Iodine is an essential trace element for growth and development of fetus. More and more surveys currently show that the urinary iodine level of pregnant women is between 100 - 149 μg/L, which is lower than the level recommended by the World Health Organization (WHO). Under this level of iodine nutrition, different investigations have shown different effects on thyroid function of pregnant women, and the impact on growth and development of the fetus requires long-term objective evaluation. At present, there are three aspects: development, intelligence quotient (IQ), and psychological behavior. There are not many studies on comprehensive evaluation. This article combs the related published research to provide a reference for further clarifying the influence of iodine nutrition level during pregnancy on thyroid function of pregnant women and the growth and development of their offspring.
3.One case of pediatric obstructive sleep apnea hypopnea syndrome with pulmonary hypertension.

The clinical manifestation included snoring and mouth breathing for 2 years, repeated coughing and shortness of breath in action for more than 1 year. Physical examination of oral cavity showed tonsils were in grade III. The endoscopy showed 2/3 of postnaris were blocked by the adenoids. The preoperative ultrasonic cardiogram revealed the right atrial and right ventricular dilatation, pulmonary artery widened. The preoperative polysomnography (PSG) showed apnea-hypopnea index (AHI) was 28.5 events an hour, and the lowest oxygen saturation (LSaO2) was 39%. The patient was diagnosed as severe obstructive sleep apnea hypopnea syndrome with pulmonary hypertension. The postoperative PSG showed the AHI was 11.7 events an hour, and the LSaO2 was 86%. The ultrasonic cardiogram at 5 months after surgery didn't show any abnormalities.

4.ISLAND SKIN FLAP WITH SIMPLE VENOUS PEDICLE:AN EXPERIMENTAL STUDY IN RATS

126 adult male Wistar rats were used.A 4?6 cm~2 standard island skin flap wasraised on one side of the abdominal wall of the rats.In the flaps of the experim-ental group,only the superficial epigastric and lateral thoracic vein were preserved.The process of revascularization,histological changes,glycogen content and lactatedehydrogenase (LDH) activity of the flap were observed postoperatively with Indian ink injection,X-ray,microscopy and microspectrometry.The shrinking rateand degree of edema of the experimental flap were also observed and all comparedwith those of the control flaps.The results indicated that the flaps of experimentalgroup had a higher survival rate and lower degree of edema,and its glycogencontent and LDH activity decreased markedly on the lst postoperative day,andincreased gradully afterwards.Revascularization started on the 2nd postoperative day,such as capillary anastomoses passing through the cut edges of the flap,vesselsentering the flap from the recipient bed,vessels becoming tortous,dilated,andmarkedly proliferated.The results suggest that there was a true plasmatic circulationat the early stage in the simple venous flap and turns to a normal vascular type byrevascularization.The vividness of a flap may be indicated by the variance of themetabolism of carbohydrate in the skin flap.
5.New Insights into Restrictive Transfusion Strategy in Gastrointestinal Bleeding

Gastrointestinal bleeding is a common acute and critical illness that may result in hemorrhagic shock.In conventional concept, sufficient blood transfusion to ensure volume resuscitation was necessary for this emergency condition.With the propose and application of restrictive transfusion strategy in clinical practice, the new concept has been applied in patients with gastrointestinal bleeding.Restrictive transfusion strategy could maintain the perfusion of vital organs with a minimized blood transfusion, and avoiding the coagulopathy and acidosis caused by massive transfusion.This article reviewed the advances in study on gastrointestinal bleeding and restrictive transfusion strategy.
6.The Management of the Intractable Exposure of Hydroxyapatite Inplants by Transposition of Conjunctiva-müller Muscular Flap

ObjectiveTo investigate a repair method for intractable exposure of hydroxyapatite implants.MethodsAt the upper edge of conjunctival defect,the conjunctival tissues were separated by scissors along the surface of exposed base plate of ocular prosthesis to make a conjunctival flap in the size as same as the exposed area.This procedure of separation was continued between the müller muscle and lavator through the fornix till the upper edge of tarsal plate to form a flap consisting of conjunctiva,aponeurosis and müller muscle,which was transposed to cover the defect area.Results5 patients with intractable exposure of hydroxyapatite implants treated with the above procedure,no case was found to be recurred in 5 to 6 months of follow-up.ConclusionThe transposition of conjunctiva-müller muscular flap is an effective method for the management of the intractable exposure of hydroxyapatite implants
7.Epidemiological characteristics of pulmonary tuberculosis in Ningbo Cityfrom 2011 to 2020

Objective:
To investigate the epidemiological characteristics of pulmonary tuberculosis in Ningbo City from 2011 to 2020, so as to provide the evidence for developing the control measures of pulmonary tuberculosis.
Methods:
The data pertaining to pulmonary tuberculosis cases in Ningbo City from 2011 to 2020 were collected from the Tuberculosis Management Information System of the Chinese Disease Control and Prevention Information System. The temporal distribution, population distribution, regional distribution and case report by hospitals were descriptively analyzed.
Results:
Totally 38 305 pulmonary tuberculosis cases were reported in Ningbo City from 2011 to 2020, with an incidence rate of 48.71/105. The incidence of pulmonary tuberculosis appeared a tendency towards a decline ( P<0.05 ), with an annual reduction rate of 5.29%. There were 15 784 smear-positive cases ( 41.21% ), and the proportion of smear-positive cases appeared a tendency towards a reduction from 2011 to 2015 ( P<0.05 ), and a rise from 2016 to 2020 ( P<0.05 ). The number of reported pulmonary tuberculosis cases peaked from March to May (10 973 cases, 28.65%), and a higher incidence rate of pulmonary tuberculosis was found in men than in women ( 64.55/105 vs. 32.20/105, P<0.05 ). The highest incidence of pulmonary tuberculosis was found in cases at ages of 15 to 25 years ( 72.93/105 ), followed by in cases at ages of 65 years and greater ( 69.97/105 ), and farmers were the predominant occupation among the pulmonary tuberculosis patients ( 12 279 cases, 32.06% ). During the period from 2011 to 2020, pulmonary tuberculosis cases were reported across the 10 districts or counties of Ningbo City, and the top three high incidence of pulmonary tuberculosis were seen in Yinzhou District ( 59.91/105 ), Fenghua District ( 56.19/105 ) and Beilun District ( 56.01/105 ). The proportion of pulmonary tuberculosis cases reported by designated pulmonary tuberculosis hospitals increased from 72.16% in 2011 to 99.74% in 2020 in Ningbo City.
Conclusions
The incidence of pulmonary tuberculosis appeared a tendency towards a decline in Ningbo City from 2011 to 2020, and the annual decline rate meets the requirements of the WHO strategic goal of ending pulmonary tuberculosis; however, the control measures for pulmonary tuberculosis require to be intensified among men, youths, and farmers.
8.Observation of the therapeutic effect of ganciclovir in the treatment of children with viral encephalitis

Objective To observe the clinical efficacy of ganciclovir in treatment of children with viral encephalitis.Methods 80 children with viral encephalitis were randomly divided into observation group and control group.All patients received the comprehensive treatment including anti-spasm,dehydration,control of body temperature,decreased intracranial pressure and drugs of improving brain metabolism.40 children in the observation group received intravenous ganciclovir.40 patients in the control group received intravenous ribavirin.The clinical effect of the two groups was compared.Results The total effective rate of observation group was 92.5%,that of the control group was 70.0%,the difference was statistically significant (x2 =6.721,P < 0.05).In the observation group,the main clinical symptoms including fever,headache,vomiting,convulsions and the disappearance time of unconsciousness were significantly shorter than the control group(t =5.270,9.730,5.749,5.903,5.421,all P < 0.05).The incidence rate of adverse reactions in the observation group was 7.5%,which was lower than 17.5% in the control group,but the difference was not statistically significant (all P > 0.05).Conclusion Ganciclovir in the treatment of children with viral encephalitis can improve clinical symptoms in a short time,and adverse reactions are mild,which can be used as the ideal choice for treatment of children with viral encephalitis.
9.Risk factors of peripherally inserted central catheter occlusion in adults

The peripherally inserted central catheter occlusion is not rare in adults.Its occurence is related with the catheter's tip position,catheter type and the skills of medical staff.Personnel training is particularly important in the prevention of occlusion.Urokinase and heparin can be used for thrombolysis.
10.Effect of different concentrations of fetal bovine serum on bone marrow mesenchymal stem cell purity and cycle

Objective To compare the influence of three kinds of complete media with 0. 10 ,0. 15 ,0. 20 fetal bo-vine serum( FBS) on purity and cycle of rat bone marrow mesenchymal stem cells ( BMSCs) cultured in vitro and to seek suitable FBS concentration for the cultivation of the stem cells. Methods SD rats were executed by cervical dislocation method and used whole bone marrow adherence to isolate rat BMSCs. Experiment was divided into A, B,C,3 groups. Compared the expression of CD45, CD29, CD90, CD44 in the three groups in 2,3,4,5 passages ( P2 , P3 , P4 , P5 );the cells of P3 in group A were digested and cultured in three different concentration of com-plete culture media for four days, measuring cell cycle in 24,48,72,96 h by flow cytometry instrument. BMSCs of P3 were collected and inoculated to 6 pieces of 96-well plates, then vaccinated with complete media with 0. 10, 0. 15,0. 20 FBS in every plate, one culture plate was taken out for optical density(OD) meaturement every day with CCK-8. Results CD45 was negative, CD29, CD90, CD44 were positive. The difference of BMSCs surface markers cultured in the three kinds of complete media was bigger in the first two passages, but the difference was less in P3 , P4 , all could obtain pure BMSCs in P4 relatively;according to the results of the cell cycle at the same time, G0/G1 phase:with the increase of concentration of fetal bovine, G0/G1 phase reduced and had no diference in A, B, C groups;G2/M phase:there was difference between them after 24 h(P<0. 05,F=12. 412), but with the extension of time, the differences disappeared;S phase:there was no difference in the three groups;S+G2/M phase increased with the concentration of FBS. According to the result of cell vitality, the OD of ABC three groups increased in turn in 24 h, and there was difference(P<0. 05,F=5. 002), but with the extension of time, there was no obvious difference between them. Conclusion The three kinds of culture media in P4 can obtain pure BM-SCs, cell cycle and vitality show three complete media can promote the growth of BMSCs. There is no difference between them. Culture media with 0. 10 FBS can satisfy the isolation and amplification of BMSCs, in order to ob-tain pure BMSCs in the short term, using culture media with 0. 15 and 0. 10 FBS in the primary culture and subcul-ture respectively.
