Objective To longitudinally analyze the unit costs and technical efficiency of the model of male circumcision in the different kinds of persons .Methods Unit costs were calculated by the person and period using longitudinal data from 3 kinds of persons ,and then technical efficiency and Malmquist indices were measured with an approach to data envelopment analysis . Results Theunit costs for changing the willingness to accept surgery changed dramatically ,decreasing from 7 166 .67 yuan(mean) to 737 .31 yuan ,while the costs for changing the ratio of the surgery increased from 666 .64 yuan (mean) to 744 .58 yuan ,and its technical efficiency was averaging between 0 .95-0 .96 .Conclusion The time series of unit costs for changing the willingness to ac-cept surgery dramatically dropped ,while changing the ratio of the surgery formed a U-shape curve with an inflection point before which unit costs dramatically dropped and another inflection point beyond which unit costs went up .These findings can inform pro-gram managers of the changing unit costs when extending or expanding the program .

Objective To observe the therapeutic effects of stellate ganglion block (SGB) with oxycodone and acetaminophen tablets on the treatment of migraine. Methods 100 patients with migraine were divided into two groups randomly. They received SGB with oxycodone and acetaminophen tablets (experimental group) or only SGB (control group) respectively. The parameters included pain intensity before and 1 d,3 d,1 w,1 m and 3 m after the treatment, and pain relief rate of each patient. The adverse effects were also evaluated. Results The pain intensity described as numeric rating score (NRS) decreased in the two groups at each time point. In the experimental group, the value of NRS was significantly less than that in the control group at 3d,1m and 3m after the treatment. The effec-tive rate ha the experimental group and the control group were 88% and 68%. Conclusion SGB with oxycodone and acetaminophen tablets is more effective for migraine and deserves widely used in patients.

Chinese Journal of Medical Aesthetics and Cosmetology 2001;0(04):-
Objective To introduce a better operation method to repair the skin and soft tissue defects in the forehead. Methods This kind of frontalis musculocutaneous flap was applied to repair the skin and soft tissue defects in the forehead in 8 cases. The flap was designed carefully according to the shape and the size of the focal lesion before the operation. The maximum area of the flap was 2.9cm?1.6cm, and the minimum area was 1.2cm?1cm. The full flap contained muscle in its proximal area and skin, muscle, and fascia in its distal one. A muscular pedicle with appropriate length was outlined on the paramedian and lateral forehead. The pedicle was composed of frontalis muscle fibers, the terminal part of the frontal branches of the superficial temporal artery, and some small branches from the supraorbital and the supratrocheal arteries. The body of the flap was designed into triangle shape. Full thickness skin was incised around the inverted pattern of the defect. The skin of the paramedian and lateral aspect was then undermined laterally. Superficial dissection was carried out in a subcutaneous layer. The flap was undermined beneath the galea and frontalis muscle just external to the skull. The muscle pedicle was prepared by incising a parallel horizontal incision. Careful attention should be taken so as not to injure the vascular pedicle. At that time, the supraorbital and supratrocheal arteries were carefully ligated to obtain complete hemostasis. Then the flap was advanced and transferred down to cover the wound in the forehead. The donor site could be sutured directedly by undermining the margins of the defect without much difficuties. Results All flaps in 8 cases survived. Venous congestion was observed in early postoperative period in only one case. But the flap survived completely after proper management in final healing. Follow-ups were made from 6 months and 12 months after operations. The color and texture of the flaps were similar to the adjacent skin in the forehead. The shapes were satisfying. Conclusions This kind of flap is a much better method for the repairing of some small or moderate defects in the forehead. It can be easily operated and allow a rapid aesthetic restoration.
9.Role of Apoptosis in Acute Rejection of Pancreaticoduodenal Transplantation in Rats
Chinese Journal of Bases and Clinics in General Surgery 2004;0(01):-
Objective To investigate the roles of cell apoptosis and the gene expressions of Fas, FasL, bcl-2 and bax in acute rejection of pancreaticoduodenal transplantation and to evaluate the function of duodenum biopsy for early detection of rejection in rats. Methods Wistar and SD rats were divided into two groups: ①Wistar rats that underwent allogenic pancreaticoduodenal transplantation from the organs of SD rats; ②Wistar rats that received homogenic transplantation. The grafts were then harvested on day 3, 5 and 7 after the transplantation, and all graft samples were observed with HE staining and TUNEL was also used to detect apoptotic cells. The expressions of Fas, FasL, bcl-2 and bax were measured by immunochemical method. According to Nakhleh’s score, pathologic features of transplanted pancreas and duodenum were ranged from one to three scores in order. Results The percentage of same or different scores between the pathological scores of pancreas and duodenum in allogenic pancreaticoduodenal transplantation group were 61.1% (11/18) and 38.9% (7/18) respectively, and there were 6 specimens of pancreatic tissue got higher scores with only one higher score for duodenum. There were significant differences of histopathologic rejection scores and apoptotic indices between the two groups, respectively (P
