1.Study on Safety and Feasibility of Termination of Early Pregnancy with Cicatrix Uterus by Combined Use of Mifepristone and Misoprostol

To study the safety and feasibility of Mifepristone combined with Misoprostol terminating early pregnancy after cesarean section, mifepristone combined with misoprostol was used to terminate early pregnancy in 188 women requesting medical abortion. Among them, 27 cases were uterine cicatrix. The results showed that the complete abortion rate in 188 cases was 91.48 % after administration, that in 27 cases of unterine cicatrix was 100 % and that in 161 cases of no uterine cicatrix was 90. 06 % (P>0. 05). There were no differences in the duration of vaginal bleeding and time from the Misoprostol intake to the expulsion of gestational sac and chorionic villi between the two groups (P>0. 05). It was suggested that combined administration of Mifepristone and Misoprostol to terminate early pregancy is safe and effective after cesarean section.
2.THE INHIBITORY EFFECT OF NIMESULIDE ON GASTRIC CANCER CELLS

The aims of this study were to compare the inhibitory effect of nimesulide and indomethacin on gastric cancer cells, to investigate the effect of nimesulide on SGC7901 and 7901-AS, and to evaluate its probable mechanism. After incubated with nimesulide (0~200?mol/L) or indomethacin (0~25?mol/L), the proliferation of gastric cancer cells was measured by MTT assay. The cell cycle of SGC7901, 7901-P, 7901-AS was respectively observed after being incubated with nimesulide and vehicle control, by using fluorescence activated cell sorter (FACS). Ultramicrostructure of gastric cancer cells (SGC7901,7901-AS,SGC7901+nimesulide) was observed by electronmicroscopy. The results showed that by MTT assay nimesulide, as indomethacin, had dose-dependent inhibitory effects on proliferation of gastric cancer cells. 3.The inhibitory effect of nonsteroid anti-inflammatory drugs on gastric cancer

Objective To compare the effect of seven commonly used nonsteroidal anti inflammatory drugs (NSAIDs) on proliferation, apoptosis, and neoplasmagenesis of gastric cancer cells in vivo. Methods Gastric cancer cell lines were treated with NSAIDs (aspirin 0-400 ?mol/L, indomethacin 0-25 ?mol/L, and ibuprofen, naproxen, sulindac, nimesulide, celecoxib 0-200 ?mol/L, respectively). Proliferation of the cells was detected by using MTT assay. Apoptosis of cells was measured by using fluorescence activated cell sorter (FACS). Nude mice bearing gastric cancer xenografts were administrated with NSAIDs (indomethacin 3 mg/kg, sulindac 8 mg/kg, nimesulide 6 mg/kg, celecoxib 15 mg/kg) for 30 days, and then the weight of implanted tumors was measured. Results There was a dose dependent inhibition of cell proliferation by majority of NSAIDs used, celecoxib the most, except for ibuprofen and naproxen. In nude mice, NSAIDs also showed a suppressive effect on tumor growth with inhibitory rate of celecoxib as (93.8?0.97)%, nimesulide (93.1?1.78)%, indomethacin (89.9?5.61)% and sulindac (89.3 ? 2.07)%. Once incubated with celecoxib and indomethacin, the gastric cancer cells went to apoptosis with an increase in percentage of apoptotic cells up to 30.4% and 23.9%, respectively. Conclusions Many NSAIDs, celecoxib in particular, appeared to be suppressive to gastric cancer cells with exception for ibuprofen and naproxen. Induction of apoptosis might be one of the mechanisms that NSAIDs inhibit gastric cancer.
4.Expression of TGF-betal in placenta of the patients with pregnancy-induced hypertension and its relationship with serum VCAM-1.

The expression of transforming growth factor-beta1 (TGF-beta1) in placental tissue of pregnancy-induced hypertension (PIH) and the relationship between the level of expression of TGF-beta1 and the amount of vascular cell adhesion molecule-1 (VCAM-1) in serum was studied. Immunohistochemistry ABC was used to detect the expression and distribution of TGF-beta1 in placental tissues in 40 PIH women and 20 normal pregnancy women. High resolution pathological image analysis system was used to determine the quality of TGF-beta1. The VCAM-1 in serum was examined by enzyme linked immunoabsorbent assay (ELISA). The results showed that TGF-beta1 could be express in syncytiotrophoblast. The levels of TGF-beta1 expression in placental tissues of the patients with moderate and severe PIH were significantly higher (P < 0.05), while the serum VCAM-1 was significantly lower than in normal group (P < 0.01). There was a significant positive correlation between the expression of TGF-beta1 in placental tissues and the serum VCAM-1 (r = 0.969, P < 0.01). It was concluded that the level of TGF-beta1 expression in PIH was increased and was positively correlated with the amount of serum VCAM-1, indicating that they might be involved in the pathogenesis of PIH.

5.Insights on public hospital corporate governance in Shenzhen

Analysis of setbacks found in the existing system and mechanism of public hospitals. Based on the health reform principles of Separation of administration and management, Separation of administration and operation, Separation of prescribing and dispensing, and Separation of for-profit hospitals and nonprofit hospitals, the authors named the following roadmap for public hospital governance reform. First, set up a management committee for each public hospital to coordinate the government function for medical service; set up a public hospital authority to upgrade the management level of these hospitals; transform functionality of health authorities to encourage diversity in medical service providers. Second, establish the tiered decision making mechanism, autonomous operation system and diversified supervision system, to separate administration and management. Third, formulate articles of association for hospitals to encourage their evolution to legal entities, consolidating the government reforms by legal means.
6.Watchful questions on diagnosis and treatment of traumatic diaphragmatic rupture and diaphragmatic hernia

Objective To explore the clinical characteristics and the treatments on traumatic diaphragmatic rupture and diaphragmatic hernia.Method A retrospective study was carried out in 49 cases of traumatic diaphragmatic rupture and diaphragmatic hernia.Results There were 32 cases with left diaphragmatic hernia,13 cases with right diaphragmatic hernia and 4 cases with bilateral diaphragmatic hernia.One case with closed combined thoraco-abdominal wound and shock,died of multiple organ failure after the surgery,3 cases were diagnosed more than 24 h after injury,the others closed injury patients were diagnosed within 24 h and cured.In patients with open diaphragmatic rupture and diaphragmatic hernia,16 cases underwent laparotomy surgery,5 cases of misdiagnosis experienced re-thoracotomy,7 cases thoracotomy,2 cases changed the thoraco-abdominal surgery.Three cases underwent thoraco-abdominal surgery.Twenty-five cases were cured; 1 patient with suppurative costal chondritis,duration up to 13months.Conclusion Traumatic diaphragmatic rupture and diaphragmatic hernia is easily missed and misdiagnosed,the establish of rational examination preoperatively can reduce complications,misdiagnosis and mortality.
7.Decreased Cyr61 under hypoxia induces extravillous trophoblasts apoptosis and preeclampsia.

During placental development, oxygen environment is not only critical for trophoblasts migration and invasion, but also fundamental for appropriate placental perfusion. Cysteine-rich 61 (Cyr61, CCN1) was expressed in the extravillous trophoblasts (EVTs) and decreased in preeclampsia. Its regulatory properties in human first-trimester extravillous trophoblast cell line (TEV-1 cells) upon a low oxygen tension were investigated. The present study examined functional changes involved in adaptation to hypoxia of the TEV-1 cells, using cobalt chloride (CoCl(2)) as hypoxic mimic. It was found that hypoxia inhibited growth of TEV-1 cells and induced the increase of cell apoptosis (P<0.05). The Cyr61 expression in human EVTs was transcriptionally induced by CoCl(2). Inappropriate EVTs apoptosis has been implicated in the failure of trophoblasts to fully invade and modify the uterine environment and Cyr61 down-regulation, potentially leading to preeclampsia.
8.Endothelial nitric oxide synthase traffic inducer in the umbilical vessels of the patients with pre-eclampsia.

The expression of endothelial nitric oxide synthase traffic inducer (NOSTRIN) was examined in the umbilical vessels of the patients with pre-eclampsia (PE) to explore its possible role in the pathogenesis of PE. The NOSTRIN mRNA in umbilical tissues was determined by RT-PCR. The eNOS activity in umbilical vessels was spectrophotometrically detected. NO2-/NO3-, the stable metabolic end products of NO, was measured by using nitrate reductase. RT-PCR showed that the expression level of NOSTRIN was significantly higher in women with PE than in the normal group (P<0.01). The activity of eNOS was significantly decreased in PE group [(12.83+/-3.61) U/mg] than in normal group [(21.72+/-3.83) U/mg] (P<0.01). The level of NO2-/NO3- in PE patients (27.53+/-7.48) micromol/mg was significantly lower than that of normal group (54.27+/-9.53) micromol/mg (P<0.01). The significant negative correlation existed between the expression of NOSTRIN and the activity of eNOS in umbilical vessels of women with PE (r=-0.58, P<0.01). It was concluded that the level of NOSTRIN expression was increased in umbilical vessel of women with PE, indicating that it may be involved in the pathogenesis of PE.

9.Molecular cloning and nucleotide sequence analysis of the S-segment in Hantavirus isolated in Zhejiang province

One strain of Hantavirus(HV) was isolated from patients with hemorrhagic fever with renal syndrome (HFRS) in Zhejiang province and its S-segment was cloned and submitted to nucleotide sequence analysis in order to determine the type of strain and extent of genetic variation for further study on its evolution and mutation .The HV antigen was detected from mouse lungs in endemic area by direct immunofluorescene test and the HV-positive sample of mouse lung was inoculated to Vero-E6 cells to isolate virus. The total cellular RNA was extracted from infected cell culture and the S segment gene was amplicated by RT-PCR. Then, the purified PCR product was cloned into T vector for sequencing. The result showed that the full-length sequence of the S segment was 1 700 bp with one open reading frame (ORF) encoding 429 amino acids. Comparison with Hantavirus HTN type showed 85.7%-91.9% homology at the nucleotide level. In comparison with the SEO type of viruses, the homology at nucleotide level was shown to be 71.2%-75%. This new HV strain may be typed as to HTN virus and may be a new subtype of this virus.
