1.Clinical utility of mammography,ultrasound and ~(18)F-FDG dual-head coincidence imaging for diagnosis of primary breast cancer

Objective To assess the diagnostic value of mammography,ultrasound and 18F-fluorodeoxyglucose(18F-FDG) dual-head coincidence(DHC) imaging in the detection of primary breast cancer. Methods The results of 54 female patients with 57 breast lesion sites examined by mammography,ultrasound and 18F-FDG dual-head coincidence(DHC) imaging were analysed and compared with pathologic findings.The sensitivity of mammography was compared with combined mammography with ultrasound or triple-tests,and the sensitivity of 18F-FDG DHC imaging was compared with combined mammography and ultrasound. Results The individual sensitivities of mammography,ultrasound and 18F-FDG DHC imaging in the diagnosis of primary breast cancer were 89.13%,91.30% and 91.30%,respectively,those for specificities were 72.73%,72.73% and 63.64%,respectively,and those for accuracies were 85.96%,87.72% and 85.96%,respectively.The sensitivity,specificity and accuracy of combined mammography with ultrasound were 100%,63.64% and 92.98%,respectively,and those of triple-tests were 97.83%,81.82% and 94.74%,respectively.Combined mammography with ultrasound and triple-tests were more sensitive than mammography(P0.05).Triple-tests were more sensitive than combined mammography with ultrasound(P
2.Imbalance of MMP-9/TIMP-1 during the cutaneous "underlying disorder" in diabetic rats

Objective To investigate the change of matrix metalloproteinase-9 (MMP-9) and tissue inhibitor of metalloproteinase-1 (TIMP-1) in the skin of diabetic rats, and to explore the potential role of MMP-9/TIMP-1. Methods Diabetic rats were induced with streptozotocin (STZ). Then all rats were maintained for 6 weeks. Routine pathological examination and immnnohistochemistry were made to reveal the histological and cytological appearances. RT-PCR and Western blotting were used to detect the expression of mRNA and protein of MMP-9 and TIMP-1 in the skin. Results Six weeks after STZ treatment, examination of HE-stained skin sections from normal and diabetic animals revealed that the epidermis and dermis layers were thinner in diabetic rats than those in control rats. The skin of diabetic rats showed features of atrophy such as disorganization of connective tissue fiber bundles and enlarged space between collagen fiber bundles. In contrast, thick bundles of connective tissue were observed in the dermis of normal rat skin. In normal skin, cells had a bipolar, spindle-shaped appearance in the thick collagen bundles, while in the skin of diabetic animals the interstitial cells had a rounded, shrunken and crenated appearance. The relative values of expression of MMP-9 in diabetic group were higher than those in normal group with significant difference, however, the relative values of expression of TIMP-I in diabetic group were lower than those in control group. Conclusion The changes in cutaneous histology and cytology appear earlier than skin wound. These "underlying disorders" may be associated with the imbalance of MMP-9/TIMP-1.
4.Efficacy of group cognitive-behavioral therapy for diabetic patients with anxiety and depression

Objective To explore the efficacy of group cognitive-behavioral therapy(GCBT) for diabetic patients with anxiety and depression. Methods In this randomized control study, 42 patients was recruited for treatment of diabetes and allocated them in a randomized manner (1:1) into two groups to receive either 10-weeks GCBT plus regular education or regular education alone.Hamilton Anxiety Scale (HAMA), Hamilton Depression Scale (HAMD), Self-rating Depression Scale(SDS), and fasting blood glucose were measured and evaluated before and after treatment. Objective To study the influence of carboxymethyl-chitosan(CM-chitosan)on chondro- cyte apoptosis induced by recombinant human interleukin-1?(rhIL-1?)and explore its mechanism.Methods Rabbit chondrocytes were isolated and cultured.Chondrocytes were pretreated with different concentrations of CM-chitosan for 1 h,then 10 ng/ml IL-1?were added into the culture medium.After 24 h,the apoptotic rates of chondrocytes were measured by flow cytometry with AnnexinⅤ-FITC and PI staining.The morphology of nuclei was observed by fluorescent microscopy with Hoechst 33342 staining.The mitochondrial membrane po- tentials were tested by confocal laser scanning microsocopy with Rhodamine-123 and ATP contents were mea- sured by luciferase reaction.Results CM-chitosan could inhibit chondrocyte apoptosis and restore the func- tion of mitochondria induced by IL-113 in a dose-dependent manner.Conclusion CM-ehitosan can inhibit chondrocyte apoptosis by protecting mitochondrial function.
6.Theoretical basis of regulating mind and relieving the depressed liver needling method for treatment of depression.

The mechanism of depression attack was studied from the point of view of TCM theory, literature and clinical practice. It is held that the brain and liver are the key zang- and fu-organs for attack of depression, and it is first put forward that dysfunction of mind and the liver failing in soothing and regulating the circulation of qi are the TCM pathogenesis of this disease. Needling method for regulating mind and soothing the liver is established with the acupoints of The Governor Vessel and The Liver Channel selected mainly.

7.Cytochrome C oxidase subunit 1 mutation result in mitochondrial encephaiomyopathy with lactic acidosis and stroke-like episodes

Objective To report a 33-year-old man with post-headache stroke-like episodes,with whom ischemic changes were found in basal ganglia and occipital-temporal lobe and muscle biopsy revealed abnormal mitochondrial structure and function without regular mutations detected in mtDNA.Methods Gene chip technique was used to detect the mutation of whole sequence of mtDNA,and direct sequencing technique was used to confirm the mutations.Results Three mutations were found.A new mutation in the mitochondrial cytochrome C oxidase subunit 1 (MTCO1),a T→C transition at nucleotide position 6253 resulted in conservative methionine transferring to threonine.His mother also held the mutation,which was not found in 98 control samples.So T6253C was considered the nosogenetic mutation.Conclusion This is the first time to report a mutation in MTCO1 responsible of MELAS.
8.Comparison of bone marrow and blood cell morphology between refractory anemia and other anemia disease.

This study was purposed to investigate the cell morphological features of bone marrow and peripheral blood in patients with myelodysplastic syndrome, mainly with refractory anemia, and to compare them with other anemia diseases including chronic aplastic anemia, hemolytic anemia and megaloblastic anemia. The bone marrow and peripheral blood were taken from patients for preparing the smears with Wright staining. 500 karyocytes in bone marrow and 100 karyocytes in peripheral blood were detected, and the features of morbid cells of erythrocyte, granulocyte and megakaryocytic series were observed. The results showed that differences between refractory anemia, chronic aplastic anemias and hemolytic anemia as well as megaloblastic anemia were statistically significant (P < 0.05) in the granules scarce and absence in the intracytoplasm of segmented neutrocyte in peripheral blood, Pelger dyskaryosis, the numbers and detected rate of immature granulocytes, monocyte detected rate, the granules scarce in all stage of granulocytic series in bone marrow, odd number and prolification of nucleolus in erythrocytic series, little macronucleus and single circle nucleus macronucleus. It is concluded that cell morphology is the foundation of diagnosing the MDS, the abnormality morphology both in peripheral blood and bone marrow play the consequence role in the diagnosis of MDS.

9.Clinical study on needling method for regulating mental activities and soothing liver for treatment of melancholia.

OBJECTIVETo observe clinical therapeutic effect of needling method for regulating mental activities and soothing liver on melancholia.
METHODSOne hundred and ten cases of depression were selected according to the diagnostic criteria of depression in mood disturbance (emotional and mental disturbance) and randomly divided into a regulating mental activities and soothing liver needling group (n=50), a traditional acupuncture group (n=30) and a prozac treatment group (n=30). Their therapeutic effects were evaluated by Hamilton depression scale, Self-rating depression scale (SDS), psychiatric self-rating scale (SCL-90).
RESULTSIn the regulating mental activities and soothing liver needling group, 34 cases were cured, 10 cases were markedly effective, 4 cases were effective and 1 cases were ineffective, with the cured rate of 69.4% and the total effective rate of 98.0%, which were significantly better than those in the traditional acupuncture group (the cured rate was 48.3% and the total effective rate was 89.7%), and was not significantly different with the prozac treatment group (the cured rate was 64.0% and the total effective rate was 96.0%).
CONCLUSIONThe needling method for regulating mental activities and soothing liver has a better therapeutic effect on depression.
