Objective To explore the application effect of implementing the accountability nursing mode in pediatric surgical ward.Methods On the basis of the transformation of nursing concept and the improving of understanding of High Quality Nursing Service Demonstration Project activities,strengthening basic nursing,establishing two-level nurse management framework,implementing the accountability nursing mode.The satisfaction degree of children's parents before and after the implementation was compared.Results The satisfaction degree of children's parents after the implementation of accountability nursing mode significantly improved.Conclusions After implementing accountability nursing mode in pediatric surgical ward,the satisfaction degree of children's parents improved,the harmonious development of nurse-patient relationship was promoted,the professional identity of nursing personnel was enhanced,and guaranteed the continuous improvement of quality of care.

Objective To evaluate regional and global left ventricular systolic function after coronary artery bypass grafting (CABG) by speckle tracking imaging (STI).Methods Transthoracic echocardiography (TTE) was performed in 32 patients with coronary heart disease (CHD) preoperative and 3 months after CABG.The two-dimensional data were obtained in apical 4-chamble,2-chamble and long axis view.Peak systolic longitudinal strain (SLs),global longitudinal systolic strain (GLS) and average global strain(GLS-Avg) were measured using STI.30 healthy volunteers were involved as controls.Results SLs in the control group increased from the basal to the apical ventricular.SLs in the control group and CHD group apical segment were significantly higher than the basal and middle segments.Compared to the control group,SLs,GLS in part of myocardial ischemic segments were lower in CHD group before CABG (P ＜0.05).SLs and GSL in the corresponding segments showed significantly improving 3 months after CABG in CHD group (P ＜ 0.05).There was good relationship between GSL-Avg and left ventricular ejection fraction.Conclusions STI technology can be used to objectively and quantitatively evaluate the change of regional and global left ventricular systolic function.Therefore,it provide a non-invasive means for evaluation of curative effect and prognosis judgement of CHD after CABG.

To investigate the protective effect of hirudin on kidneys of diabetic rats and its molecular mechanism.

Eighteen Sprague-Dawley rats were randomly divided into control group (n=5) and model group (n=13). The model group was injected intraperitoneally with streptozotocin(STZ) to establish a diabetic rat model. Eight weeks after STZ injection, successfully modeled diabetic rats were randomly divided into diabetic model group (n=6, with 1 rat dead due to hyperglycemia) and hirudin treatment group (n=5). The hirudin treatment group was administered 5 U hirudin while the control group and diabetic model group were injected subcutaneously with equal volume of phosphate buffered saline once daily for six weeks. During the experiment, the rats' body weight and random blood glucose levels were monitored every two weeks. After 6 weeks of treatment, renal function parameters were measured, and the levels of tumor necrosis factor α (TNF-α), transforming growth factor β1 (TGF-β1), interleukin 6 (IL-6), fibronectin (FN), nephrin, collagen-Ⅳ (COL-Ⅳ), and proteins related to the Janus kinase 2 (JAK2)/signal transduction and transcriptional activator 3 (STAT3) signaling pathway were assessed. Renal histopathological changes were also observed.

Compared with the control group, the diabetic model group showed significantly elevated random blood glucose and renal function parameters during the 6-week treatment (both P < 0.05), along with a significant decrease in body weight(P < 0.05) and renal pathological damage. Compared with the diabetic model group, the renal function parameters of hirudin treatment group were significantly decreased (P < 0.05), the renal pathological damage was ameliorated. Compared with the control group, the expression levels of FN, COL-Ⅳ, TNF-α, TGF-β1, IL-6, p-JAK2, and p-STAT3 in the kidneys of diabetic model group were significantly increased (all P < 0.05), while the expression of nephrin was reduced (P < 0.05). Compared with the diabetic group, the expression level of FN, COL-Ⅳ, TNF-α, TGF-β1, IL-6, p-JAK2, and p-STAT3 in hirudin treatment group were significantly reduced (all P < 0.05), while the nephrin expression increased (P < 0.05).

Hirudin improved renal pathological changes in diabetic rats, and its mechanism may be related to the decrease of fibrosis-related factors and inhibition of the activation of the JAK2/STAT3 pathway.

Objective To explore anti-EV71 effects of Ganlu Xiaodudan in vitro. Methods Ribavirin was taken as control drug, with the help of cell culture to observe anti-EV71 inhibition rate of Ganlu Xiaodudan in inhibiting-virus-directly experiment, therapeutic-inhibiting-virus experiment, preventive-inhibiting-virus experiment and preventive-therapeutic-inhibiting-virus experiment. Results In inhibiting-virus-directly experiment, therapeutic-inhibiting-virus experiment and preventive-therapeutic-inhibiting-virus experiment, virus inhibition rate of Ganlu Xiaodudan was higher than ribavirin. In preventive-inhibiting-virus experiment, virus inhibition rates of Ganlu Xiaodudan and ribavirin both were almost zero. Conclusion Ganlu Xiaodudan has better antiviral effects on EV71 than ribavirin, and it can affect more than one link of multiplication of EV71.

OBJECTIVETo observe improved effects of Jingjin acupuncture on fine activity of hemiplegic hand in recovery period of stroke.

METHODSFifty cases were randomly divided into an observation group and a control group, 25 cases in each one. Regular western medicine treatment, rehabilitation training and regular acupuncture (in which Shuigou (GV 26), Baihui (GV 20), Neiguan (PC 6), etc. were selected) were applied in both groups. Additionally, muscles in palm side of affected hand, dorsal metacarpophalangeal joints and proximal interphalangeal joints were treated with acupuncture in the observation group, once every other day and electroacupuncture was applied when arrival of qi was acquired. Baxie (EX-UE 9) in the affected hand were needled in the control group, and electroacupuncture was added when arrival of qi was acquired. Ten days of treatment was considered a treatment course, and after two courses Lindmark score, Brunnstrom movement function grade, joint range of hand and Barthel index (BI) were observed in two groups.

RESULTSCompared before the treatment, the Lindmark score in two groups were both improved after the treatment (both P < 0.01). Compared with the control group, the motor coordination ability, sensory function and total score of Lindmark in observation group were obviously improved (differences before and after treatment: 8.24 +/- 3.07 vs 6.84 +/- 2.43, 3.52 +/- 2.33 vs 2.16 +/- 2.12, 11.76 +/- 3.55 vs 9.00 +/- 3.62, all P < 0.05). The Brunnstrom movement function grade was significantly improved in both groups after treatment (both P < 0.01), which was more obvious in the observation group (P < 0.05). The joint range of hemiplegic hand was improved in both groups after treatment (both P < 0.01), which was more obvious in the observation group [differences before and after treatment: (25.35 +/- 10.91) degrees vs (18.65 +/- 7.86) degrees, p < 0.05]. The score of BI was also significantly improved after treatment in two groups (both P < 0.01).

CONCLUSIONThe Jingjin acupuncture could effectively improve fine activity of hemiplegic hand in recovery period of stroke prove daily life ability.

OBJECTIVETo investigate the dosage-efficacy/toxicity relationship of prepared rhubarb, in order to explore the bidirectional effects in hepatoprotection and hepatotoxicity of prepared rhubarb and the objective authenticity for attenuating toxicity by processing.

METHODNormal and pathological animals were adopted simultaneous to investigate the effect of total extracts from prepared rhubarb within a high dose range (2.0, 5.4, 14.7, 40.0 g x kg(-1) x d(-1)) on normal state, biochemical index and histopathology of experimental animals. The factor analytic approach was used to analyze the dosage-efficacy/toxicity relationship of prepared rhubarb.

RESULTThe factor analytic approach was used to extract two common factors from the nine biochemical indexes. The firs common factor was mainly dominated by HA, LN and TGF-β1, and could be explained as fibrotic factors. The second common factor was mainly dominated by ALT, AST and ALP, and could be explained as cellular factor. The results of the factor analysis suggested that prepared rhubarb showed significant bidirectional effects in hepatoprotection and hepatotoxicity, which could protect liver in CC14 injured chronic hepatic injury, but had a certain hepatotoxic effect to normal animals. The pathological examination showed consistent results with the factor analysis. Under comparable dosages, prepared rhubarb showed a stronger liver protecting effect than crude rhubarb, with a lower toxicity.

CONCLUSIONAlthough prepared rhubarb has a certain hepatotoxic effect to normal animals, it has also a significant therapeutic effect to animals with liver injury. The results proved the symptom-based prescription theory and the scientificity of the symptom-based medication. The symptom-based prescription theory is important to correctly realize the dosage-efficacy/toxicity relationship of traditional Chinese medicines and guide the symptom-based medication.
