Factors of spa therapy in relation to clinical effects were studied on 36 patients, who were admitted to Misasa Branch Hospital in 1987 with respiratory diseases.
1. The patients who received the spa therapy comprised 30 cases of bronchial asthma, 3 cases of diffuse panbronchiolitis, 3 cases of allergic granulomatous angitis, and 1 case of bronchiectasia.
2. Most bronchial asthma patients were over 40 years old, and 16 cases (53.3%) of 30 patients had long-term glucocorticoids. The serum cortisol level was generally low and was especially low in the cases with glucocorticoids therapy (4.9±5.6mcg/dl in female patients and 3.5±3.2mcg/dl in male patients). The incidence of each clinical type of asthma was as follows: 12 cases (66.7%) of bronchospasm type (Ia), 6 cases (33.3%) of bronchospasm plus hypersecresion type (Ib), and no case (0%) of bronchiolar obstruction type (II) out of 18 female patients. For male patients, there were 5 cases (41.7%) of Ia, 5 cases (41.7%) of Ib, and 2 cases (16.7%) of II. Spa therapy was obviously effective in 25 out of 30 cases (83.3%).
3. Spa therapy was also effective for respiratory diseases other than asthma, especially for diffuse panbronchiolitis.

Examinations to some extent were made on yearly transitions of 52 patients with bronchial asthma who were hospitalized in Misasa Medical Branch, Okayama University Medical School and received spa therapy during the five years from 1982 to 1986.
1) In the first two years (1982 to 1983), we had many patients from Okayama Prefecture. The majority of them had suffered from severe asthma attacks. They included a higher rate of cases of dependency on steroid-therapy as compared to those patients in the last three years (1984 to 1986). Some cases exhibited a remarkable airways obstruction in spite of high dosage of steroid hormone.
2) From 1984 to 1986, the patients with bronchial asthma from Tottori Prefecture increased. Their involvements were less severe with a lower dependency on steroid-therapy as compared to those hospitalized during the first two years.
3) Terms of hospitalization were generally reduced in the last two years as compared to the preceding three years. The average term was about two months from 1985 to 1986.
4) No difference in clinical symptoms based on the patient's age, age at onset, or asthma types was found between the first two years and the last three years.

Allergological characteristics were studied on 36 patients with respiratory diseases (30 cases of bronchial asthma, 3 cases of diffuse panbronchiolitis, 3 cases of allergic granulomatous angitis, and 1 case of bronchiectasia) who received spa therapy at Misasa Branch Hospital.
1. The results obtained in patients with bronchial asthma were as follows:
Serum IgE levels were generally low, and many cases with less than 301IU/ml were observed (185±184/ml in female patients and 469±532IU/ml in male patients). The rate of positive skin reactions to various allergens was low in these asthma cases. A rather low rate of positive skin reaction to house dust, in particular, was characteristic. In regard to the rate of releasing chemical mediators such as bistamine and leukotrienes from leukocytes stimulated by Ca ionophore A 23187, a wide variety of cases were observed (histamine, 3.4-51.1%; LT B₄ 9.8-119.8ng/10⁶ cell; LT C₄, 3.5-43.8ng/10⁶ cells). By analyzing cell components in bronchoalveolar lavage fluids (BALF), notable increase in the number of eosinophils were observed especially in male patients, although no other significant differences were shown between the two groups.
2. In those patients with respiratory diseases other than asthma, a remarkable increase in the number of neutrophils in BALF was seen in diffuse panbronchiolitis cases and a remarkable increase in the number of eosinophils in BALF was seen in the cases of allergic granulomatous angities. Serum IgE levels were low in both of these cases.

Background
 According to the state of the world's children 2006, 28% of children under five years of age in sub-Sahara Africa are malnourished, which has serious effects on these children's health and lives.
 Extensive research has been conducted on child malnutrition in a variety of developing countries. These studies have established scientific indexes to lead and coordinate international action to assist the needy. Central African Republic, however, has been a blind spot. Therefore, the present study was conducted in order to clarify the nutritional status and associated factors of 6 to 24-month-old children in the district of Boy-rabe, Bangui, Central African Republic.
Methods
 Participants were mothers with children aged 6-24 months who visited either a government-run clinic or the NGO-run clinic, Amis d'Afrique, between August 26th and September 16th of 2006. Mothers were interviewed using a structured questionnaire and measurements of the children's weight and length were collected. Weight, length, and age data were compared with child growth standards proposed by the WHO to calculated z-scores for evaluatinglength-for-age (wasting), weight-for-length (stunted growth), and weight-for-age (underweight). The following factors were investigated for their association to malnutrition in children: “demographic characteristics”, “access to food”, “maternal and child-care practices”, and “poor water/sanitation and inadequate health services”.
Results
 A total of 126 mother-child pairs participated in this study and valid data from 109 pairs were subjected to analysis. The rates of wasting, stunted growth, and underweight children were 20.2%, 61.5% and 42.2%, respectively.
 Incomplete vaccination (p=0.043) and the mother not having a partner (p=0.046) were significantly associated with wasting. Stunted growth was found to be associated with older child's age (p<0.001), older mothers' age (p=0.005), mothers who had stopped breast-feeding (p=0.031), insufficient breast-feeding (p=0.032), mothers with child death experience (p=0.022), mothers with a number of delivery experiences (p=0.026) and mothers with a partner (p=0.042). Underweight children were associated with incomplete vaccination (p=0.043) and mothers with child death experience (p=0.046).
Conclusion
 In total, 8 factors were found to be significantly associated with child's malnutrition and household/family level. In particular, severe acute malnutrition, or wasting, was related to insufficient vaccination, while chronic malnutrition, or stunted growth, was significantly associated with breast-feeding.

The expansion of life-style related diseases, such as metabolic syndrome (MS) and type 2 diabetes mellitus (T2DM), appears to be unstoppable. It is also difficult to cease their complications in spite of many antidiabetic medications or intervention of public administration. We and our collaborators found that physical medicine using simultaneous stimulation of heat with mild electric current activates heat shock response, thereby reducing visceral adiposity, insulin resistance, chronic inflammation and improving glucose homeostasis in mice models of T2DM, as well as in humans with MS or T2DM. This combination therapy exerts novel action on insulin signaling, beta-cell protection and body compositions, and may provide a new therapeutic alternative in diabetic treatment strategy.

OBJECTIVE: To analyze patient characteristics of cancer rehabilitation and outcomes at our hospital. METHODS: This retrospective study analyzed 580 patients, who underwent cancer rehabilitation at our hospital and rehabilitation outcome after therapy were investigated. The relationship between the initial Barthel index and discharge outcomes was investigated, with a special focus on cancer patients with bone metastasis. The Barthel index and performance status (Eastern Cooperative Oncology Group) before and after rehabilitation were analyzed, and threshold value of home discharge was calculated from a receiver operating characteristic curve (ROC). General criteria for home discharge from our hospital included independence in performing basic activities of daily living such as bathing, feeding, and toileting or availability of home support from a family member/caregiver. RESULTS: The outcomes after rehabilitation among all the patients were as follows: discharge home 59%, death 13%, and others 27%. Statistical differences were observed between the initial and final values of the Barthel index in patients with bone metastasis, who could be discharged home (p=0.012). ROC analysis of the initial Barthel index for predicting home discharge revealed a threshold value of 60, sensitivity of 0.76, and specificity of 0.72. CONCLUSION: The patients with bone metastasis had a lower rate of home discharge and a higher rate of mortality than all the study patients who underwent cancer rehabilitation at our hospital. It is proposed that at the time of initiation of rehabilitation for patients with bone metastasis, an initial Barthel index lower than 60 might predict a worse outcome than home discharge.

Since a skin infiltration of breast cancer may increase hemorrhage and malodorous exudates, it significantly reduces the quality of life. Local treatment is usually performed with zinc oxide starch, Mohs paste or its alternatives. However, the Mohs paste causes pain due to necrobiosis of normal tissues, while the topical therapy with zinc oxide starch ineffectively suppresses hemostasis and odor. We hereby report a case of an 86-year old woman with skin infiltration by breast cancer. The patient’s symptoms including bleeding, infection signs, odor, exudates, necrotic tissue objectively improved with Shiunko ointment, Zinc oxide starch, and Metronidazole (MNZ) combination therapy. The gauze was exchanged once a day and this treatment was simple and easy. The combination therapy with Shiunko ointment, Zinc oxide starch, and MNZ may become an effective new regimen for bleeding and odor from skin involvement in breast cancer.
