2.Detection of bilirubin: from laboratory to clinical practice

Jaundice (abnormal elevation of bilirubin) is common in clinical practice.At present bilirubin is detected by measuring total bilirubin (TB) and direct bilirubin (DB) in hospital.Indirect bilirubin (IB) is the difference of TB and DB.Direct bilirubin reflects mainly the conjugated bilirubin but they are not all equivalent.Indirect bilirubin and unconjugated bilirubin are the same condition.In clinical practice,the proportion of DB (or IB) in TB is more significant than their level of elevation.The cause of jaundice could be roughly determined by analyzing the proportion of DB (or IB) in TB.The methodology and quality of bilirubin detection are quite different in hospitals in our country.The proportion of DB aud IB in TB in detection of bilirubin is also quite different in hospitals.It leads to a big puzzle to the diagnosis and differential diagnosis of jaundice for clinical doctors.It is suggested that the quality control of bilirubin detection should be strengthened in laboratory in hospital.On the bases of strict quality control of bilirubin detection,proper adjustment of the proportion of DB and IB as to make it consistiug with clinical practice and pathogenesis of diseases,and making the proportion stable for long time are also suggested.
3.Progress on diagnositic methods of bronchiectasis in children

Bronchiectasis is one of the common chronic lung diseases in children.Now chest high resolution CT is the best method for the diagnosis of bronchiectasis.Chest MRI is an accurate,radiation-free technique,and it contributes to finding early lung lesions.Lung function and airway inflammation test are used for effectively assessing the disease status.This review focuses on the main diagnostic methods of bronchiectasis in children.

This paper reviews the application of nebulized hypertonic saline and adrenaline in the treatment of children with bronchiolitis.The efficacy of hypertonic saline and epinephrine in the treatment of bronchiolitis has been analyzed in different experiments.Confirming hypertonic saline and epinephrine respectively in the treatment of bronchiolitis has a significant effect.Prompted each inhalation and combined use of both play an important role in the treatment of bronchiolitis.
5.A study on 6-gingerol attenuate vascular smooth muscle cells senescence through inhibition of mTOR pathway molecular

Objective To investigate the effects of 6‐gingerol on the rat aortic vascular smooth muscle cells (VSMCs)senescence induced by angiotensin Ⅱ(Ang Ⅱ) .Methods Culturing rat VSMCs were stimulated with Ang Ⅱ with or without pretreating with 6‐gingerol .The senescent status of cells was verified by in situ staining for senescence‐associated β‐galactosidase(SA‐β‐gal) .The cell cycle distribution was analyzed by FACS .The protein expression of mTOR and phosphorylated P70‐S6K were detected by Western blot .Results When VSMCs were exposed to AngⅡ ,the senescence marker such as SA‐β‐gal staining and cell cycle arrest‐ing in G0/G1 phage were significantly increased .Furthermore ,the protein level of mTOR ,phosphorylated P70‐S6K were also en‐hanced significantly .However ,6‐gingerol was found to markedly inhibit the senescence‐like phenotype and related signal molecular induced by AngⅡ as mentioned above .Conclusion 6‐gingerol attenuate VSMCs senescence through inhibition of mTOR/P70S6K pathway molecular .

Fascia iliaca compartment block is described for blocking the femoral,lateral cutaneous,and obturator nerves if the local anesthetic is introduced behind the fascia iliaca,which circumscribes a potential space which contains three nevers.In recent years,an increasing number of fascia iliaca compartment blocks are performed in children with the development of the pediatric peripheral nerve block,and the analgesia effect on the thighs,hips and knees is satisfactory.In combination with the anatomy of the fascia iliaca compartment,this paper reviews the indications,methods and effect of fascia iliaca compartment block in children.
7.The mechanism of nebulized drugs in the treatment of patients with exacerbation of bronchiolitis

Bronchiolitis is the most common acute infection of the lower respiratory tract in infancy,characterized by rhinorrhea,cough,Wheezing,respiratory distress and hypoxemia,and is most often caused by the respiratory syncytial virus (RSV).Some patients may develop into asthma,which is serious threat to the health of infants and young children.It is one of the focal points of pediatric clinical research.For the treatment of bronchiolitis,there is still no specially good effects of the treatment,the goal is to remove the obstruction of respiratory tract,improve ventilation and control the wheezing,prevent complicated by heart failure and respiratory failure.Medication mainly includes antiviral medication,and asthma and immunotherapy.The inhalation therapy is to put the drugs as a particulate directly into target tissue,acting on the trachea with drug receptors to play curative efficacy,which has rapid absorption.Thus nebulized medication therapy has been more and more recognized clinically.There are a wide variety of nebulized drugs.Commonly used in the literature are β2 agonists,epinephrine,corticosteroids,magnesium sulfate,ambroxol,ribavirin and hypertonic saline,etc.The Mechanism of various nebulized inhalation drug commonly used in the treatment of bronchiolitis recent years are reviewed.

Objective:To study the regulatory mechanism of Wnt/?-Catenin passageway influenced apoptosis of human gastric cancer line SGC-7901 by detecting the expression of?-Catenin mRNA,Caspase-3 mRNA and bcl-2 mRNA.Methods:(1) Methyl thiazolyl tetrazolium(MTT)assay was used to measure the anticancer activity of indomethaci.(2)Expression of?-Catenin, Caspase-3 mRNA and bcl-2 mRNA in groups of without induced or induced by indomethaci were detected by RT-PCR. Results:Indomethacin of 50?mol/L at 24h couldn't induce apoptosis of gastric cancer cells.Indomethacin of 50?mol/L at 48 and 72h and of 100?mol/L and 200?mol/L at 24,48 and 72h could induce apoptosis of gastric cancer cells in adose- and time-dependent manner;Compared with groups of without induced by indomethaci,expression of?-Catenin mRNA and bcl-2 mRNA were lower,however,increased expression of Caspase-3 mRNA were found in SGC7901 cells after treatment with indomethaci.Conclusion:Apoptosisin inbalance may be caused by activating expression of bcl-2 proto-oncogene protein and inhibiting expression of Caspase-3 protein through Wnt/?-Catenin passageway in human gastric cancer line SGC7901.

0.05)in group B.Conclusions The method of intraluminal irradiation ~(125)I seeds stent in the treatment of unresectable esophageal cancer is effective.No significant difference was found between two implantation ways of ~(125)I seeds.Implantation with a distance of 1cm between the layers of ~(125)I seeds stent was more helpful to decreased relapes of malignant esophageal obstruction.
