1.Risk factors of pulmonary non-tuberculous mycobacterial infection in 489 patients with sputum acid-fast bacilli positive and/or mycobacteria culture positive

Objective To analyze the risk factors of pulmonary non-tuberculous mycobacterial (NTM) infection in sputum acid-fast bacilli positive and/or mycobacteria culture positive patients.Methods One hundred and sixty-three patients with pulmonary NTM infection were recruited from Jan 2006 to Jun 2011 in Shanghai Pulmonary Hospital and 326 patients with sputum positive pulmonary tuberculosis who were selected by random systemic sampling method in the same period were recruited as control.The data were retrospectively analyzed.The related factors were compared between groups by chi-square test.The risk factors of pulmonary NTM infection were analyzed by binary Logistic regression model.Results There were statistically significant differences of age,history of smoking,chronic obstructive pulmonary disease (COPD),bronchiectasis,thin walled cavities focus,purified protein derivative (PPD) test between patients with pulmonary NTM infection and patients with pulmonary tuberculosis (all P<0.05).Univariate analysis showed that age [45-60year (OR=2.637,95%CI:1.631-4.264; P<0.001); >60 year (OR=4.194,95%CI:2.581-6,813 ; P<0.001)],history of smoking [10-20 year (OR=1.842,95%CI:1.0843.070; P=0.024),>20 year (OR=2.040,95%CI:1.167-3.567; P=0.012)],COPD (OR=2.698,95%CI:1.588-4.583; P<0.001),bronchiectasis (OR=3.566,95%CI:2.343-5.427;P<0.001),thin walled cavities focus (OR=2.592,95%CI:1.581-4.250; P<0.001) and a weak-positive reaction of PPD test (OR=2.389,95%CI:1.276-4.472; P=0.006) were all risk factors of pulmonary NTM infection.Multivariate analysis showed that age>60 year (OR=3.961,95%CI:2.183-7.189 ; P<0.001),bronchiectasis (OR =3.880,95 % CI:2.342-6.487 ; P<0.001),thin walled cavities focus (OR=2.898,95%CI:1.567-5.360; P<0.001),COPD (OR=2.503,95% CI:1.289-4.857; P=0.007),age45-60 year (OR=2.452,95%CI:1.391 4.325; P=0.002)anda weak-positive reaction of PPD test (OR=2.295,95%CI..1.132-4.652; P=0.021) were independent risk factors of pulmonary NTM infection.Conclusion In sputum acid-fast bacilli positive and/or mycobacteria culture positive patients,age≥ 45 year,COPD,bronchiectasis,thin walled cavities focus and a weak-positive reaction of PPD test are risk factors of pulmonary NTM infection.The clinicians should pay close attention to the results of species identification.
2.Clinical application of first line anti-tuberculous drug susceptibility test by phage amplified biologically assay

Objective To evaluate clinical application of phage amplified biologically assay (PhaB) in susceptibility test of Mycobacterium tuberculosis (MTB) in sputum. Methods The drug susceptibility of MTB was detected by PhaB in 143 patients with sputum-positive pulmonary tuberculosis (PTB),and the chemotherapy regimens were adjusted according to the results of susceptibility test.Independent samples t-tests were used for comparison of means.Count numbers were compared with Chisquare test.If there were count number of 0,Fisher probabilities should be used.ResultsThe total positive rate of PhaB was 94.4% (135/143) with no differences between three types of PTB (x2 =1.886,P > 0.05 ).The duration of testing for PhaB group was (6.6 ± 1.8) days,while for control was (29.4 ±8.7) days (t =29.01,P < 0.01 ).Compared with control group,the 2-month negative-conversion rate (63.2% vs.35.1%,x2 =3.989,P < 0.05 ) and cure rate ( 100% vs.78.4%,P < 0.05 ) of PhaB group in type Ⅱ patients were significantly higher.But there were no differences between PhaB and control groups in type Ⅰ and Ⅲ PTB patients.ConclusionThe results of PhaB drug susceptibility test can be helpful for choosing effective chemotherapy regimen for PTB patients rapidly.
3.Expression of bone morphogenetic protein-2 during fracture healing in osteoporotic rats

AIM:Mesenchymal cells migrating towards fracture site and differentiating into chondrocytes and osteoblasts is the key process during fracture healing. This article is aimed to observe the changes of the expression of bone morphogenetic protein(BMP)-2 in the early stage of tibia fracture in osteoporotic rats so as to evaluate the reason of delayed fracture healing in osteoporotic rats. METHODS:The experiment was conducted at the Key Laboratory for Oral Biomedical Engineering of Ministry of Education from August 2005 to October 2006. ①Sixty-four 6-month-old female SD rats were randomly divided into ovariectomized(OVX) group and control group with 32 in each group. ②The rats in the OVX group underwent ovariectomy to establish type Ⅰ osteoporotic models. A small quantity of fat tissues was removed in the control group. Three months later,standardized tibia fracture was introduced. ③Eight rats were selected at days 7,14,21 and 28 after operation in the two groups. Some soft tissues around tibia and broken ends of fractured bone,bony callus,cortical bone and medullary canal were collected and treated with histological analysis,immunohistochemical staining,semi-quantitative reverse transcription-polymerase chain reaction(RT-PCR),respectively. RESULTS:①Histological analysis showed membrane bone formation and endochondral bone. A mass of chondrocytes appeared in bony callus of the OVX group at days 21 and 28 after operation. ②The results of immunohistochemical staining showed that mean absorbance(A) of BMP-2 was higher in the control group than the OVX group at days 7 and 14,but lower at day 21. ③The findings of RT-PCR revealed that BMP 2 level was higher in the control group than the OVX group at day 7,but lower at day 14. CONCLUSION:BMP-2 plays a key role in the early phase of bone callus formation during fracture healing. The declined and delayed expression of BMP-2 in osteoporotic rats may be an important reason for delayed fracture healing in osteoporotic rats.
4.Detection of peripheral blood NK cell subsets in patients with malignant hematological diseases

Objective To study the clinical value of detection of peripheral blood natural killer(NK) cells subsets in patients with malignant hematological diseases.Methods A total of 100 patients with malignant blood disease were enrolled from March 2014 to April 2016 as the observation group,including 50 cases in acute stage and 50 cases in mild stage.At the same time,100 health subjects were enrolled as the control group.The number of NK cells in peripheral blood and the changes of subgroups were compared.Results The number of NK cells in the different groups were compared,the absolute numbers and relative numbers of NK cells in the acute phase of leukemia and lymphoma were much lower than those in the control group(P<0.05).While the absolute number and relative number of NK cells in mild stage of leukemia and lymphoma were higher than that in acute phase(P<0.05).After treatment,the absolute number and relative number of NK cells in patients with leukemia and lymphoma were higher than before treatment(P<0.05).Before treatment,levels of CD56bright and CD56dim in patients with leukemia and lymphoma were statistically different with those of control group(P<0.05).After treatment,CD56bright and CD56dim levels were significantly improved in both groups(P<0.05).Conclusion The number of peripheral blood NK cells in patients with hematological malignancies could be reduced,and the cells function might be compromised.Detection of NK cell subsets could be with guiding significance in the treatment of malignant hematologic disease.

Objective To evaluate the value of urinary normal and modified nucleosides in the diagnosis of primary gallbladder carcinoma.Methods Between March 2011 to September 2012,28 patients with primary carcinoma of the gallbladder (PCG) were included in this study.Spontaneous urine samples were collected and 10 kinds of urinary nucleosides in the sample were determined by reversed-phase high performance liquid chromatography method.Another 62 patients with benign gallbladder disease and 70 healthy volunteers were enrolled as controls.Routine clinical tumor markers,including serum CA19-9 and CA125 levels of the PCG patients were also evaluated.Results 10 kinds of nucleoside had a good linear correlation (r>0.99),and the recovery rate was between 87.4% ~121.5% range,and the accuracy rate was between 87.7% ~121.3%,and the RSD of precision was between 1.5%~10.5% range.In addition to adenosine and cytidine,the mean levels of the rest of the urinary nucleosides in the PCG group were much higher than those of the controls (P<0.01).Based on principal component analysis,89.3% of the PCG patients were correctly identified,which was much higher than those detected by CA19-9 (60.7%) and CA125 (67.9%) (P<0.01).Conclusion Urinary nucleosides may become additional tumor markers which when combined with other clinical methods may help in the screening and early diagnosis of primary gallbladder cancer.
8.The construction of bioartificial liver by BMSC and alginate scaffold

Objective To construct the bioartificial liver by bone mesenchymal stem cell (BMSC) and alginate scaffold. Method Alginate scaffold was used as the cell carrier for the cultivation of BMSC and the differentiation from BMSC into hepatic like cells was induced by the cell factors of HGF, EGF and FGF-4 in the scaffold in vitro. The compatibility of the cells and the scaffold was observed by microscopy and the function of the differentiatd cells was tested. The gene of AFP and ALB was detected by RT-PCR. The secretion of ALB and the urea synthesis of the cells were tested by ALB kit and urea kit respectively. The glycogen synthesis and the CK-18 was tested by the glycogen stanning method and the immunofluorescence test. Results BMSC was able to attach, grow and proliferate well in the alginate scaffold, the well compatibility was observed by microscopy. ALB and urea were detected in the cultivating medium, the gene of ALB and AFP was identified by RT-PCR. The glycogen synthesis ability and the expression of CK-18 were induced during the differentiation. Conclusion The three dimensional atginate scaffold exhibited well compatibility with BMSC, BMSC could be differentiated into the hepatic like cell in the scaffold. BMSC and the alginate scaffold could be used to construct the bioartificial liver for the hepatic tissue engineering.
9.Alterations of calcium channel gene expression and function in rat ventricular myocytes infected by virus

AIM: To investigate the changes of the L-type calcium channel subunit expression and calcium currents (I_~Ca -L) in cultured rat ventricular myocytes infected by coxsackie virus B_3 (CVB_3). METHODS: Primary cultured neonatal rat ventricular myocytes were infected with CVB_3. The changes of L-type calcium channel subunits mRNA in normal and infected myocytes were measured by semi-quantitative reverse transcription-polymerase chain reaction. I_~Ca -L was recorded in two groups respectively using whole cell patch-clamp techniques. RESULTS: The expression of ?_1 and ? subunits of L-type calcium channel mRNA increased in the infected group compared with the normal one (4.00?0.07 vs 2.21?0.41, P0.05). 10.Narrow-band imaging system with magnifying endoscopy in differentiating colorectal neoplasms from non-neoplasms

Objective To observe the meshed capillary pattern(CP)on the surface of colorectal lesions by narrow-band imaging system with magnifying endoscopy(NBI-ME),and to distinguish neoplasm from non-neoplasm by the change of capillary patterns.Methods A total of 144 colorectal lesions in 102 patients detected by conventional colonoscopy were evaluated by NBI-ME to observe the CP on surface,and by staining magnifying colonoscopy to observe the pit pattern.Results All lesions were resected endoscopically (129/144)or by surgery(15/144),and the pathological evaluation diagnosed 30 cases of non-neoplasm (including 20 cases of hyperproliferative polyps and 10 of inflammatory polyps)and 1 14 cases of neoplasm (including 95 cases of adenoma and 19 cases of adenocarcinoma).The diagnostic accuracy rate,sensitivity and specificity of conventional colonoscopy were 75.7%,85.1%and 40.O%,respectively,which were significantly lower than those of NBI-ME and staining magnifying colonoscopy(P<0.005),while there was no significant difference between NBI-ME and staining magnifying colonoscopy.The CP of type Ⅰ,Ⅱ,Ⅳ and Ⅵa were totally correspondent with pit pattern of type Ⅰ,Ⅱ,Ⅳ and ⅤI. Conclusion NBI-ME findings of colorectal lesions correlated with those of staining magnifying colonoscopy.These two techniques are both helpful in differentiating colorectal neoplasms from non-neoplasms.
