Chinese Journal of Clinical Oncology 2014;(18):1141-1145
This paper introduces a new treatment method for cancer, namely, cancer nutrition therapy (CNT). CNT is the plan-ning and delivery of nutrition intervention to treat cancers and their complications and improve therapeutic outcomes. The proposed treatment has three phases:(a) Assessing nutritional status;(b) Providing therapy, ranging from counseling, dietary modification, and oral supplementation to nutrient delivery by tube or parenteral feeding;and (c) Monitoring and follow-up, which are critical to assure that CNT could achieve its goals and appropriate for the dynamic nutritional status of the patient.

Objective To identify problems and reduce death and disability rates in emergency care so as to provide basis for upgrading the emergency care network. Methods Questionnaires were filled out with information from the medical records of 1446 cases that had been admitted for emergency care to 60 hospitals of the city's emergency care network and that had died in 2002, a database was set up with Profox 6 0, and a statistical analysis was made with SPSS 11 0. Results The median time for various emergency care procedures was: 5 minutes for the issuing and execution of the first medical order respectively and also for the arrival of a senior doctor; 10 minutes for the arrival of the doctor on call; 120 and 180 minutes after admission respectively for case discussion and intra-hospital consultation; 60 minutes after admission for the start of blood transfusion. It was found that some doctors had a poor grasp of emergency care techniques and procedures, the entry of time was not detailed enough in some medical documents, and some rules and regulations failed to be carried out. In addition, differences existed in the spectrum of causes of post-emergency hospitalization death and pre-hospitalization death. Conelusion Improvements need to be made in such aspects of emergency hospitalization care as the detailing of time, the grasp of techniques and the guarantee of quality. And more attention needs to be attached to emergency care.

Objective To estimate the clinical characteristic and relevant problems of Roux-en-y surgical therapy for patients with gastric carcinoma in elderly patients. Method The clinical data of 86 elderly patients with gastric carcinoma over over years old were analyzed retrospectively. Result 86 cases received operations research,59 eases received distal end subtotal gastreetomy, 5 cases received total gastrectomy ,20 cases received proximal endsub-total gastrectomy, and 2 cases received short circuit, stoma or exploratory laparotomy. Conduslon Gastric carcinoma in elderly patients is not easy to find, and it is with more exist diseases, more postoperative complication. So it ineces-sary to enforce perioperative period management, and the options of operation methods should follow individual princi-ple.

Objective Pulmonary benign metastasizing leiomyoma (PBML) is a rare entity that leiomyoma of uterus metastasized to the lung. The clinical characteristics of this rare disease were analyzed in this article. Methods The detailed clinical records of 7 patients diagnosed as PBML at Peking Union Medical College Hospital between January 2001 and June 2015 were reviewed. Results All patients were women with median age of 44 years (range 28-62).Symptoms included dyspnea (2/7),chest pain (1/7),cyanosis (1/7),cough (1/7) and bloody sputum (1/7),while 4/7 cases were asymptomatic.Six patients had the past-history of leiomyoma of uterus 20 months to 14 years ago among whom 5 patients received hysterectomy.Chest CT showed bilateral,random-distributed multiple round solid nodules,or diffuse-distributed miliary nodules,or single solid nodule, even some small cavities.Extra-pulmonary metastasis was found in left superclavicular lymph node (1 case) and right heart (1 case).Histological tissues were obtained by video-assisted thoracic surgery lung biopsy (4/7),mass resection on tricuspid valve (1/7),transbronchil lung biopsy (1/7),and CT-guided percutaneous lung biopsy (1/7).Pathology showed an interlacing pattern by spindle cells having elongated nuclei without cellular atypia.Ki-67 index was less than 1%.Molecules such as smooth muscle antibody, estrogen receptor (ER) and progestrone receptor (PR) were positive in immunohistochemistry staining.Neither letrozole nor zoladex was effective. Two patients responded to bilateral adnexectomy, presenting as shrunk nodules. No relapsed disease was seen in one patient with single nodule after resection. There was only one patient with disease-related mortality, whose chest CT showed milliary nodules. Conclusion Although CT findings of PBML are similar to malignancies, the clinical outcome is good.Despite the positive expression of ER and PR,the effectiveness of hormone related treatment is limited. And periodical follow up is suggested even to those uneventful patients.

Chinese Journal of Clinical Nutrition 2009;17(6):368-372
Lipids metabolism can change after traumas such as operation or injury. The process is com-prised of accelerated hydrolysis of triglyceride in adipose tissue, accelerated absorption of fatty acids by extra-he-patic tissue, improved lipid oxidation rate, and augmented re-esterification of fatty acids in liver and adipose tis-sue.Mechanisms of lipid mobilization include sympathetic stimulation, insulin resistance, and inflammation.How-ever,lipid absorption by liver and intestine is inhibited after trauma.Through post-trauma nutritional support,glu-cose administration improves re-esterification of fatty acids and inhibits lipid oxidation without affecting lipolysis.Fat emulsion infusion inhibits both absorption and secretion of triglyceride in liver. Therefore, clinicians should a-void over-alimentation of glucose and fat to prevent lipid overload, particularly in liver. Intervention of post-trau-matic lipid metabolism improves the outcomes:administration of long-chain triglycerides can prevent exhaustion of essential fatty acids, and administration of cholesterol can improve absorption and oxidation of fat emulsion.

Objective:To find out the role of JNK/SAPK signaling-transduction pathway in the effect of elemene against hepatocarcinoma, offering the clue to ilustrate the molecular mechanisms of antitumor effects of elemene. Methods: The detection of the distribution of elemene in Hca-F cells was detected by gas chomatography and apoptotic changes in elemene treated. SMMC7721 cells were examined by TEM. After elemene treatment, the activation of JNK/SAPK in HepG2 cellls and the DAXX gene expression in SMMC7721 cells were detected by Western blotting and RT-PCR respectively. Results: Gas chomatography showed that elemene was detected at 8. 42 minute. The SMMMC7721 cells treated by elemene for 3 hours began to show typical apoptotic changes . The JNK/SAPK activity in HepG2 cell treated with heat shock was the highest of all groups and the group treated with elemene was the next and the control group is the lowest one. There was no DAXX gene expression in SMMC7721 cells treated with elemene. Conclusion: Elemene can diffuse into cells. Tumor cell apoptosis treated with elemene may be induced by JNK/SAPK activating and DAXX signal pathway may not play key role in JNK/SAPK activation induced by elemene.

[ ABSTRACT] AIM:To cultivate stem-like spheres from SW620 cell line in the specific serum-free medium and evaluate the features of the cancer stem cells, and to investigate the effects of docosahexaenoic acid ( DHA) and eicosapen-taenoic acid ( EPA) on the growth of SW620 stem cell-like cells.METHODS: Human colon cancer stem cell-like cells ( CSCLC) were obtained from SW620 spheres cultured in serum-free medium. These cells were tested for the expression of SSEA-1 and TRA-1-81 by immunofluorescence staining. The mRNA expression of Sox-2 and Oct-4 was detected by real-time PCR.The efficiency of colony formation on a soft agar gel and tumor formation in the nude mice was compared between SW620 adherent cells and CSCLC.The inhibitory effects of 5-fluorouracil (5-FU) and mitomycin C on both types of cells were measured by MTS assay.MTS assay, Annexin V/PI staining and trypan blue staining were used to determine the effects of DHA and EPA on both types of cells.MTS assay was also used to analyze the combined effect of DHA or EPA with chemotherapeutic drugs on SW620 CSCLC.RESULTS:SW620 cells formed spheres in serum-free culture. The cells from spheres highly expressed SSEA-1 and TRA-1-81, transiently expressed Sox-2 and Oct-4 genes and were more resistant to 5-FU and mitomycin C treatments. These cells exhibited a greater ability in clone formation and tumorigenicity, indica-ting that these cells carried stem cell-like features, hence were considered SW620-derived CSCLC.DHA and/or EPA sup-pressed SW620 CSCLC by inhibiting cell growth, inducing cell apoptosis and sensitizing them to chemotherapeutic drugs. CONCLUSION:The cells with stem cell-like features, such as high efficiency in clonogenicity, tumorigenicity and resist-ance to chemotherapeutic drugs, can be obtained from SW620 spheres cultured in serum-free condition.DHA and EPA in-duce apoptosis in SW620-derived CSCLC and sensitize them to chemotherapeutic drugs.

Objective To observe the therapeutic effects and action mechanism of moxibustion in the treatment of intestinal fibrosis in ulcerative colitis. Methods Tianshu (ST 25) and Qihai ( CV 6) were the main acupoint; other points were used according to syndrome differentiation. Sixty-five patients were randomized into two groups: Group A in which 32 cases were treated by herbpartitioned moxibustion and group B in which 33 cases were treated by bran-partitioned moxibustion. The therapeutic effects and TGF-β1, Ⅰ and Ⅲ collagen expressions were measured before and after treatment. Results In group A, 17 cases were cured, 12 cases improved and 3 failed; in group B, 1 1 cases were cured, 16 cases improved and 6 cases failed. Ⅰ , Ⅲ collagen expressions were obviously inhibited in the two groups, in group A in particular. Conclusion In the prevention and treatment of intestinal fibrosis, moxibustion may reduce the expression of TGF-β1, and hence to block or inhibit the synthesis of Ⅰ、Ⅲ collagens, and improve the structure and function.
