1.Research on the Related Problems of Small Interfering Ribonucleic Acid R & D and Registration in China

OBJECTIVE:To provide reference for small interfering ribonucleic acid (SiRNA) R&D and registration in China. METHODS:The situation of SiRNA R&D at home and abroad were introduced,and problems encountered in the evaluation and approval process were summed up to put forward suggestions. RESULTS & CONCLUSIONS:SiRNA has developed slowly in Chi-na,with a gap of 5-10 years at least compared with foreign developed countries. The registration category of SiRNA is not clear;applicants lack of guidance documents;production workshop which meet requirement is in shortage;the efficiency of drug registra-tion allows of no optimism. It is recommended that pharmaceutical registration departments carry out the pilot reform for SiRNA registration,publish SiRNA registration guide,relax regulations for IND gradually,encourage applicants to participate in the inter-national multicenter clinical trials in order to improve the efficiency of SiRNA evaluation and approval efficiency.
3.Discussion on modif ied Shenxian decoction treating connective tissue disease (CTD) combined with pulmonary interstitial f ibrosis (PIF)

Pulmonary interstitial fibrosis (PIF) is a common complication of connective tissue disease (CTD). The patients with PIF usually manifest with intensive dry cough, progressive dyspnea aggravated by activities, and even weight loss and very fatigued. In this paper, we have applied the traditional methods of "invigorating qi for ascending" and "strengthening earth to generate metal" with modif ied Shenxian decoction for treating CTD combined with PIF to support healthy energy, ascend thoracic qi, and ascend both qi of lung and spleen. We have observed Shenxian decoction could enhance body immune system, benefit the repair of damaged lung tissues, improve microcirculations, and treat both branch and root of body, which has been giving patients prominent effects.
4.Identification of Hypotensive Drugs Illegally Added to Flower Buds of Panax Notoginseng by Laser Raman Spectroscopy

Objective:To fast detect hypotensive drugs illegally added to the flower buds of Panax notoginseng. Methods:A laser Raman spectrometer was used to detect several chemical drugs added to the flower buds of Panax notoginseng, and the spectrum was qualitatively analyzed. Results:Raman spectroscopy could detect chemical antihypertensive drugs added to the flower buds of Panax notoginseng, and the test results were in accordance with those of the traditional method. Conclusion: Raman spectroscopy can be used for the determination of illegal additives in the flower buds of Panax notoginseng.
5.Influence of pan retinal photocoagulation on diabetic retinal and corneal

Diabetic retinopathy ( DR ) is one of the most serious microvascular complications of diabetes. It also is a world's major causes of blind eye diseases. Although the injection of anti - vascular endothelial growth factor intraocular drug is becoming a hotspot of current research. Pan retinal photocoagulation (PRP) remains the effective and economical treatment for DR. As a destructive treatment, PRP will inevitably cause some changes on the normal morphology and function of the retinal and cornea. Currently, with the development of the optical coherence tomography, confocal microscopy and other auxiliary examination, we have more accurate and objective research to better understand the laser-neural tissue interaction. This may help us to protect the patient's visual function more efficiently.
6.Research on the application of BioImp Vet Body composition in of the second class in the experiment teaching of medical function

Objective:Through the introduction of BioImp Vet Body Composition principle and application of the second classroom experimental teaching practice in medical function, analyzer application characteristics of BioImp Vet Body Composition. Methods:We studied the distribution of body fluid and fat content determination of rats using BioImp Vet Body Composition. The device using the complex impedance measurement technique in vivo, within the second 4 k-1000 kHz scope, scanned the 256 frequency bands. We measured body water content (TBW), extracellular fluid (ECF) and intracellular fluid (ICF), in addition to the amount of fat (FFM) and fat mass (FM). Results:This measurement technique is very convenient, reliable and quick data acquisition in our second classroom teaching and laboratory. Conclusion:the measurement of BioImp Vet Body Composition can be used in the field of medical experimental teaching, has the application and characteristics of portable and feasible of the second class in the experimental teaching of Medical Function.
7.Operational efficiency of county level public hospitals in Hubei:a Bootstrap-data envelopment analysis

Objective To estimate the operational efficiency of 37 county level public general hospitals in Hubei province using the world popular method of Bootstrap-data envelopment analysis.The aim is to make up for the theory and methodology gaps on DEA research for hospitals in China,and to provide policy and management recommendations for their further improvement.Methods The input and output indicators of hospital efficiency evaluation were selected based on our previous research.R Software was used to describe the current input and output indicators,while FEAR package used to measure Bootstrap-DEA efficiency value of the sample hospitals with bias corrected,along with such applied analysis as efficiency benchmarking for the technical efficiency outcomes.Results There are great differences among the healthcare input and output of these hospitals.In particular,the difference in the number of actual beds was 4.67 times between the highest hospital and the lowest one.All of the bias corrected efficiency scores were lower than those before correction,and the average bias corrected score of the sample hospitals is 0.717 9.Moreover,only 20 hospitals(54.05%)have their efficiency scores above the median level.Conclusions The healthcare resources allocation of these hospitals needs to be further optimized.Hospitals still have big potential to improve their efficiency.Besides providing policy support,the government is advised to guide hospitals to innovate their management mechanisms,such as introducing and applying benchmarking,ranking,inter-hospital learning among others,so as to continuously improve their operational efficiency.
8.Prediction of the grade of acute cholecystitis and plasma level of C-reactive protein

Objective To investigate the relationship between C-reactive protein and clinical significance of Tokyo Guidelines for acute cholangitis.Methods Analyzed 739 cases with acute cholecystitis in Xianyang Central Hospital of Shaanxi province.Records of patients diagnosed with acute cholecystitis were screened between September 2013 and October 2015.The criteria of Tokyo guidelines were used in grading the severity of acute cholecystitis.Patients were divided into 3 groups,Group Ⅰ (n =450),Group Ⅱ (n =260) and Group Ⅲ (n =29).C-reactive protein values at the time of admission were analyzed and compared among the groups.Results Mean C-reactive protein levels of groups were found to be significantly different,18.74 mg/L in Group Ⅰ,133.67 mg/L in Group Ⅱ,and 257.43 mg/L in Group Ⅲ.C-reactive protein values among the groups were found to be highly and significantly correlated with the disease grade (P < 0.01).After evaluating C-reactive protein levels according to the grade of the disease,Group Ⅱ was distinguished from Group Ⅰ with a cutoff C-reactive protein level of 72.54 mg/L,and from Group Ⅲ with a value of 195.85 mg/L.Those results were found to be statistically significant (P < 0.01).Conclusion C-reactive protein can be accepted as a strong predictor in classifying different grades of the disease,and treatment can be reliably planned according to this classification.
9.Isomeric M flap for facial rhombus defect

Objective To explore the possibility of repairing face rhombus defect after resection of pathological tissues on the special regions. Methods After rhombus resection of a pathological tissues on the special regions of the face, using four triangular flaps at the any sides of the defect to stagger suturing each other and to constitute peculiar "M" flap repairing defect in 26 patients (12 males and 14 females). Results The flaps were all survived. No straight line scars were seen at the repairing regions and no scars contraction for the face contour was occurred. Conclusion This is a simple and effective operation procedure that can be used for rhombus defect of the special regions of the face.
10.Case analysis of hospital merger and acquisition

It presented in detail the acquisition of Qingdao Wan Jie Hospital by the Affiliated Hospital of Medical College Qingdao University, describing the operational mechanisms in organizational setup, specialty setup, personnel management, logistic management, hospital culture and branding of the newly acquired section. It also summarized the achievements in medical resources reorganization and advantages complementary without government investment in a fast and low-cost manner. It is demonstrated that hospital acquisitions are in line with the medical reform, as they expand the service coverage of public hospitals, and extend medical service to the communities and lower levels, thus enhancing hospitals' impact and reach to integrate medical resources in a short timeframe.
