AIMTo investigate the effect of morp hine on purine nucleotides catabolism and its possible mechanisms. 2.Clinical analysis and CT features of microscopic polyangiitis with lung involvement

Objective To investigate the pathologic foundation,clinical characteristics and CT features of microscopic polyangiitis(MPA)with lung involvement.Methods Clinical data of 6 patients of MPA with lung involvement in 2003-2006 were retrospectively analyzed.Results The types of lung imaging manifestations as the followings:(1)Mutiple flaky shadow in both lung in 3 cases.(2)Ground-glass shadow in 1 case.(3)Disseminated interstitial lung involvement in 2 cases.The main respiratory symptom was haemoptysis except cough and expectoration.Renal damaging often occurred besides the lung.Six cases with positive antineutrophil cytoplasmic antibody(ANCA)and 5 cases with positive P-ANCA/MPO-ANCA were found in laboratory examination.Conclusions The CT manifestations of MPA with lung involvement have no characteristic features.Clinical characteristics and laboratory examination can help to understand and diagnose MPA with lung involvement early.
3.Clinical Efficacy of Oxycontin Combined with Gabapentin in the Treatment of Neuropathic Cancer Pain and the Effect on the Immune Function of Patients

Objective:To explore the effect of oxycontin combined with gabapentin on the clinical cure and immunity for patients with neuropathic cancer pain.Methods:80 patients with neuropathic cancer pain were enrolled in our hospital from June to 2016 July,of which patients divided into two groups randomly,Group A(n=40) accepted oxycontin treatment,and Group B (n=40) adopted gabapentin based on the patients in Group A.The VAS score and curative effect of the patients were compared between two groups;The quality of life of all patients were evaluated post-treatment,and the change of immunity indexes were compared and analyzed.Results:The VAS score of all patients was decreased significantly compared with pre-treatment (P<0.05),and the score of Group B was lower than those patients in Group A (P<0.05);The total remission rate of Group B was significantly higher than those of Group A (P<0.05);after treatment,the score of appetite,emotion,sleep,daily activities,social communications of all patients decreased significantly compared with pre-treatment (P<0.05),and the change of Group B was decrease significantly higher than those patients in Group A (P<0.05);the immune index of two groups was significantly increased (P<0.05),and the level of the indexes including IgG,IgA,IgM,CD4+,CD4+/CD8+ and circulating immune complex (CIC) increased compared with pre-treatment remarkably (P<0.05),and which change in Group B was significantly higher than Group A (P<0.05).Conclusions:Oxycontin combined with gabapentin for patients with neuropathic cancer pain deserved popularization in clinical,and which not only possessed well clinical effect,but also increased the quality of life.
5.Application of Magnetic Resonance Spectroscopy in Diagnosis of Intracranial Lesions

Objective To evaluate the clinical value of MR spectroscopy in intracranial neoplasm. Methods 52 cases with brain neoplasm or recurrent neoplasm or non-tumor lesion confirmed by pathology were undergone proton MRS before stereotactic biopsy, or operation. Results Of 52 cases, 38 were neoplasm and 14 were nonneoplastic lesion, 34 of 38 brain tumor, 11 of 14 nonneoplastic lesion were diagnosed correctly. The characteristics on MRS of glioma were remarkable Cho increase and NAA decrease, Lac in the tumors and the central necrotic area was elevated. Metastasis: Cho increased,NAA disappeared or obviously decreased and Lac increased, but the peritumoral region was normal. Lymphoma: Cho increased, NAA decreased moderately, Lip presented, but the peritumoral region was normal. Demyelination lesion: slight or median decreased NAA and normal Cr, Lac increased slightly. All metabolism decreased or disappeared in radiation necrosis, Cho was relative high compared with NAA. Brain abscess: Lac increased in the centre of abscess and perilesion was normal.Conclusion MRS has significant value in detecting and differentiating brain lesions.

8.Colonoscopy with narrow band imaging in diagnosis of colorectal polypoid lesions

Objective To investigate the value of colonoscopy with narrow band imaging (NBI) in predicting the histology of colorectal polypoid lesions. Methods A total of 173 colorectal polypoid lesions from 125 patients were included in the study. The lesions were diagnosed as neoplastic (adenoma or cancer)or non-neoplastic based on pit patterns and/or capillary patterns (CP), which was compared with the results of pathology, and the sensitivity, specificity and accuracy of each method were evaluated. Results In differentiation between neoplastic and non-neoplastic lesions, the sensitivity, specificity and accuracy of CP (94. 83% , 91. 23% and 93.64% , respectively), and those of CP combined with pit patterns (95. 69% ,96.49% and 95. 59% , respectively) were significantly higher than those of conventional colonoscopy (80. 17% , 84.21% and 81.50% , respectively, P<0.05). In differentiation between adenoma and cancer,the sensitivity, specificity and accuracy of CP were 86.90% , 100.00% and 87.93% , respectively.Conclusion NBI is superior to conventional colonoscopy in differentiation between neoplastic and non-neoplastic lesions, as well as in differentiation between adenoma and carcinoma.

Objective To observe the expression of Na+/I- symporter(NIS) in cultured lactating mammary cells with different levels of iodine and the effect of tumor necrosis factor-α(TNF-α). Methods Original generation of mouse lactating mammary cells cultured in vitro were divided into low iodine group Ⅰ (LI-Ⅰ), low iodine group Ⅱ (LI-Ⅱ), adequate iodine group(AI), high iodine group Ⅰ(HI-Ⅰ), and high iodine group Ⅱ(HI-Ⅱ). Cells were cultured in DEME/F12 culture medium for 24 h with different concentrations of iodine (0,5,50,3000 and 10 000 μg/L, respectively), and TNF-α( 10-2 mg/L) was added to some of cultured cells for 24 h. The expression of NIS mRNA of lactating mammary cells was determined by real-time quantitative PCR and the expression of NIS protein was detected by In-Cell Western. Results In iodine alone group, the expression of NIS mRNA in LI-Ⅰ group [ (64.66 ± 14.99) x 10-4] was higher than that of AI group[ (22.76 ± 7.36) × 10-4, P < 0.01 ]; HI-I group[ (10.18 ±3.53) × 10-4] and HI-Ⅱ group[ (8.59 ± 2.89) × 10-4] were lower than that of AI group(all P < .0.05); With increased iodine concentration, the expression of NIS mRNA decreased. The expression of NIS mRNA in LI-Ⅰ group [(2.72 ± 0.45) × 10-4], LI-Ⅱ group[ (2.69 ± 0.68) × 10-4] and AI group[(1.80 ± 0.67) × 10-4] with iodine plus TNF-o were all lower than that of LI-Ⅰ group, LI-Ⅱ group[ (29.82 ± 4.47 ) × 10-4], and AI group without TNF-α (all P < 0.01). In iodine plus TNF-α, the expression of NIS mRNA in HI-Ⅰ group[(6.58 ± 2.87) × 10-4] and HI-Ⅱ[(7.04 ± 1.36) × 10-4] group were all higher than that of AI group(all P < 0.05); With increased iodine deficiency or iodine excess, the expression of NIS mRNA increased. With increased iodine concentration, the expression of NIS protein decreased in iodine alone group. The expression of NIS protein in iodine plus TNF-α was all lower than that in iodine alone group. In iodine plus TNF-α, the expression of NIS protein increased in both iodine deficiency and iodine excess conditions. Conclusions Iodine may decrease the expression of NIS mRNA and protein of lactating mammary cells. The expression of NIS mRNA and protein of lactating mammary cells was inhibited by TNF-α under different levels of iodine.
