive surgery group was 9.8%, traditional surgical complication rate was 18.4%, and the incidence of postoperative concurrent with minimally invasive surgery group was significantly lower than traditional surgical group (x2=12.325,P<0.05). Minimally invasive surgical patients’ satisfaction was 96.08%, traditional surgical operation patients’ satisfaction was 83.67%, the minimally invasive surgery group’s satisfaction was higher than traditional surgery group (H=16.465,P<0.05).Conclusion: Clinical curative effect of minimally invasive technique is better, and easy to be accepted by the patients. It is more popular in clinical application.
3.Comparison of medical coping styles in renal transplant patients and hemodialysis patients☆

BACKGROUND: The perception evaluation of treatment can influence the patients' coping activities and psychosomatic reactions. Therefore, the medical coping mode adopted by patients is one of the most important medium factors affecting clinical treatment.OBJECTIVE: To explore the characteristics and influencing factors of medical coping styles in hemodialysis patients and renal transplant patients.DESIGN, TIME AND SETTING: Questionnaire investigation was performed from January 2005 to January 2006 in Department of Urology, Beijing Friendship Hospital, Capital Medical University (Beijing, China).PARTICIPANTS: Sixty hemodialysis outpatients and 60 renal transplant outpatients with normal graft function from Beijing Friendship Hospital of Capital Medical University, were enrolled into this study. All the patients had to fulfill the questionnaire investigation.METHODS: Questionnaire investigation was carried out in 60 hemodialysis patients and 60 renal transplant patients.Patients completed the questionnaires following instructions by investigators.The interview scale included variables such as gender, age education, occupation, marital status, children, family income, payment mode, influence of medical cost on family, time receiving hemodialysis and time after renal transplantation, etc. Medical coping mode questionnaire contains three subscales: Confrontation. Avoidance and Acceptance-Resignation. Patients scored the questions over a range of 1 to 4 points. A higher score indicated a higher coping tendency adopted by patients.MAIN OUTCOME MEASURES: All the patients were scored by each scale of Confrontation, Avoidance and Acceptance-Resignation, and were compared with the norms. Pearson's correlation analysis was used to explore the correlation between coping modes and influencing factors in hemodialysis patients and renal transplant patients.RESULTS: All 120 patients were involved in the result analysis. There were significant differences between hemodialysis patients and renal transplant patients in medical payment modes, the influence of medical cost on family, and the time on hemodialysis/time after renal transplantation (P<0.05).The average scores of Confrontation and Acceptance-Resignation in hemodialysis patients were lower than those in renal transplant patients (P<0.05). The Avoidance scores showed no significant differences in two groups of the patients (P>0.05). The average score of Confrontation in hemodialysis patients was lower than the norm (P<0.05), and the average scores of Avoidance and Acceptance-Resignation in hemodialysis patients were higher than the norms (P<0.05). The renal transplant patients exhibited similar scores of Confrontation as the norm, without significant differences (P>0.05). The average scores of Avoidance and Acceptance-Resignation in renal transplant patients were higher than the norms (P<0.05). In hemodialysis patients, the score of Confrontation was correlated with the gender of patients (r=-0.277, P<0.05); the score of Acceptance-Resignation was correlated with the family income (r=-0.287, P<0.05). In renal transplant patients, the score of Confrontation was correlated with the marital status (r=0.282, P<0.05).CONCLUSION: The medical coping style adopted by end-stage renal disease patients is influenced by the clinical treatment methods and psychosocial factors.Compared to the hemodialysis patients, renal transplant patients are prone to the Confrontation and Acceptance-Resignation. As for the hemodialysis patients, the score of Confrontation is correlated with the gender of patients, the score of Acceptance-Resignation is correlated with the family income.In renal transplant patients,the score of Confrontation is correlated with the marital status.
4.Therapeutic effects of tacrolimus substituting for cyclosporin in renal transplant recipients with hepatic dysfunction

Objective To evaluate the efficacy and safety of CsA substituted by tacrolimus(FK506) combined with MMF and prednisone in prevention of rejection in renal transplant recipents with hepatic dysfunction. Methods Eight patients with hepatic dysfunction received the treatment of FK506 substituting for CsA. FK506 was administeted at least 24 h after the last dose of cyclosporine.The initial dose of FK506 was based on body weight,the degree of hepatic dysfunetion and the period after operation of the patients.After administration of one week.the dose of FK506 was subsequently adjusted to maintain its whole blood trough levels between 5~15μg/L. Results Substitute of tacrolimus for cycbsporin resulted in a markedly reduced blood index,direct-bilirubin from(22.66±17.19)μmol/L to(7.05±2.32)μmol/L(P<0.05), indirect-bilirubin from(42.15±34.15)μmol/L to(14.54±2.59)μmol/L(P<0.05), sAL T from (83.00±93,14)IU/L to(29,50±15.41)IU/L(P>0.05),Cr from (177.91±86.41)μmol/L to(135.92±34.05)μmol/L(P>0.05)in one month.The ascites of 3 patients disappeared in one month. Astriction,anorexia,upper limbs trembling appeared only in one patient.Conclusion It is effective and safe to substitute CsA by FK506 combined with MMF and prednisone in prevention of rejection in renal transplant recipients with hepatic dvsfunction.

Objective To approach the influence of erythropoietin on proliferation and differentiation of neural stem cells originated from the hippocampi of adult rats in vitro. Methods Neural stem cells were isolated from the hippocampi of adult rats and cultured in vitro with serum-free incubation and single-cell cloning technique; immunochemistry technique was employed to identify the neural stem cells and neural stem cells-derived progeny. Erythropoietin (EPO) were added to cultures in different concentrations during proliferation and differentiation of neural stem cells, respectively. Results Compared with the control group, EPO supplementation group displayed an average twofold to fourfold increase in Nestin positive cells over a wide range of plating densities during proliferation, whereas showed a striking decrease in Nestin positive cells during differentiation,following an early appearance of Tuj1 or GFAP positive cells. 7.Erectile Dysfunction in Men with Moderate and Severe Lower Urinary Tract Symptoms

Objective To evaluate the degree of erectile dysfunction in the benign prostate hyperplasia (BPH) of men with moderate and severe lower urinary tract symptoms (LUTS) .Methods Total 140 patients with moderate and severe LUTS of BPH were investigated using the International Prostate Symptom Score (IPSS) and International Index of Erectile Function-5 (IIEF-5). Erectile dysfunction was less than 21 scores, but erection was more than 21 scores. Regression analysis was used to determine the degree of any correlation among age, the total IPSS, the moderate and severe IPSS score and the various erectile function scores.Results The mean age of the patients was (69 2?7 18) years. The mean total IPSS score was 23 3?7 7, consisting of 45 patients (32%) for moderate IPSS score and 95 patients (68%) for severe IPSS score. The mean IIEF-5 score was 6 8?8 02. There were erection patients in 11 cases (7 9%) and erectile dysfunction patients in 129 cases (92 1%). On the other hand, 41 cases (91%) for moderate IPSS score and 88 cases (93 6%) for severe IPSS score exists erectile dysfunction. There were no significant correlation among IIEF-5 score and patients age (?=0 417), the total IPSS (?=0 235), the moderate (?=0 07) and severe IPSS score (?=0 332). Conclusions About 90% BPH men complicated with LUTS have erectile dysfunction. Erectile dysfunction is related to age and the extent of LUTS. LUTS is not an important factor of erectile dysfunction. The high morbidity of erectile dysfunction in BPH men with LUTS should be consider in the treatment of their urinary symptoms.
8.Effect of Shenqihuang Steady Plaque Caps Combining Simvastatin on Stabilization of Atherosclerosis Plaque in Rabbits

Objective To investigate the role of Shenqihuang steady plaque caps (SQH) and simvastatin on stabilization of atherosclerosis plaque in rabbits whit atherosclerosis (AS). Methods Forty-five rabbits were randomly divided into five groups: normal food group (A), instability plaque group (B), SQH group (C), simvastatin group (D), SQH add simvastatin group. A group was fed with common feeds, and B, C, D, E groups were given high cholesterol feeds. An atherosclerotic rabbit model was established by feeding high cholesterol diet supplemented for 12 weeks in B, C, D, E groups. Then C, D, E groups received corresponding drugs. At the end of 24~(th) week, the drug were injected into the aortic segments rich in plaque. Two weeks later, the concentration in serum of lipids, vascular cellular adhesion molecule-1 (VCAM-1), intercellular cellular adhesion molecule-1 (ICAM-1) and high sensitivity C reaction protein (hs-CRP), and area of plaque, the thickness of endomembrane, ttmica media, fibrous cap were detected. Objective To evaluate the clinical value of 18FDG PET/CT in the diagnosis of lymph node metastasis and evaluation of node staging for esophageal carcinoma. Methods A prospective study was performed to access whether 18F FDG PET/CT can improve the diagnostic accuracy in lymph node metastasis for patients with esophageal carcinoma, 30 patients underwent esophagectomy with extensive dissection of lymph nodes. PET/CT findings were compared with that of CT. Results The pathological examination confirmed metastasis in 22 patients and 49 out of 243 excised lymph nodes. In PET/CT analysis, the sensitivity in the detection of lymph node metastasis was 93.9%, specificity was 91.2%, with a diagnostic accuracy of 91.8%. The positive and negative predictive value was 73.0% and 98.3%, respectively. 10.Detection of HBVDNA and HBVcccDNA in Peripheral Blood Mononuclear Cells by a Multiplex Polymerase Chain Reaction

Objective To establish a multiplex polymerase chain reaction (M-PCR) assay to detect HBV DNA in the peripheral blood mononuclear cells(PBMCs) of chronic HB patients. Methods One pair of primer amplifying HBV genome DNA and another pair of primer amplifying HBV covalently closed circular DNA (cccDNA ) were added to one PCR reaction to detect HBV DNA in PBMCs. Results Various forms of HBVDNA including total DNA and cccDNA could be amplified simultaneously. Among the 30 chronic HB patients, both the HBVDNA and HBVcccDNA in the PBMCs of 23 patients were detected, the positive rate of which was 76.6%. The positive rate of HBV cccDNA accounted for 82.1% of total HBV DNA positive rate. Conclusion HBVDNA in the PBMCs could partially replicate. The M-PCR was successfully set up to amplify HBV genome DNA and HBV cccDNA simultaneously.
