2.The treatment options for retinoblastoma

Treatment options of retinoblastoma should be individualized. Patients with early intraocular stage may be managed with focal treatments, including cryotherapy, thermotherapy, laser photocoagulation, brachytherapy and so on. Patients with middle in- traocular stage may be treated with chemoreduction plus focal therapy. In patients with advanced stage, enucleation and chemotherapy are recommended, with the addition of orbital radiation for patients with extraocular retinoblastoma. Patients with systemic metastases require intensive chemotherapy and consolidation with autologous hematopoietic stem cell rescue.
3.Phacofragmentation combined with vitrectomy in the treatment of traumatic dislocation lens

AlM: To explore the clinical application effect of phacofragmentation combined with vitrectomy in the treatment of traumatic lens dislocation.
METHODS:Totally 16 cases (16 eyes) of traumatic lens dislocation treated with phacofragmentation combined with vitrectomy were retrospectively analyzed, including 6 cases of high intraocular pressure ( lOP) and 10 cases of vitreous hemorrhage, and 1 case of retinal detachment. All patients were given conventional flat line standard three channel vitreous operation incision to remove the anterior, middle part and peripheral vitreous around lens dislocation. The crystalline lens were drawn to the center cavity of vitreous body and treated by ultrasonic disintegrator. ln the operation, the retina was examined and 8 of them had no retinal damage and in the first stage underwent fixation of posterior chamber intraocular lenses.
RESULTS: All crystalline lens dislocated were completely grinded and suctioned. There was no retinal detachment occurred in 3mo followed up. 16 eyes had normal lOP (12-20mmHg) at 1wk after operation. The average visual acuity was improved and with 8 cases got improved of 0. 2 or more 1wk after operation.
CONCLUSlON: Our research shows that phacofragmentation combined with vitrectomy is a safe and effective method for the treatment of traumatic lens dislocation. The patients with nondestructive retina in the operation are feasible to do first stage operation of intraocular lens suture fixation, which contributes to the best visual acuity.
4.Upregulated autophagy in myocardium of infants with cyanotic congenital heart defects

Objective In the heart,autophagy is important for the turnover of organelles at low basal levels under normal conditions,but is upregulated in response to stresses such as ischemia/reperfusion and in cardiovascular diseases such as heart failure. It can prevent the heart from injury under stress conditions. This study was designed to test the hypothesis that autophagy would be upregulated in the myocardium of children with cyanotic congenital cardiac defects. Methods Eighteen children with cyanotic (n=10) or acyanotic cardiac defects (n=8) who were admitted in our hospital from October 2008 to April 2009 and received surgical treatment were investigated. Samples from the right ventricular myocardium taken immediately after aortic clamping were morphologically studied with transmission electron microscopy for the ultrastructure. Microtubule-associated protein 1 light chain 3 (LC-3) was detected by Western blot analysis in the obtained samples. Results Children with cyanotic cardiac defects had higher oxyhemoglobin saturation before operation than those with acyanotic cardiac defects. Electron microscopy showed that the former group had more mitochondria,disordered arrangement,swollen endoplasmic reticulum and typical autophagosomes in the myocardial cytosol. 8.Clinical study of Basen's effect on insulin resistance in type 2 diabetic patients

Objectives:To study the effect of Basen on the sensitivity of insulin in diabetes mellitus.Methods:Blood glucose,lipoprotein,insulin,C-peptide were assayed after and before the administration of Basen.Results:Levels of insulin,blood glucose,and C-peptide were lowered after four weeks administration of Basen,which changed contrarily to ISI.Conclusions:Basen has effects not only on lowering blood glucose but also on improving the insulin resistance.
9.The capacity of MTD method for distinguishing Mycobacterium tuberculosis and nontuberculosis mycobacteria

Objective To evaluate the capacity of MTD method to distinguish between Myeobacterium tuberculosis and nontuberculosis mycobacteria.Methods Ten standard strains(including 1 H_(37)Rv strain and 9 nontuberculosis mycobacteria strains),94 clinical strains(including 48 Mycobacterium tuberculosis and 46 nontuberculosis mycobaeteria strains)and 40 sputum specimens were tested by MTD method(AMPLIFIED-MTI))and traditional methods.The results of these methods were compared.Results For all Myeobacterium tuberculosis and nontuberculosis mycobacteria strains,the agreement of MTD method and traditional method was 100%.And the positive detectable rate for sputum samples was 65% that was obviously higher than that for the direct smear(5/40),concentration smear(10/ 40)or culture(5/40).Conclusion MTD is a rapid test for identification of Mycobacterium tuberculosis and nontuberculosis mycobaeteria with high sensitivity and specificity.
