Objective To study the application of carcinoembryonic antigen (CEA),carbohydrate antigens19-9 (CA19-9) and carbohydrate antigens72-4 (CA72-4) in the diagnosis of gastric cancer.Methods A total of 78 patients with gastric cancer underwent treatment in our hospital from August,2007 to June,2011 were enrolled as the case group.A total of 78 patients with benign gastric diseases and 78 healthy residents of the same period and district were randomly selected as the benign group and healthy control group,respectively.Measurements of serum CA72-4,CA19-9,CEA of all the cases and control group were performed.And the measurements of the three groups were statistically compared.Results The Serum concentration of CA72-4,CA19-9,CEA were significantly higher than the other two groups [CEA:(27.56 ± 18.36) μg/L vs (2.44 ±0.97) μg/L vs (2.37 ± 1.25) μg/L,F =145.346,P ＜0.0001 ; CA19-9:(99.87 ±80.53) kU/L vs (18.21 ±10.36) kU/Lvs (17.48 ±9.66) kU/L,F=78.503,P＜0.0001; CA72-4:(56.13 ±39.26)kU/L vs (5.77 ±2.95) kU/L vs (3.62 ±2.18) kU/L,F =133.892,P ＜0.0001].And the positive rate of the gastric cancer group were significantly higher than those of the other two groups [CEA:69.23％,10.26％and 7.69％; CA19-9:57.69％,23.07％ and 20.51％; CA72-4:83.33％,10.26％ and 8.97％].The combination of the 3 measurements obtained the highest accuracy of diagnosis,while for single measurement,CA72-4 had the highest accuracy.Conclusion CEA,CA19-9 and CA72-4 have a significant meaning in the early diagnosis of gastric cancer,which has high sensitivity and specificity.The detection rate of the gastric cancer will be improved if the test of CEA,CA19-9 and CA72-4 are combined.

Objective To investigate the effect of psychological intervention combined with gliclazide and sitagliptin on the treatment of poorly obese obese patients with type 2 diabetes mellitus. Methods 96 obese patients with type 2 diabetes mellitus who had poor control of blood glucose after insulin treatment were randomly divided into control group and experimental group,48 cases in each groups. Gliclazide and sitagliptin (FBG), glycosylated hemoglobin (HbA1c), 2h postprandial blood glucose (2hPG), fasting insulin (FIns) were measured before and after 15 weeks of treatment in the control group. situation. Results FBG, HbA1c, 2hPG and FIns were significantly lower in the experimental group than in the control group (P<0.05). Conclusion The combination of gliclazide and sitagliptin on the treatment of poorly obese type 2 diabetes mellitus with insulin treatment can significantly reduce the blood glucose level of patients and promote it.

Objective To explore the clinic character of immune thrombocytopenia (ITP) children infected with helicobacter py‐lori (Hp) and the effects of anti‐helicobacter pylori treatment on those children .Methods One hundred and forty‐two ITP children and 92 healthy children were conducted the 13C‐Urea breath test to determine the infections of Hp .The ITP children infected with Hp were divided into two groups :single drug group (48 cases ,treated only with corticosteroid ) and combined drugs group (49 ca‐ses ,treated with corticosteroid and anti‐helicobacter pylori treatment) .The platelet parameters ,platelet associated immunoglobulin and some lymphocyte subsets were analyzed .Results Ninety seven children infected with Hp were found in 142 ITP children (69 . 7% ) .In the mean time ,37 children infected with Hp were observed in 92 healthy children (40 .2% ) .After treatment of 6 months , the PLT ,PCT ,CD3+ ,CD3+CD4+ and CD3+ CD4+ /CD3+ CD8+ of ITP children in both group were increased ,and the increased degree of those parameters were much higher in combined drug group than those in single drug group (P< 0 .01) .However ,the MPV ,PDW ,PAIgG ,PAIgA and CD3+ CD8+ were more decreased than that of the single drug group(P<0 .01) .Conclusion The ITP children have a higher infection rate of Hp ,which may be involved in the pathogenesis of ITP .Anti‐helicobacter pylori therapy would improve the therapy efficacy of ITP children infected with Hp by improving their immunity .

Objective To explore the preventive effect of early bowel intervention on the gastrointestinal dysfunction in patients with invasive mechanical ventilation.Methods Sixty patients with invasive mechanical ventilation were equally randomly divided into the observation group and the control group.patlents in the control group were treated by conventional nursing care,while Patients in the observation group were given the early bowel intervention including abdominal massage and stimulation of rectum.The two groups were compared in terms of gastrointestinal dysfunction.Result The rate of gastrointestinal dysfunction in the observation group was significantly lower than that of the control group(P<0.05).Conclusion The early bowel intervention may effectively promote intestinal peristalsis,protect the intestinal digestion, absorption of nutrients and mucosal barrier function and prevent gastrointestinal dysfunction in patients with invasive mechanical ventilation during enteral nutrition.

Objective To evaluate the influence of preoperative quitting time on coronary artery bypass grafting (CABG) patients with postoperative hypoxemia incidence.Methods 151 patients with coronary bypass surgery and preoperative history of smoking who preparation of CABG in hospital were recruited from September 2011 to September 2013.According to the preoperative smoking cessation time patients were divided into five groups:0 days,1-30 days,31-60 days,61-90 days,more than 90 days.Single factor regression and Logistic analysis were used to analyse the influence of preoperative quitting time on CABG patients with postoperative hypoxemia incidence.Results Age,weight,smoking habit,quitting time,hypertension,diabetes mellitus were risk factors of hypoxemia after coronary artery bypass grafting.Logistic regression analysis showed that age,body weight,smoking habit,smoking time were independent risk factors of hypoxemia after coronary artery bypass grafting.The incidences of hypoxemia of the five groups 0 days,1-30 days,31-60 days,61-90 days and more than 90 days were 55.56％ (15/27),59.26％ (16/27),27.58％ (8/29),22.73％ (5/22),15.63％ (5/32).The incidence of hypoxemia had significant difference (x2=19.212,P ＜ 0.05).Conclusions Age,weight,smoking habit,quitting time were independent risk factors of hypoxemia after CABG.With the quitting time increase,hypoxemia after CABG overall downward trend.Difference quitting time before the operation,the hypoxemia occurred difference rate influence,On the preoperative smoking CABG patients were smoking cessation intervention timely helps to reduce the occurrence of postoperative hypoxemia.

Objective:To set up the HPLC fingerprints of Scutellaria baicalensis collected from different regions and develop a HPLC method for the quantitative analysis of the chief Flavonoids in Scutellaria baicalensis.Methods:The HPLC separation was performed on SHIMADZU VP-ODS column(250mm?4.6mm,5?m),gradient eluted using 0.1% citric acid-acetonitrile solvent system and detected at 276nm and 200nm to 400nm with a DAD detector.The flow rate was 1.0ml/min.The fingerprints of different samples were compared with similarity evaluation software.Results The HPLC fingerprints of Scutellaria baicalensis was set up,in the fingerprints,17common peaks of flavonoids were confi rmed.But the contents of the chief Flavonoids had a big difference,in which the baicalin had the biggest content(18.32%) in cottage Scutellaria baicalensis of Yiyuan County;the baicalein and wogonin had the biggest content(2.31%&0.73%) in wild Scutellaria baicalensis of Northeastern China.Conclusion:This method showed high precision,good repeatability,stability and all of the content were separated well,so it can be useful as the means to assess the quality of Scutellaria baicalensis.

Objective To investigate awareness about tuberculosis (TB) prevention and treatment and influence of professional training on TB detection among health-care workers (HCWs) in general hospitals.Methods In total,750 HCWs were trained for TB-related knowledge for eight class-hours in a two-day course in three general hospitals,and 20 HCWs from each of the three hospitals classified as grade 3A and grade 2A in urban Beijing and grade B at suburban Beijing,respectively,were interviewed with questionnaire designed to understand their awareness about TB prevention policy,epidemiology,diagnosis and treatment,and to evaluate effectiveness of the training,respectively.All the trainees responded before and after the training,with a hundred percent of response rate.TB diagnosis and reporting one year before training in the three hospitals were compared to those one year after it.Results Scores of knowledge about TB diagnosis and treatment averaged 64-80 for HCWs before training,with statistically significant difference among three hospital (F = 5.984,P ＜ 0.01).Scores increased after training,but without significant difference from those before it (P ＞ 0.05).Awareness of TB prevention policy,regulations and epidemiology was insufficient in most HCWs of those hospitals,with lowest and highest average scores of 38.3 and 71.7 before training,respectively,but scores increased significantly after training (P ＜ 0.01).Proportion of TB diagnosis with chest roentgenograph at grades 3A and 2A hospitals was significantly higher one month,three months and six months after training,as compared to that at suburban hospitals (P ＜0.01).There was significant decrease (P ＜ 0.01) in proportion of chest roentgenograph at respiratory departments in hospitals grade 3A and grade 2A after training.There was no significant difference in reporting of pulmonary TB and positive sputum smear (P ＞ 0.05) before and after training.Conclusions HCWs in general hospitals had experience and capacity in diagnosis and treatment of pulmonary TB,but their knowledge of TB prevention policy and epidemiology was insufficient.Their ability in finding and reporting TB can not been improved with short-term training.

Objective To investigate the possibility of plastic and cosmetic operation incorporating epicanthoplasty with the double-eyelid procedure in one stage to achieve a more subtle scar in epicanthus patients with single eyelid. Methods After designing incision line of the epicanthic flap, double-eyelid line and the lower eyelid incision curved line, we cut the medial canthus skin transversely to reach a new medial canthus point, incised the part of front angle of medial palpebral ligamentum longitudinally and fixed the end point of front angle of medial palpebral ligamentum to nasal dorsum fascia firmly. Double eyelid operation was routinely performed to remove redundant small triangle skin as well as some patchy of orbicularis on the new medial canthus point. Along with eyelid edge about 1 to 2 mm to eyelash, the temporal side was cut with curved line till its disappearance by eyelid's "cat ear". The lower eyelid flap was separated downward and the superfluous flap and some patchy of orbicularis cut. The skin was su-tured to make postoperative scar hidden, blepharophimosis increased and the fold disappeared. Results 46 eyes (23 cases) were operated and satisfactory aesthetic results were obtained. Palpebral fissure was enlarged to 2 to 4 mm with epicanthic scar disappearance and formation of double-eyelid. Conclusions This is a simple and effective procedure with hidden epicanthic scar and the double-eyelid blepharoplasty could be performed simultaneously. Most patients receive satisfactory results during the 0.5 to 2 years' follow-up period except 2 cases with mild proliferation of epicanthus in half a year. It is especially suitable to correct the severe epicanthus palpebralis or epieanthus tarsalis.

Objective To explore the appropriate frequency of ventilator tube replacement by researching the influence of different ventilator circuit change frequency on ventilator-associated pneumonia (VAP).Methods A prospective randomized sampling study was conducted.The patients undergoing invasive mechanical ventilation over or equal to 3 days admitted to emergency intensive care unit (EICU) of the First Affiliated Hospital of Kunming Medical University from December 2012 to December 2015 were enrolled.The patients were divided into 3,7 and 10 days group according to the frequency of ventilator tube replacement.Bacteriology of ventilator tube and the incidence of VAP were compared among the groups.Results Ninety-eight patients were enrolled,mainly with the artificial airway of endotracheal intubation or tracheotomy,with 56 male and 42 female,aging 8 to 86 years with mean of (51.97 ± 17.56) years.There were no statistical differences in gender,age,Glasgow coma scale (GCS) score,cough function and application of glucocorticoid,enteral nutrition,atomization and sedative therapy among three groups,indicating that the risk factors among three groups were consistent.The bacteria detection rates of extension tube,breathe out tube,breathe in tube,and hydrops collection cup were 36.7％,36.7％,33.3％,and 33.3％ respectively in 3 days group,and they were 73.0％,67.6％,62.2％,and 62.2％ in 7 days group respectively,and were all 90.3％ in 10 days group.It was showed that the bacteria detection rate in different pipe parts was almost the same with the same change frequency,and the rate was higher with the longer usage of ventilator tube (x2 values were 20.599,19.879,21.975,21.975,all P =0.000).The longer of the tube used time,the higher incidence of VAP.The incidence of VAP in 3,7,10 days groups were 26.7％,59.5％ and 77.4％,respectively,but there was statistically significant difference among all groups (x2 =30.486,P ＜ 0.001).Based on the value of 3 days group,the incidence of VAP in the 7 days group was 15.950 folds of 3 days group,and the incidence of VAP in the 10 days group was 18.333 folds of the 3 days group (both P ＜ 0.001).Conclusion This study suggests that the longer of pipeline using time,the more serious degree of bacterial contamination of pipeline,the higher incidence of VAP.

Objective: To investigate the relationship between rs4373814 polymorphisms of calcium channel (CACNB2) gene and the susceptibility of essential hypertension (EH) occurrence in Han population at northeastern area of China. Methods: A case-control study in Han population at northeastern of China was carried out and the research included 1998 participants as 2 groups: EH group,n=1006 patients treated in our hospital, and Control group,n=992 subjects with normal blood pressure from physical examination. The rs4373814 polymorphisms were examined by Snapshot technique and the statistical analysis was performed by SPSS 19.0 software. Results: The rs4373814 risk allele C frequency of CACNB2 gene was signiifcantly different between EH group and Control group,P=0.039, OR=1.068, 95% CI (1.004-1.137). Logistic regression analysis showed that with adjusted gender, age, BMI, smoking, drinking, in dominant model, the genotype rs4373814 carring CC + GC/GG obviously increased the risk of EH occurrence,P=0.037, OR=1.260, 95% CI (1.013-1.567). Conclusion: The rs4373814 polymorphisms of CACNB2 gene are related to the susceptibility of EH occurrence in Han population at northeastern area of China.
