1.Surgery treatment of 146 cases of clavicle fracture with internal fixation

[Objective]To explore clinical class,surgical treatment and clinical results of clavicle fracture with internal fixation.[Method]A retrospective analysis of 146 cases of clavicle fracture was reported.Totally 146 cases of clavicle fracture accepted by our department from January of 1996 to June of 2006 were carried out the operation using plate shaft.There were 98 males and 78 females.Aged from 8 to 84 years,average of 38.6 years.The average age of the patients at surgery were 38.6 years(ranged 8 to 84 years).There were 9 cases in proximal 1/3,115 cases in middle 1/3 and 12 cases in distal 1/3 of clavicle,accompanied by 7 cases of brachial-plexus injury,4 cases of vascular injury,16 cases of another fracture.There were 7 cases of old clavicle fracture.Surgical methods: Both proximal 1/3 and middle 1/3 of clavicle fracture were treated by AO reconstruction plate or similar plate.Fracture pieces of clavicle were enlaced with screws or steel wire before fixation with plate.The distal of clavicle fracture should be fixed with 3 screws.The distal 1/3 of clavicle fracture with acromioclavicular luxation was treated by reconstruction plate or c clavicle hook plate.Patient could exercise their shoulder joint at 48 hours postoperatively.Post operative evaluation: Clinical results of clavicle fracture with internal fixation were evaluated with appearance,fracture healing time, shoulder joint function and living quality around fracture.[Result]Patients were followed up more than 6 months after operation.Adopt Neer standard to evaluate the patient's function after the operation.There were excellent results in 96 cases and good in 50 without injury of subclavicular nerve and blood vessel,infection,loose and prolapse of internal fixation devices and fracture disunion and malunion and macrosis bony callus.X-ray shows clavicle fracture healing completely.The patients can get painless free-running.[Conclusion]Internal fixation for treatment of clavicle fracture is characterized by little wound,firm fixation and early movement of the joint and is an effective method to cure clavicle fracture.
2.Investigation and Evaluation of Selenium Nutrition in Huaibei Inhabitants

Objective To evaluate the selenium nutrition level of Huaibei inhabitants and to supply valid data to Chinese Microelement Nutrition Database. Methods 513 healthy people living in Huaibei for more than 10 years were selected randomly from September 2003 to June 2004. The selenium content in hair was measured by catalytic polarographic method to delimit selenium richness region. According to the selenium content in hair,the intake of selenium from food was estimated. The investigation results were evaluated by the standard of Chinese Dietary Reference Intakes. Results The selenium content in hair was (0.390 3?0.104 5) ?g/g in Huaibei inhabitants,only 51.9% of the people reached the proper level. The average intake of selenium of Huaibei inhabitants was (31.92?9.93) ?g/d,which showed that the selenium nutrition level of Huaibei inhabitants was lower than the standard of Chinese Dietary Reference Intakes except children between 0 to 6 years old. The percent which satisfied RNI and EAR standard occupied 19.9% (102/513) and 32.9% (169/513) respectively,which were also much lower than the 50% of physiological necessary level. Conclusion The selenium intake from food in Huaibei inhabitants was scarce obviously. To increase the selenium intake properly will be a wise selection.
3.Mediastinal Tumors:Imaging Characters

Objective To study the value of imaging characters in diagnosing mediastinal tumors.Methods X-ray and CT findings of pathologically proved mediastinal tumors in 58 cases were analysed,the findings enabling qualitative diagnosis were recommended. Results There were 40 cases of anterior mediastinal tumors including intrathoracic thyroid tumors [n=32,5 tyroid carcinomas and 1 thyroid cyst ,thymomas (n=3) and teratomas (n=5, 3 cases ruptured)]. All the other 18 cases in posterior mediastinum were neurogenic tumors.Conclusion Based on the combination of the mediastinal compartment knowledge, X-ray and CT findings of the tumors and the clinical information, the qualitative diagnosis of mediastinal tumors will be characterized correctly.
4.Effects of Nitric Oxide on Growth and Metastasis of Tumor

Objective To study the effects of nitric oxide (NO) on the growth and metastasis of tumor.Methods The literatures of recent years were reviewed.Results NO had double effects on the growth and metastasis of tumor. NO promoted the growth and metastasis by regulating the expression of tumor proliferation gene and inducing tumor angiogenesis. On the other hand, NO had antitumor effects by interfering with the metabolism of tumor cells, inducing the damage of DNA, forming high toxic free radical, inducing apoptosis of tumor cells and mediating the antitumor action of endothelial cells and macrophages.Conclusion Selective blockage or induction of synthesis of NO may be a new way for tumor therapy.
5.Therapeutic Observation of JIN's Three-needle Therapy in Improving Intelligence in Mental Retardation as An Assistant Treatment

Objective To explore the effect of Jin's three-needle therapy, as an assistant treatment, in improving the intelligence of children with mental retardation (MR).Method Seventy-six MR patients were divided into a control group and an observationgroup, 38 cases in each group.The control group was treated with rehabilitation, while the observation group with JIN's three needle therapy plus the rehabilitation training.Prof. JIN Rui's 10-item observation index was adopted to evaluate the memory ofthe patients before and after the treatment; Gesell Developmental Schedule (GDS) was used to measure the intelligence before and after the treatment. The clinical efficacies in the two groups were statistically analyzed.Result After the treatment, Prof.JIN Rui's 10-item index scores dropped obviously and the GDS scores increased significantly in both groups, while the decrease and increase of scores in the observation group were more significant than those in the control group (P<0.05). The total effective rate was 94.7% in the observation group, significantly higher than 71.1% in the control group (P<0.05).Conclusion As an assistant treatment, JIN's three-needle therapy can help rehabilitation training to improve the intelligence of MR patients.

The development of economics and medical science will surely make the conception of medical ethics changing constantly, in return the formation of new medical ethics may improve the development of medical science. The society requires that the medical students of new century should become personals of high quality having both ethics and abilities and undertaking medical practice law-abiding. The teachers must renew their conception of medical ethics and pay attention to the infiltration of medical ethics in clinical teaching. The author will make a research through six aspects in the strengthening and infiltrating of medical ethics in clinical teaching so as to find the way of developing by the society qualified medical personals needed.

8.Clinical values of B-ultrasound,CT,MRCP and ERCP in diagnosing cholangiocarcinoma:a meta-analysis

Objective To systematically assess the values of B-ultrasound(US),computed tomography(CT),magnetic resonance cholangiopancreatography(MRCP)and endoscopic retrograde cholangiopancreatography(ERCP)in diagnosing cholangiocarcinoma(CCA).Methods All published literatures on bile duct cancer were collected from the literature databases of PubMed,Medline,Embase,CNKI and Wanfang.Then the patients with bile duct cancer suspected by US,CT,MRCP and ERCP were performed the systematic review.The evaluation indicators included sensitivity and specificity.The summary receiver operating characteristic(SROC)curve was adopted to obtain the area under the curve(AUC)and Q values.The diagnostic values of 4 kinds of method were analyzed.Results Forty-four articles were included,involving 4 364 patients.The pooled sensitivities of US,CT and MRCP were 0.75,0.82 and 0.85 respectively,the specificities were 0.72,0.80 and 0.87 respectively(sensitivity and specificity of ERCP cytology and image were 0.62,0.98;0.88,0.84),the Q values were 0.690 8,0.787 8 and 0.841 7(0.846 1,0.833 2 in ERCP cytology and image).Conclusion The accuracy of CT,MRCP and ERCP for diagnosing bile duct cancer is similar and higher than that of US,which is an effective assisted examination method when compared with US.The pooled specificity of ERCP cytology is obviously higher than that of other examination method and has high clinical diagnostic value in the patients with highly suspected bile duct malignant tumor.
9.Myocardial protective effects of rosuvastatin in patients with unstable angina pectoris undergoing coronary stent implantation

Objective To investigate the myocardial protective effects of rosuvastatin in patients with unstable angina pectoris undergoing coronary stent implantation.Methods Patients (90 cases) with unstable angina pectoris undergoing coronary stent implantation in No.215 Hospital of Shanxi Nuclear Industry from January 2014 to May 2016 were divided into observation group and control group,45 cases in each group.Patients in the observation group were treated with rosuvastatin,and patients in the control group were treated with atorvastatin.The levels of CK-MB,cTnⅠ,h-FABP,hs-CRP,TNF-α,INF-γ,IL-6,IL-10,and IL-18 were compared between two groups.Adverse drug reaction and major adverse cardiac events of two groups were recorded.Results After 24 h operation,the levels of CK-MB,cTnⅠ,hs-CRP,TNF-α,INF-γ,IL-10,and IL-18 of two group were significantly higher than those before operation (P < 0.05),and those in observation group were significantly lower than control group (P < 0.05).There was no difference in h-FABP between two groups before and after operation (P < 0.05).The level of IL-6 in the observation group after operation was higher than that before operation,but the difference was not significant (P < 0.05);The level of IL-6 in the control group was significantly higher than that before operation and that of observation group (P <0.05);The rates of drug adverse reactions and major adverse cardiac events were similar in two groups.Conclusion Compared with same doseatorvastatin,the effects of rosuvastatin in muscle damage and inflammatory response are better,and do not increase adverse reactions.
10.Enhancement of cisplatin -induced apoptosis in HL-60 cells by human telomerase reverse transcriptase antisense oligodeoxynucleotide

AIM: To explore the effects of the combined use of human telomerase reverse transcriptase (hTERT) gene antisense oligodeoxynucleotide (ASODN) and cisplatin on apoptosis of HL-60 cells. METHODS: PCR-ELISA was used to determine telomerase activity in HL-60 cells untreated or treated with ASODN, the expression levels of hTERT protein were assayed by immunofluorescence using fluoresce isothiocyanate (FITC) label. Apoptosis was detected by DNA electrophoresis and flow cytomertric cell cycle analysis. RESULTS: The expression levels of hTERT protein and telomerase activity in HL-60 cells treated with ASODN were shown to decrease with time, the DNA fragments were obviously found and the percentage of apoptosis cells was significantly enhanced in HL-60 cells with the combined use of hTERT ASODN and cisplatin compared with the combined use of hTERT sense oligodeoxynucleotide and cisplatin or cisplatin alone, respectively. CONCLUSION: Inhibition of telomerase activity by hTERT ASODN increases the cisplatin-induced apoptosis of HL-60 cells.
