1.Analysis of risk factors for postoperative nausea and vomiting in spinal anesthesia patients

Objective To investigate the risk factors for postoperative nausea and vomiting (PONV) in spinal anesthesia patients.Methods A total of 841 patients received spinal anesthesia were visited after operation.Data were analyzed using univariate analysis and multivariate Logistic regression to identify risk factors related to PONV.Results PONV occurred in 94 patients (11.2%,94/841).Univariate analysis showed that PONV was unrelated with gender,age,ASA classification,anesthesia mode (P > 0.05),related with operation department (P =0.026),body mass index (P =0.020),education level (P =0.000),history of previous surgery anesthesia (P =0.005),history of PONV (P =0.000),history of kinesia (P =0.002),smoke (P =0.019),intraoperative using of tramadol (P =0.018).Multivariate analysis showed that operation department (OR =4.039,95% CI 1.331-12.259,P=0.048),education level (OR =3.504,95% CI 1486-8.260,P=0.015),history of PONV (OR =5.113,95% CI 1.790-14.606,P =0.002),intraoperative using of tramadol (OR =5.316,95% CI 1.091-25.908,P =0.039) were identified as independent risk factors for PONV.Conclusions The independent factors associated with PONV following spinal anesthesia include operation department,education level,history of PONV,intraoperative using of tramadol.Identifying patients who are at high risk for PONV will enable the formation of more timely management project.
2.Changes of Blood Lipid,Blood Rheology and Microcirculation of Model Rats of Diabetes Ⅱ with Heart Blood Stasis Syndrome

Objective To explore the relations among blood lipid,blood rheology and microcirculation of bulbar conjunctiva of model rats of diabetesⅡwith heart blood stasis syndrome.Methods The rats were fed with high fat and caloric feed.Intraperitoneal injection of streptozotocin(STZ)in small dosage was applied to make the model of diabetesⅡ.Then the model of heart blood stasis syndrome was made through introperitoneal injecting dexamethasone and adrenalin.Eight hours after empty stomach,the rats were anaesthetized to measure the microcireulation of bulbar conjunctiva and test the blood lipid and carotid blood rheology.Results The indices of blood lipid,blood rheology and bulbar eonjunetiva microcirculation of model rats were all changed to abnormal,tallying with the changes of cardiovascular complications of diabetes.
3.SNP in Differentially Methylated Region Upstream of H19 Gene in Chinese Kore-an Nationality

Objective To investigate SNP and distribution of haplotypes in differentially m ethylated region (DMR) upstream of H19 gene in Chinese Korean nationality in order to provide basic data for forensic application and population genetics research. Methods One hundred and one blood sam ples from unrelat-ed Chinese Korean individuals and 14 blood sam ples from 5 Chinese Korean intergenerational fam ilies which known genetic relationship were collected. The SNP in DMRupstream of H19 gene were investi-gated by PCR-cycle sequencing and McrB Cdigestion followed by PCR . The haplotypes detected by parentally im printed allele (PIA) m ethod and relevant genetic param eters were calculated. Results Thirteen SNPs (rs10840167, rs2525883, rs12417375, rs4930101, rs2525882, rs2735970, rs2735971, rs11042170, rs2735972, rs10732516, rs2071094, rs2107425, and rs4930098) and five haplotypes were detected in 1 174 bp target product in DMRupstream of H19 gene, with 9 SNPs having high discrim ination power as good genetic markers. The average gene diversity (GD) of haplotypes was 0.714. The maternal haplotype was confirm ed correctly by PIAm ethod from McrB C-digested products of genom ic DNA. Conclusion High polym orphism sexist in DMRupstream of H19 gene in Chinese Korean nationality. And determ ination of the maternal haplotype could furtherm ore enhance the forensic identification efficiency of im printed gene.
4.Observation on therapeutic effect of herb-partitioned spread moxibustion for treatment of chronic nonspecific ulcerative colitis.

OBJECTIVETo compare therapeutic effects of herb-partitioned spread moxibustion and western medicine on chronic nonspecific ulcerative colitis.
METHODSSixty cases were randomly divided into a spread moxibustion group (n = 28) and a western medicine group (n = 32). The spread moxibustion group was treated with herb-partitioned spread moxibustion at lower limb around stomach meridian, abdomen region around Guanyuan (CV 4) and lower Jiaji (EX B 2) points; and the western medicine group was treated with oral administration of Sulfasalazine. Their therapeutic effects were observed after treatment.
RESULTSThe cured-markedly effective rate was 71.4% (20/ 28) in the spread moxibustion group, and 25.0% (8/32) in the western medicine group, the former was better than the latter (P < 0.05).
CONCLUSIONSThe therapeutic effect of herb-partitioned spread moxibustion for treatment of chronic nonspecific ulcerative colitis is better than that of the oral administration of Sulfasalazine with less adverse reaction, and is worth popularizing in clinic.

5.Safety and efficacy of the different doses of sufentanil combined with propofol target controlled infusion for modified electronic convulsive therapy

Objective To compare the safety and efficacy of the different doses of sufentanil combined with propofol target controlled infusion for modified electronic convulsive therapy ( MECT ) .Methods A total of 100 patients with mental disorders were randomly divided into four groups:control group (group C), sufentanil 1 group (S1), sufentanil 2 group ( S2 ) and sufentanil 3 group ( S3 ) , 25 cases in each group.In sufentanil groups, patients were respectively intravenously injected with 0.1, 0.2 and 0.3 μg? kg -1 sufentanil 10 mL, and those in control group were given 0.9%NaCl solution.Propofol was given through target controlled infusion, and the initial target density of 3.0 μg? mL-1 was set, and increased by 0.5μg? mL-1.When eyelash reflex disappeared, succinylcholine 1.0 mg? kg -1 was injected by intravenous injection.The vital signs of patients before anesthesia ( T1 ) , target control infusion stop immediately ( T2 ) , immediately after MECT finished ( T3 ) and consciousness recovery ( T4 ) , clinical effects,the seizure energy index, the seizure duration, awakening time, assisted ventilation time and adverse reactions were recorded.Results The difference had no statistical significance in vital signs, the seizure energy index, the seizure duration of all patients ( P >0.05 ) .Compared with group C, assisted ventilation in group S2 and S3 was prolonged, and the waking time was extended in group S3, total effective rate was increased in group S1 to S3 ( P<0.05 ) .Compared with group S1 , assisted ventilation time and waking time were extended, myalgia was decreased, and the total effective rate was increased in group S3(P<0.05).Compared with group S2 , the assisted ventilation was prolonged in group S3 ( P<0.05 ) .The incidences of nausea and vomiting and sleepiness were increased, while muscle pain and headache incidence were decreased as sufentanil dose increased. Conclusion Sufentanil combined with anesthesia of propofol target controlled infusion can be safe and effective for MECT, and 0.2 μg? kg-1 sufentanil is preferred.
6.The genetic diversity in the full coding region of human FUT5 gene in a Chinese Han population.

OBJECTIVETo reveal the sequence variations of the full coding region of the human alpha (1,beta/1,4) fucosyltransferase 5 gene (FUT5) in a Chinese Han population.
METHODSThe whole coding region of the FUT5 gene was amplified and sequenced in a total of 30 unrelated Chinese Han individuals. The PCR products containing the nucleotide variants observed in the study were subcloned into plasmid pcDNA to determine all potential haplotypes in the investigated population. Genetic polymorphisms of C560T (rs778970) and C484A loci were further analyzed by polymerase chain reaction-restriction fragment length polymorphism(PCR-RLFP) method.
RESULTSIn addition to seven previously reported base substitutions, two novel polymorphisms, namely C484A (Leu162Met) and T684C, were found in the coding region of the FUT5 gene in the 30 individuals. Seven haplotypes were identified by subcloning the variants into plasmid and subsequent DNA sequencing. The allele frequencies in the rs778970 locus in 160 Chinese Han individuals was 0.3031 for 560C and 0.6969 for 560T, while no polymorphism was detected in the C484A locus.
CONCLUSIONThe sequence of the coding region in the human FUT5 gene demonstrated high genetic diversity, and the allelic distribution of the rs778970 locus in the Chinese populations is polymorphic.

7.Clinical evidence in concurrence of retinitis pigmentosa and glaucoma.

Glaucoma is rarely complicated by retinitis pigmentosa (RP). To provide clinical evidences for this rare situation, we report the concurrence of these two diseases in two children of a Chinese family. In the present two-child Chinese family without positive history, the older sister presented with bilateral sector RP and coexisting chronic angle-closure glaucoma, and the brother with bilateral whole RP but without coexisting glaucoma. Clinical evidences in concurrence of variants of RP and glaucoma because of possible different gene mutations from the same genetic background represent a rare situation, which may provide clues for future researches in molecular pathogenesis of these rare diseases.

8.Association between two polymorphisms of the bone morpho-genetic protein-2 gene with genetic susceptibility to ossification of the posterior longitudinal ligament of the cervical spine and its severity.

BACKGROUNDOssification of the posterior longitudinal ligament (OPLL) has a strong genetic background. Previous studies have shown that bone morphogenetic protein-2 (BMP2) and BMP2 mRNA are expressed in ossifying matrix and chondrocytes adjacent to cartilaginous areas of OPLL tissues and mesenchymal cells with fibroblastic features in the immediate vicinity of the cartilaginous areas. It is suggested that BMP2 plays different roles in the different stages of development of OPLL. However, it remains unknown which factors induce ligament cells to produce BMP2.
METHODSOPLL patients (n = 192) and non-OPLL controls (n = 304) were studied. Radiographs of the cervical spine were analyzed for extent of OPLL. We investigated whether single nucleotide polymorphisms of exons 3 (-726) T/C and 3 (-583) A/G in the BMP2 gene are statistically associated with genetic susceptibility to OPLL in Chinese Han subjects.
RESULTSThere was no statistical difference between the occurrence of exons 3 (-726) T/C and 3 (-583) A/G and the occurrence of OPLL in the cervical spine. However, there was a significant association between occurrence of exon 3 (-726) T/C polymorphism and occurrence of OPLL in males of cases and controls in the cervical spine. In addition, no significant association was found between the exons 3 (-726) T/C and 3 (-583) A/G with number of ossified cervical vertebrae in OPLL patients.
CONCLUSIONSExon 3 (-583) A/G polymorphism in BMP2 gene is not associated with the occurrence and the extent of OPLL in the cervical spine. Chinese Han male patients with TC and CC genotypes in exon 3 (-726) T/C have genetic susceptibility to OPLL but not to more extensive OPLL in the cervical spine.

9.Fluid-attenuated inversion recovery vascular hyperintensity: Correlation with other radiologic findings in acute ischemic stroke with middle cerebral artery occlusion

Objective: To assess fluid-attenuated inversion recovery (FLAIR) vascular hyper-intensity (FVH) and
explore its relationship with CT perfusion (CTP) penumbral/infarct core mismatch ratio and diffusion
weighted imaging (DWI) final infarct volume in acute ischemic stroke (AIS) patients with middle
cerebral artery occlusion (MCAO). Methods: The CTP and MRI images of 38 AIS patients with MCAO
were reviewed. The FVH score (longitudinal direction) [FVH score (L)] and FVH score (transverse
direction) [FVH score (T)] were quantified on the FLAIR images. The FVH score (L) (range, 0-16)
was based on a rostrocaudal extension of FVH and the FVH score (T) (range, 0-3) was based on FVH
supply of the occluded MCA territory. The mismatch ratio was calculated from the ratio of the [mean
transit time - cerebral blood volume (CBV)] lesion/CBV lesion on the CTP images. The DWI infarct
volume was measured on the DWI images. Results: The mismatch ratio was larger for the group of
FVH score (L)=7~8 than those of FVH score (L)=5~6 and FVH score (L)=3~4 (p=0.03), whereas
the DWI infarct volume was smaller (p=0.04). Similarly, the mismatch ratio of FVH score (T)=2~3
group was larger than FVH score (T)=1 group (p=0.01), whereas the DWI infarct volume was smaller
(p=0.02). Both FVH score (L) and FVH score (T) correlated positively with mismatch ratio (P=0.02,
P=0.001, respectively), but negatively with DWI infarct volume (P=0.03, P=0.004, respectively).
Conclusions: Higher FVH score is associated with larger mismatch ratio and smaller DWI infarct
volume in AIS patients with MCAO. FLAIR vascular hyperintensity may represent collateral arterial
circulation, and may play a role in protecting the ischemic penumbra.

10.Determine resource chemical component in ginkgo pollen simultaneously by UPLC-TQ-MS.

The present study is to determine the flavonoid glycosides, terpene lactones, biflavones, gingko acid and procyanidins of ginkgo pollen. UPLC-TQ-MS technology was used for the determination of 24 kinds of resource chemical composition in ginkgo pollen qualitatively and quantitatively. The results shows that the contents of rutin, quercetion 3-O-[4-O-(α-L-rhamnosyl )-β-D-glucoside] and kaempferolis were 120.9, 114.0, 222.1 μg x g(-1). In this paper, the contents of 24 kinds of chemical components of ginkgo pollen were determinated by UPLC-TQ-MS for the first time. This method is simple and quick, which will be benefit for recycling utilization of ginkgo pollen.
