1.Laparoscopic Cholecystectomy Combined with Endoscopic Retrograde Cholangiopancreaticography for Gallbladder and Bile Duct Stones

Objective To investigate the efficacy of laparoscopic cholecystectomy(LC) followed by endoscopic retrograde cholangiopancreaticography(ERCP) for gallbladder and bile duct stones.Methods A total of 58 patients with both gallbladder and bile duct stones received endoscopic sphincterotomy(EST) in our hospital.After the stones were removed,LC was performed on the patients.Results Of the 58 patients,56 were successfully treated with EST and LC.Two cases were unsuccessful owing to difficulty in intubation during ERCP in one and failure to remove all the stones from the bile duct in the other.These two patients were then cured by LC combined with electronic choledochoscopy.Perioperative complications included bleeding during EST in 1,acute edematous pancreatitis in 2,and mild infection of the puncture site in 1.The overall complication rate was 7.1%(4/56).Retained common bile duct stone was observed in 1 patient,and was successfully removed by subsequent ERCP.Conclusions LC combined with ERCP is a safe and effective minimal invasive method for the treatment of concurrent gallstones and bile duct stones.
2.Non-operative treatment for patients with acute uncomplicated appendicitis

Total 786 cases of acute uncomplicated appendicitis(UCAA) were included for analysis.All the cases were given antibiotics as a main treatment.Clinical cure rate and the number of cases requiring surgery were observed.Patients with conservative treatment were followed up and the recurrence was observed.Logistic regression was applied to analyze the risk factors of recurrence.Among 786 cases,774 (98.5%) were treated conservatively and achieved clinical cure;68 cases (9.0%) had recurrence during follow-up,the median time to recurrence was 11 months (1-33 months).Logistic regression analysis showed that age(OR =1.124,95% CI:1.067-3.790,x2 =2.767,P =0.041) and appendix bezoar (OR =1.649,95% CI:1.281-4.055,x2 =3.935,P =0.027) were independent risk factors for recurrence.Non-operative treatment for acute uncomplicated appendicitis shows a good short and long term effects.Surgical excision may be a better choice for the patients with high risks of recurrence.
3.Characterization of programmed cell death 5 (PDCD5) gene in human cartilage and its possible significance

Objective: To characterize the expression of programmed cell death 5 (PDCD5) in normal and osteoarthritic human cartilage. Methods: Articular cartilage specimens were obtained from 20 patients with osteoarthritis and 10 with femoral neck (normal cartilage) at the time of arthroplasty. Expression of PDCD5 was detected by flow cytometry, immunofluorescence, RT-PCR and immunohistochemical analysis. Results: PDCD5 expression in osteoarthritis cartilage was significantly higher than in normal cartilage especially in the nucleus. PDCD5 positive chondrocytes were mainly observed in the superficial and deep zone of osteoarthritis tissue sections,and in contrast, in the superficial and middle regions of normal controls. Conclusion: Since apoptotic chondrocyte death occurs more frequently in osteoarthritis compared to normal cartilage and PDCD5 is an apoptosis related protein, the different expression patterns of PDCD5 in osteoarthritis and normal cartilage suggest that it might be involved in the pathogenesis of osteoarthritis.
4.Cumulative skin dose estimates for the medical diagnostic X-ray workers in Jiangsu province from 1950 to 2011

Objective To provide the dose information from the epidemiological cohort established in 1980 in Jiangsu province using normalized workload method,and to estimate cumulative skin dose to medical diagnostic X-ray workers based on personal dosimeter were worn as members of the epidemiologicat cohort.Methods Data for fixed cohort members of X-ray exposure history from 1950 to June 2011 were collected.According to correction factors such as examination types and protection conditions,exposure doses from 1950s to June of 2011 were estimated through a Visual Basic language program.Results Average annual doses vary significantly between during different time periods:10.1 (6.1-12.3) mGy/year during 1950-1965,3.5 (2.1-5.4) mGy/year for 1966-1984 and 1.3 (0.9-1.9) mGy/year during 1985-2011,respectively.Conclusions Average annual doses received by 3 961 medical diagnostic X-ray workers during different time periods in this investigation are nearly consistent with the results reported previously and show a roughly similar trend.
5.Study of the inhibition by polymorphonuclear leukocytes of TNF-α release from human mononuclear cells and its mechanism

The present study was undertaken to investigate the effect of human PMNs on the production of TNF-α by the human peripheral blood mononuclear cells (PBMCs) and to elucidate its tentative mechanism. Human PMNs and PBMCs were isolated from the venous blood of healthy donors by dextran sedimentation and density gradient centrifugation. In the presence of lipopolysaccharide (LPS), PMNs and PBMCs were cocultured at the ratio of 2:1 for 20 h and the concentration of TNF-α in the supernatant was measured by enzyme-linked immunosorbent assay. The binding rate of monocytes with the fluorescein isothiocyanate-labeled LPS (FITC-LPS) and the mean surface fluorescence intensity of monocytes were analyzed by flow cytometry. Results showed that PMNs were capable of inhibiting the TNF-α release from PBMCs (P<0.05). PMNs suppressed the TNF-α release from PBMCs by 45% on average when PMNs and PBMCs cocultured at the ratio of 2:1. Paraformaldehyde-fixed PMNs still demonstrated the same inhibition (P<0.05),which proved that the inhibition was dependent on cell-to-cell contact and suggested that effector molecules responsible for this effect existed on the cell surface of PMNs. In the presence of PMNs, the binding rate of monocytes with the FITC-LPS and the mean surface fluorescence intensity of monocytes were not affected compared with PBMCs alone (P>0.05). As incubation time was prolonged, the binding of FITC-LPS to monocytes increased (P<0.05). Thus PMNs did not block the binding of LPS with monocytes. It was concluded that PMNs suppressed the TNF-α release from PBMCs via cell-to-cell interaction. In a cell-contact dependent manner, PMNs might interfere with the signal transduction pathway through which LPS activated PBMCs, thus attenuating the response of PBMCs to LPS and downregulating the TNF-α release.
6.Ultra Performance Liquid Chromatography-Tandem Mass Spectrometric Method for Detection of Pork Peptide Biomarkers

The samples of muscular tissue from pork,beef and lamb which were closely related in the genetic relationship were analyzed by ultra performance liquid chromatography-tandem mass spectrometric (UPLC-MS) technique.The specific peptide biomarkers of pig meat species were found and confirmed.Proteins from three pure meat samples were extracted and digested using trypsin,the digested proteins were identified by UPLC-triple time-of-flight (TOF)-MS,and the total ion chromatogram (TIC) was searched and analyzed against the UniProt database.Three high abundant homologous proteins of three species and 8 potential peptide biomarkers of pork were found.A multiple reaction monitoring (MRM) QTRAP-MS method was established to confirm the specificity of potential peptide biomarkers.As a result,five peptide biomarkers of pig species meat were confirmed,three of which were not reported.

8.Development of acupuncture manipulator and its application in animal shock model.

We have developed a mini acupuncture manipulator, which can simulate the traditional needling therapy and can inquired about the acu-related physical parameters which could not be quantitatively studied in the past. For example, we can control the depth, velocity, angle, angular velocity, frequency and interval time of acupuncture. The experimental study on Sprague-Dawley rat's acute hemorrhagic shock model was conducted to compare the curative effect of acupuncture manipulator versus that of electroacupuncture. This experiment demonstrated that the acupuncture manipulator can imitate hand acupuncture in security and can produce the effect similar to that of electroacupuncture. So this mini acupuncture manipulator can be of applications in research, teaching and clinical treatment.

9.Regulation of SOCS-3, OB, GLUT4 and PPARgamma gene expression by insulin and dexamethasone in porcine primary adipocyte.

Swine is an ideal model for diabetes studies. Insulin and insulin resistance are closely related with diabetes. To investigate the effect of SOCS-3 in insulin resistance, porcine primary adipocyte was treated with insulin (100 nmol/L) and dexamethasone (300 nmol/L) to induce insulin resistance. The simi-quantitative PCR results suggested that insulin increased GLUT4, PPARgamma and SOCS-3 gene expression in primary culture porcine adipocytes and no change of OB gene expression. Under insulin resistance conditions, SOCS-3 and OB gene expression were up-regulated, whereas GLUT4 and PPARgamma gene expression were down-regulated in primary porcine adipocytes. The overexpression of PPARgamma gene resulted in the increase of GLUT4 expression by insulin. Different expression levels of SOCS-3 determined the inhibitory effects of insulin signaling. Induction of insulin resistance by dexamethasone was not only due to inhibition of glucose transportation, but also repression of insulin signaling. SOCS-3 might be a potential gene to block the insulin resistance.

10.Inhibitory effect of 15-oxospiramilactone on renal cell carcinoma 786-0 cells in vitro

Objective Wnt signaling plays an important role in the development and progression of renal cell carcinoma (RCC).This study aimed to evaluate the effects of the Wnt signaling inhibitor 15-oxospiramilactone on the proliferation , migration, cell apoptosis, and cycles of the human RCC cell line 786-0, and to investigate the possible mechanisms of this small molecule acting on RCC in ivtro. Methods We treated 786-0 cells with DMSO ( blank control group ) and 15-oxospiramilactone at the concentrations of1.25μmol/L (low 15 -OSL), 2.5μmol/L (medium 15-OSL), and 5μmol/L (high 15-OSL), respectively, for 72 hours.Then we observed the changes in the proliferation and migration of the 786-0 cells by MTT and scratch-wound assay and determined their apopto-sis and cycles by Annexin V-FITC/PI assay and flow cytometry . Results 15-oxospiramilactone significantly inhibited the growth of the 7860-cells, with the IC 50of 1.088 μmol/L at 72 hours, and decreased their migration distance (P<0.05).After 36 hours of treatment, the apoptosis rates of the 786-0 cells in the low, medium, and high 15-OSL groups were (12.17 ±0.56), (18.54 ± 1.07), and (50.74 ±1.28) %, respectively, significantly increased as compared with (7.85 ±0.42) %in the blank control group (P<0.05), and in an obviously concentration-dependent manner.15-oxospiramilactone remarkably reduced the number of cells in the G0/G1 phase and increased that in the G 2/M phase (P<0.05). Conclusion 15-oxospiramilactone can significantly inhibit the pro -liferation and migration and induce the apoptosis of 786-0cells in vitro.It may be a potential anti-RCC agent.
