Continuous suppressive antiviral medication for the treatment of repeated genital herpes is often carried out, but few treatments with Kampo have been reported. Here, we report experience with a case in which the repeated recurrence of genital herpes was treated successfully with hochuekkito.
A 34-year-old woman had experienced pregnancy twice and delivered twice. She had also had repeated recurrences of genital herpes from several years earlier, and been treated at a nearby doctor. Her recurring herpes did not resolve, however, and she visited our department. In addition to these recurrences, general fatigue and appetite loss were observed, which were regarded as Qi deficiency, and she was started on hochuekkito 7.5 g/day. The general fatigue disappeared two weeks after the treatment. Herpes recurrences also decreased gradually, and symptoms were no longer observed after medication was stopped less than a year later.
In this case, genital herpes recurrences were decreased, while symptoms such as general fatigue and appetite loss were also improved. Here, genital herpes recurrence was regarded as Qi deficiency, which was possible to treat with hochuekkito.

We reviewed 15 cases of reconstruction of cervical esophagus by grafting with free jejunum, which has been performed in our hospital since 1987. Primary disorders were hypopharyngeal cancer (8 cases), cervical esophageal cancer (4 cases), mesopharygeal cancer (2 cases) and laryngeal cancer (1 case). Our team for this operation consisted of otolaryngologists, abdominal surgeons and a microvasucular surgeon. We took, on the average, 15 hours and a half for total operation and four hours and half for blood isolation of the jejunum. Postoperative troubles were as follows ; necrosis of the grafted jejunum (1 case), suture failure at the phyayngojejunal anastomosis (1 case), fistural formation at the pharyngoesophageal anastomosis (1 case), light dysphagia (3 cases), ileus (2 cases) and perforation of the jejunostoma (1 cace).

The purpose of this study was to examine the effects of knee flexion angles during maximum isometric hip extension. Ten healthy men performed maximum isometric hip extension in prone position at 15° and 90° knee flexion. Then, the hip extension torque was measured, and electromyographic (EMG) data were obtained from the biceps femoris long head, semitendinosus, semimembranosus, adductor magnus, and gluteus maximus muscles. The EMG data were full-wave rectified and integrated (IEMG). The IEMG values obtained during the measurement of isometric hip extension were normalized with the values collected at 90° knee flexion (normalized IEMG [NIEMG]). The hip extension torque at 15° knee flexion was significantly greater than that at 90° knee flexion. The NIEMG values from the hamstrings at 15° knee flexion significantly increased compared with those at 90° knee flexion. Meanwhile, the NIEMG values from the gluteus maximus at 90° knee flexion were significantly greater than those at 15° knee flexion. However, the NIEMG values from the adductor magnus did not significantly differ between 15° and 90° knee flexion. These results indicate that the hamstrings effectively generate contracting force during isometric hip extension and at knee extended position because its fiber length was close to the optimal length.

A 36-year-old previously healthy woman with cardiomegaly on a routine chest X-ray was given a diagnosis of primary chylopericardium after pericardial puncture revealed milky effusion. Endoscopy-assisted ligation of the thoracic duct and creation of a pericardial window was performed. The operation was greatly facilitated by the preoperative three-dimensional CT scan with lymphangiography that precisely demonstrated the distribution of the thoracic duct and other lymphatic ducts.

The incidence of wound infection and delayed wound healing was greater in neonates and infants less than 3 months old who had undergone open heart surgery through a median sternotomy than in older patients. To reduce these problems, we stopped using continuous absorbable braided suture for skin and subcutaneous tissue closure in August 2005, and used interrupted non-absorbable monofilament suture instead. Around the same time, we adopted hydrocolloid dressing as a substitute for gauze dressing. We evaluated the effectiveness of wound management by comparing 28 patients who had undergone surgery before August 2005 with 22 patients who underwent surgery after that date. The age at surgery was 45±30 and 21±23 days, respectively. The patients in the earlier period were significantly older than in the later period. There were no significant differences in body weight at surgery, operating time, or cardiopulmonary bypass time between the groups. The time for wound closure was 30±11 and 22±4 min, respectively, and the patients were hospitalized after surgery for 61±41 and 44±31 days. Both were significantly shorter in the later group of patients. There was a single case of mediastinitis, in the earlier period. Wound infection or delayed wound healing occurred in 8 patients in the earlier period and in 3 patients in the later period. The only 4 patients who required wound resuturing were all in the earlier period. The incidence of wound infection and delayed wound healing tended to be low in the later period. We believe that interrupted non-absorbable monofilament sutures improved the wound microcirculation and that the hydrocolloid dressing accelerated wound healing via its moisturizing and heat-retention action, pH buffering ability, and bacteriostatic activity, and that all these contributed to the better outcomes in the later period.

Left ventricular hypertrophy in patients with aortic valve disease has long been recognized as a significant risk factor for aortic valve replacement. Higher operative mortality in such patients has been attributed to poor myocardial preservation. In these patients improvement of left ventricular subendocardial blood flow during reperfusion seems to be mandatory to avoid subendocardial injury. Therefore, we attempted to increase subendocardial blood flow during reperfusion by terminal warm blood cardioplegia (TWBCP) followed by controlled aortic root reperfusion (CARR) in patients requiring isolated aortic valve replacement. The patients with TWBCP and CARR had a tendency towards severe left ventricular hypertrophy and more advanced NYHA function class compared to those with hypothermic cardioplegia alone. Nevertheless, the patients with TWBCP and CARR showed significantly better recovery of left ventricular function, i.e., spontaneous recovery of beating and higher cardiac index as well as left ventricular stroke work index, despite significantly less catecholamine support. These resuls suggest that TWBCP followed by CARR may offer significant benefits over unmodified reperfusion during aortic valve replacement for patients with severe left ventricular hypertrophy.

Fibroblast growth factor 23 (FGF23) is a pivotal humoral factor for the regulation of serum phosphate levels and was first identified in patients with autosomal dominant hypophosphatemic rickets and tumor-induced osteomalacia (TIO), the most common form of acquired FGF23-related hypophosphatemic rickets/osteomalacia (FGF23rHR). After the identification of FGF23, many other inherited and acquired forms of FGF23rHR were reported. In this review article, the detailed features of each acquired FGF23rHR are discussed, including TIO, ectopic FGF23 syndrome with malignancy, fibrous dysplasia/McCune-Albright syndrome, Schimmelpenning-Feuerstein-Mims syndrome/cutaneous skeletal hypophosphatemia syndrome, intravenous iron preparation-induced FGF23rHR, alcohol consumption-induced FGF23rHR, and post-kidney transplantation hypophosphatemia. Then, an approach for the differential diagnosis and therapeutic options for each disorder are concisely introduced. Currently, the majority of endocrinologists might only consider TIO when encountering patients with acquired FGF23rHR; an adequate differential diagnosis can reduce medical costs and invasive procedures such as positron emission tomography/computed tomography and venous sampling to identify FGF23-producing tumors. Furthermore, some acquired FGF23rHRs, such as intravenous iron preparation/alcohol consumption-induced FGF23rHR, require only cessation of drugs or alcohol to achieve full recovery from osteomalacia.

Three patients with hypersensitivity pneumonitis caused by inhalation of spores of Cortinus shiitake (Lentinus edodes) cultivated in vinyl houses were observed. These three patients developed the disease after harvesting shiitake for several hours in closed frame houses with a heater in the autum and winter. The frames were full of spores of shiitake and the patients suffered from a discordant feeling, systemic weakness, a feeling of cold, fever (over 38 C), a feeling of airway occlusion, a slight cough and sputum. These symptoms disappeared during rest the next day.
Case 1 was examined by the inhalation provocation test with a suspension of shiitake spores and spore-allergen. This test caused several clinical symptoms (fever, airway occlusion and various symptomatic feelings), leucocytosis, decrease of PaO₂ a positive reaction of CRP and X-ray findings (appearance of interstitial pneumonitis shadows).
The 3 patients were considered to be suffering from allergic hypersensitivity pneumonitis due to inhalation of spores of Cortinus shiitake, because of their work, the development of symptoms after work in specific occupational conditions, a positive reaction to precipitating antibody against spore-allergen of shiitake, negative reactions to precipitation antibodies to 11 molds-allergens, various abnormal values in immunological tests and a positive reaction in a provocation test in one case.
In a survey of 45 shiitake-grower, it was found that 6 (13.3%) suffered from respiratory disease. No difference was found in the incidence of intracutaneous reactions to spore-allergen or allergen of dried shiitake in non-farm workers. Among 31 growers of shiitake a precipitation antibody to spore-allergen was observed only in these three patients. These results indicate that an allergic disposition is very important for development of hypersensitivity pneumonitis.

Objective: Macrolide therapy has been recommended as an effective treatment for pediatric otolaryngology patients with conditions such as chronic sinusitis and otitis media with effusion.  However, in many cases, a relapse may occur after cessation of treatment.  Therefore, patients are compelled to continue taking antibiotics.  In this paper, we examined the relationship between the duration of therapy and period to relapse on the basis of prescription information and literature research.
Methods: To evaluate the therapeutic doses, we investigated the clinical doses of erythromycin and clarithromycin used for pediatric patients in a community pharmacy from January 2009 to July 2009.  Further, we performed literature searches on the doses of both drugs using Igaku-Chuo-Zasshi databases (from 1983 to 2011) and compared the data obtained with the clinical doses.  Accordingly, the oral doses of macrolides were classified as a low dose or normal dose.  We analyzed the relationship between the administration period and the cessation period, which was defined as the period from the cessation of the treatment to relapse.
Results: Review of the 17 selected reports and the clinical doses showed that the maximum dose of erythromycin was 15 mg/kg/day and that of clarithromycin was 8 mg/kg/day during therapy.  When both cephem or penicillin antibiotics and low-dose macrolides were taken continuously during the administration period, a weak correlation was observed between the administration period and the cessation period.
Conclusion: These results may be useful for the appropriate use of antibiotics and for preventing relapse in pediatric otolaryngology patients.

The purpose of this study was to explore issues arising from the promotion of collaboration between general practitioners (GPs) and community pharmacists in the management of leftover medicine. The 2016 Revision of Medical Fee newly added the check boxes in the prescription form in which a physician requests the correspondence of a pharmacy when confirming leftover medicine. In this study, we conducted a questionnaire survey among GPs in three wards of Tokyo in the month of November 2016. Of the 1,256 clinics queried, 283 responded to the survey. Among those GPs responding, 86.6% were concerned about leftover medicine of their patients, while 29.3% used the check boxes. GPs who received an explanation about the change of the prescription form demonstrated a significant increase in the use of the check boxes, compared to GPs who did not (p<0.01). Of the GPs surveyed, 71.0% thought that the check boxes were an advantage in recognizing medicines that the patients did not take. An overall 67.5% of the GPs who used the check boxes revised prescriptions based on the information from pharmacists. The study suggests that by increasing collaboration between GPs and pharmacists, the information from pharmacists could be utilized to revise prescriptions and lead optimization of drug treatment.
