1.Simultaneous Analysis of Thirteen Bioactive Components in Evodia rutaecarpa and Its Varieties by HPLC-DAD-MS

Objective To control the quality of Evodia rutaecarpa better. Methods An HPLC-DAD-MS/MS method was established for the rapid and efficient identification of bioactive constituents and for simultaneous quantitative analysis of four bioactive ingredients including evodiamine, rutaecarpine, dehydroevodiamine, and evodin in E.rutaecarpa, which was applied to evaluating eight samples of E. rutaecarpa and its varieties from different areas.Results Thirteen potentially bioactive constituents including one flavonoid glycoside, one limonin, four indoloquinazoline alkaloids, and seven quinolone alkaloids were identified in all samples and the contents of dehydroevodiamine, evodine, evodiamine, and rutaecarpine varied widely from 0.10% to 0.51%, 0.49% to 3.12%,0.07% to 1.56%, and 0.10% to 0.69%, respectively. Conclusion This method is found to be convenient, fast,accurate, and it is facilitated to improve the quality control standard of E. rutaecarpa and related products.
2.Impact of initial dialysis dose on the residual renal function in peritoneal dialysis patients

Objective To investigate the impact of initial dialysis dose on residual renal function of peritoneal dialysis patients. Methods Clinical data of 178 consecutive patients on initial peritoneal dialysis received follow-up for 3 months in our department were analyzed retrospectively. According to urinary volume after peritoneal dialysis, patients were divided into three groups: lower urine group (LU, n=97), decreased urine group (DU, n=19), and normal urine group (NU, n=62). Their dialysate volume, dialysate glucose content, uhrafiltration, weekly renal urea clearance normalized to total body water (Kt/V), body weight, edema degree and daily urinary volume were recorded and association among these parameters were examined. Results There were no significant differences in age, gender, serum albumin and total Kt/V among three groups. One month after dialysis, body weight and edema degree in DU group were significantly higher than those in LU and NU groups (all P<0.05); the dialysate volume, dialysate glucose content, ultrafiltration and renal Kt/V in DU group were significantly higher than those in LU group (all P<0.05), but were not significantly different from NU group. Three months after dialysis, in DU group, dialysate volume, ultrafiltration and urinary volume decreased significantly (P<0.05) as compared with LU and NU groups, but body weight and edema degree were still higher, and Kt/V decreased significantly as well. Conclusions The residual renal function (urinary volume and Kt/V value) of initial patients will be deteriorated by over ultrafihration in early stage of peritoneal dialysis. Excess uhrafiltration should be avoided for the initial peritoneal dialysis patients.
3.Microsurgical treatment of intraspinal extramedullary benign tumors

Objective To explore the clinical efficacy of using the disposable whole laminectomy, laminoplasty and microsurgical tumor removal to treat intraspinal extramedullary benign tumors. Methods Sixty-four patients, 71 cases adult intraspinal extramedullary benign tumors were removed by the disposable whole laminectomy, laminoplasty and microsurgical tumor removal operation, all postoperative patients were followed up to observe the clinical efficacy. Results Patients with small tumors and large tumors all had a faster recovery of neurological function than preoperative, recent and long-term efficacy were all better than preoperative, and long-term efficacy was superior to short-term efficacy; Patients with giant tumors had a slow-er recovery of neurological function than preoperative, short-term efficacy was not better than preoperative, but the long-term efficacy was superior to preoperative. Conclusion The disposable whole laminectomy, laminoplasty and microsurgical tumor removal is an excellent method of treating intraspinal extramedullary be-nign tumors.
4.Evaluation and treatment of altered mental status patients in the emergency department

Objective To provide a framework for understanding the need for a structured assessment of altered mental status (AMS) to better understand underlying causes of the mental status changes in adults and therefore potentially improve diagnostic skills and eventually management.Methods This is a prospective cohort observational study.We recruited consecutive adult patients with undifferentiated AMS at a single center tertiary care academic emergency department over 24 months.Demographical,clinical presentations,assessment approaches,cause factors,emergency treatments and outcomes were collected prospectively.Results One thousand nine hundred and thirty-four patients with AMS were recruited,this number of patients represented 5% of the total ED census.Out of 1934 patients,1026 (53.1%) were male,908 (46.9%) were female.Mean age was (51.95 ± 15.71) years.Etiologic factors included neurological (n =641,35.0%),pharmacologic & toxicologic (n =421,23.0%),systemic and organic (n =266,14.5%),infectious (n =167; 9.1%),endocrine/metabolic (n =145,7.9%),psychiatric (n =71,3.9%),traumatic (n =38,2.1%),gynecologic and obstetric (n =35,1.9%).Total mortality rate was 8.1% (n =156).The death rate was higher in elderly patients (≥ 60) than that in younger patients (10.8% vs.6.9%,P =0.003).Conclusions The patient with AMS poses a challenge to physicians in ED.The most frequently encountered diagnostic category causing AMS were primary CNS disorders,intoxication,organ system dysfunction and endocrine/metabolic diseases.Fatality rate is very high.Prompt evaluation and treatment are essential to decrease the morbidity and mortality associated with this condition.
5.Biocompatibility of rat osteoblasts with chitosan/hydroxyapatite composite scaffold degradation products

BACKGROUND:The in vivo degradation process of chitosan/hydroxyapatite composite porous scaffolds is not very clear. Research on the effects of rat osteoblasts and degradation products is less.
OBJECTIVE:To analyze the biocompatiblity of rat osteoblasts with degradation products of chitosan/hydroxyapatite composite porous scaffolds.
METHODS:The second generation of cultured rat osteoblasts were respectively cultured in the extract of degradation products of chitosan/hydroxyapatite composite scaffolds (experimental group) and Dulbecco’s modified Eagle’s medium containing 10%fetal bovine serum (control group). At 2, 4, 6, 8, 10 days of culture, cel counting was measured by 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide assay. Alkaline phosphatase activity was measured by the recommended method of determination of the Federation, and total protein was determined by BCA method.
RESULTS AND CONCLUSION:The proliferation speed, alkaline phosphatase activity, total cel ular protein synthesis and ratio of alkaline phosphatase to total protein in rat osteoblasts cultured in the experimental group were significantly higher than those in the control group (P<0.05). This experiment showed that the degradation products of chitosan/hydroxyapatite composite porous scaffolds cannot only promote rat osteoblast adhesion, growth and proliferation, but also enhance its ossification function, with good biocompatibility.
6.Key Techniques and Application Progress of Molecular Pharmacognosy

At the boundary between pharmacognosy and molecular biology, molecular pharmacognosy has developed as a new borderline discipline. This paper reviews the methods, application, and prospect of molecular pharmacognosy. DNA marker is one of genetic markers and some molecular marker methods which have been successfully used for genetic diversity identification and new medicinal resources development. Recombinant DNA technology provides a powerful tool that enables scientists to engineer DNA sequences. Gene chip technique could be used in determination of gene expression profiles, analyses of polymorphisms, construction of genomic library, analysis of mapping, and sequencing by hybridization. Using the methods and theory of molecular biology and pharmacognosy, molecular pharmacognosy represents an extremely prospective branch of pharmacognosy and focuses on the study of systemic growth of medicinal plants, identification and evaluation of germplasm resources, plant metabolomics and production of active compounds. Furthermore, the great breakthrough of molecular pharmacognosy could be anticipated on DNA fingerprint analysis, cultivar improvement, DNA identification, and a global DNA barcoding system in the future.
7.Exploration on scientific connotation of compatibility of Zuojin Wan based on "disease-syndromes-formulas-medicines" binding mode.

"Disease-syndromes-formulas-medicines" binding mode is benefit to reveal the scientific connotation of the compatibility and mechanism of classical prescriptions of traditional Chinese medicine (TCM). In this article, Zuojin Wan was used as a model prescription, with the etiology and pathogenesis of liver-fire invading stomach in TCM, and modern pathological and pharmacological interpretation, the compatibility, nature-flavour, channel tropism, the active substance and pharmacology of Zuojin Wan were explained, in order to reveal the compatibility, nature-flavour and channel tropism of Zuojin Wan, and to provide ideas for the development of modern compound prescription.

8.HPLC fingerprint of Euodiae Fructus.

OBJECTIVETo establish a HPLC fingerprint for quality evaluation of fructus Euodiae rutaecarpa, and to perform qualitative and quantitative analysis of its main constituents.
METHODThe 10 batches of samples were analyzed by HPLC linear gradient elution method and evaluated by similarity evaluation and system cluster analysis. The common peaks in chromatographic fingerprint were identified by LC-DAD-MS.
RESULTThe HPLC fingerprint of Euodiae Fructus showed 32 characteristic peaks, among them 13 peaks were identified, and 2 constituents were quantified.
CONCLUSIONThe HPLC fingerprint established could reflect the main constituents of Euodiae Fructus, and could combine with content determination to take an overall control of Euodiae Fructus.

9.Analysis on changes of chemical compounds in different processed products of Euodiae fructus.

OBJECTIVETo study the relationship among processing methods and chemical compounds.
METHODHPLC was used to compare the difference between pre and post processing. The main peaks in chromatogram were identified and divided into groups of chemical compounds. The contents of identified compounds and groups of chemical compounds were also analyzed.
RESULTThe chromatographic peaks were divided into three groups of chemical compounds that were flavonoid glocosides, uinazoline alkaloids and bitter principle, indoloquinazoline alkaloids. The contents of flavonoid glocosides were reduced in each processed product, and that in hot-water processing product were the least. The contents of all three groups of chemical compounds were decreased in Coptidis Rhizoma processing products. The dissolving release of quinolones alkaloids were increased in wine, salt, Glycyrrhizae Radix et Rhizoma and ginger processing products.
CONCLUSIONDifferent processing methods caused different changes of chemical compounds.

10.Clinical observation of brucea javanica oil emulsion combined with DP chemotherapy in treating advanced non-small-cell lung cancer

Objective To evaluate the clinical efficacy and adverse effects of brucea javanica oil emulsion combined with DP chemotherapy in treating advanced non-small-cell lung cancer.Methods Totally 48 patients with advanced non-small-cell lung cancer were divided into two groups randomly by mechanical sampling method.Twenty-four cases in treatment group were treated by brucea javanica oil emulsion combined with DP chemotherapy, while 24 cases in control group were treated by DP chemotherapy only.The clinical effects were evaluated after treatment of two cycles.Results The short-term effective rate was 54.2% (13/24) in treatment group and 45.8% (11/24) in control group, and there was no significant difference between two groups ( χ2 = 0.333, P = 0.564).The rate of increased and stable life quality was 87.5%(21/24) in treatment group and 58.3%(14/24) in control group,and there was significant difference between two groups (χ2 = 5.169,P = 0.023).The rate of increased and stable weight was 79.2% (19/24) in treatment group and 45.8%( 11/24) in control group, and there was significant difference between two groups (χ2 = 5.689,P = 0.017).The incidence of nausea or vomiting was 45.8% (11/24) in treatment group and 41.7%( 10/24 ) in control group, and there was no significant difference between two groups (χ2 = 0.085, P = 0.771 ).Compared with those in control group, patients in treatment group had less adverse effects in decreasing of peripheral blood leucocytes and showed better immune function.Conclusion Brucea javanica oil emulsion combined with DP chemotherapy in treating advanced non-small-cell lung cancer has good clinical effect, especially enhances the quality of life, improves immune function and decreases the adverse effects of chemotherapy.
