1.To investigate the difference between B-mode ultrasonography and color Doppler twinkling artifact for detection of kidney stone

Objective:To compare the detective effect between color Doppler twinkling artifact and B-mode ultrasonography for kidney stones in order to develop an ultrasonic model that can more easily and more accurately detect kidney stone.Methods: 9 patients with kidney stone who were confirmed by CT scan in recently were selected to accepted two methods (color Doppler twinkling artifact and B-mode ultrasonography) to detect kidney stone. The video fragments of upper pole, interpolar and lower pole were created by three radiologists and the results were diagnosed by them.Results: In dual kidneys of the nine patients, 32 stones with average size of (8.9±7.5) mm were found. The sensitivity, specificity, positive predictive value and negative predictive value of the B-mode ultrasonography were 71%, 48%, 52% and 68%, respectively, while those of color Doppler twinkling artifact were 56%, 74%, 62% and 68%, respectively.Conclusions: In the detection of kidney stones, B-mode ultrasonography is more sensitive when is used alone, but color Doppler twinkling artifact has more high specificity. The color Doppler twinkling artifact should be applied in more clinical practices, and a new commercial ultrasonography mode that used basic acoustic difference and is special for detection of kidney stone should be researched and developed so as to improve the detection of kidney stone.
2.Evaluation of free fatty acid-induced insulin resistance with euglycemic-hyperinsulinemic clamping.

Objective To prove insulin resistance-induced by free fatty acid(FFA)with hyperinsulinemic-euglycemic clamp and assess the prevention from this resistance by N-acetylcysteine(NAC).Methods Twenty-four female Wistar rats were randomly divided into 4 groups,each of which received infusion of IH(20% intralipid +20 U/mL heparin),IH+NAC,NAC,and saline respectively for 7 h.Two-hour hyperinsulinemic-euglycemic clamp was made after 5h infusion.During the infusion and clamp the rats were awake.Blood samples were taken for measuring insulin,FFA and maleic dialdehyde(MDA)at 0 min,280 min,290 min,300 min,340 min,380 min,400 min,410 min and 420 min.The insulin sensitive index was calculated.Results Plasma FFA and MDA levels in IH group were significantly elevated compared with those in control,IH+NAC and NAC groups,P

By studying medical works during the Sui and Tang dynasty,we discussed the factors that doctors of that period believed to affect the dosage of medicine.We found that doctors of that period developed the dosage theory in terms of compatibility of medicines,onset time a disease,dosage form of medicine,constitution and age of patients,and toxicity of medicine.
4.Mesenchymal stem cells modulate immune responses in a rat renal transplantation model

Objective To discuss the effect of mesenchymal stem cells (MSCs) in modulating immune responses in a rat renal transplantation model.Methods An in vivo trial of cytology was performed in one centre from March to December in 2008.Wistar rat donors and Lewis rat recipients in a renal transplantation model were randomly divided into 4 groups:MSCs (low dose,1 × 106 )therapy,MSCs (high dose,1 × 107) therapy,CsA monotherapy,and no therapy.Biochemistry methods were used to detect the levels of creatinine in serum.The survival time,renal grafting function and pathological changes of transplanted renal tissues were observed.Results Animal survival was significantly prolonged by MSCs (high dose) therapy and CsA monotherapy as compared with the no therapy group (both P<0.01).Animal survival in the MSCs (low dose) therapy group was prolonged as compared with no therapy group (P<0.01),but shortened as compared with MSCs (high dose) therapy group (P<0.05) and CsA monotherapy group (P<0.05).The MSCs (high dose) therapy and CsA therapy groups showed no special changes in histology,hut the control group showed acute rejection.Conclusion MSCs down-regulated immune responses,reduced production of some inflammatory mediators,preserved graft function in the initial stage after transplantation,and prolonged animal survival,and these effects were the same as those of CsA therapy with 1 × 107/day.
7.Study on the shifting of Th1/Th2 balance in patients with interstitial cystitis

Objective To investigate possible disturbance of Th1/Th2 immunoregulation of blood serum and bladder mucosa in patients with interstitial cystitis(IC). Methods Blood serum and bladder mucosal specimens were collected from 16 female patients with IC and 16 female normal controis.The age of IC patients was 51.3±10.2 years.The age of normal controls was 53.1±9.6years.The expressions of Th1(IFN-γ,IL-2)and Th2 cytokines(IL-4,IL-10)were determined by enIFN-γ and IL-2 in IC blood serum were(4.57±2.92),(17.52±7.52)pg/ml and in normal blood serum were(4.11±2.27),(20.99±5.09)pg/ml.Such two factors had no significant differences in these two groups.The expressions of IL-4 and IL-10 in IC blood serum were(0.34±0.22),(4.37±2.34)pg/ml and in normal blood serum were(O.14±O.07),(1.18±0.61)pg/ml.Such two factors had sighad no significant differences with normal bladder tissues(IFN-γ:χ2=1.900,IL-2:χ2=0.514).The expressions of IL-4 and IL-10 in IC bladder tissues had significant differences with normal bladder tiswere(0.16±0.11)and(2.42±1.27)pg/ml but in high PUF group were(0.53±0.12)and(6.31±1.21)pg/ml.The expressions of these two factors had significant correlations with the degree of PUF,which intensified in the IC of high PUF. Conclusion The expression of Th2 type eytokines is predominant in IC blood plasm and bladder tissues,the expression of Th2 type cytokines in IC blood plasm and bladder tissues has positive correlation with the degree of PUF.
8.Relationship of potassium sensitivity test and PUF in interstitial cystitis

Objective To discuss whether potassium sensitivity test(PST) is correlated with PUF in Interstitial Cystitis (IC). Methods The data of 14 IC patients (female 13, male 1) were an-alyzed. The mean age was 48 years (range 35-67 years). The clinical symptoms included urinary fre-quency, urgency, pelvic and peritoneal region pain after bladder filling. All the patients met the diag-nostic criteria of NIDDK for IC. Dilatations by hyponome were performed, medicine including heparin-sodium, lidocaine, NaHCO3 were used by intravesical instillation. PST and the pelvic pain and urgen-cy/frequency patient symptom (PUF) were used for evaluation. The relationship of the PST and PUF was assessed by statistics. Results PST median decreased from 4.0 to 1.0 (P<0.01). PUF medi-an decreased from 27.5 to 13.5(P<0.01). PST was directly correlated with PUF (rs=0. 868, t= 4.418, P= 0.001 before treatment, rs = 0.779, t=4.300, P = 0.001 after treatment). Conclusions PST and PUF are correlated. Both can be used as index in diagnosis, differential diagno-sis, symptom severity and treatment effectiveness evaluation of IC.
9.Correlation study on mast cells' property and PUF scale in interstitial cystitis

Objective To compare the mast cells' property and the level of pelvic pain, urgency and frequency (PUF) scale in urinary bladder between interstitial cystitis (IC) patients and controls.Methods Eighteen cystoscopy biopsy specimens of interstitial cystitis patients and 12 controls were stained with 0.5% toluidine blue and immobilizated with Osmic Acid. Then the mast cells were observed and counted with light microscope and transmission electron microscope. The PUF scale and the number of mast cells between the 2 groups were compared. Results The mast cell's number of the interstitial cystitis samples(28-76 pieces/mm2) was significantly higher than that of the non IC persons' bladder tissues(7-15 pieces/mm2) (Z=3.927,P<0.01). 75.3% mast cells were in a state of being activated degranulation. The PUF scale of IC patients( 17-35 scores)was significantly higher than that of the non IC persons' (0-8 scores) (t=14.736,P<0.01). The PUF scale of the patient group did not have a linear IC relation with the mast cell's infiltrated number among the specimens (rs=-0.618,P=0.601). Conclusions Mast cell infiltration may be one of the characteristic pathological manifestations of IC. The association of mast cell infiltration and the PUF scale may be a new diagnosis criteria for IC.
10.The value of diagnosis and treatment of pulmonary function in children with refractory mycoplasma pneumoniae pneumonia

Objective To investigate the value of pulmonary function tests in the diagnosis and treatment of children with refractory mycoplasmapneumoniae pneumonia.Methods The clinical data of 58 children with refractory mycoplasmapneumoniae pneumonia (RMPP) from May 2015 to February 2016 were retrospectively analyzed in the Department of Paediawics, Shengjing Hospital of China Medical University.They have done bronchoscopy and interventional therapy in the acute phase.Pulmonary function test was performed 24 hours before bronchoscope examinations.The endoscopic manifestation and pulmonary function data were analysed in the study.According to whether the mucosa of endoscopic manifestation is complete, fifty-eight cases were divided into mild group and severe group, record the pulmonary function indexes of each of the children.Results Severe group decreased than mild group, both of large airway pulmonary function in children with acute severe MPP (FVC, FEV1, PEF) and small airway index (FEF25, FEF50, FEF75, FEF25-75%), and the difference was significant(P < 0.05).Conclusion In the acute phase, there may be different pulmonary function,such as, expressed as a normal, performance of the restrictive or obstructive ventilation dysfunction, performance ofrnixed ventilatory dysfunction, The most common one is the change of small airway function.The more severe of the endoscopic manifestation in RMPP acute phase, the more severe the same period of pulmonary function index decreased degree.The severity of RMPP in children can be predicted by the classification and severity of pulmonary function,it can be used as an important basis for the early identification of severe MPP.Pulmonary function indicators may further guide the need for bronchoscopy intervention treatment and to evaluate the curative effect.
