There are some limits and irrationality in practical application of the clause 3.1, 3.2, 4.4.2, 4.5.2, 4.9.1 of YY 0672.1-2008 as the industry standard for laparoscopic trocar. This paper puts forward some opinion and advice.

Objective To evaluate the curative effect of normobaric oxygen therapy on fetal atrial premature contractions(PACs) with Doppler ultrasound. Methods Maternal normobaric oxygen therapy was performed in one hundred and seventy three fetus with PACs but without congenital heart disease diagnosed by Doppler echocardiography. Another 63 cases with isolated PACs receiving no therapy served as controls. Objective To explore the pathogenesis of acute lung injury in rats suffering hemorrhagic shock at plateau.Methods Seventy-two male Wistar rats, weighing 280-320 g, were randomized into 6 groups (n=12): sham group (group Sham), hemorrhagic shock for 15 min (group HS15), hemorrhagic shock for 30 min group (group HS30), hemorrhagic shock for 45 min group (group HS45), hemorrhagic shock for 60 min group (group HS60) and 90 min group (group HS90).Hemorrhagic shock model of Wistar rats was reproduced at plateau.The rats were only anesthetized, no shock and were sacrificed after 90 min in group Sham.The other groups were different in bleeding time and then were respectively sacrificed at 15, 30, 45, 60 and 90 min after shock.The pathological changes in the lungs were observed with light microscope.Wet to dry weight ratio (W/D), lung permeability index (LPI), myeloperoxidase (MPO) activity, malondialdehyde (MDA) and superoxide dismutase (SOD) in lung were measured.Enzyme-linked immunosorbent assay was used to detect the TNF-α and IL-10 in lung tissue, the expression and distribution of claudin-3 and claudin-4 in lung tissue was verified by immunohistochemistry method.Results Compared with group Sham, shock causes acute lung injury at different degree, and was positively correlated with the duration of shock, during the period of 15 to 30 min, it merely rendered a slight change in lung W/D, LPI, MPO, MDA, TNF-α, T-SOD and IL-10.Subsequently, along with time prolonged, lung W/D, neutrophils in BALF, LPI, MPO, MDA, TNF-α were significantly elevated, while T-SOD and IL-10 were notably reduced (P<0.05).Immunohistochemical results showed that claudin-3 and claudin-4 expression in lung epithelial cells and endothelial cells expressed at low levels and dislocated (P<0.05).Conclusion After a short time compensatory lesions, the change of rats' hemodynamic stability suffering severe hemorrhagic shock showed a spiral downward.Along with the extension of the shock, hemorrhagic shock at plateau results into the disturbance of inflammatory response and oxidative stress, the loss of claudin-3 and claudin-4 in lung epithelial cells, which triggers the acute lung injury.

Objective To describe the special ramp lesion, which involves the capsulomeniscal region of posterior horn of medial meniscus and most commonly occurs in association with anterior cruciate ligament(ACL) injury, pre- and intra-operative diagnosis methods, arthroscopy repair technique and follow-up results of arthroscopy second-look & MR review. Methods From April 2002 to April 2005, 89 cases of ACL injury with ramp lesions are enrolled in this series, among them 85 need operation repairing. Arthroscopic ACL reconstruction with hamstring or B-PT-B autograft or allograft and all-inside medial meniscus repair through 2 posteromedial portals and transcondylar portal have been performed. Generally, the ramp lesion accounts for 26.7% of ACL injury, 30.5% for chronic and 19.5% for acute injury respectively. Results Of the repaired ramp lesion cases, 75 cases(88.2%) are available for follow-up, with the duration from 5 to 41 months (mean, 20.2 months). 25 cases underwent arthroscopic second-look and 21 cases received MR evaluation. The study include subjective symptom, clinical examination, and arthroscopic second-look as well as MR review. All of the repaired cases present normal subjective and objective evaluations, and all the 25 second-look cases show completely healing. Of the 21 MR follow-up cases, 18 completely healed, 3 partial healed. Conclusion About 1/3 ACL injuries are associated with medial meniscus ramp lesion, and the morbidity is higher in old injuries than acute injuries. For this combined lesion, one stage ACL reconstruction and meniscus repair was recommended. All-inside meniscus repair is a satisfactory technique for ramp lesion and can get high healing rate.

Objective To evaluate the efficacy and safety of endoscopic submucosal dissection (ESD) with scissors knife in difficult cases.Methods A total of 36 sessions of ESD in 34 patients were performed from May 2010 to May 2012 with application of new scissors knife.The complications and followup outcomes were recorded.Results All lesions were removed successfully with an en bloc resection rate at 91.7％.Delayed bleeding occurred in 2 patients (5.6％),and both of them were cured sucessfully with endoscopy.No perforation happened and the average hospitalization time was 5 days.Follow-up endoscopy performed 6 or 12 months after ESD in 6 patients revealed no recurrence or residual lesions.Other patients are under follow-up now.Conclusion The scissors knife is easy to manipulate,which can ensure the safety and efficiency of ESD.
