1.Research on stomach remaining-floating sustained-release preparation of compound demethylcantharidin

AIM:To make compound demethycantharidin(including Radix astragali and Rhizoma bletillae) into stomach remaining-floating sustained-release preparation. METHODS:Radix astragal and Rhizoma betillae pulverized to superfine powders were combined with hydrophilic gel to manufacture stomach remaining-floating sustained-release tablet(floating tablet). RESULTS:The tablet started to drift in 3 min in the artificial gastric juice and float lasted for more than 8 h. By isotopic tracering,the floating tablets given on an empty stomach were retarded for 2-3 h and on large meal for 4-5 h in combination with ordinary tablet only retarded for 1 h. A pharmacopoeia dissolution method was set up to determine the release. 50% of tablets were disintegration in 5 h,70% in 8 h. The t 1/2 was 5.1 h(2-4 h for ordinary tablets),and the MRT was 8.3 h(3.3 h for ordinary tablets),and the remaining-floating sustained-release tablets showed a relative bioavailability of 230.71%. CONCLUSION:Pretreatment of raw herbs from compound demethylcantharidin and selection of the excipients will help to produce the herb-containing floating tablet.
2.Clinical characteristics and drug resistance of Elizabethkingia meningoseptica infection in a tertiary hospital in Hainan Province from 2021 to 2022

Objective To explore the clinical characteristics and drug resistance of Elizabethkingia meningoseptica (EM) nosocomial infection, so as to provide evidence for prevention of EM nosocomial infection and guiding the rational use of antibiotics. Methods A retrospective study was conducted of 67 patients with EM infection in a tertiary hospital from January 2021 to December 2022. The infective characteristics and drug resistance were analyzed. Results The cohort of 67 EM-infected patients was predominantly males aged ≥60 years, with the most frequent source being the first district of the intensive care unit (ICU), followed by the respiratory medicine and emergency department (19.40%, 13/67). The specimens were mainly isolated from respiratory tract (86.57%, 58/67), of which sputum accounted for 49.25% (33/67), and alveolar lavage fluid accounted for 37.31% (25/67). The majority of EM infections occurred in patients with pre-existing respiratory conditions (49.25%, 33/67), who generally experienced prolonged hospital stays and underwent invasive procedures, such as mechanical ventilation 94.03% (63/67), urinary catheterization (95.52%, 64/67), and central venous catheterization (97.01%, 65/67). Post-treatment, the improved rate of the 67 patients was 40.30% (27/67). Susceptibility testing demonstrated a high resistance rate of EM to cefoperazone-sulbactam, 98.39% (61/62), contrasted by significant susceptibility to compound trimethoprim-sulfamethoxazole (TMP-SMX)/cotrimoxazole, doxycycline, minocycline, and piperacillin-tazobactam, with susceptibility rates exceeding 90%. Conclusions The patients infected with EM were almost elderly men with certain underlying diseases, experienced prolonged hospital stays, and had a history of invasive operations. The specimens of EM were mainly from Intensive Care Unit and isolated from respiratory tract. The strain showed high resistance to cefoperazone-sulbactam, whereas it remained highly susceptible to cotrimoxazole, doxycycline, minocycline and piperacillin-tazobactam, which may be considered as first-line treatment options.
3.Effect comparison of three-point positioning method and traditional puncture method for patients with arteriovenous fistula in hemodialysis

Objective To compare the effect of three-point positioning method and traditional puncture method in patients with arteriovenous fistula in hemodialysis.Methods A total of 800 hemodialysis patients with malignant tumor of urinary system in our hospital were divided into three-point positioning method group (observation group) and traditional puncture method group (control group), with 400 cases in each group.All the puncture was performed by the senior nurses with color Doppler, and vein intima thickness of artery fistula before puncture and 60 times after puncture were observed, puncture failure rate, incidence of venous fistula aneurysm, arteriovenous fistula stenosis and satisfaction were compared.Results There was no significant difference in vein intima thickness of artery fistula(P<0.05);The effect degree of vein intima thickness of artery fistula in the observation group after treatment was significantly lower than that of the control group, and the difference was statistically significant(P<0.05);The puncture failure rate, incidence of arteriovenous fistula aneurysm, and arteriovenous fistula stenosis rate in the observation group after 60 times puncture were significantly lower than that in the control group, and the differences were statistically significant(P<0.05);The satisfaction in the observation group was significantly higher than that in the control group, the difference was statistically significant (P<0.05).Conclusion Three-point positioning puncture combined with losartan has significant effect for arteriovenous fistula patients with the malignant tumor of urinary system, and it can effectively prolong longevity of vascular puncture, and reduce complications of vascular with fistula.
