1.Olecranon osteotomy for treatment of intercondylar distal humeral fracture with double-plate fixation

Objective:To evaluate the clinical effect of open reduction and internal fixation for the intercondylar distal humeral fracture with double-plate fixation.Method:A total of 26 cases with intercondylar distal humeral fracture were treated with the double-plate fixation via olecroanon approach.According to the AO/ASIF classification,there were 3 cases with C1,19 cases with C2 and 4 cases with C3.Cassebaum numerical rating system was employed to evaluated postoperative function of the involved elbow.Results:The post-operative function in 26 cases was evaluated according to the Cassebaum numerical rating system,follow-up averaged 15 months,and the results were:excellent in 19 cases(65.4%);good in 9 cases(23.1%);fair in 2 cases(7.7%);poor in 1 case(3.8%).The excellent rate was 88.5%.Conclusion:Double-plate fixation is characterized by stable fixation,minor complication and high satisfaction,and hence is an effective method for intercondylar distal humeral fracture.
2.Influence of all-trans retinoic acid on proliferation and differentiation of neural stem cells from new born rat striatum

AIM: To study the effect and mechanism of all-trans retinoic acid (atRA) on neural stem cell (NSCs) proliferation and differentiation from new born Sprague- Dawley rat striatum. METHODS: NSCs were isolated from the brains of new born Sprague-Dawley rat striatum, and the features of cells were characterized by immunofluorescence staining. The effects of different culture medium on cell cycle distribution and proliferation of NSCs were determined by flow cytometry (FCM) . The effects of atRA on differentiation of NSCs were determined by immunofluorescence staining and classified count of differentiated cells. RESULTS: FCM assay indicated that atRA inhibited the proliferation of NSCs. The percentage of cells in G0/G1 phase in atRA treatment group was significantly higher than that in control, and the proliferation index (PI) was significantly low. The percentage of neurons differentiated from NSCs in atRA group was 2.5 times of the control group after induced by adding 10% FCS in culture medium. CONCLUSION: atRA counteracts the effects of bFGF on the promotion of mitosis and inhibition of differentiation of NSCs. atRA also promotes NSCs to differentiate into neurons in vitro.
3.Interventional treatment for limb and maxillofacial region low flux vascular malformation

Objective To study the clinical feasibility of the percutaneous sclerosis treatment for limb and maxillofacial region low flux vascular malformations. Methods 17 cases with limb and maxillofacial region low flux vascular malformation were collected from 1998 to 2002 accepting interventional treatment in our hospital with application of direct percutaneous sclerosis or percutaneous sclerosis through arteriography or arterial embolism three methods, using tardener's alcohol method of Hardener with alcohol or iodinated oil accompanied by pingyangmycin. Results Altogether 17 cases included with once cured in 11 cases, multple times of treatment in 5 and the last case needed necrotic tissue resection with skin graft due to localized muscular necrosis caused by inproper embolization. Conclusions The interventional treatment for limb and maxillofacial region low flux vascular malformation is an effective and safety method.

BACKGROUND: Siberian solomonseal rhizome is a sort of Chinese traditional medicine for anti-senilism. The effective component, poly gonati polysaccharide, has the effects of reducing blood glucose and glycosylhemoglobin.OBJECTIVE: To assay regulative effect of polygonati polysaccharide on expression of the key substance of non-enzymic glycosylation of proteinsglycosylated end-product receptor mRNA by reverse transcriptase polymerase chain reaction, so as to develop effective inhibitor for non-enzymic glycosylation of proteins and provide experimental evidences for preventing diabetes and its complications.DESIGN: Randomized control animal trial SETTING: Department of Geriatrics, the Second Xiangya Hospital, Central South University; Department of Cardiology, Haikou Hospital Affiliated to Xiangya Medical College, Central South University.MATERIALS: The experiment was completed in Animal Room of Second Xiangya Hospital of Central South University form March to June 2004. A total of 30 BALB/C mice of clean grade were selected and randomly divided into normal control group, model control group and polygonati polysaccharide group, with 10 in each group.METHODS: Diabetic models were established by intraperitoneal injection with streptozotocin to mice in model control group and polygonati polysaccharide group. Model establishment would be regard as successful if blood glucose of mouse was 8.0 mmoL/L or above. Mice in polygonati polysaccharide group were treated with polygonati polysaccharide (2 mL/kg per day), while mice in normal control group and model control group were treated with injection of 0.5 mL water once a day for 12 consecutive weeks.After medicine had been given to the mice, they were put to death by decapitation. Reverse transcriptase polymerase chain reaction was used to assay expression of glycosylated end-product receptor mRNA in cardiac and renal tissues of experimental animals.MAIN OUTCOME MEASURES: ① Observation of general situation of mice in each group 12 weeks later after model establishment. ② Change of blood glucose of mice in each group before and after model establishment.③ Gel electrophoretic maps of glycosylated end-product receptor mRNA in cardiac and renal tissue of mice in each group. ④ Semi-quantitative assay of glycosylated end-product receptor in cardiac and renal tissue of mice in each groupRESULTS: All the 30 mice entered the results analysis. ① Observation of general situation of mice in each group 12 weeks later after model establishment: mice in normal control group gained weight and moved freely; mice in model control group manifested the symptoms of losing weight, polyuria, listlessness, lags in response etc.; mice in polygonati polysaccharide group manifested milder symptom of polyuria and more sensitive in responses as compared with model control group.② Changes of blood glucose of mice in each group before and after model establishment: blood glucose levels were similar between normal control group and model control group before and after model establishment (P > 0.05), while blood glucose in polygonati polysaccharide group significantly decreased 12 weeks later after model establishment [(10.05±1.16), (7.18±0.84) mmoL/L, P < 0.05]. ③ Gel electrophoretic maps of glycosylated end-product receptor mRNA in cardiac and renal tissue of mice in each group: Compared with normal control group, the expression of glycosylated end-product receptor mRNA increased in cardiac and renal tissue of mice in model control group, while the expression in polygonati polysaccharide group significantly decreased as compared with model control group. ④ Semi-quantitative assay of glycosylated end-product receptor in cardiac and renal tissue of mice in each group: the relative value of glycosylated end-product receptor to β-actin in cardiac and renal tissue of mice in model control group was significantly higher than normal control group (P < 0.01); however, the relative value of polygunati polysaccharide group significantly decreased as compared with model control group (0.760±0.121,0.998±0.202;0.609±0.146;0.765±0.113; P < 0.05).CONCLUSION: Besides reducing blood glucose, polygonati polysaccharide can significantly down regulate high expression of glycosylated endproduct receptor mRNA in cardiac and renal tissue of mice with diabetes, so as to inhibit the combining sites for glycosylated end-products and a series of cytobiological reactions after combined with their receptors, and protect the target organs and tissues from injuring by hyperglycemia.
6.The effect of 3'-meisoindigo on the function of splenocytes and thymocytes of C57BL/6 mouse

BACKGROUND: The effect of Chinese traditional medicine 3'-meisoindigo as well as indimbin derivatives on normal immunocytes is less reported while it is used for antitumor.OBJECTIVE: To investigate the effect of 3'-meisoindigo on the proliferation of the splenocytes and thymocytes of C56BL/6 mouse.DESIGN, TIME AND SETTING: The randomized cytopathology observation was performed between August 2007 and January 2008 at Zhujiang Hospital of Southern Medical University, Guangzhou, Guangdong Province, China.MATERIALS: C57BL/6 mice, clean, male, 6-8 weeks old, weighing (20±2) g.METHODS: The thymus gland and the spleen of C57BL/6 mice were ground to get the single-cell suspension and cells were treated by 5, 10, 15, 20 and 25 μ mol/L 3'-meisoindigo,respectively. Cells without any drug treatment were used as blank control and cells treated by concanavalin A were used as positive control.MAIN OUTCOME MEASURES: Proliferation of splenocytes and thymocytes detected using MTT method; IL-12 activity in culture supematant detected using ELISA method; the cell cycle, apoptosis rate, cell death rate and intracellular reactive oxygen species level detected using flow cytometry; the mRNA level of bcl-2 and cdk2 detected using reverse transcription-polymerase chain reaction; the expression rate of Bcl-2, CDK2 and Bax detected using fluorescence microscope.RESULTS: After treating for 24 hours, 15, 20 and 25 μ mol/L 3'-meisoindigo can significantly inhibit the proliferation of thymocytes and splenocytes (P < 0.05) and the inhibition was dose-dependent and time-dependent. Cells resumed proliferation after removing the 3'-meisoindigo, although they had been treated by 25 μ mol/L 3'-meisoindigo for 72 hours. The secretion of IL-12 was markedly reduced in all 3'-meisoindigo groups versus control groups at each time point (P < 0.05). 15 μ mol/L 3'-meisoindigo could decrease the mRNA expression level of apoptosis-related protein bcl-2 and cyclin cdk2 gene, decrease the expression level of BCL-2 protein and CDK protein, increase Bax expression level, decrease Bcl-2/Bax ratio markedly and started apoptosis.15 μ mol/L 3'-meisoindigo arrested cells in the G2/M stage of the thymocytes and splenocytes, and intracellular reactive oxygen species level elevated dose-dependently and time-dependently.CONCLUSION: In certain concentration range, 3'-meisoindigo can reversibly inhibit the proliferation of thymocytes and splenocytes of C57BL/6 mouse and can inhibit IL-12 secretion in parallel, and start apoptosis.
7.Predictive value for coronary heart disease by epicardial adipose tissue and carotid intima-media thickness

Objective To explore the predictive value for coronary heart disease by epicardial adipose tissue(EAT) thickness and carotid intima-media thickness(IMT) measured with high-frequency ultrasound.Methods According to the results of coronary angiography, the cases were divided into the normal control group (29 patients),coronary artery disease group with single-vessel lesion (43 patients),coronary artery disease group with multi-vessels lesion (28 patients), respectively. EAT and IMT were measured by high-frequency ultrasound. Results The EAT in the three groups were (4.8 ± 1.3) mm, (7.6 ± 1.8) mm,(10.1±2.6) mm respectively, and the IMT were (0.8±0.1)mm,(1.0±0.2)mm,(1.1 ± 0.2)mmrespectively. In either coronary artery disease group with single-vessel lesion or multi-vessels lesion, the EAT and IMT were significantly higher than those in the normal control group ( P< 0.01 ). And the difference between groups of single-vessel lesion and multi-vessels lesion was also statistically significant (P< 0.01). The areas under receive operating characteristic(ROC) curve to predict coronary heart disease by EAT and IMT was 0.947 and 0.917, respectively, there was no significant difference between the two areas. For patient with coronary artery stenosis>50%, the sensitivity and specificity of EAT>6 mm were 90.1% and 86.2% ,respectively,the sensitivity and specificity of IMT>0.85 mm were 87.3% and 82.8%,respectively. Conclusions EAT and IMT measured by high-frequency ultrasound can precisely predictcoronary heart disease. EAT can be a new predictor to diagnose coronary heart disease.
9.Correlation between epicardial adipose tissue thickness and cardiovascular risk factors in patients with coronary artery disease

Objective To explore the correlation between epicardial adipose tissue(EAT) thickness and cardiovascular risk factors in patients with coronary artery disease.Methods According to the results of coronary angiography,84 cases of patients undergone coronary angiography were divided into the normal control group(28 patients),coronary artery disease group with single-vessel lesion (28 patients),coronary artery disease group with multi-vessels lesion(28 patients),respectively,then measured the EAT thickness and carotid intima-media thickness(IMT) by high-frequency ultrasound.A correlation analysis was carried out between the EAT and IMT and cardiovascular risk factors.Results In either coronary artery disease group with single-vessel lesion or multi-vessels lesion,the thicknesses of EAT and IMT were significantly higher than those in the normal control group(P <0.01),and the difference between group of single-vessel lesion and multi-vessels lesion was also statistically significant (P <0.01).A correlation analysis between the EAT and IMT and cardiovascular risk factors showed that there were significantly positive correlations between EAT and the IMT,age,weight,waist circumference,body mass index,low-density lipoprotein,Creactive protein (r = 0.124~0.790,P <0.05 or P <0.01),and the significantly negative correlation between EAT and high-density lipoprotein (r = - 0.203,P < 0.05).Correlation between EAT and the IMT was 0.678,0.713,0.737 in the normal control group,group with single-vessel lesion,group with multivessels lesion,respectively.Conclusions There were favorable correlation between EAT and carotid IMT in cardiovascular risk factors.The more severity of coronary heart disease,the more high correlations of EAT and IMT.EAT can be a valuable index in evaluating coronary artery disease.
10.Haploidentical hematopoietic stem cells transplantation for the treatment of medulloblastoma One case report

BACKGROUND: Some reports demonstrated that, autoallergic or hematopoietic stem cells transplantation combined with chemotherapy received good outcomes in treating medulloblastoma, which can prolong survival time of patients. However, whether haploidentical hematopoietic stem cells transplantation can treat medulloblastoma remains poorly understood.OBJECTIVE: To firstly report a patient receiving haploidentical hematopoietic stem cells transplantation for treating medulloblastoma.METHODS: A terminal cancer patient with bone matastases successively received six lymphocyte transfusions from an unrelated donor combined with chemotherapy and three haploidentical hematopoietic stem cell transplantations.RESULTS AND CONCLUSION: The patient presented erythra, accompanied by fever, diarrhea and yellow brown liquid stools, which was considered as graft versus host disease, and treated by urbason, gammaglobulin, CellCept, Prograf, Basiliximab (anti-CD25 antibody), Infliximab (anti-tumor necrosis factor α antibody), effective antibacterial and supportive treatments. After that, the erythra and diarrhea were remised. But the patient died from cerebral hemorrhage. Allogeneic lymphocyte transfusion can kill or damage tumor cells, improve life quality, but the outcome is restrained for patient with a high tumor burden. Immunosuppressant, such as anti-CD25 antibody and anti-tumor necrosis factor α antibody should be timely used in consideration of allogeneic hematopoietic stem cell transplantation.
