1.Clinical characteristics of systemic lupus erythematosus patients with cutaneous vasculitis

Objective To compare clinical and laboratory characteristics between systemic lupus erythematosus (SLE) patients with and without cutaneous vasculitis,and to investigate the correlation of cutaneous vasculitis with severe visceral involvement and laboratory biomarkers.Methods A total of 152 SLE patients with various skin manifestations were enrolled from Department of Dermatology of Huashan Hospital affiliated to Fudan University from July 2011 to October 2014.The clinical and laboratory data were collected and retrospectively analyzed.SLE patients with cutaneous vasculitis were divided into upper/lower extremity vasculitis group and livedo reticularis group.A logistic regression model was used to analyze the correlation between cutaneous vasculitis and various clinical and laboratory variables.Results Of 152 SLE patients,62 (41%) presented with cutaneous vasculitis,including 55 with upper/lower extremity vasculitis and 7 with livedo reticularis,and 90 (59%) did not have cutaneous vasculitis.Patients with upper/lower extremity vasculitis showed significantly younger age (30.54 ± 12.67 years vs.37.77 ± 12.17 years),and lower prevalence of aberrantly elevated 24-hour protein excretion (39.39% vs.64.00%) and serum urea level (2.08% vs.16.43%),but significantly higher percentage of females (98.18% vs.84.44%),higher proportions of patients with abnormal brain MRI (37.5% vs.12.19%),anemia (87.03% vs.70.93%) and positive antiribosomal P protein antibodies (77.77% vs.53.65%),and higher SLE disease activity index (SLEDAI) (14.71 ± 7.75 vs.10.68 ± 5.61) than those without vasculitis (all P < 0.05).The proportion of patients with decreased C3 level did not differ between patients with upper/lower extremity vasculitis and those without cutaneous vasculitis (P =0.362),but was significantly lower in the patients with livedo reticularis than in those without cutaneous vasculitis (28.57% vs.79.76%,P =0.008).However,no significant differences in the other variables were observed between patients with livedo reticularis and those without cutaneous vasculitis (all P > 0.05).Additionally,body mass index (BMI),abnormal lung function and other laboratory variables all did not differ among patients with upper/lower extremity vasculitis,patients with livedo reticularis and patients without cutaneous vasculitis (all P > 0.05).Logistic regression analysis revealed that after exclusion of potential effects of age and gender,cutaneous vasculitis was significantly positively correlated with abnormal brain MRI (OR =4.24,95% CI:1.17-16.13,P =0.028),and positive anti-ribosomal P protein antibodies (OR =3.97,95% CI:1.86-8.47,P =0.0004),but negatively correlated with abnormally elevated 24-hour protein excretion (OR =0.25,95% CI:0.09-0.69,P =0.009).Furthermore,cutaneous vasculitis showed no significant associations with abnormal serum urea level (OR =0.12,95% CI:0.01-1.06),decreased C3 level (OR =0.93,95% CI:0.38-2.28),anemia (OR =1.38,95% CI:0.56-3.40) or SLEDAI (OR =1.05,95% CI:0.98-1.14).Conclusions Cutaneous vasculitis is closely associated with central nervous system damage and emergence of anti-ribosomal P protein antibodies,so SLE patients with cutaneous vasculitis should be closely monitored for central nervous system damage.SLE patients without cutaneous vasculitis are more liable to kidney injury,so they also need to be closely monitored.
2.Analysis on Current Status and Countermeasures of Health Personnel Development of Public TCM Hospitals

Objective To grasp the current situation and trend of health personnel development in public TCM hospitals through the analysis of health personnel statistics data of public TCM hospitals in China; To provide decision-making references for management departments. Methods Data were from the National Health and Family Planning Commission Statistical Information Center. This article analyzed staff number, the average speed of development, the average annual growth rate, health staff age, academic qualifications and job titles and other indicators of TCM public hospitals during 2010-2015. Results From 2010 to 2015, there was an annual increasing tendency of the number of TCM health personnel, and the average annual growth rate was 5.9%, but the growth of the percentage of the TCM health personnel to the total health personnel was only 0.3%. In 2015, the proportion of TCM health personnel was less than 50%; the doctor-to-nurse ratio was 1:1.18; the educational level of postgraduate and undergraduate were 14.0% and 48.0% respectively, less than the average level of national hospitals. Conclusion From 2010 to 2015, there is an increasing tendency of the TCM health personnel of public TCM hospitals, but the proportion of TCM health personnel is low. The doctor-to-nurse ratio is reasonable gradually, but there is still a gap compared with the average level of the national hospitals. The average age of technical health personnel tends to be younger. The proportion of high educational level health personnel is increasing, but the proportion of senior technical position is reduced. The management departments should take appropriate measures to promote the rational allocation of human resources and development of TCM hospitals.
3. Relationship between impaired myocardial untwisting and left ventricular diastolic dysfunction in patients with autoimmune diseases

Objective:
To observe the relationship between impaired myocardial untwisting and left ventricular diastolic dysfunction in patients with autoimmune diseases (AD).
Methods:
In this retrospective study, 95 AD patients (27 males, (38.6±14.2) years old) were enrolled as AD group and 71 gender and age matched healthy subjects (24 males, (37.6±12.2) years old) were enrolled as control group, all underwent transthoracic echocardiography and two-dimensional speckle-tracking echocardiography (STE) in our hospital between January 2014 and June 2018. Left ventricular untwisting and diastolic function parameters were measured. Multiple logistic regression analysis was used to identify related factors of left ventricular diastolic dysfunction in AD patients. Receiver operating characteristic (ROC) curve was used to identify the diagnosis value of untwisting parameters for left ventricular diastolic dysfunction in AD patients.
Results:
Compared with control group, left ventricular ejection fraction was lower (58(47, 66)% vs. 67 (62, 71) %,
4.Experience of XUE Boshou in Using Fangfeng (Radix Saposhnikoviae) in Clinical Practice

This paper summarized professor XUE Boshou's clinical experience of using Fangfeng (Radix Saposhnikoviae). Firstly, Fangfeng is used to scatter wind and release the exterior, disseminate and penetrate the pathogens, and can treat the early-staged externally contracted heat diseases with exterior qi constraint and blocking, and serious internal heat through Xiaodu Xijiao Beverage (消毒犀角饮) from Beneficial Formulas from the Taiping Imperial Pharmacy (《太平惠民和剂局方》)combined with Yinqiao Powder (银翘散), Shengjiang Powder (升降散) and Zhizichi Decoction (栀子豉汤) in their modifications. Secondly, it eliminates wind and dispels dampness, relieves itching, and heals sores, with self-made Xue's Guomin Decoction (薛氏过敏煎) in modification being formulated to treat allergic skin diseases caused by pathogenic wind attacking the exterior. Thirdly, it disperses liver qi and soothes the spleen, lifts yang and stops diarrhea, and can be used in modified Tong Xie Yao Formula (痛泻要方) from Teachings of ZHU Danxi (《丹溪心法》) when treating diarrhea caused by liver constraint and spleen deficiency. Fourthly, it can disperse stagnated qi throughout the body and unblock the meridians of the five zang (脏) organs, commonly in formulation of modified Huangqi Chifeng Decoction (黄芪赤风汤) from Correction of Errors in Medical Works (《医林改错》) to treat patients with qi deficiency and blood stasis after radiotherapy and chemotherapy of tumors, as well as alopecia areata or even alopecia universalis, chronic rhinitis and others.
5.Study on Sustainable Development Ability Evaluation Index System for Ethnomedicine Enterprises in Guizhou Province

OBJECTIVE:To establish sustainable development ability evaluation index system for ethnomedicine enterprises in Guizhou province ,and to promote the sustainable development of the ethnomedicine industry. METHODS :The draft of evaluation index system had been made by documents collection and a meeting of focus group discussion ,the final indexes and weight had been determined by Delphi method for conducting 2 rounds expert questionnaire survey ;the index system was used to measure and compare the results with the evaluation results of the drug regulatory department and the authoritative experts of the industry. RESULTS:The draft of evaluation index system included 6 indexes in the first level indicators and 49 indexes in the second. The expert’s positive coefficients was 100% after 2 rounds of consultation ;the authoritative coefficients on the opinions of experts for the levels of indicators were 0.86,the coordination coefficient of experts was 0.22 in the first round and 0.48 in the second. After 2 rounds of expert consultation ,the final established evaluation index system contained 6 indexes in the first level indicators and 33 indexes in the second. Fifteen ethnomedicine enterprises in Guizhou province were selected for on-site testing. The evaluation results were not much different from those of the drug regulatory department and the authoritative experts of the industry. CONCLUSIONS:Established sustainable development ability evaluation index system for ethnomedicine enterprises is scientific , reasonable and feasible ,and can provide standardized reference for regular monitoring and evaluation.
