1.Clinical analysis of imatinib mesylate in the treatment of chronic myeloid leukemia

Objective To study and explore the clinical effect of imatinib mesylate in the treatment of chronic myeloid leukemia. Methods From January 2013 to January 2017,60 patients with chronic myeloid leukemia in the Second Hospital of Jiaxing were included in the study. The patients were randomly divided into control group and observation group,with 30 cases in each group. The control group was given routine chemotherapy, the observation group was given routine chemotherapy combined wth imatinib mesylate orally. After 6 months of treatment,the clinical curative effect, the incidence of adverse reactions, immune function, quality of life score of the two groups were compared. Results The response rate of the observation group was 76. 67% (23/30),which was higher than 50. 00%(15/30) of the control group (χ2 =4. 593,P <0. 05). The incidence rate of adverse reactions in the observation group was 13. 33%(4/30),which was significantly lower than 36. 67%(11/30) in the control group (χ2 =4. 356,P<0. 05). Before treatment,the immune function indicators between the two groups had no statistically significant differences (t=0. 168,0. 287,0. 156,all P>0. 05). After treatment,the indicators of immune function in the observation group were higher than those in the control group(t =4. 482,3. 731,3. 361,all P <0. 05). After treatment,the quality of life scores in the observation group were higher than those in the control group( t=8. 898, 5. 945,9. 309,5. 679,all P<0. 05). Conclusion Imatinib mesylate in the treatment of patients with chronic myelog-enous leukemia can effectively improve the efficacy and safety of chemotherapy,reduce the effect of chemotherapy on the immune function of patients,improve their quality of life.
2.Epidemiological characteristics of infected patients aged 60 years and older during the outbreak of SARS-CoV-2 Omicron variant in Sanya, Hainan in 2022

or older during the outbreak epidemic of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) Omicron variant in Sanya City, Hainan Province, August-September 2022, and to provide scientific and effective basis for formulating strategies for the prevention and treatment of COVID-19 in the elderly in the future. Methods The data of 1 785 COVID-19 cases aged ≥60 years reported from August 1 to September 8, 2022 in Sanya City, and 537 local COVID-19 inpatients aged ≥60 years treated in Sanya Central Hospital were collected and analyzed using WPS Office 2018 software for data cleaning, database assistance, and drawing, and SPSS 22.0 software was used to statistically describe and analyze the counting data. Results A total of 1 785 local COVID-19 cases aged ≥60 years were reported in China's Disease Prevention and Control Information System, including 1 509 confirmed cases (84.54%) and 276 asymptomatic cases (15.46%). Of the confirmed cases, 1 448 were mild cases (95.96%), 40 were common cases (2.65%), 10 were severe cases (0.66%), and 11 were critical cases (0.73%), and no deaths were reported. The ratio of male to female was 0.88∶1. The youngest was 60 years old, the oldest age was 107 years old, with a median age of 67 years old. Most of them (1 559 cases, 87.34%) were non-social employed personnel. Among them, 952 (53.33%) were from Tianya District and 733 (41.06%) from Jiyang District. Among the 537 COVID-19 elderly infected patients hospitalized in Sanya Central Hospital, infected patients with underlying diseases accounted for 287 (53.45%) cases of hypertension, diabetes and other types of basic diseases (chronic kidney disease, hyperlipidemia, chronic gastritis, gastric ulcer, etc.), and there were serious infections. Conclusions Most of the elderly infected with Omicron variants were mild cases with few symptoms, but the elderly infected with Omicron variants were difficult to recover or even worsen the condition due to old age and underlying diseases. Therefore, by analyzing the various aspects of elderly infected cases, scientific and effective basis can be provided to prevent and control the epidemic situation in the future, and reduce the occurrence of severe, critical and death cases of COVID-19 in the elderly.
3.Endoscopic retrograde cholangiopancreatography in management of biliary complications after liver transplantation

Objective To evaluate the effect of endoscopic retrograde cholangiopancreatography (ERCP) in treatment of biliary complications after liver transplantation. Methods Data of 39 patients who underwent ERCP between January 2005 and December 2007 because of biliary complications after liver trans-plantation were retrospectively evaluated. Endoscopic sphincterotomy, dilatation, nasal-biliary drainage (ENBD) and stent placement were performed in 25 patients with biliary strictures (14 at anastomosis site and 11 at non-anastomosis site). ENBD and stent placement were applied in 6 patients with biliary leakage, while endoscopic sphincterotomy, ENBD and stone extraction with baskets were performed in 16 patients with biliary stones. Procedures were repeated when necessary. Results ERCP was successfully performed at a rate of 95.9% (94/98) without any severe complications. Strictures at anastomosis site were resolved in all patients (100%, 14/14), while for strictures at non-anastomosis site, only 27.3% (3/11) were cured. Biliary leakage was resolved in 83.3% (5/6) patients. Complete bile duet clearance was achieved in 81.3% (13/16) of the patients with biliary stones. Conclusion ERCP proves to be safe and effective in the treatment of post liver transplantation biliary complications with low incidence of severe complications.
4.The effect of alpha lipoic acid and Shuxuetong on islet function in patient with type 2 diabetes mellitus

Objective To explore the effect of alpha lipoic acid and Shuxuetong on oxidative stress and insulin resistance in patient with type 2 diabetes mellitus(T2DM)and to guide the clinical therapy for T2DM.Methods Totally 90 cases of T2DM pa-tients were sellected from Hainan Province Nongken Hospital.The patients were divided into Shuxuetong group(group A,n=30), alpha lipoic acid group(group B,n=30)and alpha lipoic acid and Shuxuetong group(group C,n =30).HOMA-IR,HOMA-β,and HOMA-ISI was calculated.SOD was determined by xanthine oxidase method.MDA was detected by Thiobarbituric acid method. The expression of IL-6 and TNF-αwas determined by ELISA analysis.Results Before treatment,SOD,MDA,IL-6,TNF-α,HO-MA-IR,HOMA-ISI,and HOMA-βwas no significant difference among the three groups(P >0.05).After treatment,all of which was significantly changed(P <0.05 and P <0.01).SOD,HOMA-ISI and HOMA-βin group C was significantly higher than that in group B(P <0.05),and which was significantly higher in group B than that in group A (P <0.05).MDA and HOMA-IR in group C was significantly lower than that in group B(P <0.05),and which was significantly lower in group B than that in group A (P <0.05).IL-6 and TNF-αin group C was lower than that in group A and group B(P <0.01),and which was no significant difference between group A and group B(P >0.05).Conclusion Alpha lipoic acid combined with Shuxuetong injection can significantly im-prove the oxidative stress state of T2DM patients,inhibit the inflammatory reaction,improve insulin resistance and patient′s sensi-tivity to insulin.Which was contribute to clinical treatment for T2DM.
5.Accurate Detemination of Isotopic Abundance of Intracellular Metabolites of Saccharopolysporaerythraea Based on Ultra Performance Liquid Chromatography-Triple Quadrupole Mass Spectrometry

A method for measuring 13C isotopic abundance of intracellular metabolites of Saccharopolysporaerythraea by ultra-high performance liquid chromatography (UPLC)-triple quadrupole mass spectrometry was established.First, the chromatographic conditions of UPLC were optimized, and then the MS conditions such as unique tube lens voltage, collision energy, and ion pair were optimized.On the bases of length of the parent and daughter ions carbon chains and whether the daughter ions contain 13C atoms, the one-to-one method, one-to-many method and SIM method were established for measuring 13C isotopic abundance.Then these methods were used to measure naturally labeled intracellular metabolite standards and 13C labeled samples, and according to the gap between the experimental value and the theoretical value, the best method was established for each metabolite of different characteristics.The results showed that one-to-one method was most effective for measuring the metabolites of daughter ions not containing 13C atoms represented by sugar phosphates, one-to-many method was the best for measuring the metabolites of both parent and daughter ions containing 13C short carbon chains represented by carboxylic acids, SIM method could play a role in measuring the metabolites of both parent and daughter ions containing 13C long carbon chains represented by coenzyme A.This method had a good measurement precision and could be applied to the measurement of Saccharopolysporaerythraea intracellular metabolites, which contributed to the consequent study of metabolic mechanism and the efficient expression of erythromycin.
6.The clinical study of effect on prevention postpartum hemorrhage of caesarean section by used calcium gluconate combined with oxytocin,misoprostol

Objective To discuss effect on prevention of postpartum hemorrhage of caesarean section by us- ing calcium gluconate combined with oxytocin and misoprostol.Methods 385 cases of caesarean section were select- ed and randomized into O(Oxytocin) group and OM(Oxytocin+ Misoprostol) group and COM (Calcium gluconate+ Oxytocin+Misoprostol)group.Results The mean operative blood loss in O group and OM group and COM group were (300?50.24)ml,(220?30.83) ml,(150?45.52) ml.The amount of the mean operative blood loss of COM group was significantly lower than those of O group and OM group(P<0.05).The amount of bleeding of 2 hours after delivery in O group and OM group and COM group were (400?45.52)ml,(260?60.43)mi and(210?50.54) ml.The amount of bleeding of COM group was significantly lower than those of O group and OM group (P<0.05).Conclusion The prevention by used calcium gluconate combined with oxytocin and misoprostol is efficient in reducing the amount of postpartum hemorrhage of caesarean section.The operation of medicine is easy and safe and economic.
7.Effect of Hepatitis C Virus Core Protein on Interferon-Induced Antiviral Genes Expression and Its Mechanisms

Emerging data indicated that HCV subverts the antiviral activity of interferon (IF); however,whether HCV core protein contributes to the process remains controversial. In the present study, we examined the effect of HCV core protein on interferon-induced antiviral gene expression and whether the effect is involved in the activation and negative regulation of the Jak/STAT signaling pathway. Our results showed that, following treatment with IFN-α, the transcription of PKR, MxA and 2'-5'OAS were down-regulated in HepG2 cells expressing the core protein. In the presence of HCV core protein,ISRE-dependent luciferase activity also decreased. Further study indicated that the core protein could inhibit the tyrosine phosphorylation of STAT1, whereas the level of STAT1 expression was unchanged.Accordingly, SOCS3, the negative regulator of the Jak/STAT pathway, was induced by HCV core protein. These results suggests that HCV core protein may interfere with the expression of some interferon-induced antiviral genes by inhibiting STAT1 phosphorylation and induction of SOCS3.
8.Botulinum toxin type A repeated injections into detrusor to treat detrusor hyperreflexia and neurogenic incontinence in spinal cord injury patients: 6 cases report

ObjectiveTo evaluate the effects of Botulinum toxin type A(BTX-A) repeated injection into detrusor to treat detrusor hyperreflexia and neurogenic incontinence in spinal cord injury(SCI) patients.Methods6 patients with SCI were included into the study.300 U of BTX-A was dissolved in saline 15 ml,and injected into 30 different sites in detrusor using a flexible cystoscopic needle.BTX-A injection was repeated 3 times for all the patients.Urodynamic examination,voiding diary and toxic effects were recorded.ResultsThe improvement for symptoms was observed within 5 to 21 days.The effects of injections lasted averagely 8.9 and 5.8 months for each injection.After the first injection,the mean frequency of incontinence decreased from 10.1 /d to 3.3 /d.The mean volume of intermittent catheterization(IC) increased from 98.5 ml to 404.2 ml each time.Mean cystometric volume increased from 95.4 ml to 385.6 ml,mean maximum storage detrusor pressure decreased from 105.8 cmH2O to 31 cmH2O.After the second injection,the mean frequency of incontinence decreased from 9.7/d to 3.7/d.The mean volume of IC increased from 108.3 ml to 387.2 ml each time.Mean cystometric bladder storage volume increased from 105.4 ml to 375.6 ml.Mean maximum storage detrusor pressure decreased from 97.8 cmH2O to 33 cmH2O.After the third injection,the mean frequency of incontinence decreased from 9.2/d to 3.9/d.The mean volume of IC increased from 115.7 ml to 363.9 ml each time.Mean cystometric volume increased from 102 ml to 357.6 ml.Mean maximum storage detrusor pressure decreased from 98.1 cmH2O to 36.9 cmH2O.The patients were followed up for 6 months.No adverse and toxic effect was observed.ConclusionBotulinum toxin type A repeated injection into detrusor is effective,safe,feasible and mini-invasive to treat detrusor hyperreflexia and neurogenic incontinence in patients with SCI.Botulinum toxin type A repeated injection into detrusor is effective,safe,feasible and mini-invasive to treat detrusor hyperreflexia and neurogenic incontinence in patients with SCI.
9.Preliminary clinical report of treatment for neurogenic bladder by sacral neuromodulation using a new tined-lead electrode

ObjectiveTo explore the effects of sacral neuromodulation using a new tined-lead electrode on neurogenic bladder.MethodsThe use of a new tined-lead electrode for sacral neuromodulation was evaluated in a study including 5 consecutive patients with neurogenic bladder.The tined leads were implanted at the S3 foramen under the X-ray screening.Subjects completed the recording of detailed voiding diary pre-and post-operation including fluid intake,voided volume,leaked volume,catheterized volume,frequency,accompanying symptoms and sensation.Vesicourethral function was assessed by video-urodynamics.ResultsUrinary frequency and voided volume were improved 22% and 49% respectively in one patient with spinal bifida.Urinary frequency,voided volume and residual volume were improved 0.7%,11% and 46% respectively in another one.Urinary frequency,voided volume and residual volume were improved 0.4%,18% and 44% respectively in the third one.Frequency of leakage and leaked volume were improved 36% and 54% respectively in the patient with brain trauma.Frequency of CIC and catheterized volume were improved 42% and 54% respectively,and indexes of urodynamics were improved 37%~45% in the patient with spinal cord injury.ConclusionA new tined-lead electrode for sacral neuromodulation provide a new alterative and minimally invasive procedure to treat neurogenic bladder.
10.Application of transurethral holmium laser enucleation of the prostate to the aging patients: 50 cases report

ObjectiveTo investigate the safety and efficiency of transurethral holmium laser enucleation of the prostate(HoLEP) for the aged.Methods50 old patients(above 70) with benign prostatic hyperplasia(BPH) accepted HoLEP.All patients were assessed with IPSS,QOL,Qmax and PVR before and 3 months after operation.ResultsBefore the treatment,the mean IPSS was 22.9 and was 9.7 3 months after operation,while the mean QOL was 5.3 and 2.1,Qmax was 7.2 ml/s and 14.3 ml/s,PVR was 127.5 ml and 19.3 ml.ConclusionHoLEP is a safe,effective procedures for treating BPH.
