Objective To observe the mechanism of the change in runx3 in gastric carcinoma its difference and with p53 mutation.Methods A total of 30 cases of gastric carcinoma were taken from clinical operation.PCR-SSCP method was used to detect mutations in exons 2,3,4 of runx3 gene in the gastric carcinoma.The methylation status of runx3 gene was examined by methylation-specific polymerase chain reaction(MS-PCR),PCR-SSCP method was used to detect mutations in exons 5,6,7,8 of p53 gene in the gastric carcinoma.Results Two cases with mutation were found,methylation of runx3 promoter region was confirmed in 87%(26/30) specimens of gastric carcinoma,53.3%(16/30)cases were confirmed with mutation in the p53 gene.The rate of methylation of runx3 was higher than the rate of mutation of p53(P

Objective To introduce a poisoning accident caused by clearing up the disuced bloat pool of green plums. Methods In a certain area in Fujian happened a poisoning accident in which many people poisoned. The site of monitoring was carried out immediately. Results The concentrations of hydrocyanic acid, acetonitrile, methane, and carbon dioxide were very high in the air of the pool. Conclusion The main cause of the poisoning was that the cyanogenic glycosides contained in the nucleolus of the green plums. After broken down, the cyanide was released, and then the cyanide synergistic effect with the asphyxiating gas such as methane and carbon dioxide developed also. Treated the sediment and waste water with bleaching powder and lime, cyanide had been eliminated efficiently.

Objective To evaluate the value of transthoracic echocardionraphy(TTE)and angiographic appearance for measurement of the diameter of patent ductus arteriosus(PDA)in transcatheter interventions.Methods In 52 patients,the position,diameter and length of ductus arteriosus between the descending aorta and the pulmonary artery were observed and the narrowest diameter of PDA was measured by TTE.In angiographic appearance,pig catheter was selected to perform left lateral aortography and measure the narrowest diameter of PDA.Comparative studies were performed on the numbers measured by 2 methods.Results Aortography showed that 45 patients had funnel shape PDA and 5 patients had tubular shape PDA,while 1 patient had funnel plus tubular shape PDA and 1 patient had residual shunt after operation.TTE showed that 46 patients had funnel shape PDA,4 patients had tubular shape PDA and 1 patient had window shape PDA,1 patient had residual shunt after operation.The mean diameter of PDA measured by angiographic appearance was(3.72?2.51)mm,which was statistically lower than those by TTE(4.47?2.31)mm(t=5.17 P

ObjectiveTo investigate the high risk factors,etiology,diagnosis,treatment and precaution of venous thromboembolism (VTE) in pregnancy and puerperium. MethodsData of 16 cases of VTE admitted in Department of Obstetrics and Gynecology,Peking Union Medical College Hospital from January 1992 to April 2011 were analyzed retrospectively.The differences of blood routine test and coagulation function between VTE patients (study group) and normal pregnant women (control group) were compared by paired t test in this case-control study.Results All patients were diagnosed by color Doppler ultrasound or CT pulmonary artery angiography.Five cases (31.2 ％) occurred before delivery and 11 (68.8 ％) occurred during postpartum.Two cases (12.5 ％)complicated by pulmonary thromboembolism,and one of them died.Six cases (37.5％) were older than or equal to 35 years old,and 12 cases (75.0％) had gestational complications.The hematocrit in study group were lower than that in control group (0.29±0.06 vs 0.39±0.02,t=4.56,P=0.01).There was no statistical difference between the coagulation function of the two groups.After anticoagulant therapy or surgery,15 cases recovered.One pulmonary thromboembolism patients complicated with rheumatic cardiac disease remained shock after anti-coagulants was administered,then died of hemorrhage of respiratory tract after inferior vena cava filter placement. Conclusions VTE is likcly to happen in puerperium than in pregnancy.There are no good screening indicators for thromboembolism during pregnancy and puerperium.Anticoagulation is recommended to be the first line therapy.For women with high risk of thromboembolism,it is suggested to prevent actively and treat early to decrease the complications and long-term sequelea.

ObjectiveTo investigate the clinical features and pregnant outcomes of the pregnant women with congenital adrenal hyperplasia (CAH) 21-hydroxylase deficiency (21-OHD).MethodsThe clinical features,therapies,pregnant outcomes of the pregnant women with 21-OHD were retrospectively reviewed in Peking Union Medical College Hospital,from January 2005 to April 2011.ResultsThere were 8 pregnant womenwith 21-OHD including 5siinplevirilizing patientsand 3nonclassical 21-OHD women.Eightpatientswereacceptedprogestationalandprenatalcontinuallowerglucocorticoid treatment.During the gestational period,the dosage of glucocorticoid was adjusted in one pregnancy.The serum level of 17-alpha hydroxyprogesterone (17-OHP) were elevated after pregnancy [ (70 ± 38 ) versus (24 ±23) nmol/L,P ＜ 0.05].The fertility and offspring rate of 8 patients was 8/12,the fertility and offspring rate of patients who started treatment at preadolescence was significantly increased (4/5 versus 4/7).Four patients were accepted genital reconstructive surgery ( clitorectomy,clitoroplasty,vulvoplasty) before pregnancy.The incidence of GDM was 1/8.All patients selected caesarean at from 37 +6 gestation weeks to 39+6 gestation weeks.The average newborn birth weight was (3210 ± 447 ) g,and height was (48 ±2) cm of 8 neonates,none of them was CAH.Conclusions Medical and surgical therapy provides satisfactory fertility and pregnancy outcomes for women with 21-OHD.It is safe to pregnant women with 21-OHD and their fetus in continual lower glucocorticoid treatment.The dosage of glucocorticoid should be carefully adjusted during the pregnancy individually according to serum level of 17-OHP.

BackgroundThe favorable effect of bone marrow mesenchymal stem cells (BMSCs) on the reconstruction of injured corneas have been reported,but the mechanism remains unclear.ObjectiveThis study was to evaluate the anti-inflammatory and antiangiogenic effect after transplantation of BMSCs in chemically burned corneas.Methods BMSCs were isolated and extracted from the bone marrow.The cells were cultured and passaged and then were seeded on the amniotic membrane.Corneal alkali injury models were created in 18 clean SD rats by sticking the filter paper containing 1 mol/L NaOH at the central cornea for 40 seconds.The rats were then randomized into 3 groups.Amniotic membrane with BMSCs or amniotic membrane without BMSCs were transplanted in 1 week after the establishment of models,and the rats without transplantation were used as the control group.The severity of corneal lesion was graded,and angiogenesis area was measured 2 weeks after the transplantation.The expression of interleukin-2 ( IL-2 ),interferon-γ(IFN-γ),IL-10 and transforming growth factor-β ( TGF-β ) were examined by ELISA,and the mRNA of the matrix metalloproteinase-2 ( MMP-2 ),vascular endothelial growth factor (VEGF),epidermal growth factor(EGF) and basic fibroblast growth factor(bFGF) were analyzed by real-time PCR.ResultsThe positive rates of the cells were 99.78％ and 99.79％ for CD90 and CD29,7.90％,1.16％ and 1.28％ for CD34,CD45 and CDllba.The cells grew well on the amniotic membrane.The corneal inflammatory score and neovascularization area were similar among the three groups ( F =0.021,P-- 0.979 ; F =0.076,P =0.927 ).However,the corneal inflammatory score was significantly reduced and neovascularization area was significantly less in the amniotic membrane group compared with the BMSCs group and control group(P=0.011,0.001 ;P=0.005,0.000).The levels of IL-2 and IFN-γ secreted by Th1 cells were decreased (P =0.000,0.002;P =0.003,0.045 ) and the levels of IL-10 and TGF-β secreted by Th2 cells were increased in the BMSCs group compared with the amniotic membrane and control group ( P =0.000,0.000 ; P =0.000,0.021 ).No significant difference was found in VEGF expression among three groups( F=4.880,P =0.056).But the mRNA of the MMP-2 and bFGF were lower in the BMSCs group than the amniotic membrane group(P=0.009,0.003 ) and control group(P＜0.01 ).Conclusions BMSCs modulate the expression of inflammatory-related and angiogenic-related cytokines and therefore play the antiinflammatory and anti-angiogenic effects in the chemically burned cornea.

Objective To investigate the therapy and influencing factors for prognosis of ventilator-associated pneumonia(VAP) caused by multidrug-resistant organisms(MDROs).Methods 169 patients with VAP who were admitted to a hospital between January 2012 and December 2013 were included in analysis, 125 were in MDRO infection group and 44 in non-MDRO infection group.MDRO infection group was subdivided into MDR-A group(n=78, resistant to selected antimicrobial agents) and MDR-B group (n=47, sensitive to at least one kind of selected antimicrobial agent).Antimicrobial choice and prognosis between each group were analyzed and compared.Results 242 strains of pathogenic bacteria were isolated from airway secretion of VAP patients, 173(71.49％) were MDROs.The major pathogens causing VAP were Klebsiella spp.(n=66), Pseudomonas aeruginosa(n=64), Acinetobacter spp.(n=60), Staphylococcus aureus(n=27), and Escherichia coli (n=17), the percentages of MDROs of above pathogens were 68.18％, 50.00％, 91.67％, 88.89％, and 76.47％ respectively.The prognosis of MDRO infection group was poorer than that of non-MDRO infection group, MDR-A group had the worst prognosis(P<0.001).Persistent fever, leukocytosis, and progress of pulmonary inflammation in VAP patients suggested poor prognosis(all P<0.001);antimicrobial use in patients with effective therapy was higher than those in a worsened condition before onset, at the beginning of onset, and after culture of specimens(all P<0.001), while coma, early-onset VAP and multiple bacterial infection had no prognostic significance in patients with VAP(all P>0.05).Conclusion There is high incidence of MDRO infection in patients with VAP, effective antimicrobial therapy can improve the prognosis.
