1.Analysis of and inspiration from differences in technical appraisals of malpractice

The adoption of the system of multi-technical appraisals of malpractice means that the appraisals made each time, which constitute the key basis and evidence for the health administrative departments and the judicial organs to deal with medical disputes, are of legal validity as proof. By analyzing the causes of the differences in the appraisals of certain cases, the authors argue that a correct understanding of the definition of malpractice is the key to a correct determination of it; an appropriate application of the principle of allocating the burden of proof is the key link in the technical appraisal of malpractice; perfection of Regulations on Handling Malpractice, Standards for the Classification of Malpractices, and Temporary Provisions on the Technical Appraisal of Malpractice is the basis of correct and impartial appraisals; and a good grasp of the gist of rules and regulations and appraisal according to law is the guarantee for a correct determination of the nature of malpractices.
2.Correlation Between Consumption Amount of Commonly Used Antimicrobial Agents and Antibiotic Resistance Rate of Pseudomonas Aeruginosa

OBJECTIVE:To analyze the correlation between the consumption amount of antimicrobial agents and antibiotic resistance rate of Pseudomonas aeruginosa(PA) in our hospital for clinical reference of rational use of antimicrobial agents.METHODS:The antibiotic resistance rate of PA and the consumption amount of anti-PA antimicrobial agents in our hospital from 2001 to 2007 were retrospectively analyzed.The correlation between the two was statistical analyzed by using SPSS12.0.RESULTS:The antibiotic resistance rate of PA increased year by year.However,the DDDs of the antimicrobial agents assumed diversified change.The antibiotic resistance rate of PA was positively correlated with the consumption amount of piperacillin/tazobactam(TZP),meropenem(MPM),ciprofloxacin(CIP) and moxifloxacin(MOX).CONCLUSION:The antibiotic resistance rate of PA showed an increasing trend in our hospital and which was positively correlated with the consumption sum of multi-kind of antimicrobial agents.
3.A situation study about teacher preparation for nursing undergraduates in clinical field in Shandong Province

Objective To know the situation of teaching and teacher preparation for nursing under-graduates, and then reference to reasonable clinical teaching mode. Methods Investigated clinical pre-ceptors from 30 hospitals by serf-designed questionnair to know their basic situation, and then investigated nursing undergruaduates who had enter school in 2004 to know their evaluation about their preceptors in the aspect of basic quality. Results In professional tides, most of the teachers are nurse-in-charge, and have been working over 10 years, but qualification certificates levels were low and 50% of them teaching. Experience was short and not qualified to the popurse of bachelors degree, nurse-in-charge and more than five years nursing experience. The ratio in thesis writing, teaching training and awards are rather low. There were teaching assessment from students, mainly in teaching and research initiative. Conclusions The qualities of nursing teachers should be defined and should be established standard teaching training system and qualification organization, the capabilities of teaching and research innovations should be improved.
4.Antihypertensives Used in Our Hospital During 2004~2005:Utilization Analysis

OBJECTIVE:To investigate the utilization situation and tendency of antihypertensive drugs in our hospital during the period 2004~ 2005.METHODS:The consumption sum,DDDs and the defined daily cost of antihypertensives during 2004~ 2005 were analyzed statistically.RESULTS:The consumption sum of antihypertensive drugs increased year on year,dominating the first 4 places were calcium antagonists,ACEI,angiotensin receptor antagonists and ? adrenoceptor antagonists.Dominating the first 10 places in terms of consumption sum were chiefly newly marketed,expensive but safe and effective varieties.Leading the top 15 places in terms of DDDs were those drugs that proved to be of confirmed efficacy and lower prices.CONCLUSION:The utilization of antihypertensive drugs in our hospital is basically in line with the general utilization condition home and abroad,and it is up to the current medication principle of antihypertensive drugs
5.Telephone and texting interventions for medication adherence of discharged patients with schizophrenia:A one-year follow-up study

Objective:To explore the effects of problem-solving based telephone and texting interventions for medication adherence of discharged patients with schizophrenia.Method:A total of 178 discharged patients with schizophrenia were randomly assigned to telephone intervention (TI)group (n =63),text-messaging intervention (TMI)group (n =61),and control group (n =54).All patients were routinely treated,and patients in TI and TMI groups were given problem-solving based interventions.Medication adherence and psychotic symptoms were as-sessed by pill counting and the Positive and Negative Symptom Scale (PANSS)at baseline and the first,third, sixth,ninth and twelfth month.Results:Overall,patients with TI had higher medication adherence than those with TMI (P <0.001),and patients with TMI had higher medication adherence than that in control group (P <0.001). PANSS scores in TI and TMI groups were not significantly different (P >0.05),but significantly lower than control group (P <0.01).There was higher rate of re-admission in control group in comparison with TI and TMI groups (9.3% vs.[0% and 1.6%],P <0.05).Conclusion:It suggests that problem-solving based telephone and texting interventions are effective for improving medication adherence in patients with schizophrenia after their discharge from hospital,so as to reducing psychotic symptoms and lowering re-hospitalization risk.In comparison with TMI, TI has a better effect in improving medication adherence.
6.Infantile spinal muscular atrophy in 39 children

Objective To improve the recognition for infantile spinal muscular atrophy (SMA-Ⅰ) and the level of early diagnosis,intervention and treatment for SMA-Ⅰ.Methods The clinical data of 39 patients with SMA-Ⅰ were analyzed retrospectively, including the clinical manifestations, neural electrophysiological characteristics, geno-type, diagnosis,treatment and prognosis of SMA-Ⅰ.Results Of the 39 cases with SMA-Ⅰ , 37 cases (94.9%) had onset in 6 months after birth.The paralyses of the limbs were symmetrical and flaccid.The lower was more severe than the upper , and the proximal was more severe than the distal, hypotonia and tendinous reflex disappears.Thirty-two cases (82.1%) had normal serum creatine kinase, and 7 cases (17.9%) increased slightly.Nerve electrophysiological examination showed that 169 (96.0%) had spontaneous potentials in 176 muscles.Of 160 limb muscles,35 (21.8%) released few motor unit potential (MUP) ,117 (73.1%) extended the duration of MUP and 104 (65.0%) increased the amplitude of M UP.Of 167 peripheral motor nerves, 160 (95.8%)decreased the amplitude of the compound muscle action potential and 162 (97.0%) had normal motor conduction velocity.Of 93 peripheral sensory nerves, 93 (100.0%) had normal range of the conduction.The gene detection showed that 38 cases had homozygous deletion of exon 7,8, and 1 case had homozygous deletion of exon 7 and heterozygous deletion of exon 8.In the effective follow-up of 30 cases,6 cases died in the 2-3 months after birth,4 cases died in 10 months after birth, 12 cases died in 12-18 months after birth.Six cases survived to 2 years old,2 cases survived to 3 years old,and all of them died of pulmonary infection.Conclusions There are typical clinical and nerve electrophysiological characteristics for SMA-I.Nerve electrophysiological examination can be used as an important method for diagnosis and differential diagnosis for SMA-I.Genetic testing can be used to identify the disease and make prenatal diagnosis.Through comprehensive intervention the quality of life can be improved in SMA-Ⅰ children.
7.Interventional effect of sodium hyaluronate on inducible nitric oxide synthase mRNA expression in cartilage of rabbits with traumatic osteoarthritis

BACKGROUND: Traumatic osteoarthritis (OA) resulted from the injury of joints and postoperation of joints is commonly observed. Intra-articular injection of sodium hyaluronate (Na-HA) has been considered as effective method for OA. OBJECTIVE: To observe the influence of intra-articular injection of NaHA on mRNA expression of inducible nitric oxide synthase (iNOS) in cartilage of traumatic OA induced by transection of anterior cruciate ligament. DESIGN: Randomized controlled animal experiment. SETTING: Department of Orthopaedics, Renmin Hospital, Wuhan Uni ersity. MATERIALS: The experiment was performed in Laboratory of Depart ment of Orthopaedics, Renmin Hospital, Wuhan University from April to December 2003, in which, 16 clean healthy flat-eared white rabbits, aged 5-6 months were employed. The rabbits were randomly divided into Na- HA injection group and saline control group with 8 rabbits in each group. Na-HA (2000, No 366095) was provided by Shanghai Jiahua Fine Biologi- cal Products Co. METHODS: ①OA model was established in rabbits of the two groups. Each rabbit was anesthetized intravenously with 1.0 mg/kg ketamine hy- drochloride and underwent unilateral anterior cruciate ligament transection. ②5 weeks after transection, Na-HA injection group rabbits received 0.3 mL of intra-articular 10 g/L Na-HA injection, once a week for 5 weeks. Ani mals in saline controlled group were treated with saline of the same vol ume. ③The rabbits were killed at week 10 after operation, general morphology and histopathological changes of articular cartilage degeneration of medial femoral condyle were evaluated (0 points as smooth articular surface with normal color and luster; 1 point as minimal fibrillation or a slight crevice and dark grey color of the surface; 2 points as erosion extending into superficial or middle layers of cartilage; 3 points as ulceration and erosion extending into the deep layers, and 4 points as denudation of cartilage, erosion extending to the sub-chondral bone). The mRNA expression of iNOS in cartilages was examined with reverse transcription-polymerase chain reaction (RT-PCR). MAIN OUTCOME MEASURES: ①Observation of articular cartilage degeneration of medial femoral condyle, ②observation of articular cartilage degeneration of medial femoral condyle at light microscopic level, ③expression of iNOS in cartilages of each group. RESULTS: A total of 16 clean healthy rabbits entered the result analysis. ① Result of articular cartilage degeneration of medial femoral condyle: Pathological change of articular surface of femoral condyle was observed under anatomic microscope. Cartilage degradation in experimental group was significantly less severe than that in saline control group. ②Histological changes of articular cartilage degeneration of medial femoral condyle at light microscopic level: The Na-HA injection group showed cartilage changes: Membrane of cartilage presented denaturalization and abscission. Chondrocytes of superficial zone presented denaturalization, necrosis, turbulence and erosion. Animals treated with saline showed denaturalization, necrosis and disorder of chondrocytes, ulceration penetrating into the middle or deep zone of the cartilage. New hyperplasia of capillary vessels and fibroblasts were more obvious. Proliferation of fibrous tissue appeared at the bottom of ulcer. ③Expression of iNOS in cartilages of two group:The gene expression of iNOS in cartilage of Na-HA injection group was(1.09±0.18) and the expression of saline control group was (1.26±0.23). Nosignificant difference of iNOS expression was found between the two groups(P > 0.05).CONCLUSION: Intra-articular injection of Na-HA has protective and repairing effect on cartilage with early OA and can significantly reduce the severity of cartilage degradation during early stage of traumatic OA. Intra-articular injection of Na-HA does not down-regulated iNOS expression in cartilage.
8.Discussion of establishment of professional team in the neostomy and its effect

Objective To improve the specific nursing standards of neostomy and chronic wounds,make patients receive professional guidance and standard treatment. Methods To establish professional team in the neostomy and conduct systematic and specialized training for team members, to work out nursing standards in the neostomy and execute nursing consultation and discussion in team on special types of chronic wounds. Results The professional knowledge of the team members and clinical nurses was enhanced, 18lessons about 50 class-hours were given, the attendance rote reached above 96% since the establishment of professional team in the neostomy, the quality of clinical nursing was improved and the complications were reduced. Conclusions The establishment of professional team provides clinical nurses with a platform of communication and learning, reflect their own value, and promote development of nursing science.
9.The expressions of hypoxia inducible factor -1 and angiopoietin -2 in patients with angiodysplasia related gastrointestinal bleeding and treatment with thalidomide

Objective To study the expressions of hypoxia inducible factor (HIF)-1 and angiopoietin (Ang)-2 in repeated gastrointestinal bleeding due to vascular malformation, and the efficacy of treatment with thalidomide. Methods Twenty-five patients with repeated gastrointestinal bleeding due to vascular malformation confirmed by capsule endoscopy or enteroscopy were collected and 18 subjects without severe diseases were served as controls. Ten patients with gastrointestinal vascular malformation, who received 25 mg of thalidomide 4 times daily for 4 months and were followed up for at least one year, were also enrolled. The serum samples from all participauts were detected for expressions of HIF-1 and Ang-2 using enzyme-linked immunosorbent assay (ELISA).The expressions of HIF-1 and Ang-2 were compared between angiodysplasia group and control group.The expressions of HIF-1 and Ang-2 were comparatively evaluated before and after treatment with thalidomide in treatment group. Results The expressions of HIF-1 and Ang-2 in vascular malformation group [( 113. 84 ± 26. 66 ) ng/ml and ( 652. 11 ± 140. 39) ng/ml, respectively] were significantly higher than that of control group [(43.28±17.30) ng/ml and (265.60±53.88) ng/ml,respectively, P=0. 000]. The expression of HIF-1 was positively associated with that of Ang-2. (r=0. 700, P= 0. 000). There was no difference in expressions of HIF-1 and Ang-2 before and after treatment with thalidomide (P=0. 498 and =0. 136, respectively). However, significant reduction of Ang-2 [(113. 80±73. 60) ng/ml(P=0. 003)] was found in 8 effectively treated patients after thalidomide treatment. Conclusions HIF-1 and Ang-2 might play an important role in the formation of vascular malformation. The extent of Ang-2 reduction after thalidomide treatment may be helpful in evaluating its efficacy or prognosis.
10.Simultaneous determination of ten constituents in Zhenqi Fuzheng Capsules by UPLC-MS/MS

AIM To develop a UPLC-MS/MS method for the simultaneous content determination of adenosine,salidroside,chlorogenic acid,calycosin-7-O-β-D-glycoside,specnuezhenide,calycosin,formononetin,quercetin,apigenin and astragaloside Ⅳ in Zhenqi Fuzheng Capsules (Ligustri lucidi Fructus and Astragali Radix).METHODS The analysis of methanol extract of this drug was performed on a 40 ℃ thermostatic Inertsutain C18 column (75 mm × 3.0 mm,2 μm),with the mobile phase comprising of methanol-acetonitrile-4 mmol/L ammonium acetate flowing at 0.5 mL/min in a gradient elution manner.RESULTS Ten constituents showed good linear relationships within their own ranges (r≥0.996 0),whose average recoveries were 95.1%-104.3% with the RSDs of less than 4.20%.CONCLUSION This sensitive and specific method can be used for the rapid quality control of Zhenqi Fuzheng Capsules.
