2.Clinical application of template designed by three-dimensional reconstruction and rapid prototyping in locating acetabular prosthesis

[Objective]To provide a precise individualized design proposal for locating acetabular prosthesis by means of 3D reconstruction,reverse engineering software and rapid prototyping technology.[Method]3D CT scan pelvis image data were obtained from six patients,which displayed osteonecrosis of femoral head on one side.The data were transferred via a DICOM network into a computer workstation.A 3D model of hip was reconstructed using Amira 3.1 software,and saved in STL format.Then the 3D model was imported into Imageware 12.0 software.The normal hip joint centre of abnormal side was determined and the best orientation of the prosthesis(which was central axial ray of acetabula)was defined using reverse engineering.Locating template was designed according to the anatomic features of the acetabula and the central axial ray.Entity model was produced by rapid prototyping technology.The orientation of the prosthesis was located by matching the model to acetabular concavity at operation.[Result]By measuring postoperative standard hip anteroposterior film,the abduction angle of acetabular prosthesis was(46.00?2.49)? and the anteversion angle was(15.68?2.85)?.The expected result was obtained.[Conclusion]The navigation template designed and constructed preoperatively can provide precise location for acetabular prosthesis and avoid procedural errors at operation.
3.Efficacy of celecoxib in the treatment of peri-operative pain of orthopaedic patients

[Objective]To observe the effect and the safety of the COX-2 inhibitor Celecoxib as the analgesia in orthopaedic post-operative patients.[Method]Sixty-four operative patients were selected in all between 2004~2005 in hospital,and divided into two groups randomly.Celecoxib was used as the postoperative analgesia in the experiment group and the "Patient-controlled analgesia" in the control group.The administration of Celecoxib: 8-12 hours before operation generally,just before the abrosia,and 6 hours after operation when patients can take food.drug withdrawal was depended on the complex of the operation and the severety of pain in 3 to 5 days.Dosage:400 mg first time,if a complicated operation,larger dose would be given.VAS score,adverse reaction and the satisfaction of patients were observed.[Result]The effect of Celecoxib is similar as the PCA,with less adverse reaction and more satisfaction.[Conclusion]Celecoxib has a satisfactory effect and safety as a postoperative analgesia,and is suitable to be a peri-operative analgesia of orthopaedic patients.

5.Clinical Observations on Influences of Spleen-strengthening Decoction on the Expression of GLP-1 in Patients with Type Ⅱ Diabetes

OBJECTIVE: To observe the effects of Spleen- strengthening decoction on GLP- 1 in patients with type II diabetes. METHODS: A randomized double- blind placebo- controlled test was conducted, and through observations for 8 wks, the changes of the two patient groups in plasma glucose, HbA1c, plasma GLP- 1 and Glucagon as well as TCM symptom score were measured for comparison. RESULTS: Before administration of Spleen- strengthening decoction, the two groups showed no significant difference in all of the indexes. After administration, the treatment group recorded lower indexes in fasting plasma glucose, HbA1c and plasma Glucagon, but higher index in fasting plasma GLP- 1, with no significant difference seen in other indexes. CONCLUSIONS: Spleen- strengthening decoction can improve glucose metabolism through heightening fasting plasma GLP- 1, and lowering both Glucagon and fasting plasma glucose.
6.Study of terminal disinfection before and after the object surface temporarily bacteria in clinical ward

Objective To understand the change of transient bacteria on surface in clinical ward before and after terminal disin‐fection ,provide the basis for controlling of hospital infection .Methods Surface samples were collected before and after terminal dis‐infection in infected patch of our hospital ,and then bacterial in the samples were cultured and identified .Compared changes about number and type of samples bacterial ,distribution of common clinical pathogenic bacteria before and after of the terminal disinfec‐tion .Results The surface colony number < 10 CFU /cm2 accounted for 63 .54% after terminal disinfection ,compared with the dis‐infection before 56 .29% ,increased 7 .25 percentage points .Surface sampling microorganism detecting rate decreased by 6 .74% . Surface average bacteria colony had different degree decreased before and after disinfection ,except the bed frame and quilt cover . Water tap ,which was the largest amount of bacteria surface ,followed by the bedside table .Before and after disinfection ,the mainly common microorganism was environment bacteria in infected patch ,including coagulase negative staphylococcus ,gram positive ba‐cilli ,Micrococcus ,Acinetobacter spp .Clinical common pathogenic bacteria mainly isolated from the department of brain surgery (9 .49% ) ,department of hepatology(8 .76% ) ,department of dermatology (8 .76% ) ,department of pediatrics (8 .03% ) ,emergency department (7 .30% ) .Pathogenic bacteria living areas were mainly the bedside table (21 .17 % ) ,water tap (18 .25% ) ,bed rest (12 .41% ) .Conclusion Terminal disinfection could effectively reduce the number of bacteria in the infected patch ,improve the ward environmental sanitation quality ,it have an important significance in the prevention of hospital infection control .
7.Application of ATP bioluminescence assay in surveillance of terminal disinfection of effects

Objective To summarize the application of ATP bioluminescence assay in surveillance of terminal disinfection of effects ,so as to provide the basis for intervention of disinfected effects .Methods ATP bioluminescence assay were employed to randomly test the surfaces of operating objects in therapeutic rooms and beside tables in wards ,total 144 object surfaces ,of each clinical departments in the whole hospital .The values of ATP bioluminescence assay were read on‐site ,0-250 RLU was recognized as qualification ,while disqualification when >250 RLU .The disqualified object surfaces were performed on‐site intervention that all of them were re‐disinfected ,the results were compared .Results Both the surfaces of operating objects and beside tables were dis‐qualified before disinfection ,and the values of ATP bioluminescence assay were 780 ± 10 .34 RL and 853 ± 13 .29 RLU respectively . The pass rates of ATP bioluminescence assay was 61 .97% of operating surfaces and 79 .45% of beside table surfaces the first dis‐infection .The disqualified sites were retested following on‐site intervention .The values of ATP bioluminescence assay were 431 .02 ± 0 .53 before intervention and 1 .43 ± 0 .59 after intervention ,and the difference was statistically significant .Conclusion ATP bi‐oluminescence assay can get more immediately ,simple and timesaving in evaluating the effect of disinfection and estimate the effi‐ciency of disinfection timely ,which can also provide the scientific basis on on‐site intervention so as to improve the execution power of hospital infection management .

9.Relation of oxygen-induced apoptosis of alveolar epithelial cells with surfactant protein C and surfactant protein D in neonatal rats

ObjectiveTo study the change of the pulmonary surfactant protein C, D (SP-C, SP-D) and apoptosis of alveolar epithelium cells in neonatal rats with hyperoxia-induced lung injury.MethodsThe neonatal rats born within 24 hours were divided into the air group (n=50) and the hyperoxia group (n=50). The lung tissue was collected on the ifrst, third, seventh, tenth, fourteenth day after the hyperoxia exposure. The pathological changes were observed by HE staining. The apoptosis rate of lung epithelial cells was detected by TUNEL (terminal deoxyn ucleotidyl transfer-mediated end labeling). The content of SP-C and SP-D in broncho alveolar lavage lfuid (BALF) was detected by enzyme-linked immunosorbent assay.ResultsIn the air group, as age increased, the alveolar were gradually more completely formed with the regular shape and uniform size. Mean-while, in the hyperoxia group, as age increased, the number of alveolar was reduced, the small blood vessels expanded, the alve-olar hemorrhage was increased, the interstitial cells were increased and the lung tissue was swelling. The levels of SP-C, SP-D decreased with the increase of age in the air group. The level of SP-C in hyperoxia group was lower than that in the air group on the ifrst day. It was higher than that in the air group on the third day, peaked on the seventh day, and then it began to decline on the tenth day and decreased more obviously on the fourteenth day. The level of SP-D in hyperoxia group was not signiifcantly dif-ferent from that in the air group on the ifrst day, was higher than that in the air group on the third day and peaked on the seventh day. Then it began to decline on the tenth day and decreased more on the fourteenth day. ConclusionsLong-term inhalation of high concentrations of oxygen inhibits alveolar development. With the prolonged time of oxygen inhalation, the apoptosis of lung epithelial cells is increased, and the level of SP-C and SP-D in BALF was increased ifrst and then decreased.
10.Influence of group psychological training on the mental health and self consistency of nurses from operating rooms

Objective To explore the effect of group psychological training on the mental health and self consistency of nurses from operating rooms.Methods 52 nurses of an operating room were randomized into the treatment group(n=25)and the control group(n=27): The latter received humane care and instructions for mental self care and the former received group psychological training for 8 weeks based on the same treatment as in the control group.Symptom checklist 90(SCL-90)and self consistency and congruence scale(SCCS)were used to assess and compare the difference of the two groups before and after training.Results The intra-and inter-group comparisons between pre-and post-intervention showed statistical differences in terms of the total score by SCL,interpersonal sensitivity,depression and hostility,total score by SCCS and the score on the subscale of inconsistency between themselves and their experience(all P<0.05 or P<0.01).Conclusion Group psychological training can effectively improve the level of mental health and self consistency of OR nurses and thus can improve the total psychological status of the OR nurses.
