1.Nosocomial Infection Control in Local General Hospitals:Current Situations

OBJECTIVE To ascertain the current situations of nosocomial infection control in local general hospitals,and to provide reliable data for future work.METHODS A random sampling questionnaire(method) was adopted to investigate the current situations of nosocomial infection control in 15 local hospitals in eight counties and cities.RESULTS Altogether 15 hospitals at the county and urban levels have been surveyed,(among) which 14 hospitals have fewer than 300 sickbeds and only 1 hospital has over 500 sickbeds.Seven hospitals did not have full-time staff of infection control till 2003.In the 15 hospitals,each full-time staff was(responsible) for an average of 143.9(sickbeds);in terms of the constitution of the full-time staff,nurses accounted for 73.7%,doctors,21.9%,and technicians,5.2%;of the full-time staff,63.2% held an intermediate(professional) position,and 36.8% held a junior professional position;with regard to the chances of further professional training in other places,19 full-time staff had 56 chances.The applications of sterilized agents and protective equipments were increasing each year.CONCLUSIONS The(infection) control in local general hospitals is gradually on the right track,but in some aspects,improvements are still needed.The prerequisites for improving infection control work in local general hospitals are that leaders should pay more attention to nosocomial infection control,and that more human and(material) resources should be pooled in it.
2.Natural infection of 4-Lyme disease Borrelia burgdorferi in Diebu and Huajian areas of Gansu province

Objective In order to offer scientific evidence for prevention and therapy of Lyme disease, we had investigated the natural infection of lyme disease Borrelia burgdorferi(Bb) of 4 ticks in Diebu and Huajian areas of Gansu province. MethodsEpidemiology detection of natural infection of lyme disease Bb was accomplished for four dominant tick ( Dermacetor silvarum , Dermacentor nuttalli , Haemaphysalis Japonica and Ixodes Crenulatus ) at Diebu, Diebu forest zone(Qin mountain), north of Min Mountains of Huajian region and Huajian forest zone, north of Qilian Mountains of Sunan region, Gansu, from March to June, 2010. The methods of dispersion of clamp every 10 m which was put in the morning and retrieved at night were used to capture rodent animal and gnawer retrorse hair inspection insect methods were used to collect parasitic tick. Flagging methods were used to collect free tick.Four kinds of live adult ticks were dissected after cleaning and disinfection. Intestinal contents were smeared, lyme disease Bb was observed under dark-field microscope. Etiological agent was cultivated and separated. Separated spirochete was confirmed by spirochete monoclonal and polyclonal antibody. Results The ticks collected were classified as 2 families, 8 genera, 36 species, i.e. Ixodidae 6 genera, 33 species and Argasidae 2 genera, 3 species. The method of dark-field microscope was used to detect the lyme disease spirochete in 201 ticks intestines after dissection of Dermacentor silvarum, Dermacentor nuttalli, Haemaphysalis Japonica and Ixodes Crenulatus, the lyme disease Bb-positive were 25 with positive rate of 12.44%(25/201). The lyme disease Bb was cultivated and separated from 12 ticks with the positive rate of 18.46%(12/65) after inoculation and cultivation of 3 species, 65 ticks of Dermacentor silvarum, Dermacentor nuttalli and Haemaphysalis Japonica. ConclusionsDermacentor silvarum,Dermacentor nuttalli and Haemaphysalis Japonica have different degrees of natural infection of lyme disease Bb.
3.Construction and prokaryotic expression of SLC

Objective To construct an expression vector pET32a(+)/SLC and express the fusion protein TrxA SLC. Methods Total RNA from human inflammatory tonsil was extracted and its cDNA was generated with reverse transcription. Mature secondary lymphoid tissue chemokine (SLC) gene was amplified by RT PCR and cloned into plasmid pET32a(+) with Nco Ⅰ and Eco RⅠ sites added to the 5′ and 3′ ends respectively. E. coli DH5? was transformed with the recombinant plasmid, and the grown clones were selected. The inserted DNA was verified by enzyme digestion and DNA sequencing. The 3 amino acids between enterokinase site and target gene were deleted with mutation and the new vector was verified by sequencing. Expression of SLC was analysed by SDS PAGE. The fusion protein was purified by metal affinity chromatography and weak cation exchange chromatography, which was analysed by SDS PAGE and Western blotting. Results Trx SLC fusion protein expression vector was successfully construced, and the fusion protein was expressed with solubility. The purified fusion protein displayed the ability of binding the goat anti human SLC polyclonal antibody. Conclusion The SLC fusion protein can be expressed with stability and solubility and primary purification is performed with metal affinity chromatography and weak cation exchange chromatography.
4.Establishment of diabetic cystopathy guinea pig model and its urodynamic evaluation

BACKGROUND:Diabetic cystopathy is one of the most common chronic diabetic complications. The establishment of animal models of diabetic cystopathy wil provide experimental animal platform for relevant research.
OBJECTIVE:To establish a guinea pig model of diabetic cystopathy and to evaluate its urodynamic characteristics.
METHODS:Fifty short-hair Britain female guinea pigs were randomly divided into two groups, 42 as the experiment group and the other 8 as the control group. The experiment group was intraperitoneal y injected with streptozotocin to induce diabetes. The control group received injection of blank citric acid buffered solution. Diabetic guinea pigs were detected by urinary dynamics test at 9 and 12 weeks. Diabetic guinea pigs were further assigned into diabetic cystopathy subgroup and compensated subgroup. The urodynamic parameters of three groups were compared.
RESULTS AND CONCLUSION:Twenty of 42 guinea pigs were successful y induced diabetes by the injection of streptozotocin. At 9 weeks after the injection, bladder function compensation was present in six diabetic guinea pigs while bladder function was decompensated in another three diabetic guinea pigs. At 12 weeks, bladder function compensation was present in one diabetic guinea pig, while another eight guinea pigs were confirmed with diabetic cystopathy (88.89%). In the diabetic cystopathy subgroup, the residual urine volume was increased (0.72±0.08) mL, maximal detrusor pressure was decreased (0.63±0.05) kPa, maximum bladder capacity was increased (2.01±0.05) mL, and bladder compliance was increased (0.34±0.04) mL/kPa. There were significant differences compared with the compensated subgroup and the control group (P<0.001). Diabetic cystopathy occurs at 12 weeks after diabetic models are successful y established in guinea pigs, and urodynamic changes are mainly the increase of residual urine volume.
5.Rational Treatment Course of Omeprazole for Critical Patients

Objective: To study the rational treatment course of proton pump inhibitor omeprazole for critical patients in order to improve the prognosis and reduce the hospital stay and treatment expense. Methods:Totally 180 critical patients with APACHEⅡ>10 and mechanical ventilation >48 hours in ICU were selected and randomly divided into 3 groups treated by omeprazole for 1 day, 3 days and 5 days , respectively. The incidence rate of stress ulcer, gastrointestinal hemorrhage and ventilator-associated pneumonia ( VAP) in the three groups was studied, and the ICU stay and expense were also investigated. Results:The incidence rate of stress ul-cer and gastrointestinal hemorrhage in the group with treatment course of 3 days and 5 days were significantly lower than those in the group with 1 day treatment(P<0. 05). The incidence rate of VAP with 3-days treatment was the lowest, compared with 1 day and 5 days treatment, the different was satatistically significant(P<0. 05). The ICU hospitalization time and treatment expense were also significantly different among the three groups, and that in 3-days, group was the lowest(P<0. 05). Conclusion:The 3-day treatment is the most reasonable, which can not only prevent stress ulcer and gastrointestinal bleeding in critical patients, but also reduce the in-cidence of VAP with lowered hospital stay and expense.
6.Characteristics and choice of surgical treatments for severe liver trauma

Objective To analyze characteristics of severe liver trauma and efficacy of different surgical procedures. Methods Clinical data of 109 patients with severe liver trauma treated in the recent 10 years were retrospectively analyzed. Debriding suture was performed in 32 patients, gauze tamponade in 5, debridement hepatectomy in 59 and anatomical hepatectomy in 13 patients. Results In all the 109 patients, 92 were cured and 17 died. The dead patients included 3 with grade Ⅲ trauma,9 with grade Ⅳ trauma, and 5 with grade V trauma. Among the dead patients, there were 3 patients with simple liver injury (17.6%) and 14 with associated injury (82.4%). Conclusion Right hepatic serious damage is the main type of severe liver trauma and is always complicated with associated injury and needs emergency treatment. Application of the most appropriate surgical approach according to the traumatic condition is important to promote the successful rate of treatment.
7.Effects of bone marrow mesenchymal stem cells supernatant on hippocampal GFAP expression and learning and memory in rats with neonatal hypoxic ischemic brain damage

Objective To analyze the expression of glial fibrillary acidic protein(GFAP) in hippocampal of rats with hypoxic-ischemic brain damage(HIBD) after injection of bone marrow stromal cells(BMSCs) supernatant into the lateral ventricle of the neonate rats,assess the ability of learning and memory,and explore the mechanism.Methods 7-day-old SD rats were randomly divided into sham operation group,model group,cells and supernatant group,with 20 rats in each group.The HIBD model was established via the ligation of left carotid arteries followed by 2-hour hypoxia.One week later,rats in control group were injected with 0.01 mol/L PBS 2 μl via the left ventricle.Meanwhile,the cell and the supernatant groups were injected with BMSCs and supernatant harvested from BMSC culture,respectively,via the same route.In the sham operation group,the left carotid arteries were separated but not ligated,and no hypoxia treatment was imposed on this group.They also received 0.01 mol/L PBS injection 2 μl per weak post surgery.8 weeks later,Morris water maze test was performed to assess the learning and memory,and the expression of GFAP in the dentate gyrus of rats was measured by immunohistochemistry.Results Morris water maze showed that the searching time of the probe trail(T1:(15.40±2.80) s) and reversal probe trail (T2:(16.45± 1.16) s) of the model group was shorter than that of the sham operation group (T1:(19.96±2.57) s,T2:(2:25.32±2.54)s,P<0.05),while the searching time of the cell group(T1:(17.54± 1.80)s,T2:(18.99± 1.47) s) and supernatant group (T1:(17.40±2.37) s,T2:(17.96± 1.09) s) was prolonged compared with that of the model group (P<0.05).No significant difference between the cell group and the supernatant group (P>0.05).The integral optical density (IOD) value of GFAP positive cells was higher in the model group than that in the sham operation group(15.26±1.49 vs 12.82±2.56,P<0.05),while the IOD of cell group(18.13±1.97) and that of the supernatant group(17.38± 1.64) were higher than that of the model group (P<0.05).However,there was no significant difference between the cell group and the supernatant group (9>0.05).Conclusion BMSC supernatant injected through lateral ventricle can improve the HIBD rats' abilities of learning and memory.The mechanism might be that MSCs secrete some cytokines to promote central nervous system repair.
8.Ultrasonography of Normal Fetal Fourth Ventricle in the Second Trimester of Pregnancy

Purpose To characterize normal growth of the fetal fourth ventricle in the second trimester of pregnancy by standardized ultrasonography.Materials and Methods 516 cases of pregnant women with single fetus aged 18-27 weeks and 6 days were examined by conventional ultrasound.At the standard section of the fourth ventricle,the transverse dimension,anteroposterior dimension,circumference and area of the fourth ventricle were measured.Results The display rate of the fourth ventricles of fetuses aged 18-27 weeks and 6 days was up to 100%.There were no significant differences in the values of the fourth ventricles between fetuses with different gender (P>0.05).The values of the fourth ventricle,including transverse dimension,anteroposterior dimension,circumference and area,increased linearly with the increase of gestational age,and there were positive correlations (r=0.374,0.378,0.387,0.379,P<0.05).Conclusion The normal size of the fourth ventricle can be obtained by standardized measurement,which can be used as a reference for the clinical evaluation of the size of the fourth ventricle.
9.Prenatal ultrasonographic diagnosis of fetal absent pulmonary valve syndrome

ObjectiveTo summarize the value of prenatal ultrasonography in the diagnosis of fetal absent pulmonary valve syndrome (APVS).MethodsBetween January 2010 and January 2014, ifve cases with fetal APVS diagnosed by prenatal ultrasound and conifrmed by autopsy in West China Second Hospital of Sichuan University were reviewed. Four conventional views of fetal echocardiography were used to diagnose APVS, including two-dimentional images, color Doppler lfow imaging (CDFI) and pulsed Doppler lfow spectrum.ResultsTypical signs of fetal APVS in 3 of 5 cases were found: no pulmonary valve was found in the main pulmonary artery, and pulmonary annulus was relatively smaller compared with a tumor-like expansion in pulmonary trunk; “to and fro” sign in the pulmonary trunk were observed steadily by CDFI and “to and fro” spectrum were detected by pulsed Doppler. Another 2 of 5 cases of fetal APVS have no pulmonary expansion, but “to and fro” sign could be detected by color and spectral Doppler, including 1 case with tricuspid atresia and 1 case with Ebstein’s anomaly.ConclusionsPrenatal ultrasonography has important diagnostic value for Fetal APVS. Fetal APVS on prenatal ultrasound has many characteristics and is easy to diagnose. Color and spectral doppler can detect and diagnose fetal APVS effectively.
10.A Retrospective Study:High Intensity Focused Ultrasound (HIFU) Associated A Microbubble-enhancing Contrast (“SonoVue”) Therapy for Uterine Fibroids with Pelvic Operation History

Objective To evaluate the feasibility of high intensity focused ultrasound (HIFU) associated microbubble-enhancing contrast (“SonoVue”) therapeutic ablation of uterine fibroids in patients with pelvic operation history. Methods A total of 100 patients with uterine fibroid from October 2010 to December 2012 in Suining Central Hospital of Sichuan were treated with HIFU associated SonoVue. Among them, 50 patients had pelvic operation history. The no-perfused volume, median fractional ablation, ablation power rate, ablation dose, treatment time and adverse reaction were recorded. Results There was no statistical significance between patients with operation history and without in no-perfused volume and ablation does. The ablation power was lower in patients with operation than without. The treatment time and room time were longer in patients with operation than without. The adverse reactions were higher in patients with operation than without, especially skin pain and lower abdominal pain ( <0.05) . According to short- and long-term follow-up results, no major complications were observed in any patients. Conclusion Pelvic operation history increases the difficulty in treatment of HIFU ablation uterine fibroids. HIFU associated contrast agent SonoVue ablation can be considered a safe and feasible approach to the management of uterine fibroids in patients with pelvic operation history after grasping the indications and contraindications.
