1.The application of reengineering of the dispense process of discharge medications for elderly cardiac patients

Objective The study aimed to improve the medication safety of aged patients through the reengineering of the dispense process of discharge medications for elderly cardiac patients.Methods The original dispense process was analyzed to find the existing problems,changing the order of process,increasing or decomposing steps of process in order to redesign,perfect and implement the distribution process of medications.Time and correction rate of drug dispensing for three times a day,mastering degree of related knowledge of oral medication and satisfaction degree with medication instruction were compared before and after reengineering of the process.Results Compared with the original process,patients' ability to understand the drug knowledge,the timing when the patients take three medications per day and the satisfaction degree with the instructions of taking medications all had statistical significance.Conclusions Reengineering of the dispense process of discharge medications for elderly cardiac patients can help to improve patients' understanding of drug knowledge,improve the security of taking medications outside the hospital as well as their satisfaction degree with nursing.
2.Expression of Breast Cancer Resistance Protein Gene in Breast Cancer Patients and Its Value in Evaluating the Therapeutic Effect of Chemotherapy for Breast Cancer

Objective To observe the expression of breast cancer resistance protein(BCRP) gene in breast cancer patients,and to explore its value in evaluating the therapeutic effect of chemotherapy for breast cancer.Methods Semi-quantitative reverse-transcription polymerase chain reaction(RT-PCR) was applied for the detection of mammary BCRP gene expression level in 60 female breast cancer patients who have received chemical therapeutic regimen of CEF(cyclophosphamide,epirubicin and 5-fluorouracil).The patients were divided into two groups according to the BCRP gene expression before CEF therapy.After treatment,the therapeutic effect in the two groups was observed and the BCRP gene expression level was also measured.Results In 17(17/60,28.3%) patients with positive BCRP gene expression,11 were relieved,with a relief rate of 64.7%,lower than 93.0% in patients with negative BCRP gene expression(P
3.Effect of fosinopril on the expression of NADPH oxidase p22phox mRNA in the kidneys of diabetic rats

Objective To study the effect of fosinopril on the expression of NADPH oxidase subunit p22phox mRNA and the extracellular matrix (ECM) accumulation in the kidneys of rats with diabetes mellitus . Methods Diabetic rats induced by streptozotocin were randomly divided into control group(DM group) and fosinopril group (fosinopril 10 mg'kg-1·d-1) (DM+Fosin group) and treated for 12 weeks . Expression of p22phox mRNA of NADPH oxidase in kidneys was measured by RT-PCR . The expression of fibroneetin was studied by immunohistochemistry and matrix metalloproteinases 9 activity was detected by Zymography . Meanwhile, the kidney hypertrophy index, serum creatinine level and 24-hour urinary protein excretion were evaluated . Results The expression level of p22phox mRNA in the kidneys of DM+Fosin group rats was decreased by 45% than that of DM group at week 4 (P<0 .05) . At week 8 fosinopril significantly decreased the expression of glomerular and tubulointerstitial fibronectin by 52,5% and 42 .9% respectively (P<0 .05), while increased MMP-9 activity by 29 .6% (P<0 .05) compared with DM group . Fosinopril significantly decreased 24-hour urinary protein excretion of diabetic rats from week 8 . Serum creatinine level, 24-hour urinary protein excretion and kidney hypertrophy index were significantly decreased by 35 .9%, 50 .2% and 17 .2% in rats of DM+Fosin group than those of DM group at week 12 (P<0 .05) . Fosinopril did not affect blood sugar significantly . Conclusion Fosinopril has beneficial effect on diabetic nephropathy partly through inhibiting the expression of NADPH oxidase p22 phox mRNA .

Objective To investigate the effects of metabolic syndrome (MS) and the cardiocerebrovascular risk factors such as essential hypertension (EH), diabetes mellitus (DM), and lipid metabolise disturbance on the subtypes of acute cerebrovascular disease (ACVD). Methods A total of 502 ACVD patients were studied, including 121 cases of intracerebral hemorrhage (ICH) and 381 cases of cerebral infarct (CI). The effects of MS, EH, DM, and blood lipids on the subtypes of ACVD were analyzed. 7.A System for Measurement of Pulse Wave Transit Time Continuously Based on Wireless Sensor Network

Objective To design a system for monitoring pulse wave transit time (PWTT) in working condition non-intrusively and continuously. Method The system was composed of wireless ECG sensor and wireless pulse wave sensor which measure pulse wave signal from the temporal artery and ECG signal from body synchronously and calculates PWTT continuously. Result Both the wireless ECG sensor and the wireless pulse wave sensor were small sized and powered by button battery. And the accuracy of time synchronization about sensors was less than 1 ms. The calculated PWTT changed slowly with deep breathing. Conclusion The system works smoothly for continuous monitoring of PWTT in working condition.
8.Detecting KRAS,EGFR mutations in nonsmall-cell lung cancer/colorectal cancer patients by high resolution melting curve analysis

ObjectiveTo evaluate the feasibility of detecting the mutations of KRAS,EGFR in nonsmall-cell lung cancer and colorectal cancer patients with high resolution melting curve analysis. MethodsAt first,the mutations in KRAS exon 2 and EGFR exons 18 to 21 of 5 patients with non-small cell lung cancer and 5 patients with colorectal cancer were detected by high resolution melting curve analysis.Then the results were confirmed with direct sequencing. ResultsBy high resolution melting curve analysis,1 of 10 patients was found to carry EGFR 19 mutation who was harboring 2236-2250del mutation.By direct sequencing,consistent results of the patients with mutations orwithout mutations were got. Conclusion s The new high resolution melting curve analysis was more efficient,more convenient than direct sequencing.Moreover,it was a low-cost test,which was suitable for clinic test.
9.Efficacy of ultra-short-acting β-blocker on cardiac ischemia in patients with serious coronary lesions

Objective: To explore the efficacy of esmolol on cardiac ischemia in patients with serious coronary lesions who are not candidates for coronary revascularizations. Methods: Fifty seven aged patients (34 male) with serious coronary artery disease not candidates for coronary revascularizations were included. All patients had classical angina involved in cardiac ischemia, and definitely ischemic ST-segment depressions in surface electrocardiogram, however which were not relieved by routine treatment including intravenous nitrate or even morphine. All patients received loading dose injection and continuously intravenous infusion of Esmolol, and the baseline characteristics before treatment and the effects at 4 hours after infusion of Esmolol were carefully recorded. Results: After infusion of Esmolol, the ischemia-related symptoms of 41 patients completely relieved, and their ST depressions recovered to the baseline accompanied with significant reduction in blood pressure and heart rate (P<0.0001 all) within (66±23) min. Moreover, nine patients were observed that their ischemia-related symptoms were relieved and ST alteration recovered partially (P<0.0001 both) at four h after continuous infusion of esmolol. Conclusion: Esmolol is effective to relieve the serious cardiac ischemia-related symptoms and ST-T alteration of ECG in patients with serious coronary lesions.
