1.Genetic algorithm and support vector machine-based gene microarray analysis

BACKGROUND: Gene microarray data has small sample size and large numbers of variates.Traditional statistical method is not effective.Genetic algorithm(GA)and support vector machine(SVM)are machine learning algorithms developed rapidly in recent years,which can decrease the dimension of features.OBJECTIVE: To combine GA and SVM to classify samples and compare with other two processes in which all genes and difference expression genes are taken as classifiers,respectively.METHODS: We applied golub data set provided by Bioconductor,which included gene expression data of leukaemia samples and normal samples.All genes were used to classify samples with SVM.SAM software was used to extract difference expression genes and estimate False Discovery Rate.Finally,76 difference expression genes were used as feature gene set to classify samples with SVM and GA-SVM respectively.Three classification effects were compared.Additionally,the distribution and function about feature genes in KEGG pathways were also discussed.RESULTS AND CONCLUSION: The accuracy of classification of SVM was improved by decreasing dimension with genetic algorithm.In particular,this process eliminated a great deal of redundant genes and noises,which improves the classification performance.Results show that GA-SVM algorithm is effective in classifying samples.In addition,the pathway analysis shows that signal transmission and amino acid metabolism are two major functions of feature genes.

3.Introduction to nurse-staffing ladder and compensation system in NHS UK

Objective To explore ladder and compensation system of nurse-staffing which is suitable for the situation of our country.Methods The ladder management and compensation system to nursestaffing of National Health Service (NHS) in UK were given detailed introduction and analysis through the official documents,references and personal study experiences,and summarize its scientific and referenced aspects.Results NHS has formed a set of mature competency-based ladder system,and linked the ladder system to compensation system,made a relative scientific performance appraisal system,realized the scientific competency matches the level,salary changes along with the level model.Conclusions It provides references and thinking to the in-depth reform of public hospitals in our country,and the development of high quality nursing service,as well as the implementation of nursing ladder management and compensation reform.
4.Identification and Treatment of Depression in Patients with Functional Dyspepsia

Objective: To investigate the rate of depression in patients with functional dyspepsia (FD) and corresponding treatments Method:285 out-patients diagnosed as FD were assessed by two physician trained for depression assessment Depression was diagnosed by criteria of CCMD-2-R Patients with depression were divided into three groups: group A (n=28) received venlafaxine, group B (n=31) received doxepine, group C (n=26) received no antidepressant as control SDS (self-rate depression scale) and FD score were used to assess the effect of antidepressants Result:The rate of depression in our sample was 32% (92/285) Among 92 with depression, 85 consent to participate the study FD scores of both group A and group B decreased after 2 weeks treatment, and were significantly lower than that of group C at the end of 8 weeks The score of SDS in group decreased from the end of second week after treatment, and both group A and group B had significantly lower scores in SDS than group C at the end of 8 weeks Group A had fewer side effects than group B Conclusion:Depression is common in patients with FD Antidepressant can improve FD symptoms as well as depressive symptoms Venlafaxine has fewer side effects than doxepine
5.Dynamic monitoring of virus antibody during the establishment of SPF cynomolgus monkey population

Objective To investigate the positive and suspicious rates of BV, SRV, SIV and STLV-1 antibody of the cynomolgus monkeys introduced from Laos, dynamic monitoring of virus antibody during the establishment of SPF cyno-molgus monkey population, and then comparing the positive rate of virus antibody in the baby monkeys between the common with SPF populations.Methods Continuous monitoring four kinds of virus in the monkeys was carried out using special reagent kits and analyzed the data.Results In the introduced cynomolgus monkeys (n=1998), the positive rate of BV antibody was 52.35%, suspicious rate of BV antibody was 8.31%, and negative rate of BV antibody was only 39.34%, the positive rates of SRV and STLV-1 antibody were 7.45%and 8.56%, respectively, and positive or suspicious SIV anti-body in the cynomolgus monkeys was not detected.After screening and establishment of the SPF population, the positive rates of BV, SRV and STLV-1 antibody were 5.24%, 1.01%and 0.4%, respectively, in the year 2010.Then after five years of screening and elimination, by the end of 2014, the positive rates of BV, SRV and STLV-1 antibodies were 0.82%, 0.27%and 0.27%, respectively, and positive or suspicious SIV antibody in the cynomolgus monkeys was not de-tected.The BV antibody positive rate of the baby monkeys was 9.71%and the suspicious rate was 1.85%in the common population, but only 0.22%of the baby monkeys in the SPF population.Conclusions Continuous monitoring virus anti-bodies and eliminating positive and suspicious animals have important significance in the establishment of a SPF cynomolgus monkey population.

7.Influence of Prenatal Diagnosis and Immediate Surgery Intervention on the Prognosis of Congenital Hydronephrosis in Children

Objective To explore the influence of prenatal diagnosis,active follow up and immediate surgery intervention on the prognosis of congenital hydronephrosis in children.Methods The clinical data in 85 cases were analyzed retrospectively.They were divided into prenatal diagnosed group and postnatal diagnosed group.Prognostic parameters such as preoperative statuse,pathology during surgery,operation process,status before discharging and complications were collected and statistically analyzed.Results The numbers of grade 4 hydronephrosis,secondary renal calculus,opposite kidney hydronephrosis,preoperative infectioin,perinephric adhesion and postoperative complication in prenatal diagnosed group were significantly lower than those in postnatal diagnosed group(Pa0.05).Conclusions 1.Ultrasonagraphy examination and follow-up are indispensable both before and after delivery which will help doctors to diagnose the disease at early stage.2.Surgical operation on congenital hydronephrosis at early stage is reliable and will achieve the same effect with the children undergo surgical operation at older age.3.Immediate surgical operation is of great clinical significance on preventing serious complication and irreversible kidney damage.
9.Clinical Features of Cord and Artery Blood Gas Analysis in Neonate with Acid-Bas eImbalance

Objective To discuss clinical features and harmfulness of acid-base imbalance in newborn by investigating the changes in blood gas of cord and artery blood,and to establish more appropriate prevention measures. Methods One hundred and ninety-two cord blood and 113 peripheral artery blood of neonates were measured for blood gas analysis by automatic blood gas analysis instrument from America;meanwhile,the concentrations of K~+,Na~+ and Cl~- were detected by biochemical analyzer.Results Among 192 neonates,179 were normal labor and 13 neonates underwent acidosis of different degrees.The incidence was 79.9%.Among the 13 ca-(ses),2 cases had severe asphyxia and both of them got serious mixed acidosis.For all the neonates, 156 cases got acidosis, 84 neonates underwent respiratory acidosis (43.8%), 47 cases with metabolic acidosis (24.5%) and 25 cases with mixed acidosis (13.0%).The left 113 neonates were abnormal labor.Among them,99 cases had acid-base disturbance of different degrees (87.6%). Among the 99 acid-base disturbance cases, 50 cases were single metabolic acidosis (44.2%) and presented the most common changes; the second was single respiratory acidosis, totally 20 cases (17.7%).The third was metabolic acidosis combined with respiratory alkalosis, totally 16 neonates (14.2%).Eleven cases had mixed acidosis (9.7%) and 2 cases were respiratory alkalosis combined with metabolic alkalosis (1.8%).Conclusions While the normal labor neonates still got acidosis of different degrees,the acid-base imbalance of abnormal labor is much more prominent,and the critical neonates often got mixed acid-base imbalance.Serious acid-base imbalance can cause fatal pathological changes in body.The dangers of acidosis and alkalosis can not ignore either. The condition of acid-base imbalance can be judged more accurately by the combination of blood gas analysis and anion gap.The perinatal health cares are important measures to prevent acid-base imbalance.
10. Protective effect of N-acetylcysteine on lung dendritic cells against damage in multiple organ dysfunction syndrome model

Objective To explore the protective effect of N-acetylcysteine (NAC) on lung dendritic cells (DCs) from damage in multiple organ dysfunction syndrome (MODS) mouse model. Methods Animal model of MODS was established by injecting zymosan into the peritoneal cavity of BALB/c mice, and the mice were thereafter randomly divided into zymosan group, zymosan + NAC group and control group, 20 for each. MPO activity in lung tissue was measured by biochemical analysis 48 hours after modeling. Pathological changes of the lung were observed under light microscope. Lung DCs were separated by density gradient centrifugation and CD11c+ immunomagnetic beads. DCs' phenotypes of MHC-II/IAd and CD86 were analyzed by flow cytometry. Apoptosis of DCs was detected by flow cytometry with double labeling of Annexin V and 7-AAD. Results Compared with control group, the MPO activity in lung tissue remarkably increased in zymosan group (P<0.05). Infiltration by a large number of neutrophilic granulocytes was found in lungs, suggesting serious injuries in lung. The expressions of MHC-II/I-Ad and CD86 on DC surface and the percentage of DC apoptosis increased dramatically (P<0.05). Compared with zymosan group, the MPO activity remarkably declined and the lung injury was mitigatory (P<0.05); the number of infiltrating neutrophilic granulocytes decreased greatly, and the expressions of MHC-II/I-Ad and CD86 on the DC surface and the percentage of DC apoptosis decreased dramatically (P<0.05) in zymosan + NAC group. Conclusion NAC in vivo could inhibit DC activation-induced apoptosis and relieve lung injury, thus being of protective effect on lung DCs against damage in MODS model.
