1.Application of Painless Nursing in Postoperative Pain at Night Patients after Calcaneal Fractures Surgery

Objective:To explore the effect of painless nursing in patients with calcaneal fractures postoperative pain at night.Methods:Total 60 cases of orthopaedic hospital patients were randomly divided into control group and experimental group, 30 cases in each group.The control group had routine nursing, the painless nursing was used in experimental group.By updating the paramedics painless concept, clear pain assessment method and pain nursing records, the implementation of procedural pain in health education, will be painless care effectively applied in postoperative patients with calcaneal fractures pain at night.Joint pain evaluation method is used and the night sleep stage method compare two groups of postoperative pain at night is happening.Results:After the painless nurs-ing, the patients pain at night to sleep affect significantly reduce, the incidence of pain at night fell, the experi-mental group effectiveness were significantly higher than that of control group, the difference was statistically signifi-cant ( P<0.05) .Conclusions:Painless nursing can effectively prevent or reduce the occurrence of calcaneal frac-tures patients postoperative pain at night, promote the body′s recovery, improve patient′s satisfaction, is helpful to improve the quality of nursing.
2.Analysis on the Causes of Drug Repercussion and Study on Its Countermeasures in Our Outpatient Pharmacy

OBJECTIVE: To reduce and standardize drug repercussion and to guarantee rational,safe,economic and effective drug use on patients.METHODS: 449 records of drug repercussion in outpatient pharmacy of our hospital from Mar.to Nov.in 2009 were statistically analyzed to explore corresponding countermeasures.RESULTS: Drug repercussion was mainly cause by physicians (inadequate communication and false computer operation),patients (medication refusal resulting from inadequate knowledge,admission,discharge and death),drug shortage,wrong charge and other aspects (ADR,medical insurance).CONCLUSION: It is fact that there is drug repercussion.It requires the improvement of relevant system and skill of medical staff,strengthening of service awareness,enhancement of medication guide to avoid occurrence of drug repercussion.
3.Investigation and analysis on depression symptom in elderly inpatients

Objective To investigate and analyze the depression situation and the influencing factors in elderly inpatients.Meth-ods The cross-sectional method was used to conduct the investigation on randomly selected 125 elderly inpatients by the geriatric depressive scale(GDS)and the general personal questionnaire.Results Among 125 elderly inpatients,41 cases were depressed eld-erly inpatients,accounting for 32.8%.Disease,marital status,length of hospital stay,self-care ability,medical expenses and hobbies were the influence factors of the elderly inpatients(P<0.05).Conclusion The prevalence of depression in the hospitalized elderly patients is relatively high,the geriatrics should pay attention to it.
4.Application of hematology analysis combined with genetic diagnosis in prenatal diagnosis of thalassemia

Objective To evaluate the application value of the hematology analysis combined with genetic diagnosis in the prena-tal diagnosis of thalassemia.Methods 2 368 parturients were performed the prenatal diagnosis of thalassemia in the prenatal diag-nosis center of our hospital from July 2010 to April 2013,and those with positive hematological screening detection were performed the genetic diagnosis.Results The hematology analysis results showed 124 parturients of abnormal HbA2.The hematology analy-sis results in the abnormal parturients had the statistical difference compared with those in the rest healthy parturients (P <0.01). Among 124 pregnant women of hematological positive phenotype,there were 16 couples with the same genotype,in these 16 preg-nant women,15 cases were performed the prenatal diagnosis of thalassemia,there were 7 cases of αthalassemia gene,2 cases of βgenotype,6 cases of normal genotype,3 cases of severe fetal thalassemia,including 2 cases of α-thalassemia(genotype--SEA/--SEA ),1 case ofβ-thalassemia(genotype 41-42/IVS-Ⅱ-654).All pregnant women terminated pregnancy after informing the patients.Conclu-sion The hematology analysis combined with genetic diagnosis plays an important role in the construction of perfect thalassemia prenatal detection and diagnosis intervention mode,and is worthy of popularization and application.
5.Application of fluorescence in situ hybridization and chromosome karyotype analysis in prenatal diagnosis

Objective To investigate the application value of fluorescence in situ hybridization (FISH)in prenatal diagnosis and compared with karyotype analysis of cells from amniotic fluid.Methods A total of 183 high-risk pregnant women received for a FISH and karyotype analysis who taken prenatal disgnosis in Meizhou people′s hospital between April 2012 and November 2013. Results According to the test results of karyotype analysis.There were 9 cases of chromosomal abnormality in 183 pregnant women.Among 9 cases,3 cases were trisomy 21,3 cases were trisomy 18,2 cases were XO and one case of XXY.One case of tri-somy 18 was showed normal using the FISH.Other cases were consistent with karyotype analysis result.Conclusion FISH is a rapid,reliable and prompt prenatal diagnosis method,combined with karyotype analysis will maximize the detection efficacy in pre-natal diagnosis of chromosome abnormalities.
6.Effects of pegylated interferon on cellular immune function in patients with chronic hepatitis C

Objective To investigate the clinical effect of pegylated interferon(IFN)on chronic hepatitis C and its effect on cellular immune function.Methods A total of 92 patients with chronic hepatitis C were enrolled in our hospital from June 2013 to October 2016.They were randomly divided into study group and control group by single and double numbers(admission order),with 46 cases in each group.The control group was treated with recombinant human interferon alpha-2b on the basis of conventional treatment.The study group was treated with pegylated interferon alpha-2a on the basis of conventional treatment.Results After treatment, the rates of RVR, EVR, ETVR and SVR in the study group(52.17%, 71.74%, 76.09% and 54.35%, respectively)were significantly higher than those in the control group(34.78%, 50%, 58.70%, 39.13%, respectively), The difference was statistically significant(P<0.05).There were no significant difference in the cellular immune function index between the two groups before treatment;After treatment,the CD3+, CD4+, CD4+/CD8+ and other immune function indexes of the study group were significantly higher than those before treatment, while the results of CD8+ were significantly lower than before treatment(P<0.05);There was no significant difference in the cellular immune function in the control group compared with before, and the difference of the cellular immune function between the 2 groups after treatment was statistically significant(P<0.05).Conclusion Pegylated interferon(IFN)on the basis of routine treatment can improve the virologic response and cellular immune function in patients with chronic hepatitis C.

9.Multi-factor analysis of the central nervous system infection after invasive intracranial pressure monitoring

Objective To investigate the related factors of central nervous system infection in patients who received invasive intracranial pressure monitoring.Methods The clinical data of 63 patients who received invasive intracranial pressure monitoring were retrospectively analyzed.Possible related factors including gender,age,disease,Glasgow coma scale score,emergency operation or selective operation,duration of the operation,drainage tube placement site,drainage tube lifetime,sensor insertion site,sensor indwelling duration,electrolytes,glucose and other complications were studied and compared retrospectively between patients with and without central nervous system infection.All data were analyzed with univariate analysis and multivariate Logistic regression analysis to identify the related factors and independent risk factors associated with central nervous system infection.Results In univariate analysis,drainage tube placement site (x2 =12.345,P =0.002),drainage tube lifetime ((2.5 ± 0.9) d and (6.3 ± 5.7) d in patients without and with central nervous system infection,respectively,t =-4.434,P =0.000),sensor insertion site (x2 =4.701,P =0.030),sensor indwelling duration ((6.5 ± 3.0) d and (8.9 ± 4.6) d in patients without and with central nervous system infection,respectively,t =-2.420,P =0.019) and other complications (x2 =13.086,P =0.000) were the relevant factors of central nervous system infection (P <0.05).In multivariate Logistic regression analysis,the sensor insertion site (OR =0.047,P =0.000) and other complications (OR =10.953,P =0.019) were the independent risk factors for the central nervous system infection.Conclusions There are a variety of factors related to central nervous system infection in patients with placement of intracranial pressure monitoring.The sensor insertion site and other complications were the independent risk factors for the central nervous system infection.
