Objective To investigate the influence of psychological intervention in emotional disorder on blood pressure,lipids and glucose in hypertensive patients in community.Methods 599 hypertensive in community were divided to intervention group (n =302) and control group (n =297) by randomization.The control group received routine medication.Additionally the intervention group got another psychological intervention which lasted 6 months.An SCL-90 scale was adopted to evaluate patients'psychological change of mental health,blood sugar,low density lipoprotein cholesterol,total cholesterol,systolic pressure and diastolic blood pressure before and after treatment.Results Somatization,compulsion,interpersonal relationship,depression,anxiety,hostility and terror in intervention group((1.44±0.29),(1.78 ±0.39),(1.66±0.38),(1.63 ±0.48),(1.43 ±0.35),(1.37 ± 0.52) and (1.28 ± 0.41) respectively) had a lower score than control group ((2.01 ± 0.59),(2.04±0.68),(1.88 ±0.38),(1.84±0.59),(1.92±0.54),(1.61 ±0.53)and(1.60±0.50)).The differences were significant(P＜ 0.01).After intervention,blood glueose,low density lipoprotein,total cholesterol,systolic pressure,and diastolic pressure were ((4.65 ± 0.811) mmol/L,(2.97 ± 0.702) mmol/L,(4.86 ± 0.884)mmol/L,(134.11 ± 13.39)mm Hg(1 mm Hg =0.133 kPa) and (93.73 ±7.61)mm Hg respectively) and before intervention((5.09 ± 0.710) mmol/L,(3.23 ± 0.584) mmol/L,(5.36 ± 1.138) mmol/L,(150.13 ± 12.23) mm Hg and (89.64 ± 8.35) mm Hg respectively).The differences were significant (P＜ 0.05).Conclusion Cognitive behavioral therapy can improve community hypertension patients' physiology and psychology,which is good for blood pressure,lipids,glucose control.

DNA polymerase iota (Polt),as well as Revl,Polκ and Polη,are all Y family DNA polymerases,which are able to replicate damaged DNA via translesion synthesis pathway.However,Pol(t) has the lowest fidelity among all DNA polymerases in both correct and inaccurate DNA templates.Also Pol(t) can bypass certain DNA damages and accumulate mutations.Recent studies show that the aberrant expression of Pol(t) is observed in human uveal melanoma,breast cancer,bladder cancer,lung cancer and esophageal cancer,which may contribute to the tumorigenesis and progression of tumor.The special role of Pol(t) in replicating damaged DNA may contribute to the resistance in oncotherapy.

Objective:To examine the preliminary clinical efficacy of Chinese magnetic sphincter augmentation (MSA) in the treatment of gastroesophageal reflux disease (GERD).Methods:According to the enrollment criteria for the MSA developed by ShengJieKang Co. and Shanghai Chest Hospital (SS-MSA) clinical trial, a total of 19 GERD patients were treated with SS-MSA from August 2018 to January 2020 at Department of Thoracic Surgery, Shanghai Chest Hospital, Shanghai Jiao Tong University. The majority of registered cases were male patients with age of (32.2±7.3) years (range: 22 to 50 years), height of (170.7±6.2) cm (range: 160 to 179 cm) and weight of (65.2±10.3) kg (range: 47.5 to 90.0 kg). SS-MSA was implanted via laparoscopy. The major evaluation indexs of postoperative efficacy were the total time of acid exposure within 24 hours and the total number of reflux. Secondary efficacy indicators included: (1) evaluation of the average daily dose of proton pump inhibitor medications; (2) the score of GERD health related quality of life questionnaire (GERD-Q) before and after MSA implantation. Paired design t-test was used to evaluate the efficacy of the SS-MSA. Results:A total of 19 patients underwent SS-MSA surgery successfully. The history of the GERD were 19 (54) months ( M( QR)). The operation time was 63 (22) minutes and the in-hospital stay was 3 (2) days. No obvious surgical complications occurred. Postoperative adverse events included 14 cases with mild to moderate dysphagia exited after surgery, gradually eased within 1 to 3 months, 1 case with the removal of the device after 1 month of severe swallowing difficulties, 1 case of diarrhea. No corrosion, perforation, displacement occurred. The GERD-Q score (11.0(4.5) vs. 6(1.0), t=4.274, P=0.013), 24-hour acid exposure time (6.2(4.8)% vs. 0.1(0.9)%, t=5.814, P=0.004), and Demeester score (23.72(16.20) vs. 0.96(3.10), t=6.678, P=0.003) were significantly decreased 1 year after surgery( n=5). Proton pump inhibitor reuse rates were 6/18, 5/15, 3/10, and 1/5 in 1, 3, 6 and 12 months after the operation, respectively. Conclusions:SS-MSA implantation is feasible and safe with short hospital stay and rare perioperative complications. The preliminary results is good after 1 year follow-up. It could be expected to be an ideal substitutive for future GERD treatment.

Objective    To give an annual data report of surgical treatment of esophageal cancer in Shanghai Chest Hospital to provide reference for treatment and research on esophageal cancer. Methods    The clinical data of 414 patients with esophageal cancer who underwent endoscopy (9 patients) and esophagectomy (405 patients) in Shanghai Chest Hospital in 2015 were reviewed, including 334 males and 80 females. Their tumor biological characteristics and short-term and long-term treatment results were analyzed. Results    Patients in this group were predominantly aged 60 to 69 years (46.9%), and the tumor was mainly located in the middle thorax (50.7%). About 79.9% of the patients received trans-right thoracic esophagectomy, 44.4% received minimally invasive surgery; 388 (93.7%) patients accepted upfront esophagectomy without induction therapy, and 179 (43.2%) patients received postoperative adjuvant therapy. The R0 resection rate was 90.6%, and the 30- and 90-day mortality rates were 1.4% and 2.4%, respectively. The 4-year overall survival rate of the R0 resection patients was 65.7%. Conclusion    Satisfactory long-term survival results can be obtained for thoracic esophageal cancer if R0 resection can be achieved by trans-right thoracic esophagectomy and extended lymphadenectomy combined with appropriate postoperative adjuvant treatment.
