1.On the advantage of RBL teaching mode for medical students in undergraduate years and in foreign academic exchanges

Research based learning (RBL) is a brand new teaching mode implicated by Shanghai Jiaotong University School of Medicine specifically designed for clinical undergraduates.The nature of RBL is about exploring unknown knowledge and designing and executing comprehensive experiments.RBL aims at creating an open,active student participation,and close student-mentor interaction teaching mode.IBL includes literature study,research aim selection,experiment design and implementation,statistical analysis,results and conclusions,final report writing and defense of the report.Our experience indicates that RBL can substantially improve students' scientific logic and critical thinking and experimental skills,and develop the spirit of a team-player.This scientific training enables students to receive more comprehensive scientific training,facilitate their subsequent clinical research and practice,and assist them to participate in more international scientific exchanges.We propose that the RBL mode should be adopted for clinical student education in other universities.
2.Association of single nucleotide polymorphism in interleukin 6 gene with lumbar intervebral disc disease

AIM: To explore the association of single nucleotide polymorphisms of promoter G~ -597 -A, G~ -572 -C, G~ -174 -C and T~ 15 -A, C~ 132 -T in exon 5 of interleukin 6 and lumbar intervebral disc disease. METHODS: The single nucleotide polymorphisms of interleukin 6 gene, including polymorphisms of G~ -597 -A, G~ -572 -C in promoter, G~ -174 -C and T~ 15 -A, C~ 132 -T in exon 5 were analyzed by the polymerase chain reaction and sequencing methods in 81 cases with lumbar intervebral disc disease and 101 healthy controls. The association of single nucleotide polymorphisms of interleukin 6 gene with lumbar intervebral disc disease in two groups was measured. The association of single nucleotide polymorphisms of interleukin 6 gene with lumbar intervebral disc degeneration in those younger than 45-year-old were also measured. RESULTS: The G~ -572 -C polymorphism of interleukin 6 gene was observed, but no single nucleotide polymorphism of G~ -597 -A, G~ -174 -C in promoter and T~ 15 -A, C~ 132 -T in exon 5 in two groups was detected. There was no difference between the distribution of the G~ -572 -C polymorphism of interleukin 6 gene in two groups. In those younger than 45-year-old the association of the single nucleotide polymorphism of interlukin-6 gene and lumbar intervebral disc degeneration was not significant. CONCLUSION: There is G~ -572 -C polymorphism in Chinese. No relation between G~ -572 -C polymorphism of interleukin 6 with lumbar intervebral disc disease and lumbar intervebral disc degeneration was observed.
3.Antitumoral bibenzyl derivatives from tuber of Arundina graminifolia.

OBJECTIVETo isolate the bibenzyl derivatives from the tuber of Arundina graminifolia and evaluate the anti-tumor activity of these compounds in vitro.
METHODThe constituents have been extracted by 95% alcohol and then isolated by column chromatography on silica gel and Sephedax LH-20. The structures were determined by UV, IR, NMR and MS spectral analysis.
RESULTSix constituents have been isolated, and their structures have been established as 2,7-dihydroxy-1-(p-hydroxylbenzyl)-4-methoxy-9, 10-dihydrophenanthrene (1), 4,7-dihydroxy-1- (p-hydroxylbenzyl)-2-methoxy-9,10-dihydrophenanthrene (2), 3, 3'-dihydroxy-5-methoxybibenzyl (3), (2E) -2- propenoic acid-3-(4-hydroxy-3-methoxyphenyl) -tetracosyl ester (4), (2E) -2-propenoic acid-3- (4-hydroxy-3- methoxyphenyl) -pentacosyl ester (5) and pentadecyl acid (6), respectively.
CONCLUSIONAll compounds except for 3 were isolated from the tuber of A. graminifolia for the first time. Compound 3 with bibenzyl ring opening exhibits stronger anti-tumor activity than that of compounds 1 and 2 with bibenzyl ring closing.

4.Measurement of body weight, blood parameters and main organ coefficients of germ-free piglets

Objective To establish the background information of physiological parameters for germ?free ( GF ) Taihu piglets. Methods In this study we selected 25 days old GF Taihu piglets and 4 conventional ( CV) littermates, the male and female ratio was 1∶3, to measure the normal clinical values of hematology and serum biochemistry, immunoglobu?lin concentration and main organ coefficients. The analysis of relative growths of main organ weight to body weight was con?ducted in the Taihu GF and CV pigs by allometric scaling model. Results (1) Twelve hematological parameters and 8 blood biochemical parameters in the GF piglets were significantly lower than those in CV pigs (P<0?05). (2) The aver?age body weight, IgM concentration of GF pigs and CV pigs had significant difference ( P <0?05 ) , and no mesenteric lymph nodes were found in the GF pigs. (3) The gut weight had the largest allometric association with body weight in the GF pigs, while spleen weight has the largest allometric association with body weight in the CV pigs. Both the weight of heart and stomach in CV and GF pigs had a negative allometric association with body weight (allometric coefficient b<1), respectively. Conclusions Different microbe control grades affect the body weight, hematology and serum biochemistry, expression of immunoglobulin and development of main organs in laboratory pigs.
5. Correlation between FOXO3 gene polymorphisms and susceptibility to occupational noise-induced deafness

Objective:
To investigate the association between the single nucleotide polymorphisms of rs12212067 in FOXO3 gene and the susceptibility to occupational noise-induced deafness in a Chinese Han population.
Methods:
A total of 1 066 cases of noise exposure workers from a large chemical fiber factory in Jiangsu Province were selected as the study subjects. All subjects’ basic data and field exposure data were collected through questionnaires and occupational health surveys. The subjects were divided into case group (531 persons, double ear high frequency average hearing threshold>25 dB) and control group (535 persons, double ear high frequency average hearing threshold≤25 dB) according to their results of pure tone hearing test .2ml fasting venous blood was collected for DNA extraction and genotyping was performed by TaqMan-PCR technique.

6.The effect of reduction and in situ fusion on postoperative imaging parameters of degenerative lumbar spondylolisthesis

Objective:To compare the efficacy of reduction and in situ intervertebral fusion fixation in the treatment of degenerative lumbar spondylolisthesis.Methods:A total of 182 patients (92 males and 90 females) with L 4 degenerative lumbar spondylolisthesis of Meyerding's classification of grade I and grade II, aged (62.6±6.8) years (range, 57-73 years), who underwent posterior L 4, 5 internal fixation and interbody fusion in the Department of Spinal Surgery, the Second Hospital of Shanxi Medical University, were retrospectively analyzed from January 2019 to December 2022. There were 105 cases of I-degree spondylolisthesis and 77 cases of II-degree spondylolisthesis. According to the operation method, the patients were divided into reduction intervertebral fusion fixation (reduction group) and in situ intervertebral fusion fixation group (in situ group). Imaging parameters such as lumber lordosis (LL), pelvic incidence (PI)-LL, L 3, 4 intervertebral space heights, fusion segment angle, and sagittal vertical axis (SVA) were measured on the pre- and post-surgical lumbar spine lateral radiographs. The visual analogue scale (VAS) and Oswestry Disability Index (ODI) of low back pain were recorded before and after surgery. The differences in clinical and imaging parameters were compared between reduction and in situ fusion group. Results:All 182 patients successfully completed the surgery and were followed up for 12.0±2.4 months (range, 9-15 months). The LL of the reduction group before surgery, immediately after surgery, and at the last follow-up were 46.9°±7.1°, 57.2°±5.9°, 55.6°±5.5°, respectively, with statistically significant differences ( F=87.61, P<0.001), with immediate and final follow-up being smaller than those in the in situ fixation group. The LL of the in situ fixation group before surgery, immediately after surgery, and at the last follow-up were 47.8°±7.2°, 50.5°±7.0°, and 48.7°± 6.4°, respectively, with no statistically significant difference ( F=2.83, P=0.062). The immediate and final follow-up of LL in the reduction group was lower than those in the in situ fixation group ( P<0.05). The fusion segment angles of the reduction group before surgery, immediately after surgery, and at the last follow-up were 14.2°±5.1°, 23.2°±4.7°, 23.2°±4.7°, respectively, with statistically significant differences ( F=152.87, P<0.001), with immediate and final follow-up after surgery being greater than before surgery. The fusion segment angles of the in situ fixation group before surgery, immediately after surgery, and at the last follow-up were 15.4°±5.9°, 18.2°±5.5°, and 17.4°±5.1°, respectively, with statistically significant differences ( F=4.69, P=0.009), with immediate and final follow-up being greater than before surgery. The fusion segment angulation in the reduction group was greater than that in the in situ fixation group at both the immediate and final follow-up ( P<0.05). The SVA of the reduction group before surgery, immediately after surgery, and at the last follow-up were 16.9±18.2 mm, 9.5±12.0 mm, and 8.7±11.3 mm, respectively, with statistically significant differences ( F=11.32, P<0.001), with immediate and final follow-up being smaller than before surgery. The SVA of immediately after surgery and at the last follow-up were both smaller than before surgery. The SVA of the in situ fixation group before surgery, immediately after surgery, and at the last follow-up were 16.4±17.2 mm, 14.3±15.5 mm, and 13.8±15.0 mm, respectively, with no statistically significant difference ( F=0.57, P=0.576). The SVA of the reduction group at immediate and final follow-up was lower than that of the in situ fixation group ( P<0.05). Conclusion:Both reduction and in situ intervertebral fusion fixation can effectively relieve the clinical symptoms of patients. Fusion fixation after reduction can improve the angulation of fusion segments to form segmental kyphosis, which is more conducive to improving SVA.
7.Clinical Application Advances of Antipsychotic Drugs in Female Schizophrenic Patients

Female patients with schizophrenia exhibit a slower rate of absorption and excretion,leading to higher blood levels of antipsychotic drugs in their bodies and consequently causing more drug side effects.Women also have different levels of estrogen at different stages of life,and estrogen improves the efficacy of antipsychotic drugs by increasing dopamine D2receptor sensitivity.It is necessary to adjust the dosage of antipsychotic medication based on the level of estrogen,while also monitoring indicators such as blood glucose,triglycerides,cholesterol,and prolactin to minimize medication side effects.Nevertheless,the current literatures do not provide specific treatment plans for schizophrenia tailored to different genders,which is not the optimal treatment strategy for female patients.This review aims to summarize and categorize the differences in prescription,pharmacokinetics,pharmacodynamics,efficacy,and side effects in female patients with schizophrenia,and to provide scientific recommendations for drug treatment strategies in different stages of a woman's life(premenopausal,pregnancy,lactation,postmenopausal).
8.Study on HPTLC identification of one-plate multi-drug and HPLC content determination method for Jizhi syrup

OBJECTIVE To optimize the existing t hin layer chromatography (TLC)identification and content determination methods of Jizhi syrup. METHODS High performance thin-layer chromatography (HPTLC)was used to identify five medicinal materials in Jizhi syrup ,such as Ilex chinensis ,Houttuynia cordata ,Peucedanum praeruptorum ,Citrus aurantium ,Glycyrrhiza uralensis. High performance liquid chromatography (HPLC)was used to determine the contents of procatechuic acid ,ephedrine hydrochloride and naringin in Jizhi syrup. RESULTS HPTLC results showed that the identification spots of pedunculoside , praeruptorin A ,naringin,and liquiritin were clearly displayed ,and the retention factors were in the range of 0.2 to 0.8. After validation,the method had been proved to be strongly specific ,robust and repeatable. HPLC results showed that the linear ranges of protocatechuic acid ,ephedrine hydrochloride and naringin were 4.32-431.67,1.14-114.17 and 7.02-702.33 μg/mL(all r> 0.996),respectively. The average recoveries were 100.61%,100.40% and 99.22%,and RSDs were all less than 2.00%. RSDs of precision(n=6),stability(24 h,n=7)and repeatability (n=6)were all less than 2.00%. The average contents of the three components in 10 batches were 623.3,152.1,1 213.9 μg/mL(RSD<10.00%),respectively. CONCLUSIONS In this study , HPTLC method of one-plate multi-drug is established for the identification of Jizhi syrup. One sample pretreatment method and two TLC conditions are used to realize the rapid identification of five kinds of medicinal materials. An HPLC method is established to determine the content of Jizhi syrup ,which realizes the fast quantification of three active components in Jizhi syrup ,and can be used to optimize the identification and content determination items in the existing legal quality standards of Jizhi syrup.
9.Effect of Jianpi-bushen-huoxue Decoction Combining with Kinesiotherapy on Circulation in Ischemic Area of Patients with Cerebral Infarction: A Perfusion Weighted Imaging Study

Objective To explore the effect of Jianpi-bushen-huoxue Decoction combining with kinesiotherapy on blood perfusion in the ischemic area in patients with cerebral infarction. Methods From May, 2016 to May 2017, 65 spleen-kidney-deficiency patients with cerebral infarction at convalescence were randomly divided into group A (n=22, accepting routine treatment), group B (n=22, accepting routine treatment and Jianpi-bushen-huoxue Decoction) and group C (n=21, accepting routine treatment, Jianpi-bushen-huoxue Decoction and kinesiotherapy), and were observed for 14 days. On the first day and the 14th day of treatment, they were scanned with perfusion weighted imaging, and the regional cerebral blood flows (rCBF) of region of interest were measured.Results rCBF increased after treatment in each group (t>3.572, P<0.05), and it ranked as group C, group B and group A with the rCBF from more to less after treatment (P<0.05).Conclusion Jianpi-bushen-huoxue Decoction is effective on blood flow in ischemic area of spleen-kidney-deficiency patients with cerebral infarction at convalescence, and it is even more effective when combining with kinesiotherapy.
10. Investigation on the influencing factors of low back pain in automobile factory workers

OBJECTIVE: To investigate the prevalence and influencing factors of low back pain in workers in automobile manufacturing plants. METHODS: A judgment sampling method was used to select 394 workers with working length ≥1 year in an automobile manufacturing plant as study subjects. A questionnaire survey was conducted to investigate the prevalence of low back pain in these workers using the Nordic Musculoskeletal Disorders Questionnaire. RESULTS: The prevalence of low back pain in these study subjects was 28. 9%( 114/394). The multivariate logistic regression analysis results showed that the relative risk ranked from high to low is as follow: uncomfortable working posture [Odds ratio( OR) =7. 31,95% confidence interval( 95% CI) = 2. 92-18. 28],feeling tired after work( OR = 5. 34,95% CI = 1. 22-23. 32],the commu time more than 30 minutes( OR = 2. 76,95% CI = 1. 38-5. 52),insufficient operating space( OR = 2. 22,95% CI = 1. 28-3. 83),hand or arm frequently used at work( OR = 2. 08,95% CI = 1. 14-3. 78) and age( OR = 2. 00,95% CI = 1. 19-3. 35),that were all risk factors of low back pain( P < 0. 05). CONCLUSION: The frequent use of hand or arm at work,insufficient operating space,uncomfortable working posture,feeling tired after work,and long commute time are the main influencing factors affecting the low back pain of automobile manufacturing workers.
