1.Rehabilitation of diabetic peripheral neuropathy with integrated traditional Chinese medicine therapy

ObjectiveTo observe the effect of integrated traditional Chinese medicine therapy on diabetic peripheral neuropathy.Methods22 patients with diabetic peripheral neuropathy were as the group of integrated traditional Chinese medicine therapy ,who received Chinese herbal medicine treatment(orally),acupuncture,etc. Other 20 cases were as the control group,disease natural course. The effect of two groups were compared with the means.ResultsThe patients in group of integrated traditional Chinese medicine therapy got obviously better results than that of the control group(P<0.05).ConclusionThe integrated traditional Chinese medicine therapy is effective on diabetic peripheral neuropathy.
2.The value of ROX index in evaluating the efficacy of high-flow nasal cannula oxygen therapy in patients with COVID-19

Objective:To assess the value of the ROX index in evaluating the efficacy of high-flow nasal cannula oxygen therapy (HFNC) in patients with coronavirus infected disease (COVID-19).Methods:This is a retrospective study. The included patients were diagnosed as COVID-19 in the intensive care unit (ICU) of the Cancer Center of Union Hospital affiliated to Tongji Medical College of Huazhong University of Science and Technology from February 15, 2020 to March 15, 2020. All the patients were treated by HFNC. According to whether the patient subsequently received non-invasive positive pressure ventilation or invasive positive pressure ventilation, patients were divided into the HFNC success group and the HFNC failure group. Parameters in the two groups such as basic characteristics, lactic acid, number of chest radiographs, APACHE II, lymphocyte count, baseline respiratory rate, baseline percutaneous oxygen saturation, baseline PaO 2/FiO 2, baseline ROX index, and ROX index after 2, 6 and 12 h HFNC treatment were analyzed with t test, Chi-square test or rank sum test. Results:A total of 57 cases were included in this study. There were no significant differences in sex, age, comorbidities, lactic acid, quadrants of chest radiograph lung infection, APACHE II, lymphocyte count, and baseline respiratory frequency, transcutaneous oxygen saturation, oxygenation index, and ROX index between the HFNC success group and the HFNC faliure group ( P>0.05). Logistic regression analysis showed that ROX index after 2 h HFNC treatment ( OR=0.069), ROX index after 6 h HFNC treatment ( OR=0.194) and ROX index after 12 h HFNC treatment ( OR=0.036) were all protective factors for the therapeutic effect of HFNC treatment in COVID-19 patients. ROC curve showed that there were significant differences in ROX index after 2 h HFNC treatment, ROX index after 6 h HFNC treatment, and ROX index after 12 h HFNC treatment ( P<0.05). In the evaluation index, the area under the ROC curve of the ROX index after 2 h HFNC treatment was 0.838, the sensitivity was 64.5%, and the specificity was 100%. After 6 h HFNC treatment, the area under the ROX index ROC curve was 0.762, the sensitivity was 64.5%, and the specificity was 92.3%. After 12 h HFNC treatment, the ROX index ROC curve area was 0.866, the sensitivity was 67.7%, and the specificity was 100%. Conclusions:The ROX index can be used to evaluate the efficacy of HFNC in COVID-19 patients in a timely, simple and real-time manner.
3.Significance of urinary protein in evaluating the damage degree of diseased kidneys in children with hydronephrosis

Objective To evaluate the damage degree of diseased kidneys in children with hydrone- phrosis using urinary?2-microglobulin(?2-MG),albumin(Alb),T-H glycoprotein(THP)and immuno- globulin(IgG).Methods The?2-MG,Alb,THP and IgG in pelvis urine in diseased kidneys and in healthy kidneys(as controls)were detected in 78 children with congenital hydronephrosis by radioimmuno- assay;at the same time urinary creatinine(Cr)was detected in 36 cases of them.Pathologic changes of the diseased kidneys were graded;and the correlations of pathologic grades with?2-MG,Alb,THP,IgG and?2-MG/Cr,Alb/Cr,THP/Cr,IgG/Cr were analyzed by Spearman's test.Results In healthy and hydro- nephrotic pelvises,the relevant parameters were as follows.Urine?2-MG was(130.06?22.02)ng/ml vs(386.18?42.15)ng/ml;Alb was(80.52?9.32)ng/ml vs(275.54?69.46)ng/ml,respectively (both P<0.05).Urine?2-MG/Cr was(0.16?0.04)ng/?mol vs(1.57?0.39)ng/?mol;Alb/Cr was (0.11?0.03)ng/?mol vs(1.07?0.56)ng/?mol(both P<0.01).THP was(25.80?9.02)ng/ml vs(31.43?10.56)ng/ml;and IgG was(15.21?3.56)ng/ml vs(20.52?10.85)ng/ml(P>0.05). THP/Cr was(0.05?0.01)ng/?mol vs(0.14?0.06)ng/?mol;and IgG/Cr was(0.04?0.01)ng/?mol vs(0.12?0.05)ng/?mol(P<0.05).There was significant correlation between urine?2-MG,Alb lev- els and pathologic grades(P<0.05),also between?2-MG/Cr,Alb/Cr levels and pathologic grades(P<0.01).There was positive correlation between THP,IgG and pathologic grades but with no significance(P>0.05),also between THP/Cr,IgG/Cr and pathologic grades with significance(P<0.05).Conclu- sions For hydronephrotic kidneys,urine?2-MG/Cr can evaluate the damage degree of nephric tubules, whereas urine Alb/Cr can evaluate that of glomeruli.Increase in THP/Cr and IgG/Cr respectively indicate the impaired nephric tubules and glomeruli in children with congenital hydronephrosis.

Poly(β-amino esters) (PBAE) are used for drug carrier and have many advantages, such as pH-sensitivity, low toxicity, structural diversity and the synthetic method of PBAE is easy. Therefore they are possessed broad application prospect in tumor-targeted drugs delivery systems. In this paper, the structural features and target drugs delivery property of PBAE are reviewed. The application forms of PBAE and different anti-cancer drugs loaded in the copolymer for tumor-targeted drugs delivery systems are introduced particularly.

5.Growth and developmental rhythm of Scrophularia ningpoensisin southwest middle mountain area of China.

Plant samples were collected and investigated periodically. According to the growth of different parts and the characteristics of dry substance accumulation of Scrophularia ningpoensis, the development of S. ningpoensis could be divided into four stages: seeding stage, stem and leaf growth stage, expanding period of root tubers, and dry substance accumulation stage of root tuber. Leaf numbers of S. ningpoensis grew gradually from one at first to 370 at the final stage, main stem leaf were 50 pieces. Leaf size increasesed with the fastest growth at the stem and leaf growth stage, average daily increase amount was 225 cm2. By the middle of August, leaf size reached to 16,270 cm2. Leaf area indexrose sharply in the seeding stage, and remained above 8 among stem and leaf growth stage and expanding period of root tubers, and rapidly reduced to zero in the stage of dry substance accumulation of root tuber. Leaf area ratio has a tendency of obvious dropping. The net assimilation rate had a small change ranges, two small peak were seeding stage and dry substance accumulation of root tuber. The value of specific leaf area was higher in seeding stage, and in the earlier stage of dry substance accumulation of root tuber. Relative growth rate changed with large ranges, higher in seeding stage, rapid decrease in stem and leaf growth stage, rose in expanding period of root tubers, and declined again in the stage of dry substance accumulation of root tuber. Crop growth rate was higher in the first and last stages, and smaller in interim stage. The growth parameters of S. ningpoensis such as relative growth rate, net assimilation rate, leaf area index, leaf area ratio, specific leaf area, crop growth rate changed along with the growth. The rule of dry matter accumulation was as follows: the dry matter increased slowly during the seeding stage and speeded up in the middle and late stages, and in dry substance accumulation of root tuber increased slower, the growth of dry matter all appeared an "S" curve, and accorded with logistic equation. Cultivation technologies of S. ningpoensis and the relevant management methods could be established according to the growth of different parts of S. ningpoensis and the characteristics of dry substance accumulation in different stage.

6.Hemangioma of Small Intestine in Children Causing Recurrent Gastrointestinal Hemorrhage:3 Cases Report and Literature Review

Objective To summarize the clinical characteristics and diagnosis and treatment of hemangioma of small intestine in children.Methods Location,pathology,clinical manifestation,diagnosis and treatment of 3 cases from our hospital and 44 cases reported in our courtry with hemangioma of the small bowel were analyzed from 1994 to 2004.Results These tumors locating in jejunum,ileum,duodenum as well as parts small intestine were 31.9%,42.6%,4.2% and 21.3%,respectively.Solitary and multiple tumors were 29.8% and 70.2% separately.The histopatholical report of 10 cases revealed that capillary,cavernous,mixed type hemangioma and hematolymphangioma were 2,6,1 and 1 cases,respectively.Thirty nine children presented with recurrent black stool.Five patients manifest in the form of intussusception.One child passed a bloody stool so massive as to cause shock.Small-bowel obstruction occurred in 5 cases.The hemoglobin of 38 patients were lower than 90 g/L.The incidence of preoperative diagnosis was only 10.6%.All of patients were performed operation.Conclusions The frequent locations of hemangioma of small intestine are in ileum and jejunum,and multiple tumors are common.The diseases are characterized as recurrent hematochezia with painlessness.Preoperative diagnosis of a small bowel hemangioma can be very difficult.The segments of small bowel with hemangioma resection are the most method of the treatment.
7.Polymorphism of the Bacillus anthracis BclA Protein

Bacillus anthracis collagen-like protein(BclA) is a structural component of the exosporium filaments,as well as the immunodominant antigen on the spore surface.The genes encoding BclA proteins were cloned and sequenced from three Bacillus anthracis strains separated from China.It was founded that the BclA proteins of strain A16R and 40048,containing 388 and 322 amino acids,72 and 50 copies of GXX repeat,5 and 3 copies of 21-amino-acid sequence(GPT)_(5)GDTGTT(BclA repeat) respectively,are different from those reported by foreign scholars;while the BclA protein of strain 40022,containing 370 amino acids,66 copies of GXX repeat,and 5 copies of BclA repeat,is identical with that of strain 53169 reported by others.The results are helpful for the molecular typing of B.anthracis strains,and provide a basis for the elucidation of the pathogenesis and immunogenicity of B.anthracis spore.
8.Structures of four new triterpenoid saponins from the leaves of Oplopanax elatus Nakai.

AIMIsolation and structural elucidation of the triterpenoid saponins of Oplopanax elatus Nakai.
METHODSSolvent extraction and column chromatography were used to isolate the triterpenoid saponins, physico-chemical constants and spectroscopic analysis were employed for structural elucidation.
RESULTSFour newtriterpenoid saponins named cirenshenoside S (1), cirenshenoside T (2), cirenshenoside U (3) and cirenshenoside V (4) were isolated, and their structures were elucidated to be 3-O-beta-D-glucopyranosyl 3beta,23-dihydroxylup-20 (29)-en-28-oic acid 28-O-alpha-L-rhamnopyranosyl (1 --> 4)-beta-D-glucopyranosyl (1 --> 6)-beta-D-glucopyranoside (1), 3-O-beta-D-glucopyranosyl hederagenin 28-O-alpha-L-rhamnopyranosyl (1 --> 4)-beta-D-glucopyranosyl (1 --> 6)-beta-D-glucopyranoside (2), 3-O-beta-D-glucopyranosyl 3beta-hydroxyolean-9(11),12-dien-28-oic acid 28-O-alpha-L-rhamnopyranosyl (1 --> 4)-beta-D-glucopyranosyl (1 --> 6)-beta-D-glucopyranoside (3) and 3alpha-hydroxyolean-12-dien-23,28-dioic acid 28-O-alpha-L-rhamnopyranosyl (1 --> 4 )-beta-D-glucopyranosyl (1 --> 6)-beta-D-glucopyranoside (4), respectively.
CONCLUSIONCompounds 1-4 are new triterpenoid saponins and isolated from the leaves of Oplopanax elatus Nakai for the first time.

10.Mismatch pair defective phenotype in hereditary nonpolyposis colorectal cancer in the Chinese.

OBJECTIVETo study the protein expression pattern of DNA mismatch repair genes hMSH(2), hMLH(1) and the microsatellite instability (MSI) status in the tumor tissue from hereditary nonpolyposis colorectal cancer in the Chinese.
METHODSFifty-eight families fulfilling different clinical criteria including Amsterdam Criteria (AC) (22/24 families, 38 tumors), Japanese Criteria (JC) (12/15 families, 16 tumors) and Bethesda Guidelines (BG) (12/19 patients, 13 tumors) were studied. Monoclonal antibodies against hMSH(2), hMLH(1) proteins and a panel of microsatellite markers (5 loci) including BAT26, BAT25, D2S123, D5S346 and D17S250 were used for study.
RESULTSMSI-H was identified in all 22 (100%) AC tumors, with 81.8% (18/22) showing altered hMSH(2) or hMLH(1) expression; in 14/15 (93.8%) JC cancer, 1/1 (100%) JC adenoma, with 45.5% (5/11) showing altered hMSH(2) or hMLH(1) expression; and in 7/13 (53.8%) BG tumors, with 4/7 showing loss of hMSH(2) or hMLH(1) gene expression.
CONCLUSIONThe frequency of MSI-H and loss of mismatch repair protein are different in the families fulfilling different clinical criteria. Amsterdam Criteria and Japanese Criteria are the two most useful criterion systems for identifying mismatched repair defective tumors. However, Bethesda Guidelines should also be used for detecting more such tumors. The combination of immunohistochemical methods and microsatellite instability analysis is an effective strategy to detect the mismatch repair defective tumors. A close correlation does exist between hMSH(2), hMLH(1) protein expression pattern and MSI status.
