1.Comment about paraneoplastic syndrome in patients with lung cancer at Department of Respiratory in Bach Mai Hospital from 2003 to 2005

Background: Paraneoplastic syndrome is a rare syndrome on lung cancer patients in Vietnam and in the world. Objective: To define the percentage of the common paraneoplastic syndromes on lung cancer patients and research the clinical and investigations of these syndromes. Subjects and method: A study included 570 patients diagnosed lung cancer based on histopathological and cytological evidences, treated in Department of Respiratory in Bach Mai Hospital from 2003 to 2005. This was a descriptive, retrospective study. Results and conclusions: 19.47% of the patients had common paraneoplastic syndromes. Pierre Marie's syndrome was the most common (65.8%), followed by Schwartz-Bartter's syndrome (25.2%). 64.8% of the patients were 50 to 69 years old, 88.3% were men, 83.8% were smokers. 66.6% of patients were the severe periods. 30.7% of patients were diagnosed histologically adenocarcinoma. 12.8% of patients had Pierre Marie's syndrome, 5.3% had Schwartz-Bartter's syndrome, 1.9% had hypercalcemia of malignancy and 1.9% had thromboembolism.

2.Evaluation of the efficacy of cynoff 25 ULV to eradicate aedes species, dengue vectors in Vietnam, 2004

Background: Dengue fever remains a major public health problem in Vietnam. It was transmitted through two main Aedes species, namely Aedes aegypti and Aedes albopictus in which Aedes aegypti played the predominant role in transmitting the dengue viruses (accounted for 94% of the Aedes genus).Different groups of insecticides have been widely used in the community. As a consequence, it increased the resistance level of Aedes vectors to the insecticides. Therefore, a new synthesized insecticide was required for future control and prevention of dengue epidemics. \r\n', u'Objectives: To evaluate the efficacy of Cynoff 25 ULV in suppressing Aedes species in Vietnam. \r\n', u'Subjects and methods: The study was conducted in the entomology laboratory and in the fields of Ha Tay province. ULV spraying was implemented in Hiengiang commune, Thuongtin district, in comparison with a control area, Van Mo commune in Ha Dong town.\r\n', u'Results and conclusions: : Cynoff 25 ULV had a high efficacy that kills over 90% of 2 dengue vector species, namely Aedes aegypti and Aedes albopicctus after 24 hours exposure at the distance of 30 meters from the brass nozzle of a STIHL SR 400 sprayer. In the field trials, Cynoff 25 ULV also had high efficacy in killing Aedes species, e.g. 100% of Aedes species were killed after spraying and the effects lasted for 3 months for Aedes aegypti and one month for Aedes albopictus species. 3.Possible association between recent migration and hospitalisation for dengue in an urban population: A prospective case-control study in northern Vietnam

A prospective case–control study was conducted in urban districts in Hanoi,
northern Vietnam to evaluate the effect of migration on the risk of hospitalisation for dengue
in a Vietnamese urban population. We enrolled laboratory-confirmed dengue patients aged
>18 years who were hospitalised in local hospitals in November and December 2010. Four
neighbourhood-matched controls for each case were recruited within a week of hospitalisation.
Sociodemographic data were collected by interviews, and the number of immature and adult
mosquitoes within household premises was counted by entomological survey. Matched-pair
analyses were conducted using conditional logistic regression models. Among 43 cases and
168 controls, 84% and 83% were migrants from rural areas, respectively. Although statistical
significance was marginal, recent migration (residing in study area for <5 years) independently
increased the risk of hospitalisation for dengue compared with inhabitants after controlling
for potential confounders (adjusted odds ratio [aOR] = 3.78; 95% confidence interval [CI] =
0.99–14.27), whereas longer-term migration (residing in study area for >6 years) did not
change the risk (aOR = 1.1; 95% CI = 0.30–4.05). Younger age (18–34 years) (aOR = 7.26; 95%
CI = 2.39–22.06) and higher adult Aedes aegypti infestation level within household premises
(aOR = 9.25; 95% CI = 1.68–51.09) were also independently associated with hospitalisation
for dengue. Recent migration from rural areas seems to increase the risk of hospitalisation for
dengue in urban populations in endemic areas. Further research including cohort study should
be done to confirm the impact of migration on the risk of dengue in urban areas.

Background:
Highly pathogenic avian influenza viruses (HPAIVs) is an extremely contagious and high mortality rates in chickens resulting in substantial economic impact on the poultry sector. Therefore, it is necessary to elucidate the pathogenic mechanism of HPAIV for infection control.
Objective:
Gene set enrichment analysis (GSEA) can effectively avoid the limitations of subjective screening for differential gene expression. Therefore, we performed GSEA to compare HPAI-infected resistant and susceptible Ri chicken lines.
Methods:
The Ri chickens Mx(A)/BF2(B21) were chosen as resistant, and the chickens Mx(G)/ BF2(B13) were selected as susceptible by genotyping the Mx and BF2 genes. The tracheal tissues of HPAIV H5N1 infected chickens were collected for RNA sequencing followed by GSEA analysis to define gene subsets to elucidate the sequencing results.
Results:
We identified four differentially expressed pathways, which were immune-related pathways with a total of 78 genes. The expression levels of cytokines (IL-1β, IL-6, IL-12), chemokines (CCL4 and CCL5), type interferons and their receptors (IFN-β, IFNAR1, IFNAR2, and IFNGR1), Jak-STAT signaling pathway genes (STAT1, STAT2, and JAK1), MHC class I and II and their co-stimulatory molecules (CD80, CD86, CD40, DMB2, BLB2, and B2M), and interferon stimulated genes (EIF2AK2 and EIF2AK1) in resistant chickens were higher than those in susceptible chickens.
Conclusions
Resistant Ri chickens exhibit a stronger antiviral response to HPAIV H5N1 compared with susceptible chickens. Our findings provide insights into the immune responses of genetically disparate chickens against HPAIV.

Objective:
The purpose of this survey was to estimate the prevalence of viral load (VL) suppression and emergence of HIV drug resistance (HIVDR) among individuals receiving antiretroviral therapy (ART) for 36 months or longer in Viet Nam using a nationally representative sampling method.
Methods:
The survey was conducted between May and August 2014 using a two-stage cluster design. Sixteen ART clinics were selected using probability proportional to proxy size sampling, and patients receiving ART for at least 36 months were consecutively enrolled. Epidemiological information and blood specimens were collected for HIV-1 VL and HIVDR testing; HIVDR was defined by the Stanford University HIVDR algorithm.
Results:
Overall, 365 eligible individuals were recruited with a mean age of 38.2 years; 68.4% were men. The mean time on ART was 75.5 months (95% confidence interval [CI]: 69.0–81.9 months), and 93.7% of the patients were receiving non-nucleoside reverse transcriptase inhibitor-based regimens. Of the 365 individuals, 345 (94.7%, 95% CI: 64.1–99.4%) had VL below 1000 copies/mL and 19 (4.6%, 95% CI: 2.8-–7.5) had HIVDR mutations.
Discussion
Our nationally representative survey found a high level of VL suppression and a low prevalence of HIVDR among individuals who received ART for at least 36 months in Viet Nam. Continued surveillance for HIVDR is important for evaluating and improving HIV programs.

Soluble epoxide hydrolases (sEH) are enzymes present in all living organisms, metabolize epoxy fatty acids to 1,2-diols. sEH in the metabolism of polyunsaturated fatty acids plays a key role in inflammation. In addition, the endogenous lipid mediators in cardiovascular disease are also broken down to diols by the action of sEH that enhanced cardiovascular protection. In this study, sEH inhibitory guided fractionation led to the isolation of five phenolic compounds trans-resveratrol (1), trans-piceatannol (2), sulfuretin (3), (+)-balanophonin (4), and cassigarol E (5) from the ethanol extract of the seeds of Passiflora edulis Sims cultivated in Vietnam. The chemical structures of isolated compounds were determined by the interpretation of NMR spectral data, mass spectra, and comparison with data from the literature. The soluble epoxide hydrolase (sEH) inhibitory activity of isolated compounds was evaluated. Among them, trans-piceatannol (2) showed the most potent inhibitory activity on sEH with an IC₅₀ value of 3.4 µM. This study marks the first time that sulfuretin (3) was isolated from Passiflora edulis as well as (+)-balanophonin (4), and cassigarol E (5) were isolated from Passiflora genus.

9.A study of pathological characteristics and BRAF V600E status in Langerhans cell histiocytosis of Vietnamese children

Background:
Langerhans cell histiocytosis (LCH) is more common in children than adults and involves many organs. In children, the BRAF V600E mutation is associated with recurrent and high-risk LCH.
Methods:
We collected paraffin blocks of 94 pediatric LCH patients to detect BRAF V600E mutation by sequencing. The relationship between BRAF V600E status and clinicopathological parameters were also critically analyzed.
Results:
BRAF V600E mutation exon 15 was detected in 45 cases (47.9%). Multiple systems LCH showed a significantly higher BRAF V600E mutation rate than a single system (p=.001). No statistical significance was evident for other clinical characteristics such as age, sex, location, risk organs involvement, and CD1a expression.
Conclusions
In Vietnamese LCH children, the proportion of BRAF V600E mutational status was relatively high and related to multiple systems.
10.Predictors of twin pregnancy in in vitro fertilization with intracytoplasmic sperm injection cycles with day 3 double embryo transfer

Objective:
The purpose of this study was to identify factors associated with twin pregnancy following day 3 double embryo transfer (DET).
Methods:
This retrospective cohort study incorporated data from 16,972 day 3 DET cycles. The participants were women aged between 18 and 45 years who underwent in vitro fertilization with intracytoplasmic sperm injection (IVF/ICSI) at My Duc Assisted Reproduction Technique Unit (IVFMD), My Duc Hospital, located in Ho Chi Minh City, Vietnam.
Results:
Of the 16,972 day 3 DET cycles investigated, 8,812 (51.9%) resulted in pregnancy. Of these, 6,108 cycles led to clinical pregnancy, with 1,543 (25.3% of clinical pregnancies) being twin pregnancies. Factors associated with twin pregnancy included age under 35 years (odds ratio [OR], 1.5; 95% confidence interval [CI], 1.32 to 1.71; p<0.001) and cycles involving the transfer of at least one grade I embryo. Relative to the transfer of two grade III embryos, the risk of twin pregnancy was significantly elevated following the transfer of two grade I embryos (OR, 1.40; 95% CI, 1.16 to 1.69; p<0.001) or a combination of one grade I and one grade II embryo (OR, 1.27; 95% CI, 1.05 to 1.55; p=0.001).
Conclusion
By analyzing a large number of IVF/ICSI cycles, we identified several predictors of twin pregnancy. These findings can assist medical professionals in tailoring treatment strategies for couples with infertility.
