1.Seasonal Changes in the Health Problems among Women Farmers in Nagano Prefecture with Reference to Their Mode of Living

In view of the present situation of agriculture in which housewives have to handlethe bulk of farm work, a study was made on the health conditions of women engaged in farming in comparison with those of non-farmers.
The study covered a total of 28, 600 females, aged from 40 to 59, who received masshealth screenings conducted between 1978 and 1980 on rural inhabitants in Nagano Prefecture as part of the outreach program by the health service corps of our hospital.
All the examinees live in areas whose principal crops are rice, vegetables and/or fruits.
The data were arranged according to seasons based on the dates of health examination in order to clarify the seasonal variations of morbidity rates and other indices of health.
Another investigation was made into seasonal changes in the length of time used byhousewives in farming, domestic chores, rest and so on with a view to studying correlation between the mode of living and health conditions.
To sum up, our findings are as fllows:
(1) Seasonal variations in the results of various laboratory tests and morbidity rates were notable, depending on the amount of farm work, dietary habits, coldness and the height above sea level.
(2) During summer months, the incidence of anemia was exceedingly high, obesity rates plummeted, and serum cholinesterase values dropped sharply among housewives who are engaged in farm work, compared with those who are not.
(3) In summer, farm housewives worked long hours in the fields, slept less and rested less. They had not enough time to attend to household duties.
2.Surgical Management of Perimembranous or Infundibular-isolated Ventricular Septal Defect Associated with Prolapse of Aortic Cusp or Aortic Regurgitation.

Out of 104 patients with perimembranous or infundibular-isolated ventricular septal defect (VSD), causative factors of Prol and AR, and the operative indication were studied in 17 with prolapse of the aortic cusp (Prol) and 10 with aortic valve regurgitation (AR). The left to right shunt ratio and the size of VSD were smaller in patients with Prol or AR than in those with the normal aortic cusp, suggesting that hemodynamics might take part in the cause of Prol or AR. Twenty-two patients underwent VSD closure only, four valvuloplasty and one aortic valve replacement. Residual AR was occurred in three out of ten patients. After surgery, AR was disappeared in six out of seven patients with the first grade preoperative AR, but AR remained in all two patients with the second grade preoperative AR. Careful preoperative observation and early operation before the appearance of AR are the important factors for avoiding residual regurgitation after aortic valvuloplasty.
3.Arterial Reconstruction for Aorto-Iliac Obstructive Disease.

Surgical interventions for aorto-iliac obstructive diseases were studied through the operative results. Eighteen patients underwent aorto-femeral bypass (AOF) and 23 who were over 70 years of age or who had serious preoperative complications had axillofemoral bypass (AXF). No perioperative death occurred in AOF patients, while the mortality rate of AXF patients was 8%. Postoperative ankle pressure indexes were significantly higher in AOF patients than in AXF patients. Follow-up graft patency rate was 100% in AOF patients at 54 months (mean), and 85% in AXF patients at 44 months respectively. AOF should be the first choice for patients with aorto-iliac obstructive disease, and AXF is suitable only for high-risk patients.
4.Hepatic and Intestinal Circulation during Extracorporeal Circulation.

Blood oxygen saturation, keton boby ratio and endotoxin concentration of arterial and hepatic venous blood were measured in 12 adult patients before, during and after extracorporeal circulation (ECC). When rectal temperature returned to 32°C during ECC, the levels of hepatic venous blood oxygen saturation (ShvO2) and arterial keton body ratio, hepatic venous keton body ratio decreased. The serum level of endotoxin concentration was within normal limits on the operative day and increased at the first and second day after surgery. In three patients in whom the level of ShvO2 was under 50% at 60 minutes after ECC, postoperative liver dysfunction occurred frequently. Endotoxin changes on the first day after surgery is probably due to recovery differences between hepatic and gastrointestinal circulations.
5.Preoperative Pulmonary Arterial Pressure and Surgical Treatment of Secundum Atrial Septal Defect in Patients over 50 Years of Age.

The preoperative pulmonary arterial pressure of 38 patients aged over 50 undergoing surgical closure of a secundum atrial septal defect was studied. They were divided into three groups according to systolic pulmonary arterial pressure (PAP): Group A (PAP<30mmHg, n=14), Group B (30≤PAP<50mmHg, n=16), and Group C (PAP≥50mmHg, n=8). The mean age of group C patients was older than that of group A patients. With higher PAP, the Pp/Ps, Rp/Rs and cardiothoracic ratios increased, atrial fibrillation and heart failure (NYHA≥2) were more frequent, and PaO2 levels declined. There were no differences in left to right shunt ratio and Qp/Qs among the three groups. The PAP and Rp/Rs were under 70mmHg and 0.30, respectively in all patients. High pulmonary blood flow seems to be the cause of pulmonary hypertension in most elderly patients because PAP and Rp/Rs decreased after surgery in all groups. Findings of cardiomegaly and heart failure also improved after surgery. Surgical intervention is recommended even in elderly patients with a ASD.
6.Inhibition of plasminogen activator inhibitor-1 attenuates against intestinal fibrosis in mice

Background/Aims:
Intestinal fibrosis is a major complication of Crohn’s disease (CD). The profibrotic protein transforming growth factor-β (TGF-β) has been considered to be critical for the induction of the fibrotic program. TGF-β has the ability to induce not only the expression of extracellular matrix (ECM) including collagen, but also the production of plasminogen activator inhibitor-1 (PAI-1) that prevents enzymatic degradation of the ECM during the onset of fibrotic diseases. However, the significance of PAI-1 in the developing intestinal fibrosis has not been fully understood. In the present study, we examined the actual expression of PAI-1 in fibrotic legion of intestinal inflammation and its correlation with the abnormal ECM deposition.
Methods:
Chronic intestinal inflammation was induced in BALB/c mice using 8 repeated intrarectal injections of 2,4,6-trinitrobenzene sulfonic acid (TNBS). TM5275, a PAI-1 inhibitor, was orally administered as a carboxymethyl cellulose suspension each day for 2 weeks after the sixth TNBS injection.
Results:
Using a publicly available dataset (accession number, GSE75214) and TNBS-treated mice, we observed increases in PAI-1 transcripts at active fibrotic lesions in both patients with CD and mice with chronic intestinal inflammation. Oral administration of TM5275 immediately after the onset of intestinal fibrosis upregulated MMP-9 (matrix metalloproteinase 9) and decreased collagen accumulation, resulting in attenuation of the fibrogenesis in TNBS-treated mice.
Conclusions
PAI-1-mediated fibrinolytic system facilitates collagen degradation suppression. Hence, PAI-1 inhibitor could be applied as an anti-fibrotic drug in CD treatment.
