859 patients with chronic hepatitis B virus (HBV) infection had followed from February 1997 to September 1998. The natural history of chronic HBV infection was described: Seroconvertion from HBeAg to anti-HBe is 9.6% per year; in which patients with high level of ALT the rate seroconvertion is 15.8%. The highest rate of spontaneous seroconversion occurred in the 20 to 40 age groups. The rate of seroconversion per year of HBsAg was low, about 1.2%; Patients with HBeAg positive may suffer from chronic hepatitis B at the rate of 16.8%. Cirrhosis and hepatocellular carcinoma commontly happened in the patients over 40 years old

BACKGROUND: These days the Hepatitis B virus (HBV) DNA quantitation is the common tool for defining the viral replication state and antiviral therapeutic effect. However, it has a slow turn around time due to a labour intensive complex procedure. The HBeAg assay is a simple, rapid test, but it gives only qualitative information. However, Elecsys 2010 immunoanalyzer (Roche Diag-nostics GmbH, Germany) gives not only qualitative information but also semiquantitative information. So, we want to know about the clinical utility of HBeAg semiquantitation on Elecsys 2010. METHODS: HBeAg and HBV DNA were measured serially for 18 months by Elecsys 2010 and the Hybrid Capture System (HCS; Digene Corporation, Gaithersburg, USA) respectively to evaluate their general correlations (n=287) and serial-changing patterns after lamivudine treatment (n=30, 12-18 months) in 41 patients positive for HBeAg and HBV DNA. RESULTS: Positive correlation was found between the semiquantitative results of HBeAg and the quantitative results of HBV DNA (R=0.56, P<0.05). At two and five months after lamivudine treatment, significant differences (P<0.05) between HBV DNA disappearance groups (group 2, 3) and the HBV DNA remnant group (group 4) were found in the HBeAg level and a decreasing rate. The serial HBeAg and HBV DNA level and the decreasing rate showed a similar pattern in the HBV DNA disappearance groups (group 2, 3). However, a markedly delayed decreasing pattern in the HBeAg level was observed in the HBV DNA remnant group, when compared with HBV DNA. CONCLUSIONS: HBeAg semiquantitation using Elecsys 2010 would provide a cost effective, fast and valuable, and supplementary monitoring tool when the result might be reported in quantity in a dynamic range, because HBeAg semiquantitation could provide an outline of the HBV replication state HBeAg positive patients and a therapeutic predictive value in HBeAg positive, lamivudine treated patients.

BACKGROUND: After an infection with HBV, HBsAg is the first virologic marker detectable in the serum. If anti-HBs against 'a'determinant of HBsAg appears, HBsAg will disappear and the patients will recover from the HBV infection in most cases. However, we encounter not infrequently concomitant cases of HBsAg and anti-HBs. In this study we evaluated HBV DNA levels in concomitant cases to aid in the interpretation of these serologic results. METHODS: This study included 36 cases with positivity for both HBsAg and anti-HBs in an electrochemiluminescent immunoassay as well as a radioimmunoassay. They were tested for HBeAg, anti-HBe, and HBV DNA levels. RESULTS: Chronic viral hepatitis was the most frequent diagnosis (15/36 : 41.7%) and AST and ALT levels were normal in 17 (47.2%) and 20 (55.6%) cases, respectively, among total 36 concomitant cases. HBeAg was positive in 24 and anti-HBe in 17 cases. HBV DNA was positive in 33 cases (91.7%). including all 24 HBeAg positive cases and 9 (75%) of 12 HBeAg negative cases; 6 (50%) of 12 HBeAg negative cases had HBV DNA levels higher than 105 copy/mL. CONCLUSIONS: This study showed that viral replication still exists in most cases of concomitant HBsAg and anti-HBs, and even in some HBeAg negative cases. So in the concomitant cases, HBV DNA quantitation may aid in the interpretation of clinical significance of these cases.
