1.Establishment of the allograft menisci bank and preliminary clinical application

[Objective] To investigate the establishment of the Allograft Menisci Bank so as to provide suitable allograft meniscus for clinical study of allograft meniscus transplantation.[Method]The allograft menisci from the plentiful donors of the tissue bank were achieved and preserved with the deep-frozen technique.Clinical use of allograft menisci should be based on practical requirement of patients and the data recorded in the Menisci Bank.[Result]From 2005 to the end of 2006,an Allograft Menisci Bank was established which consisted with about 300 different-sized menisci and 14 allograft menisci were provided for clinical study.[Conclusion]Establishment of the Allograft Menisci Bank can provide a base for clinical meniscus transplantation.The short-term effects of the allograft menisci transplantation are satisfactory,but the long-term outcomes need more cases and further observation.
2.Influence of defatting procedure on immunogenicity of cancellous bone allograft

D and the differences between groups were statistically significant(P0.05).Proliferation index increased in the early phase and reached the peak at the 4th post-operative week.Level of proliferation index in four groups was gradiently decreased from group A to group D.At the 4th post-operative week,differences of proliferation index were statistically significant between groups(P0.05).The changing trend of ratio of CD4+/CD8+ was similar to that of proliferation index.Histological observations showed inflammatory cells infiltrated in early phase gradiently decreased from group A to group D.At the 4th post-operative week,histological observations showed neovascularization in group D.At the 8 weeks post-operatively,more new bone formation was observed in group D. 3.Influence of defatting procedure on osteogenicity of cancellous bone allograft

[Objective]To explore the influence of defatting procedure on osteogenicity of cancellous bone allograft.[Method]Thirty inbred Lewis rats were killed.Distal femoral metaphyses were resected as cancellous bone grafts.Grafts were randomly divided into four groups which were treated by four different procedures,respectively.Content of total lipid and total protein in grafts were measured.Twelve inbred Fisher344 rats were randomly divided into four groups according to different grouped grafts which were transplanted into femoral metaphysis bone defects of the recipients.Two different fluorochrome dyes were given subcutaneously to the recipients at 14th and 4th days before sacrifice respectively.All rats were killed at the 8th postoperative week.The experimental distal femoral metaphyses were harvested and nondecalcified sections were prepared.Sections were observed with a fluorescent microscope and bone histomorphometries were studied.The correlation analysis was performed between mineral apposition rate(MAR) and the content of total lipid.[Result]Results of measurement showed content of total lipid in four grouped grafts were different and the difference between groups were statistically significant(P
4.Retrospective study of 33 deaths of in-patients with percutaneous coronary intervention

Objective To discuss the reasons of death in the in-patients with percutaneous coronary intervention(PCI).Methods Retrospective analysis was done in 33 cases who died in or after the procedures among 3 252 in-patients with PCI.Results 4 patients died in procedures and the others died after procedures.The major reasons of death in order were:cardiogenic shock,ventricular fibrillation,acute left heart failure,multiple organ failure,heart rupture and acute renal failure.The in-hospital mortality rate was 1.01%.Conclusion The major reasons of death of patients with PCI are cardiogenic shock,ventricular fibrillation and acute left heart failure.Taking some measures will reduce the in-hospital mortality rate.
5.Investigation on a Water Contamination Accident in Tuojiang River Caused by Industrial Wastewater

Objective To investigate a water contamination accident caused by industrial wastewater during Feb 27th-29th ,2004,in Tuojiang river, analyze the variation of sanitary quality of drinking water during and after the accident. Methods After the occurrence of the accident,consecutive surveillance and analyses were done to discover the variation of the levels of amino-nitrogen,nitrite and nitrate in source water and finished water of Jianyang Water Works and Neijiang No 2 Water Works,which located at the lower reaches of Jianyang. Results The levels of amino-nitrogen in source water and finished water of Jianyang Water Works reached the first peak [(47.04?2.48)?(45.97?2.37)mg/L]at the 3rd day(March 3rd) after the end of discharge of industrial wastewater from contamination source,decreased at the 4th day ,reached the second peak at the 5th day[(42.26?2.88)?(39.50?2.64)mg/L], then decreased gradually, presented the normal levels at the 11th day[(0.54?0.10)?(0.30?0.12)mg/L]. The levels of nitrite in source water and finished water of Jianyang Water Works reached the first peak [(2.09?0.23)?(0.91?0.15)mg/L] at the 2 nd-3rd day after the end of discharge of industrial wastewater from contamination source,decreased at the 4th day ,reached the second peak at the 5th day[(2.18?0.33),(0.61?0.16)mg/L], then decreased gradually,presented the normal levels at the 11th day[(0.16?0.07)?(0.02?0.01)mg/L]. The levels of nitrate in source water and finished water of Jianyang Water Works(3.56~9.15 mg/L) didn't exceed the standard ruled the Sanitary Standard for Drinking Water (2001) throughout the duration of accident. The levels of amino-nitrogen and nitrite in source water and finished water of Neijiang No.2 Water Works both exceeded the related standards, the levels of nitrate met the requirement of the Sanitary Standard for Drinking Water (2001) during the period from the first day to the third day after the end of discharge of industrial wastewater from contamination source. From the forth day to the 25 th day, Neijiang No.2 Water Works was changed to be supplied by the Five-star reservoir,and produced safe and sanitary finished water meeting the requirement of the Sanitary Standard for Drinking Water (2001). Conclusion This wastewater contamination accident seriously deteriorates the quality of drinking water in Jianyang and Neijiang.
6.Anesthesia for surgery of spinal tumors

[Objective]To summarize the clinical experience on anesthesia techniques for surgery of spinal tumors.[Method]A total of 546 patients undergoing surgery of spinal tumors were retrospectively studied,and the complications of surgery were also collected.[Result]There was no perioperative death.The average degree of blood loss was 1997ml per patient who usually required a large volume of blood transfusion.Acute pumonlary edema were occurred in 3 patients who were suspected to develop larynx edema immediately after operation.A perioperative visual loss following cervical spine surgery was also founded.[Conclusion]It is important to prevent and minimize blood loss by varied techniques during operation.Peventing adverse effect of operative position on patients and protecting their airway are also crucial for spinal surgery.

Objective To study the protective effect of interleukin-33 (IL-33) on mouse warm hepatic ischemia-reperfusion (I/R) injury.Methods On a mouse warm hepatic I/R injury model IL-33 mRNA and protein levels during hepatic ischemia and reperfusion period were determined,and then mice were divided into control group,model group,recombinant IL-33 intervention group and anti ST2L antibody intervention group,and mice were sacrificed after 6 hours of reperfusion.Serum aspartate aminotransferase (AST),alanine aminotransferase (ALT) protein levels were determined.Liver pathology was observed by transmission electron microscopy and serum cytokine level (tumor necrosis factor-α,interferon-γ,IL-4,IL-5,IL-13) were measured by flow cytometry CBA method.Results The level of IL-33 mRNA and protein were significantly higher in the reperfusion stage (t2 h =-3.574,t6 h =-4.147 ; P < 0.05).After intervention by recombinant IL-33,the level of serum ALT and AST decreased significantly (tALT =4.592,tAST =3.471 ; P < 0.05),the severity of pathological damage was reduced,the levels of IL-4,IL-5,IL-13 increased and that of IFN-γ decreased,with statistically significant difference in comparison with the control groups (tIL-4 =-4.995,tIL-5 =-4.584,tIL-13 =-4.431 ; P < 0.05).Anti-ST2L antibody intervention effected the opposite.Serum TNF-α level did not change in intervention groups compared with that in model group (tTNF-α =0.261,P > 0.05).Conclusions IL-33 mRNA and protein level increased in mice with hepatic I/P injury.IL-33 exerts a protective effect on the I/R injured liver after binding to its receptor ST2L.
9.Clinical analysis of standard large trauma craniotomy in treatment of acute subdural hematoma

Objective To explore the surgical techniques and clinical outcome of standard large trauma craniotomy in the treatment of acute subdural hematoma.Methods The clinical data of 51 cases with acute subdural hematoma,who underwent standard large trauma craniotomy in recent 5 years in our department,were retrospectively analyzed.Results All the patients were followed up for 3 ~ 6 months.10 cases were dead,4 persistent vegetative status,severe disability in 6 cases,midrange disability occurred in 11 cases and 20 cases were good.Postoperative complications included tardive haematoma in 4 cases,traumatic cerebral infarction in 2 cases,subdural collection of fluid in 4 cases,cerebromalacia in depressor area in 2 cases,hydrocephalus in 2 cases,and traumatic epilepsy in 3 cases.Conclusion Standard large trauma craniotomy is an effective approach in the treatment of acute subdural hematoma,due to its advantages such as satisfactory exposure for haemostasis,rapid removal of haematoma and relief of acute intracranial hypertension.
10.Study on peritoneal micrometastasis by detection of CEA in peritoneal lavage fluit in patients underwent laparoscopy-assisted radical gastrectomy

Objective To assess the change of CEA in the peritoneal lavage fluit at pre-,post-laparoscopy-assisted radical gastrectomy, and after hyperthermic perfusion chemotherapy, and to investigate the influence of laparoscopy-assisted radical gastrectomy on drop off of cancer cells, the efficiency of hyperthermic perfusion chemotherapy. To investigate whether CEA in the peritoneal lavage fluit detected by flow cytometry (FCM) is an effective predictor of intraperitoneal free cancer cells and peritoneal metastasis. Methods Peritoneal washings of 40 patients with gastric carcinoma were collected to detect CEA using FCM. The peritoneal lavage cytology examinations ( PLC)were detected by H-E staining. Results Laparoscopic surgery in patients with gastric cancer,before resection of gastric cancer the peritoneal washing CEA positive rate 35. 0% (14/40) ,and after operation, the positive rate was 40. 0% ( 16/40) .which was not significantly higher than that before operation ( P > 0. 05 ). After intraperitoneal hyperthermic perfusion chemotherapy the CEA positive rate was 7. 5% (3/40) ,which was significantly lower than that pre-operation(P<0. 05). Before operation there were 4 cases of positive PLC in the peritoneal lavage fluid, 14 CEA-positive detected by flow cytometry, and there was significant difference ( P < 0. 05 ) . All PLC-positive cases were positive for CEA, whereas 10 PLC-negative cases showed CEA-positive. None of CEA-negative cases showed PLC-positive. Conclusions Laparoscopic radical gastrectomy does not increase the intra-abdominal gastric cancer cell shedding. Intraoperative hyperthermic perfusion chemotherapy is simple and feasible approach with high efficiency.Peritoneal washing CEA detected by flow cytometry is an effective index to predictor for intraperitoneal free cancer cells and prediction of peritoneal metastasis.
