1.A study on microsatellite instability status for colorectal serrated lesions

Objective To verify the traditional serrated pathway by comparing microsatellite instability(MSI) status among traditional serrated adenoma, traditional adenoma, serrated colorectal cancer and non-serrated coloreetal cancer. Methods Seventy-five paraffin-embedded tissue samples, including 15 with serrated adeneearcinoma (Sca), 20 with non-serrated adenocareinoma (N-Sca), 20 with traditional serrated adenoma(TSA) and 20 with villous adenoma(AD) were collected from the pathology department of our hospital. Genomic DNA was extracted from these samples and then amplified with fluorescently-labeled primer specific for BAT25 and BAT26. The MSI status was detected with DNA automatic sequencer. Results Six of 18 samples with TSA harbored MSI-H and twelve MSI-L/MSS; 18 samples with conventional adenoma were exclusively of MSS; 3 of 13 samples of serrated carcinoma harbored MSI-H and ten MSI-L/MSS; 18 of 19 N-Sca samples harbored MSI-L/MSS and only one MSI-H. With Chi-square test, the MSI frequency in AD group and N-Sca group was significantly lower than that in TSA group and Sca group ( P < 0. 05 ) ; but with no statistical difference between the TSA group and Sca groups ( P > 0. 05 ). Conclusion MSI-H frequency in AD group and N-Sca group was obviously lower than that of TSA group and Sca group. It is concluded that there might be a new traditional serrated neoplasia pathway which is different from the conventional adenoma-carcinoma carcinogenesis pathway, but we still need prospective follow-up studies to verify its existence.
2.Comparison of the behavior characteristics in SHR, WKY and SD rat models of attention deficit/hyperactivity disorder

Objective To compare the differences in behavior characteristics among SHR, WKY and SD rat models of attention deficit/hyperactivity disorder (ADHD), and explore an ideal control model of SHR rats.Methods Using open field test to analyze the rat movement distance, speed, wearing numbers and the number of grooming to evaluate the spontaneous movement in SHR, WKY and SD rats.Using the Morris water maze to test the learning and memory ability among the three rat groups.Results The result of open field test showed that the SHR rats had significantly increased (P< 0.01) total amount of exercise, average speed and wearing numbers than WKY and SD rats.Compared with the WKY rats, SD rats had a significantly higher movement distance (P< 0.01), slightly higher movement speed and wearing number (P< 0.05).In the Morris water maze hidden platform period test, the SD rats had a significantly longer latency than the SHR rats (P< 0.05).SD rats showed longer latency distance on the first, third and fourth days of training, as compared with the SHR rats (P< 0.05 or P< 0.01).Compared with the WKY group, SD rats showed a shorter latency distance in each training time (P< 0.05 or P< 0.01).In the probe trial period, the SD rats showed shorter time and distance ratio to the target quadrant than SHR rats (P< 0.05), while significantly longer than the WKY rats (P< 0.05 or P< 0.01).Conclusions There are significant behavioral differences between SHR and WKY rats, showing certain disadvantages in comparison of the two types of rats.To add SD rats as a control group for SHR rats can improve the comparability of behavior characteristics of SHR rats, and to get more objective evaluation of the behavior characteristics of SHR rats.
3.A case report of successful management of middle ear foreign body following hearing aid fitting with ossicular chains integrity.

A case of ear mold impression middle ear foreign body was reported. In the exploratory operation we found the ear mold impression surrounded the ossicular chains. During the process of removal the middle ear foreign body, the incudostapedial joint was protected from obvious external force and ossicular chains were kept integrity. Tympanoplasty was completed after management of foreign body. The patient recovered well postoperatively and was discharged as scheduled.

4.A multifunction glasses in the recovery term of cycloplegia and its primary application

Objective:To design functionality glasses in order to solve or alleviate various visual disorder during cycloplegia.Methods:Based on the principle of the polarizing lens and pinhole lens, the glasses frame was combine with them so as to be suitable for the recovery of cycloplegia and for the usage in near and distant vision situation.Results: This glasses has a series of advantages including small size, light weight, convenience and beauty. Through the combination of different lenses, the symptom of visual unclear during cycloplegia wasimproved, and the symptom of glare and squint in distant vision also was improved. Besides, the symptom that adjustment of near vision couldn't be activated was improved, and near vision blurry leaded by peripheral aberration coming from mydriasis also was improved. On the other hand, this device was easy and simple on combination and was convenient on usage.Conclusion: This design contributes to correct refractive error, and enhance the definition of near or distant vision. And it is suitable for various situation of usage including near, distant, indoor and outdoor.
5.Expression of CXCL8 and its receptors (CXCR1 and CXCR2) in peripheral blood neutrophils of chronic hepatitis B

Objective:To study on the levels of CXCL8 and its receptors (CXCR1 and CXCR2) in peripheral blood neutrophils of the patients with chronic hepatitis B.Methods:The neutrophils were isolated and purified by neutrophil isolation medium,and the loads of HBV-DNA in neutrophils were detected by PCR,and the levels of HBeAg in serum were measured by ELISA.The patients were divided into different groups according to the detective results so that the expressions of CXCL8 and its receptors ( CXCR1,CXCR2) in neutrophils were detected by the methods of streptavidin-biotin complex ( SABC ) immunocytochemistry stain.Results:The data of SABC immunocytochemical stain showed that the positive color of CXCL8 was mainly located in the cytoplasm of PMNs.However,the most positive color of CXCR1and CXCR2 was mainly expressed in the cytoplasm and cell membrane.Interestingly,the deeper immune coloring of CXCL8 and CXCR1, and relatively shallow immune coloring of CXCR2 were explored in the group with positive of HBeAg.The similar detective results also had been found in the cases with positive of HBV DNA in neutrophils.Compared with the normal control group,the levels of CXCL8 and CXCR1 in the patients were significantly increased ( P<0.05) ,but the differences were too small to be statistically significant in the level of CXCR2 (P>0.05).Conclusion:After neutrophils occult infected by HBV,not only the secretion of CXCL8 can be promoted, but also the expression of CXCR1 will be further increased.The data of immunohistochemical staining have been shown that the color degree of CXCL8 and its receptors ( CXCR1, CXCR2 ) are positive correlation to the level of HBeAg and the loads of HBV DNA.More PMNs can be chemotactic attraction to lesion so as to participate in the local inflammatory injury and tissue repair via the interactive pathway of the high expression of CXCR1 on surface of neutrophils with CXCL8.
6.Effects of Fatty Oil of Acute Common Perilla (Perilla frutescens)on the Learning and Memory of Mice

Different doses of fatty oil of Perilla frutescens var. arguta (Benth. ) Hand. -Mazz. (ig )to mice for i5 days can decrease errors in step-down test, improve percentage of accuracyand shorten the time to reach their goal in water maze experiments. lt also facilitate thesynthesis of RNA, DNA and proteins in brain and regulates the level of monoamine -like ne-urotransmitters. The above results indicated that fatty oil of P. frutescens var .arguta canimprove learning and memory in mice related to the symthesis of RNA, DNA, proteins andneurotransmitters in braiu.

Objective Adenosine receptor agonist NECA has a certain myocardial protection, but the specific mechanism is not clear.This paper aimed to study the effect and mechanism of NECA inhibiting endoplasmic reticulum stress to against myocardial ischemia reperfusion injury in rats.Methods 56 Wistar rats of SPF grade were selected and divided into Sham group, I/R group, NECA group and TUDCA group through random number table method.We established the isolated rat heart ischemia reperfusion model by using the Langendorff device.Sham group: heart threaded but not ligated, Kerb-Henseleit buffer continuous infusion 170min;I/R group: heart stability 20min, ischemia 30min, reperfusion 2h;NECA group and TUDCA group: heart stability 20min, ischemia 30min, reperfusion 2h, perfusion solutions containing 0.1μmol/L NECA and 30μmol/L TUDCA were respectively given at 5min before reperfusion, and ended at 30min after reperfusion.Transmission electron microscope was used to evaluate alterations of the myocardial ultrastructures.Western blot analysis was used to detected the expression levels of endoplasmic reticulum stress IREl-XBPl signaling pathway marker protein IRE1α, XBP1s.Immunohistochemical staining was used to detect the expression of IRE1α.Results The results of transmission electron microscopy showed that most of the myofilament ruptured, sarcomere contracture deformation, visible mitochondrial vacuoles degeneration in I/R group, and injury in NECA group and TUDCA group were less than the I/R group, appeared as the filaments arranged more neat, sarcoma only had mild contracture.Immunohistochemical results showed that IRE1αpositive staining was not found in the sham group, and the area of positive staining of IRE1α in I/R group was significantly increased, while the NECA group and TUDCA group were significantly decreased.Compared to the Sham group, the expression level of IRE1α and XBP1s was significantly increased in I/R group(P<0.05);but compared with the expression level of IREα and XBP1s in I/R group(1.72±0.27, 0.97±0.19), the NECA group(1.14±0.16, 0.6±0.13) and the TUDCA(1.07±0.27, 0.58±0.15) group were significantly decreased(P<0.05).Conclusion NECA can reduce endoplasmic reticulum stress through inhibiting IREl-XBPl pathway to protect the myocardium.
9.Application of TQC Method to Strengthen the Drug Quality Control in Hospital Pharmacy

In order to raise clinic safety and use drugs reasonably and effectively, the corresponding tactics were taken for the related problems that influence drug quality. Thus, scientific management of drugs, improvement of preparation quality, decrease of consume, and increasing income and sparing expenses were attained.
10.Impact of continuous quality improvement initiatives on clinical outcomes in peritoneal dialysis

Objective We evaluated the role of a quality improvement initiative in improving clinical outcomes in peritoneal di-alysis ( PD) . Methods In a retrospective analysis of 6 years of data from a hospital registry, the period between 1 July 2005 and 30 June 2008 ( control group) provided baseline data from before implementation of systemic outcomes monitoring, and the period between 1 July 2008 and 30 June 2011 [continuous quality improvement (CQI) group] represented the time when a CQI program was in place.Per-itonitis incidence, patient and technique survival, cardiovascular status, causes of death, and drop-out were compared between the groups. Results In the 370 patients of the CQI group and the 249 patients of the control group, the predominant underlying kidney diseases were chronic glomerulonephritis and diabetic nephropathy.After implementation of the CQI initiative, the peritonitis rate de-clined to 1 episode in 77.3 patient-months from 1 episode in 22.6 patient-months.In the CQI group, the complicance of blood pressure was more significantly improved than the control group ( 67.8% vs 47.4%,P<0.05).During the 3 years of follow-up,cardiothoracic ratio and IVST were significantly increased in the control group [0.55 ± 0.08 vs 0.51 ±0.05,P<0.05,11.07 ±1.66 mm vs 10.25 ±1.38 mm, P<0.05〗.The difference of LVID between the two groups was signifi-cant at the 2nd and 3rd year of follow-up(P<0.05).Patient survival at 1, 2, and 3 years was significantly higher in the CQI group (97.3%, 96.3%, and 96.3%respectively) than in the control group (92.6%, 82.4%, and 67.3%respectively, P<0.001).Imple-mentation of the CQI initiative also appeared to significantly improve technique survival rates:95.6%, 92.6%, and 92.6%in the CQI group compared with 89.6%, 79.2%, and 76.8%in the control group (P<0.001) after 1, 2, and 3 years respectively. Conclusion Integration of a CQI process into a PD program can significantly improve the quality of therapy and its outcomes.
