1.Pharmacokinetic Study of Magnolin in Rats

OBJECTIVE:To study the pharmacokinetics of magnolin in rats. METHODS:High performance liquid chromatog-raphy(HPLC)was adopted. The determination was performed on Kromasil C18 with mobile phase consisted of acetonitrile-tetrahy-drofuran-water (39∶1∶60),at the flow rate of 1.0 ml/min,with the measurement wavelength of 278 nm,column temperature of 35 ℃ and the sample size of 20 μl. 8 Wistar rats were docked to collect blood from the caudal vein before administration [10 mg(medicinal materials)/kg] and 0.25,0.5,0.75,1,1.5,2,4,8,12 and 20 h after administration,to determine the concentra-tion of the drug in blood. DAS2.1.1 software was used to calculate pharmacokinetic parameters. RESULTS:In the determination of magnolin,the mass concentration linear range was found to be 0.05-10.00 μg/ml(r=0.999 5);the RSD of precision test and sta-bility test were less than 13%(n=6);relative recovery rate was 97.32%-102.15%(n=6);extraction recovery rate was 84.63%-90.02%;t1/2α of magnolin in rats was(0.48±0.22)h,t1/2β was(7.96±2.57)h,CL/F was(0.09±0.032)L/(h·kg),and AUC0-20 h was(944.43±212.83)mg·h/L. CONCLUSIONS:This method conforms to the requirements for the determination of bio-logical samples with respect to precision,stability and accuracy. Magnolin demonstrates a good linear relationship between AUC0-20 h and the dose in rats,with the process compatible with two-compartment model.
2.Determination of Contents and Related Substances in Loxoprofen Sodium Tablets by RP-HPLC

OBJECTIVE:To improve the determination method for the contents of main components and related substances in Loxoprofen sodium tablets. METHODS:RP-HPLC method was adopted. The determination was performed on Inspire C18 column with mobile phase consisted of acetonitile-0.01 mol/L potassium dihydrogen phosphate(containing 0.2% triethylamine,phosphoric acid adjusted to 3.0±0.1,62 : 38,V/V)at the flow rate of 1.0 mL/min. The column temperature was 40 ℃,and the detection wave-length was set at 221 nm. The sample size was 20 μL. RESULTS:The peak of loxoprofen sodium was well separated with the peak of its related substances(R>1.5). The linear range of loxoprofen sodium ranged 30.0-90.0 μg/mL(r=0.9998). The detection lim-it of loxoprofen was 0.3 μ g/mL. RSDs of precision,stability and repeatability tests were <1.0% . The average recovery rates ranged 99.00%-99.87%(RSD=0.33%,n=9). CONCLUSIONS:This method is accurate,simple,rapid and suitable for the quali-ty control of Loxoprofen sodium tablets.
3.Inducing bone marrow mesenchymal stem cells to differentiate into cardiomyocyte-like cells via Wnt-11 in vitro

Objective To probe into the optimal concentration of Wnt-11 to induce the differentiation of bone marrow mesenchymal stem cells (BMMSCs)into cardiomyocyte-like cells in vitro.Methods BMMSCs were isolated from the bone marrow of SD rats using whole bone marrow culture method.After cultured for 48 h, BMMSCs of the second generation were utilized for directed induction.Based on the final concentration of Wnt-11 , BMMSCs were divided up into Group A (100 ng/mL),Group B (200 ng/mL),Group C (400 ng/mL)and Group D (blank control).After 72-hour induction,the cells were cultured in complete medium for 4 weeks while cells in Group D were cultured only in the complete medium.The morphological changes were observed under the phase contrast microscope.Surface antigen expression of BMMSCs was identified by flow cytometry.When cells were cultured for 4 weeks,the expressions of Desmin,Connexin43 and cTnI were detected by immunocytochemistry. Meanwhile, the ultrastructural changes were observed using transmission electron microscope. The mRNA expressions of cardiac transcription factors GATA-4,Nkx2.5 andα-MHC in BMMSCs were detected by RT-qPCR at 1,2 and 4 weeks after induction.Results Primary BMMSCs formed cell colonies at 2 weeks;the cells were mainly fusiform or star-shape,and a few irregularly-shaped ones were also visible.The passaged cells were larger than those of primary culture.After induction,the cells exhibited long shuttle-shape and were aligned in parallel. Flow cytometery displayed that the positive rate of the surface antigens of BMMSCs CD29,CD45,and CD90 was 97.9%,0.4% and 99.5%,respectively.When BMMSCs-induced via Wnt-11 were cultured for 4 weeks,Desmin, cTnI and Connexin43 were all positively expressed in induction groups.Whereas in the blank control group they were slightly positive or negative;the positive rate in Group B was the highest (P<0 .05 ).Transmission electron microscopy exhibited that organelles such as rough endoplasmic reticulum,mitochondria,as well as some ribosomes were visible in the cytoplasm of these cells in each induction group.In addition,myofilaments were arranged in parallel in the cytoplasm.The cells in induction groups could express GATA-4 and Nkx2 .5 in the first week,and then the expression of them decreased in the second week,but then increased in the fourth week;gene expression in induction Group B was significantly higher than in the other two induction groups (P<0 .05 ).The expression of GATA-4 and Nkx2 .5 in Group D was 1 ,α-MHC was not expressed in the four groups during the culture period. Conclusion Wnt-11 can induce the differentiation of BMMSCs into cardiomyocyte-like cells in vitro,and the optimal concentration of Wnt-11 is 200 ng/mL.
4.NPC1L1 contributes to the inhibition of lipid accumulation induced by ezetimibe in RAW264.7 cells

Aim To investigate the effect of intestinal cholesterol absorption inhibitor Ezetimibe on lipid accumulation in RAW264.7 cells and identify the underlying mechanism.Method RAW264.7 cells were pretreated with the indicated concentrations of Ezetimibe (0,0.003,0.01 and 0.03 mol·L~(-1))for 24 hours or pretreated with the optimal concentration(0.03 mol·L~(-1))of Ezetimibe for different periods (0,6,12 and 24 h),followed by incubation with 50 mg·L~(-1) oxLDL for 24 hours,then the number of intracellular lipid droplets and lipid content were measured by using oil red O staining and HPLC; the expression of NPC1L1 was measured by Western blot.Results Pretreatment with indicated concentrations of Ezetimibe caused a concentration-dependent inhibition of intracellular lipid accumulation;pretreatment with 0.03 mol·L~(-1) Ezetimibe caused a time-dependent inhibition of intracellular lipid accumulation.It was noted that pretreatment with 0.03 mol·L~(-1) Ezetimibe for 24 hours inhibited CE by about 47%+0.1% compared with control group(oxLDL alone).Immunoblotting results showed that NPC1L1 was expressed in RAW264.7 cells and it was down-regulated after Ezetimibe treatment.Conclusions Ezetimibe causes concentration-dependent and time-dependent inhibition of lipid accumulation in RAW264.7 cells;it also reduces NPC1L1 expression in RAW264.7 cells.
5.Association between glycated hemoglobin and plaque characteristics in patients with non-ST-elevation acute coronary syndrome

Objective To explore the association between glycated hemoglobin (HbA1c) level at admission and coronary plaque characteristics under intravenous ultrasound (IVUS) study in patients with non-ST-elevation acute coronary syndrome (NSTE-ACS). Methods 118 patients with NSTE-ACS were divided into the low(HbA1c ≤ 5.6%), medium(HbA1c 5.7%-6.4%)and high(HbA1c ≥ 6.5%)level groups based on admission HbA1c. IVUS was performed in all target lesions. Results As compared with the other two groups, patients with high level HbA1c had higher mean body weight index, higher co-morbidities of diabetes mellitus, dyslipidemia and non-ST-elevation acute myocardial infarction.The high HbA1c level group had more diffuse coronary atherosclerosis ,increased plaque burden and higher rates of positive remodeling, soft plaque and plaque rupture. Conculsions The HbAlc level of admission is associated with plaque vulnerability in NSTE-ACS patients.
6.Relationship of body mass index and blood lipid level with cancer detection on prostate biopsy

Objective To investigate the relationship between body mass index (BMI), blood lipid level and cancer de?tection in prostate biopsy. Methods A total of 214 patients undergoing a prostate biopsy during 2013.2—2014.8 were re?viewed retrospectively. They were divided into prostate cancer and non-cancer groups by biopsy results. The differences of age, prostate-specific antigen (PSA) level, prostate volume (PV), blood lipid level and BMI were analyzed between two groups. Risk factors for cancer detection of biopsy were also analyzed. Results Compared with non-cancer patients, pros?tate cancer patients were older, had higher level of PSA and BMI, but smaller PV and lower level of HDL-C ( P<0.05). Lo?gistic regression analysis showed that older, higher level of PSA and BMI were risk factors for prostate biopsy positive, but larger PV and higher level of HDL-C were protective factors (P<0.05). Conclusion Comprehensive assessment of BMI and blood lipid levels can provide important reference for prostate cancer screening at early time and establishment of pros?tate biopsy scheme, which also provide significant evidence for the prevention, diagnosis and treatment of prostate cancer at early stage for high risk population.
7.Efficacy and Safety of Salmeterol/fluticasone Combined with Montelukast versus Salmeterol/fluticasone in the Treatment of Cough Variant Asthma: A Meta-analysis

OBJECTIVE: To evaluate the efficacy and safety of salmeterol/fluticasone combined with montelukast versus salmeterol/fluticasone in the treatment of cough variant asthma, and to provide reference for evidence-based reference in clinic.METHODS: Retrieved from Cochrane library, PubMed, Chinese Journal Full-text Database, VIP and Wanfang database, randomized controlled trials (RCTs) about therapeutic efficacy (total response rate, marked improvement rate, cough disappearance time, 6-12 months recurrence rate) and safety (the incidence of ADR) of salmeterol/fluticasone combined with montelukast (trial group) vs. salmeterol/fluticasone (control group) in the treatment of cough variant asthma were included. Meta-analysis was performed by using Rev Man 5. 3 software after data extraction and quality evaluation with Cochrane system evaluation manual 5. 1. 0. RESULTS: Totally 10 RCTs were included, involving 976 patients. Results of Meta-analysis showed the total response rate [RR=1. 22, 95%CI(1. 16, 1. 29), P<0. 001] and marked improvement rate [RR= 1. 38,95%CI(1. 22,1. 56),P<0. 001] of trial group were significantly higher than those of control group; the cough disappearance time was significantly shorter than control group [MD= - 3. 07, 95% CI (- 3. 54, -2. 59),P<0. 001],and 6-12 months recurrence rate was significantly lower than control group [RR=0. 24, 95%CI(0. 11, 0. 54), P<0. 001], with statistical significance. There was no statistical significance in the incidence of ADR between 2 groups [RR=1. 58, 95% CI (0. 99, 2. 51), P=0. 05]. CONCLUSIONS: The salmeterol/fluticasone combined with montelukast is better than of salmeterol/fluticasone in the treatment of cough variant asthma, but great importance should be attached to the occurrence of adverse events when using drug combination.
8.Factors and prognosis analysis of graft renal function recovery after DCD donor renal transplantation

Objective To analyze the factors and prognosis of graft recovery after donation after citizens death (DCD) donor renal transplantation.Methods A retrospective analysis of 67 cases of DCD renal transplantation from August 2012 to September 2015 in our hospital was carried out.According to the stability of renal function after operation,the patients were divided into group A (51cases) with stable renal function,and 16 cases in group B (delayed graft function after operation).The clinical data of two groups including age,gender,cause of death,warm ischemia time,type of dialysis,and application of norepinephrine before operation were collected and analyzed.The related factors of graft function recovery were analyzed.Logistic regression analysis was used to analyze the risk factors of graft functional recovery after operation.The 3-month,6-month,1-year and 18-month survival rate after operation in the two groups was compared.Results The causes of death,the time of ischemia,the type of dialysis before operation,the application of norepinephrine before operation,infants and young donors were related factors of graft function recovery (P < 0.05).Logistic regression analysis showed that cerebral hemorrhage death donor,the long thermal ischemia time,the preoperative hemodialysis and the application of norepinephrine before operation were the risk factors of delayed graft function recovery (P<0.05).The 3-month,6-month,1-year and 18-month survival rate after operation in group A was higher than that in group B,with the difference being statistically significant (P<0.05).Conclusion Cerebral hemorrhage death donor,the long thermal ischemia time,the preoperative hemodialysis and the application of norepinephrine before operation were the independent risk factors of delayed graft function recovery.And the prognosis of patients with delayed graft function was poor.Clinical risk should be eliminated or reduced in clinical practice,which can effectively prevent the delayed graft function and further improve the prognosis of the patients.
9.Pretemporal transcavernous approach tailored surgery of cavernous sinus tumors: a consecutive series of 31 cases report.

OBJECTIVETo investigate the indications of the pretemporal transcavernous approach for cavernous sinus tumors resection and design individually tailored surgery according to the extent of tumors and operation requirements.
METHODSA retrospective analysis of clinical data, surgical outcomes and complications in a series of 31 cases with cavernous sinus tumor operated via the individually tailored pretemporal transcavernous approach between May 2012 and September 2015 in Department of Neurosurgery, Xiangya Hospital, Central South University. There were 13 male and 18 female patients, aging from 17 to 67 years with a mean of (41±14) years. The patients included 18 cases of shwannomas, 4 cases of meningiomas, 3 cases of cavernous hemangiomas, 2 cases of invasive pituitary adenomas, 1 case of chordoma, 1 case of chondroma, 1 case of recurrent teratoma, 1 case of metastatic nasopharyngeal carcinoma. The first followed-up visit was on the 3(rd) month after surgery, and if tumor progression or recurrence was observed on MRI, the Gamma knife treatment was recommended, the patient was followed up every 6 months, otherwise the patient was followed up again 6 months later, then, every 12 months.
RESULTSGross total removal of tumors was achieved in 22 cases of 31 patients (71.0%), containing 17 cases of shwannomas, 3 cases of hemangiomas, 1 case of chondroma, 1 case of teratoma; subtotal removal in 6 cases (19.3%), including 3 cases of meningiomas, 1 case of pituitary adenoma, 1 case of chordoma, 1 case of metastatic carcinoma; partial removal in 3 cases (9.7%), comprising 1 case of meningioma, 1 case of recurrent shwannoma, 1 case of recurrent pituitary adenoma. The symptoms of cranial never aggravated in 5 cases, the new postoperative cranial never palsy was observed in 7 cases. There was no surgical mortality, intracranial hematoma, intracranial infection and cerebrospinal fluid leakage cases, ect. Twenty-eight cases were followed up for more than 3 months (3 to 40 months), 1 case of chordoma had tumor progression; the nerve function was restored in 5 cases, among the 12 cases with postoperatively new occurred or deteriorated cranial nerve paralysis.
CONCLUSIONSThe pretemporal transcavernous approach can be used to resect tumors limited in cavernous sinus or tumors simultaneously involving the cavernous sinus and its vicinity areas, it can be individually tailored based on the extent and exposure of the tumor. This approach can improve the surgical results in terms of high tumor resection rate, less complication, is an ideal approach for cavernous sinus tumor resection.

10.One case report of aspergillus lumbar spine infection diagnosed by metagenomic next-generation sequencing after renal transplantation and literature review

Objective:To explore the application and value of metagenomic next-generation sequencing (mNGS) in refractory infection after organ transplantation.Methods:A case report discussed about a patient with lumbar spine infection after kidney transplantation and the relevant literature was reviewed. The recipient was a 63-year-old man with low back pain after kidney transplantation. Lumbar spine magnetic resonance imaging showed lumbar spine infection. Multiple operations plus antibacterial and antituberculosis treatments were ineffective. Before and after treatment, numerous tests of traditional pathogenic microorganisms failed to detect any positive bacteria.Results:The detection of lumbar secretion by mNGS suggested aspergillus infection. The symptoms improved after dosing of voriconazole.Conclusions:The incidence of fungal infection of lumbar spine is low. The imaging manifestations are non-typical so that it is easy to misdiagnose. mNGS helps to timely diagnose and guide treatment. With a review of the literature, mNGS has some application value for some difficult and rare infectious diseases.
