BACKGROUND: Mesh plug tension-free hernioplasty is commonly used for inguinal hernia. In the operation, different mesh materials can be selected, among which the modified Kugel mesh is extensively applied. OBJECTIVE: To observe the biocompatibility of modified Kugel mesh in mesh plug tension-free hernioplasty for inguinal hernia. METHODS: Total y 79 cases of inguinal hernia were treated by mesh plug tension-free hernioplasty. The patients were randomly divided into observation group repaired with Bard mesh (modified Kuge) (n=38) and control group repaired with Prolene mesh (n=41). After 12-month fol ow-up, levels of interleukin 6, C-reactive protein and tumor necrosis factor α were detected and compared before and after treatment;adverse events, including wound infection or infection caused by the mesh in the two groups were recorded. RESULTS AND CONCLUSION: Levels of interleukin 6, C-reactive protein and tumor necrosis factor α in the two groups were significantly decreased after treatment than before treatment (P < 0.05). No wound infection or infection caused by the mesh appeared in both two groups. Although scrotum effusion occurred in two cases of each group, their outcomes al improved significantly after active puncture and suction treatment without other serious problems. No recurrence was found during the 12-month fol ow-up. These results show that the modified Kugel mesh has good biocompatibility in mesh plug tension-free hernioplasty for inguinal hernia.

To understand the genetic origin and variation of porcine epidemic diarrhea virus (PEDV) prevailing in Henan province,a total of 22 PEDV positive samples from suckling piglets with severe diarrhea were collected from 16 pig farms and amplified by RT-PCR.ORF3 genes were then cloned into pMD18-T vector and sequenced. The result of sequencing showed that the length of ORF3 genes all were 675 bp which encoded 224 amino acids. The strains in this study had eight amino acid substitutions when compared with CV777 strain. The nucleotide homologies of the 22 strains were 95.9%-100% and amino acid homologies were 96.4%-100%.By comparing with those of European strain CV777 and vaccine strain truncated CV777,the nucleotide homologies were 97.1%-97.9% and 94.8%-95.4%,respectively.Based on the phylogenetic relationship of ORF3 genes, the PEDV field strains and PEDV reference strains could be divided into two groups, and all the field strains identified in this study belong to group 2.It suggested that prevalent PEDV strains in this study seem to be closely related to Henan isolates in previous years, domestic strains,Janpanese strains,American strains as well as Korean strains but differ genetically from European strains or vaccine strains used in China. This experiment analyzed the ORF3 gene sequence features in prevailing of Henan region in recent years, which provides a new support of epidemiology study and protein function research of PEDV ORF3 gene.

Objective To explore the standard clinical pathway and stratified cost insurance in patients with senile cataract operation. Methods Patients treatment by senile cataract phacoemulsification combined with IOL implantation were selected as research subjects by cluster sample from two hospitals from 2013 to 2014 . Patients were divided into different groups according to the year, chronic diseases, preoperative waiting time. The difference of hospitalization expenses and hospitalization time were compared. Different levels of hospital costs were established by CHAID decision tree used hospitalization cost as target variable. Results The waiting time for operation was 2 days, the total hospitalized time was 5 days and these can be used as reference time in clinical pathway. Patients uninsured or with long waiting time for operation had longer hospitalized time. Chronic disease, terms of payment and waiting time before operation were point factors that effect the hierarchical of decision tree. Conclusion The new standard of clinical path need to adjust constantly. Distinguish different patients condition and pay different clinical path costs.

OBJECTIVETo investigate the effect of ligation level of inferior mesenteric artery (IMA) on postoperative defecation function in patients with rectal cancer.
METHODSA total of 128 rectal cancer patients who were planned to undergo low anterior resection in the First Hospital of Zibo City between January 1, 2012 and December 31, 2013 were prospectively enrolled and randomly divided into IMA high ligation group(63 cases, cutting distance of 1.0 to 1.5 cm to the root of IMA) and low ligation group(65 cases, cutting distance of 0.5 to 1.0 cm to the root of left colic artery originated from IMA). The efficacy, especially the defecation function, was observed and compared 3 months and 1 year after surgery between the two groups.
RESULTSNo significant difference was found in the number of harvested lymph nodes between two groups[8(1-30) vs. 7(2-28), P=0.125], but high ligation group had greater number of metastatic lymph nodes[1(0-9) vs. 0(0-8), P=0.041]. Frequency of defecation in high ligation group was significantly higher than that in low ligation group during postoperative 3-month follow-up[5(2-10)/d vs. 3(1-8)/d, P=0.035], whereas other indexes of defecation function were not significantly different(all P>0.05). The proportion of patients needing laxatives in high ligation group was higher than that in low ligation group during postoperative 1-year follow-up [11.3%(6/53) vs. 1.7%(1/58), P=0.038], whereas other indexes of defecation function were not significantly different as well (all P>0.05). Three cases and 2 cases showed recurrence in high ligation group and low ligation group respectively during postoperative 1-year follow-up without significant difference(P=0.623).
CONCLUSIONLow ligation of IMA in low anterior resection for rectal cancer is beneficial to the protection against defecation function.

Objective To explore the effects of long-term low-protein intake on nonspecific inflammatory responses in the liver and T lymphocytes in the spleen in middle-aged and older mice and underlying mechanisms. Methods Fourteen month-old female KM(Kunming)mice were randomly divided into the control group, the low-protein group, the high-protein group and the high-protein + rapamycin group.Additionally,Two month-old female KM mice served as the adult group and were fed regular chow.Animals in the high-protein + rapamycin group received injections of rapamycin intraperitoneally once every two days.Animals were sacrificed at the end of 3 months.Liver histology slices were prepared for the examination of pathological changes and detection of the expression of CD68 and mTOR(the mechanistic target of rapamycin).Spleen lymphocyte suspensions were prepared to count the percentages of CD4+ T and CD8+ T cells. Results Compared with the control group, the low-protein group and the high-protein-+-rapamycin group in histology slides showed regularly arranged hepatocytes,no obvious sinus hepaticus expansion and only mild time-related changes. Moreover, compared with the control group, the low-protein and high-protein + rapamycin groups were associated with increased percentages of CD4+T and CD8 + T cells in the spleen(all P< 0.05)and decreased expression of CD68 in the liver(P<0.05),though the levels of mTOR were similar among the groups. Conclusions Long-term low-protein intake has beneficial effects in mitigating the reduction of T lymphocytes in the spleen and slowing age-related structural and nonspecific inflammatory changes in the liver, possibly through downregulation of the expression of the mTOR protein.

Objective:To explore the mediating effects of self-efficacy between leadership empowerment behavior and neglect of care among senior care workers. Methods:In this cross-sectional study, from July to September 2020, totally 423 senior care workers from 50 nursing institutions for the aged in Jining, Jiaozuo, and Shijiazhuang were selected by convenient sampling and investigated with the General Information Questionnaire, Leadership Empowerment Behavior Scale, Self-Efficacy Scale, and Nursing Caregiver Negligence Assessment Scale. Pearson's correlation analysis was used to analyze the correlation of each variable, and bootstrapping was employed to test the mediating effect. Results:In this study, 423 questionnaires were distributed and 386 valid questionnaires were recovered. The total score of leadership empowerment behavior of 386 care workers was (47.93±7.51) ; the total score of self-efficacy was (28.65±5.92) ; the total score of neglecting care was (73.04±8.80) . The leadership empowerment behavior, self-efficacy, and neglect of care in the senior care workers were positively correlated ( P<0.01) . Self-efficacy played a partial mediating role between leadership empowerment and neglect of care, and the mediating effect accounted for 19.73%. Conclusions:The neglect of care in the senior care workers is at a low level, and self-efficacy plays a partial mediating role between leadership empowerment and neglect of care. Leaders of nursing institutions for the aged should improve their level of empowerment behaviors and strengthen the self-efficacy of senior care workers, so as to reduce the neglect of care among senior care workers.
