1.Two Cases of Acne Vulgaris Successfully Treated with Tokikenchuto

Kampo treatment is frequently effective for patients with acne vulgaris who are unresponsive to standard treatment with western medicine. We report two cases of acne vulgaris successfully treated with tokikenchuto. In the first case, a 32-year-old woman complained of mild acne, loose stool and menstrual pain. Her abdomen was soft and a pulsation above the navel was noted. These symptoms were considerably improved after taking tokikenchuto. In the second case, a 26-year-old woman presented with moderate acne, loose stool and menstrual pain with excessive strain of the abdominal muscles and objective tenderness on the sides of the abdomen. Kamishoyosan and tokishakuyakusan were first prescribed, although these caused bowel disturbances, and eventually tokikenchuto was prescribed, which improved the acne. We suggest tokikenchuto is effective for patients with mild to moderate acne who have bowel disturbance with soft abdomen or excessive strain of the abdominal muscles, which are considered indicators of gastrointestinal weakness. In addition, yokuinin has a synergistic effect in the treatment of acne.
2.An Operative Case of Right Coronary Artery Fistula Communicated to the Left Atrium.

We report a very rare case of a coronary artery fistula with communication between the right coronary artery and the left atrium. The patient was a 45 year-old woman admitted for evaluation of heart murmur. Selective coronary angiography demonstrated right coronary artery-left atrial fistula. The operation was indicated due to volume overload of the left ventricle. At operation, the proximal portion of the coronary fistula was successfully ligated from the epicardial side and the entrance of the fistula into the left atrium was directly closed from the inside of the left atrium under the cardiopulmonary bypass. The post-operative course was uneventful. Post-operative coronary angiography showed disappearance of the fistula. Angiography 6 months later, demonstrated that the orifice of the right coronary artery remained dilated, while the diameter of the distal site was normalized.
4.Urinary Porphyrins in Patients with Endemic Chronic Arsenic Poisoning Caused by Burning Coal in China

Objective: To evaluate the effect of arsenic (As) on the porphyrin biosynthetic pathway, urine samples from patients with endemic chronic arsenic poisoning were examined.Subjects and Methods: The subjects were 16 patients, who had been exposed to As from burning coal for 8 to 25 years, and-16 controls living in the same region in Guizhou Province in southwest China. Concentrations of urinary As, porphyrins and ALA were determined by induced coupled plasma mass spectrometry (ICP-MS), high performance liquid chromatography (HPLC) with a reversed-phase column and fluorescence detector, and colorimetric spectrophotometry, respectively.Results: Concentrations of As in patients and controls, 184.40 ± 200.04 and 86.82 ± 64.20 μ g/g creatinine (mean ± SD) respectively, were significantly different (p<0.05). The concentrations of various kinds of urinary porphyrins, including isomers I and III of coproporphyrin and pentacarboxylporphyrin, were determined. Positive correlations were observed between As and porphyrins (e.g. total porphyrins, hexacarboxylporphyrin and coproporphyrin III) or between As and ALA in male and female patients. However, porphyrin and ALA concentrations were not significantly different between the patients and the controls. Urinary porphyrin concentrations in females were higher than those in males.Conclusion: Exposure to As from burning coal may influence porphyrin biosynthesis.

5.Increasing Use of Morphine in Our Hospital and Cancer Pain Relief in Our Pain Clinic.

This study was performed to evaluate management of cancer pain in 322 patients who were treated in our pain clinic between January 1986 and December 1997. For this purpose, the change in the number of cancer patients and the annual morphine consumption during the period in our hospital were investigated. We found that as morphine consumption increased, the number of cancer patients visited the pain clinic decreased. Although 90% of them who visited the pain clinic underwent neural block therapy before 1996, the parcentage dropped to only 79% in 1996 and 50% in 1997. The increasing rate of morphine consumption in our hospital was on a par with the national average between 1986 and 1996. The use of adjuvant drugs did not change throughout the 12-year period.
We believe the consumption of morphine is not enough. It is necessary for us to become more skilled in using narcotics for relief of cancer pain along the guidelines of the World Health Organization.
6.Occupational health hazards among migrant forestry and construction workers in a mountainous rural area. Part 1. Migrant forestry work.

High economic growth since the late 1950's has brought about a severe labour shortage in Japan. Japanese industry sought migrant workers as one reliable source of labour. As seasonal and unskilled workers, they were employed in a wide variety of trades ; for example, construction, manufacturing, forestry and transportation. However, some of the migrant work was becoming a principal and nonseasonal occupation, and specialized. Migrant forestry and tunnel-construction work are two examples of such specialized work.
To identify health problems among them, we examined the factors affecting workers' entry into migrant forestry and tunnel-construction work, working conditions, and health care services at the workplace by interviewing 65 migrant wokers identified at Hayama Village in Kochi Prefecture, one of the areas well known for providing migrant wokers. Moreover, we carried out health examinations for vibration disease and pneumoconiosis. In this report, we show the results of analysis on 35 migrant forestry workers.
The main factors affecting their entry into migrant forestry work were unstable and poor socioeconomic conditions in the area and the strong demand for a workforce during the rapid economic growth from the late 1950's in Japan. Most of the wage configuration was for piece and contracted work, which promoted long working hours and above-average consecutive work days. They engaged in cutting down, collecting and transporting trees, using vibrating tools such as chain saws, bush cutters, collecting machines and so on. High levels of vibration-exposure quite exceeded the limit of the Ministry of Labour and lasted continuously over 17 years. A difference in labor conditions and working situations was observed between migrant workers who devoted themselves to migrant forestry work and those who did not. Health care administration including health examinations was inadequate. There had been no case of vibration disease identified at the workplace. However, 68 percent among 25 examinees were diagnosed as suffering from vibration disease. Moreover, 40 percent of the subjects experienced absences of more than 4 days due to occupational accidents.
7.A Trial Calculation of the Costs Incurred by the Absences of Agricultural Co-opEmployees in Aichi Prefecture due to Novel Influenza

With the spread of a novel H1N1 strain of influenza A around the world, the World Health Organization declared that the situation had entered the pandemic phase. In Japan, it had a major social impact, causing a great hit to the national economy. And there is every likelihood that the H1N1 influenza is going to break out a lot of people on the staff of the agricultural cooperatives in Aichi Prefecture, seriously affecting the activities of agricultural cooperatives too. Based on the amount of damage from the flu presumed by the Japanese government and various kinds of statistical material, we worked out how much the flu would cost the agricultural co-ops in the prefecture by infecting their employees and involved their family. At an absence rate of 20%, the lowest rate presupposed by the government, our calculation showed that the extra costs would amount to 740 million yen altogether. If the rete of abscense from work was given 40%, the largest estimate by the government, the costs rocketed to highly 1.48 billion yen. A check on the situation of individual co-ops revealed that some of the co-ops would burden an extra cost of well over 100 million yen. From these findings, we concluded that to minimize the effects of the flu appropriate, effective countermeasures have to be taken after sufficient information about the novel influenza virus was collected and analyzed carefully.
8.The Clinical Utility of Diffusion Tensor Imaging and Fiber Tractography for evaluating Diffuse Axonal Injury

Diffuse axonal injury (DAI) is identified as one of the most important causes of cognitive disorders in patients with traumatic brain injury. Radiologic recognition of DAI can help in understanding the clinical syndrome and in making treatment decisions. However, CT and conventional MRI are often normal or demonstrate lesions that are poorly related to the cognitive disorders present. Recently, diffusion tensor imaging (DTI) and fiber tractography (FT) have been shown to be useful in detecting various types of white matter damage. The aim of this study was to evaluate the feasibility of using DTI and FT to detect lesions in DAI patients, and to correlate these DAI lesions with the patients' cognitive disorders. We investigated 9 normal volunteers and 9 patients with DAI. The DAI patients had impaired intelligence, as well as attention, memory and executive function disorders that restricted their activities of daily living. In the DAI patients, DTI showed abnormal brain areas in the corpus callosum, fornix, frontal and parietal lobe white matter, and FT revealed interruptions of the white matter fibers in the corpus callosum and the fornix when compared with the normal volunteers, while no lesions were found on conventional MRI. DTI and FT can directly visualize DAI lesions, which cannot be reliably detected by conventional methods. Accordingly, both DTI and FT may be useful techniques for the evaluation of DAI, and may have the potential to be applied to planning rehabilitation therapy, and predicting the neurologic prognosis in DAI patients with cognitive disorders.
9.Research on the situation of the employment needs of acupuncture, moxibustion, and massage practitioners in clinics

[Objective]To analyze the situation of clinics that want to employ acupuncturists and masseurs and to obtain basic data to find employment in the clinic in the future.
[Methods]The subjects were 52 clinics that responded to the questionnaire survey saying they wanted to employ acupuncture and massage therapists. They were divided into three groups according to the license (acupuncture group (n = 5), acupuncture &massage group (n = 25), massage group (n = 22)). The situation and the business realities of the clinic were analyzed.
[Results]Clinics with obstetrics and gynecology want to employ only acupuncturists. There were as many employment needs for both acupuncturists and masseurs in clinics with orthopedics and rehabilitation. The employment satisfaction rating is higher for the patient's need standpoint than cost-effectiveness.
[Conclusion]We analyzed the status of clinics that want to employ acupuncturists and masseurs. This study is expected to help promote the employment of medical acupuncturists and masseurs.
10.Atlases of pesticide poisoning mortality for prefectures in Japan.

Mortarity due to pesticide poisoning for prefectures in Japan is examined according to the vital statistics in 1979-1981. The atlases of pesticide poisoning mortality for prefectures in Japan are presented.
There is a wide range in mortality among prefectures and the highest mortality is 17.8 times as high as that observed in the prefecture which showed the lowest mortality. Regional characteristic is observed in the mortality. Northern part of Kanto Area and the southern part of Kyusyu Island show high mortality and low mortality is observed in Hokuriku Area.
The ratio of the deaths due to pesticide poisoning to those due to automobile accident also show wide range among prefectures. The highest ratio is 38.3% and the lowest is 2.4%. The ratio shows similar regional characteristic to that observed in mortality.
